        노래를 통한 초기 알츠하이머 치매환자의 자서전적 기억

        This study investigated the song-induced autobiographical memory of patients with early Alzheimer’s Dementia (AD) by comparing the effects of patient-selected songs (M-AD) versus music therapist-selected songs (M-MT). A total of 19 patients with early AD participated in this study. In the M-AD and M-MT conditions, each participant listened to a song and was instructed to recall memory. The time to recall memory, the specificity of the memory, mood changes, and the type of recalled memory were measured. Perceived familiarity and preference of the used songs and association of the song with the recalled memory were also analyzed. The results of the study showed that the M-AD condition elicited more specific memory and positive mood change than the M-MT condition. In addition, AD patients reported a higher level of familiarity with and preference of songs in the M-AD condition, compared to the M-MT condition. These results indicate that patient-selected songs, which have a personal meaning to an individual, could be effectively used for intervening with memory of this population, which would support music therapists to make better decision with regard to song selection. Further studies would be needed to deepen the understanding of autobiographical memory in older population with cognitive impairment and to propose more effective music therapy strategies for intervening with memory. 본 연구의 목적은 초기 알츠하이머 치매환자를 대상으로 본인이 선곡한 노래 조건과 치료사가 선곡한 노래 조건에서 자서전적 기억 회상의 내용, 회상하는데 걸린 시간과 기분변화를 측정하여 자서전적 기억회상에 효과적인 노래조건을 알아보는 것이다. 실험 동안 각 피험자는 자신이 선곡한 친숙한 노래(M-AD)와 치료사가 선곡한 노래(M-MT) 조건에서 노래를 듣다가 자신이 직접 경험한 기억이 떠오르면 손을 들게 하였고, 노래가 끝나면 인출된 기억을 이야기하도록 하였다. 피험자가 이야기한 내용은 녹음 전사하였고, 기억의 주체성과 내용, 회상 시간과 기분 변화 등과 관련하여 분석하였다. 본 연구 결과 M-AD 조건이 M-MT 조건에서 제공된 노래에 대한 친숙도와 선호도가 높았으며, 회상한 기억과 노래의 밀접함의 정도가 높았다. 또한 구체적으로 기억을 회상할 수 있었으며, 회상하는 데 걸린 시간도 짧았다. 또한 실험 전후의 기분변화 폭이 컸으며, 회상한 기억과 기분의 관련성에도 영향을 미쳤다. 본 연구는 초기 알츠하이머 환자 본인이 선곡한 노래가 자서전적 기억을 회상하는 단서가 되어 구체적이고, 긍정적인 기억을 촉진하며 회상시간이 단축됨을 보여주어 임상에서 초기 알츠하이머 환자의 자서전적 기억을 회상할 때, 본인이 선곡한 친숙한 노래가 효과적으로 사용될 수 있음을 보여줄 수 있음을 시사한다.

        COVID-19로 인한 노인의 신체활동, 사회참여 및 정신건강 변화 분석

        The purpose of this study is to figure out a change in physical activity, social participation and mental health of the elderly caused by COVID-19 and to look into which influence this change has upon mental health of old people. Targeting 184 senior citizens aged 65 or older who are dwelling in Y city of Gyeonggi Province, who have no problem with cognitive function, and who are free to communicate, the offline questionnaire survey was conducted over a total of three months from August of 2022 to October. To examine a change due to COVID-19, the Paired samples T-test was carried out. To verify the relative influence among physical activity, social participation and mental health of the elderly, the correlation analysis was implemented. According to the research results, first, elderly people’s physical activity, social participation and mental health caused by COVID-19 appeared to have a statistically significant difference(p<.001). Second, the correlation was indicated to be r=0.302, p=.000 between physical activity and mental health and to be r=0.147, p=.046 between social participation and mental health. Thus, there was a low positive(+) correlation.

        Effects of lamotrigine on attention-deficit hyperactivity disorder in pediatric epilepsy patients

        Purpose: The purpose of this study was to investigate the effects of lamotrigine for the treatment of attention-deficit hyperactivity disorder (ADHD) symptoms in children with epilepsy. Methods: Pediatric patients newly diagnosed with epilepsy (n=90 [61 boys and 29 girls]; mean age, 9.1±3.4 years) were enrolled. All patients were evaluated with the Korean ADHD rating scale (K-ARS)-IV before treatment with lamotrigine and after doses had been administered. The mean interval of ADHD testing was approximately 12.3 months. The initial dosage of lamotrigine was 1 mg/kg/day (maximum 25 mg/day for the first 2 weeks), and increased by 1 mg/kg every 2 weeks until titrated up to 7 mg/kg/day (or maximum 200 mg/day). Results: The mean ADHD test score of the 90 subjects was 17.0±1.8 at baseline. It was slightly reduced to 15.6±1.7 after lamotrigine monotherapy (P>0.01). Prior to treatment, a total of 31 patients (34.4%) met the diagnostic criteria for ADHD according to Diagnostic and Statistical Manual of Mental Disorders, 4th Edition, Text Revision, Of these 31 patients, 27 (87.1%) had significantly improved ADHD scores with lamotrigine monotherapy (28.0±1.6 reduced to 18.1±2.6, P<0.001). Among these 27 patients, 25 (92.6%) showed normalized electroencephalogram (EEG) and 26 (96.3%) achieved total freedom from seizures within 12 months of the initiation of lamotrigine monotherapy. Conclusion: The results from our study show that lamotrigine had a positive effect in pediatric epilepsy patients by reducing ADHD symptoms, preventing seizures, and normalizing EEG. However, further research is required to determine whether lamotrigine is efficacious against ADHD symptoms independent of its effects on epileptic seizures.

        비타민 D 보충이 운동선수의 신체적 퍼포먼스에 미치는 영향 : 무작위 대조 연구에 대한 체계적 문헌고찰 및 메타분석

        Purpose: The recent observational study was clarified what the vitamin D supplementation is related to the physical performance of an athlete. However, RCTs, which have been reported so far, have not been concluded yet because every researcher shows a difference as for the impact of vitamin D supplementation on the physical performance. Also, there are many existing systematic considerations or meta analyses that were conducted targeting mainly the general public. Thus, there is a limit to evaluating the role of vitamin D as the improvement in physical performance of an athlete. Accordingly, this study is aimed to examine the influence of the Vitamin D supplement therapy in an athlete upon physical performance through meta analysis. Method: A literature search was carried out the comprehensive retrieval by using MEDLINE, EMBASE, Cochrane Library. The integrated effect size of the collected research was analyzed by using statistical program R version 4.0.4 (Feb. 15, 2021). The effect size was distinguished in accordance with the guideline in Cohen’s d. Results: Totally 7 RCTs were included in meta analysis. The effect size of physical performance in an athlete after vitamin D supplementation stood at 0.39, thereby having reflected the meaningful effect size. The significant effect size was shown in counter movement jump(0.89) and Peak Power(1.24) out of the muscle power in physical performance. Conclusion: This meta analysis can be considered that the vitamin D supplementation has the impact on the counter movement jump and the Peak Power in an athlete’s physical performance. Nevertheless, the outcome needs to be interpreted carefully due to few number of usable RCTs and to the restrictions such as the research-based heterogeneity and the potential publication bias.

        Effects of low-dose topiramate on language function in children with migraine

        Purpose: This study aimed to verify the safety of low-dose topiramate on language development in pediatric patients with migraine. Methods: Thirty newly diagnosed pediatric patients with migraine who needed topiramate were enrolled and assessed twice with standard language tests, including the Test of Language Problem Solving Abilities (TOPs), Receptive and Expressive Vocabulary Test, Urimal Test of Articulation and Phonology, and computerized speech laboratory analysis. Data were collected before treatment, and topiramate as monotherapy was sustained for at least 3 months. The mean follow-up period was 4.3± 2.7 months. The mean topiramate dosage was 0.9 mg/kg/day. Results: The patient’s mean age was 144.1±42.3 months (male-to-female ratio, 9:21). The values of all the language parameters of the TOPs were not changed significantly after the topiramate treatment as follows: Determine cause, from 15.0±4.4 to 15.4±4.8 (P>0.05); making inference, from 17.6±5.6 to 17.5±6.6 (P>0.05); predicting, from 11.5±4.5 to 12.3±4.0 (P>0.05); and total TOPs score, from 44.1± 13.4 to 45.3±13.6 (P>0.05). The total mean length of utterance in words during the test decreased from 44.1±13.4 to 45.3±13.6 (P<0.05). The Receptive and Expressive Vocabulary Test results decreased from 97.7±22.1 to 96.3±19.9 months, and from 81.8±23.4 to 82.3±25.4 months, respectively (P>0.05). In the articulation and phonology validation in both groups, speech pitch and energy were not significant, and all the vowel test results showed no other significant values. Conclusion: No significant difference was found in the language-speaking ability between the patients; however, the number of vocabularies used decreased. Therefore, topiramate should be used cautiously for children with migraine.
