        돌발성 난청환자에서 전정기능의 평가

        Background and Objectives It is very important to evaluate vestibular function in patients with sudden sensorineural hearing loss (SNHL) because vertigo combined with sudden SNHL is well known as an important prognostic factor. However, the vestibular function test is not usually performed in sudden SNHL patients without vertigo. We investigated whether different vestibular function tests such as the results of vestibular evoked myogenic potential (VEMP) testing or caloric testing were correlated to prognosis in not only sudden SNHL patients with vertigo but also in sudden SNHL patients without vertigo. Subjects and Method We enrolled in our study 47 patients with idiopathic sudden SNHL with vertigo or without vertigo. The degree of initial hearing loss was categorized as mild, moderate, moderate-severe, severe, and profound group. Types of initial audiograms were categorized as high tone loss, low tone loss, flat and scale out type. We analyzed the association of the VEMP and caloric test with degree of hearing loss, type of audiogram, and hearing recovery in two month. Results The more sever the hearing loss was, the more increased was the rate of abnormal VEMP response (p=0.003). The rate VEMP and caloric abnormality was higher in hearing loss types with high tone loss than in the types with only low tone loss. The rate of accompanying vertigo was high (p=0.017), when the hearing loss was more severe. There was significant correlation between the hearing recovery and vestibular dysfunction (p=0.000, r=0.563). Conclusion VEMP and caloric test are useful in evaluating vestibular function and prognosis of sudden SNHL patients. Korean J Otorhinolaryngol-Head Neck Surg 2010;53:761-7

        기저세포암, 편평세포암 및 피부내암의 최신지견

        Incidence of basal and squamous cell carcinoma is increasing among Korean. With increasing this incidence we need to attend the management and prevention of these cutaneous malignancies. Basal cell carcinoma (BCC) is the most common skin malignancy. Sun exposure is the most important environmental cause of BCC. Daily sun protection especially avoiding sun exposure between 10 a.m. and 4 p.m. provides most effective prevention against chronic ultraviolet-induced skin damage. Hedgehog pathway inhibitors are used locally advanced and metastatic BCC lesions in Korea. Until recently, there are no reports whether it developed resistance of Hedgehog pathway inhibitors with advanced BCC in Korean patients. Among malignant skin tumors, 19.1% were squamous cell carcinoma (SCC). The incidence of SCC was rapidly increased over the past years. However, the incidence rate of SCC in Japan has not been changed from the period 1976-1980 to 1986-1990. Sentinel lymph node biopsy for SCC did not provide diagnostic value. Bowen’s disease and Erythroplasia of Queyrat are considered as carcinoma in situ of the skin. Erythroplasia of Queyrat have worse prognosis with a higher rate of malignant degeneration. Dermoscopy may helpful tool in assisting the noninvasive diagnosis of carcinoma in situ of the skin.

        사각형 구조물에 저장된 유체의 동적거동에 미치는 시간-가속도 형상의 영향

        유체 저장 구조물의 경우 지진 시 유체의 출렁임에 의하여 구조물에 동수압이 작용한다. 이 때, 유체의 동수압은 지진의 강도뿐만 아니라 유체 자유수면의 출렁임 높이(sloshing height)에 의해서도 변화한다. 이러한 하중의 변화에 영향을 미치는 인자로는 지진파의 형상, 최대지진강도, 유체 저장 구조물의 크기, 구조물의 폭, 유체의 높이 등이 있는데, 본 논문에서는 지진파의 형상과 유체 자유수면의 출렁임 형상의 상관관계를 규명하고자 하였다. 이를 위하여 구조물의 폭이 500 mm인 수조에 실지진파를 적용시켜 유체 자유수면의 출렁임 높이를 측정하고, 수치해석을 통하여 실험과 해석의 유사성을 검증하였다. 또한, 실험과 해석의 비교를 통하여 검증된 구조해석 기법을 적용하여 인공지진파의 형상을 다양하게 변화시키면서 유체 자유수면의 출렁임 형상의 변화를 측정하고, 인공지진파의 형상이 유체 자유수면의 출렁임 형상에 미치는 영향을 분석하였다. In the case of fluid storage structure, hydrostatic pressure acts on the structure due to fluid surge during an earthquake. At this time, hydrodynamic pressure of the fluid charge not only by the strength of the earthquake but also by the sloshing height of the fluid. Factors affecting the change of load include the size, width and height of the fluid storage structure and height of fluid, time-history shape, etc. This paper wanted to identify the relationship between the earthquake shape and fluid free surface shape. The sloshing height measured the height of the fluid by applying earthquake to a tank whose width 500mm and comparison of the experiment and analysis. In addition, the shape of the fluid free surface was measured while varying the shape of earthquake and effective of the shape of earthquake of the fluid was analyzed.

        GNAS1 유전자 과오돌연변이가 관찰된 가성부갑상선기능저하증 Ia형 1예

        Pseudohypoparathyroidism (PHP) is a group of genetic disorders in which the kidneys fail to respond to parathyroid hormone. Genetic defects in the GNAS complex locus lead to reduced Gsα (alpha-subunit of the heterotrimeric stimulatory G protein) activity in PHP type Ia patients. These patients exhibit characteristics of Albright hereditary osteodystrophy (AHO) and hypocalcemia, increased parathyroid hormone, and resistance to other Gsα protein-coupled hormones. AHO has a wide range of manifestations such as short stature, obesity, round face, subcutaneous ossification, and bone shortening in the hands and feet. In this study, we present the case of a 47-yr-old woman who was diagnosed with PHP type Ia with AHO. She showed tetany, dizziness, irritability to light, decreased visual acuity, cognitive impairment, and motor dysfunction. Direct sequencing identified a heterozygous missense mutation in exon 6 (c.466G>A, p.Asp156Asn) in GNAS1. To our knowledge, this case is the first report in Korea of PHP type Ia caused by a heterozygous missense mutation in exon 6 (c.466G>A, p.Asp156Asn) in GNAS1. 가성부갑상선기능저하증은 신장의 부갑상선 호르몬에 대한 반응저하를 유발하는 유전질환이다. 가성부갑상선기능저하증 Ia형에서 다양한 GNAS 복합부위의 결손들이 발견되었으며, GNS1 유전자 돌연변이는 Gsα 단백 활성도를 감소시킨다. 대부분의 가성부갑상선기능저하증 Ia형 환자는 특징적으로 알브라이트 유전성 골이영양증(Albright hereditary osteodystrophy)과 저칼슘혈증, 부갑상선호르몬의 증가, Gsα 단백이 매개하는 여러 호르몬들에 대한 저항성을 보인다. 알브라이트 유전성 골이영양증은 작은 신장, 비만, 둥근얼굴형, 피하 석회결절 그리고 발과 손의 단지증 등의 다양한 증상이 발현되는 증후군이다. 저자들은 알브라이트 유전성 골이영양증, 근강축, 어지러움, 빛에 대한 과민성과 시력저하, 인지 및 운동기능의 장애를 보이는 47세 여자환자에서 직접염기서열분석을 통해 국내에서는 최초로 GNAS1 유전자 엑손 6에서 과오돌연변이(c.466G>A,p.Asp156Asn)를 증명하여 가성부갑상선기능저하증 Ia형으로 진단한 증례를 경험하였기에 보고하는 바이다.

        콘크리트 구조물의 이상 탐지를 위한 시맨틱 분할 기반의 데이터 증강 기법

        Although innovative achievements are being made in various fields due to advances in artificial intelligence and deep learning technologies, there are still limitations in learning models using limited quantities and types of datasets. This paper proposes a Semantic Segmentation based Data Augmentation Techniques for Anomaly Detection of Concrete Structures. The proposed method extracts the defect part through semantic segmentation, and synthesizes objects and backgrounds through random positioning and alpha synthesis to generate an image dataset containing various backgrounds and defect combinations. This method was evaluated using Dice Score and IoU Score, and as a result of the experiment, this method shows higher performance than the existing method, so it can be applied practically in the field of detecting defects in concrete structures.
