        자궁경부종양에서 HPVDNAChip^(�) 및 Hybrid Capture Ⅱ^(TM)검사를 이용한 인유두종 바이러스의 검출

        목적 : 인유두종 바이러스는 자궁경부암의 의의있는 원인 인자이다. 이 연구의 목적은 HPVDNAChip??검사를 이용하여 정상, 전암병변, 침윤성 경부암의 인유두종 바이러스의 분포를 알아보고 그 결과를 Hybrid CaptureⅡ^(^(TM)) 검사와 비교하였다. 연구 방법 : 149명의 대상환자 중 57명은 병변이 없었고, 59명은 전암 병변이었으며, 33명은 침윤성 경부암이었다. HPVDNAChip??검사와 이 검사처럼 아형 66, 69를 제외한 고위험군(HPV-16/18/31/33/35/45/51/52/56/58/59/68) 인유두종 바이러스를 검사할 수 있는 Hybrid CaptureⅡ^(^(TM))검사를 이용하여 연구하였다. 결과 : 두 검사 모두 인유두종 바이러스를 검사하는 데 유용하였고, 결과가 의의있게 일치하였다(kappa value 0.721, p<0.01). Hybrid CaptureⅡ^(^(TM))검사에서 양성이나 HPVDNAChip??검사에서 음성인 경우는 18명(12.1%)으로 평균 Hybrid CaptureⅡ^(^(TM))값이 120.7±233.0 (mean±SD)이였고, 두 검사 모두 양성인 경우의 Hybrid CaptureⅡ^(^(TM))값은 448.1±732.8 (mean±SD)이였다. Hybrid CaptureⅡ^(^(TM))검사의 민감도는 94.6%이고, 특이도는 78.9%였고 HPVDNAChip??검사의 민감도는 83.7%이고, 특이도는 89.5%였다. HPVDNAChip??검사는 모두 15가지의 고위험군의 인유두종 바이러스 아형을 알아낼 수 있으며 아형 16이 전암병변(28.8%, 15/59)과 침윤성 경부암(48.5%, 19/33)에서 가장 흔한 아형이었으며 그 다음으로 흔한 아형은 58이었다. 결론 : HPVDNAChip??검사는 Hybrid CaptureⅡ^(^(TM))검사처럼 인유두종 바이러스 검사에 매우 민감하고 유용한 검사이다. HPVDNAChip??검사는 Hybrid CaptureⅡ^(^(TM))검사와 비교할 때, 인유두종 바이러스의 아형과 중복감염에 대한 정보를 준다. 앞으로 HPVDNAChip??검사의 자궁경부암의 선별검사로서의 가치에 대한 연구가 더 필요하다. Objective : Human Papillomavirus (HPV) is well known as one of the major risk HPVDNAChip?? factors for cervical cancer. The purpose of this study is to know HPV genotype distribution in women with normal cervix, precancerous lesion, and invasive cervical cancer by HPVDNAChip?? test. In addition, the result of HPVDNAChip?? test was compared with Hybrid CaptureⅡ^(TM) system for HPV detection. Methods : One hundred forty nine patients were included in this study. 57 women had normal cervix, 59 women had precancerous lesion, and 33 women had invasive cervical cancer. We tested them with two method for detection of Human Papillomavirus (HPV) by HPVDNAChip?? test and Hybrid CaptureⅡ^(TM) system. Hybrid CaptureⅡ^(TM) test can detect same high-risk HPVs (16/18/31/33/35/39/45/51/52/56/58/59/68) with HPVDNAChip?? test which can detect additional 66 & 69 high risk types of HPV. Results : Both methods for the detection of HPV were useful tests. The correlation between the results of two methods was very significant (kappa value 0.721 [p<0.01]). Positive Hybrid CaptureⅡ^(TM) test and negative HPVDNAChip?? test group were 18 women (12.1%) and average Hybrid CaptureⅡ^(TM) titer value of this group was 120.7±233.0 (mean±SD), positive Hybrid CaptureⅡ^(TM) test and positive HPVDNAChip?? test group were 84 women (54.4%) and average titer was 448.1±732.8. The Hybrid CaptureⅡ^(TM) test showed a sensitivity of 94.6% and a specificity of 78.9% and the HPVDNAChip?? test showed a sensitivity of 83.7% and a specificity of 89.5%. HPVDNAChip?? test detected total 14 genotype of HPV. HPV-16 was 28.8% (15/59) in precancerous lesion and 48.5% (19/33) in invasive cancer, most common in both groups. Next common type HPV-58 was 25.4% (12/59) in precancerous lesion and 9.1% (3/33) in invasive cancer. Conclusion : HPVDNAChip?? test is very sensitive and effective method for detection of Human Papillomavirus (HPV) infection as Hybrid CaptureⅡTMtest. In comparison with Hybrid CaptureⅡ^(TM) test, HPV genotype and multiple HPV infection information can be given by HPVDNAChip?? test. Further study will be needed to know the value of screening of cervical cancer by HPVDNAChip?? test in future.

        Jehovah's Witness comprise a unique obstetric population. Their refusal of blood stems from an interpretation of a literal translation of the Bible, and it is this belief that puts them at an increased risk of morbidity and mortality if hemorrhage occurs. We report two cases of a Jehovah's Witness who bled massively due to obstetric hemorrhage, refused blood transfusion and had profound anemia. The patients were treated with recombinant human erythropoietin, parenteral iron and oxygen. And they were treated on an intensive care unit with intermittent positive pressure ventilation. We reviewed with literature considering the therapy for acutely anemic patients who refuse transfusion to decrease the duration of the most severe anemia.

        The purpose of this study was to determine the antioxidative effects of Rhodiola sachalinensis Rhodiola methanol extract was fractionated sequentially with dichloromethane and butanol. Each Rhodiola fraction (water, MeOH, BuOH and CH_2Cl_2 fractions) showed the potent superoxide dismutase (SOD) activity, and had inhibitory effets on peroxide value of linoleic acid (40~57%) and lipid peroxidation (47~70%) in FE^2+/ascorbate system-induced rat liver microsome. Rhodiola methanol extract also recovered carbon tetrachloride-induced decrease in SOD by 42% and catalase activities by 50%, and had inhibitory effects (54%) on carbon tetrachloride-induced lipid peroxidation in rat liver microsome. These results suggest that Rhodiola sachalinensis has the antioxidative effects.

      • 길찾기 좋은 서울 (Legible Seoul)을 위한 전략 -지하철역 길찾기 시스템을 중심으로

        This study aims to explore problems of the way-finding system of Seoul subway station. In case of Seoul, there has been many problems of losing a way, looking around, going on a bootless errand, going back, asking and so forth for subway users whether they are foreigners or not. Specifically focused on four densely-used subway stations which are Seoul Station, City Hall Station, Express Bus Terminal Station, Myeong-dong Station, the research arguments are developed through site visit, investigation and analysis, discussion meeting and interviews. Investigating the real condition of signs in four subway stations, 3 types of problems are determined. The most serious problem is that the signs is not working as a way-finding system at the level of transfer guide, way-out and upper space guide, and circulation decision point guide. These problems are mostly driven because of failing to properly locate the sign and to provide the necessary level of information detail. The reason of those problems is examined from the point of planning process, production structure, and finally organization. In overall, the production of subway signs pre-empt sign system planning as well as subway sign manual do subway design guideline. Also the subway sign guideline and manual have not been developed based on behavioral and cognitive study, so it does not have any exact measuring standard and human scale to locate the signs. Policy implication is to suggest a supplimentation of subway sign guideline and manual based on user`s behavioral and cognitive characteristics, integrating interagency for better quality control of way-finding system, and several demonstration projects to improve sign system and remodeling projects.

        공공주도 근린재생사업의 로컬 거버넌스 특징 연구

        The purpose of this study was to explore characteristics of local governance in government-led neighborhood regeneration projects in Korea at the aspects of agencies and activities. Based on Rhodes’ governance theory, the research framework is composed of network and activity; analyizing agency of public·private·citizen sector, interdependence between organization through community participation, a significant degree of autonomy from the state, and continuing interaction through future cooperation intentions. 4 Case study is delivered with the interpretation of stakeholder’s interview and residents survey. The essential finding were as follow; First, government-led neighborhood regeneration projects are structuring various types of local governance including public/private/citizen sector and its intermediary agencies. Second, residents as major stakeholder are becoming interdepent on other agencies thrgouth government-led community participation programmes. Third, intermediary agencies’s role is significant to transform government-led local governance to community-led local governance. Forth, resident at citizen sector is intentionally collaborate with resident’s organisations, neighbors, public agenies, and professionals which is gearing a future continuing interactions for neighborhood regenerations.

        성인 남성의 대사증후군 예방을 위한 적정 체력수준

        전세계적으로 비만 인구의 증가와 함께 대사증후군 유병률이 증가하는 추세에 있으며, 우리나라에서도 이러한 양상이 급격하게 나타나고 있다. 이 연구에서는 성인남성 5,525명을 대상으로 조사한 결과 대사증후군 유병률이 18.2%로 나타났다. 이러한 대사증후군 유병률 추이를 감소시키고 예방하기 위해 이번 연구에서는 첫째, 비질환자와 대사증후군 질환자에서 결과적으로 체력수준에서 차이가 나타나는지를 심폐지구력, 근력, 유 연성, 순발력, 균형감각의 하위요소로 구분하여 비교하고, 둘째, 체력수준의 차이가 발행한다면 대사증후군을 예방하기 위한 체력의 적정수준 (cut- off)을 하위요소별로 산출하여, 대사증후군 유병률을 낮추기 위한 운동 프로그램의 기준을 제공하고자 하였다, 그 결과에 따르면 대사증후군 집단에서 심폐지구력, 유연성, 하지 순발력, 민첩성 (각각p<001)이 유의하게 낮은 것으로 나타났고, 근력(p<01)은 유의하게 높은 수준으로 나타났다, 이와 유사하게, 연령대별로는40~50대에서 비질환자 집단의 심폐지구력, 유연성, 하지 순발력, 균형감각(각각p<001)과 민첩성(p<01)이 모두 우수하게 나타난 반면, 근력(p<01)은 대사증후군 집단이 우수하게 나타나서 체력의 대사증후군에 대한영향력은40~50대에서 가장 의미있는 것으로 사료된다. 대사증후 군 유병률 예방을 위한 연령별 적정 체력수준을 제시하기 위해 ROC 곡선으로 하위요소별 적정체력 수준을 연령별로 산출한 결과에 따르면, 20~30대 청장년층의 심폐지구력 28.6ml/kg/min, 근력 42kg, 유연성 12cm, 하지 순발력 42cm, 민첩성 357sec, 균형감각 18sec의 적정수준이 산출되었고, 40~50대 중년층에서는 심폐지구력 27.gml/kg/min, 근력 39kg, 유연성 lcm, 하지 순발력 36cm, 민첩성 369sec, 균형감각 8sec로 나타났으며, 60~70대 노년층에서는 심폐지구력 VO2max 23.7ml/kg/min, 근력 27kg, 유연성-7cm, 하지 순발력 33cm, 민첩성 40lsec, 균형감각 8sec로 나타났다. 이 연구를 통해 산출한 적정수준의 체력수준은 연령별 운동프로그램을 작성하는데 필요한 운동 강도, 운동 시간 등의 운동량을 결정하는데 있어 중요한 기준이 될 수 있다. 이를 바탕으로 설계된 운동프로그램의 운동부하로 규칙적인 운동을 실시하면, 대사 증후군의 위험으로부터 안전한 건강상태가 유지될 수 있을 것으로 사료된다. There has been shown the rapid increase in the prevalence of metabolic syndrome along with obesity not only in worldwide but in nationwide. The prevalence rate of metabolic syndrome within adults male (n=5,525) in this study, was 18.2%. To prevent such steep increases of metabolic syndrome, we examined that, at first, the differences on fitness factors (cardiorespiratory fitness (VO2max), grip strength, flexibility, hindlimb power (vertical jump), agility, balance) between normal and metabolic syndrome group. As the first result, the level of VO2max, flexibility, vertical jump, balance were significantly low in metabolic syndrome group (p <.001), and agility (p <.001), grip strength (p <.01) were shown significant differences, For the comparison between each age groups, middle aged group (40-50s) showed significant difference by metabolic syndrome hi each fitness factors. It means that fitness level has critical impact on the prevalence of metabolic syndrome in middle aged group. As the second result, the cut-off values to prevent the incidence of metabolic syndrome were calculated by ROC curve. In youth group (20-30s) the associated cut-off values of the VO2max and grip strength and flexibility arid hindlimb power(vertical jump) and agility and balance as 28.6m1/kg/min and 42kg and 12cm arid 42cm and 357sec and l8sec respectively. In middle age group the associated cut-off values of the VO2 arid grip strength and flexibility and hindlimb power (vertical jump) and agility and balance as 27.9m1/kg/min and 39kg arid 1cm and 36cm and 369sec and 8sec respectively. In old age group (60-70s) the associated cut-off values of the VO2max and grip strength and flexibility and hiridlimb power (vertical jump) and agility and balance as 23.7m1/kg/miri and 27kg and-7cm and 33cm and 40lsec and 8sec respectively. These cut-off values would provide the optimal ranges for the age-specific exercise program to prevent metabolic syndrome. The exercise programs based on the values are expected to maintain healthy status from such chronic disease.

        사회적 경제조직의 지역공동체 역량 형성에 관한 연구

        이 연구는 지역의 생활환경 문제를 개선하기 위해 주민 중심으로 조직된 사회적 경제조직이 주민 주도형 사업 활성화에 기여할 수 있을 것을 전제로 사회적 경제조직의 현황 및 운영 실태를 파악하고, 지역공동체 역량 형성 여건과 그 특징을 살펴보는 것이 목적이다. 분석 결과를 정리하면 첫째, 우리나라에서 생활환경개선사업을 본격적으로 수행하는 사회적 경제조직은 34개로 전체 사회적 경제조직의 0.6%에 그치며, 이 중 48%가 수도권에 집중되어 있다. 둘째, 사회적 경제조직이 수행하는 생활환경개선사업은 주택건설, 주택개보수, (취약계층)집수리, 주거현물급여사업, 에너지효율개선사업, 저소득층 주거환경개선사업, 마을만들기, 텃밭/공원가꾸기, 마을카페 등 시설운영 등으로 다양하지만, 지역사회의 필요보다는 정부예산이 지원되는 취약계층 주거복지사업에 집중되어 있는 편이다. 셋째, 대부분 3~4년된 신생기업이지만 개인, 조직, 네트워크 역량 차원에서 지역사회의 인식과 참여형태의 변화, 이를 통한 조직 및 네트워크 구축이라는 차원에서는 지역공동체 역량 형성의 긍정적인 성과를 보여주고 있다. The study aims to explore the present conditions of social enterprises which are conducting residential environment improvement and to study how they build community capacity. The findings are as follows. First, there are 34 such organizations in Korea. This is only 0.6% of total social enterprises and 48% of them are located on Seoul and Koyung-gi Province. Second, their main business areas vary covering raging from housing construction, housing remodelling, housing welfare projects, building energy improvement, community building, urban gardening, community cafe business, etc. The requirements of government subsidy, however, make the organizations targets for the poor than the overall community members. Third, even though they are newly established, they already show positive impacts on community recognition, public participation, and community network. In this consideration, the social enterprises which are conducting residential environmental improvement projects contribute to build the regeneration capacity of the community.

      • 도시 준공공공간의 보행활성화 방안 -건물전면공간을 중심으로-

        While the street-and-building interface has been a key element in urban design for better street in New York, Boston, London and Tokyo, there has been a little attention to this practice in Seoul countparts. This difference stems from the identification of public realm on the street and public function for pedestrian activity. The street-and-building interface in Seoul is currently defined as a semi-public space but in reality it is a privately-owned space. This study aims to redefine the street-and-building interface as a semi-public space making a huge impact on the scenery of public space "street" and creating the clear character of urban streetscape for better pedestrian flow and pedestrian activity just like the building does. It also delivers several design principles and institutional approaches for better semi-public space. Several streets in each city are selected for an in-depth analysis of the street-and-building interface. Specific cases selected are Teheran-ro, Kangnam-daero, Echondong-gil, Garosu-gil(Seoul), 5th Avenue(New York), Newbury Street(Boston), London Wall Street, Brompton Road(London), Naka-dori, Hibiya-dori, Omotesando-dori, Aoyama-dori(Tokyo). The spatial characteristics / pedestrian function and land use / regulations of street-and-building interface has been reviewed. In-depth study is focused on several aspects of street-and-building interface; scale, spatial characteristics, pedestrian circulation, land use, policy regulation. It is an effort to capture a detailed pattern of street-and-building interaction regarding pedestrian flow and pedestrian activity. Typology of street-and-building interface is identified in terms of the spatial characteristics and functions. Spatial types of the street-and-building interface are first, street-widening type, second, front-access·front-connecting type, and third, plaza type. Functional types are first, intermediary between street and building, second, structuring pedestrian-oriented streetscape, and third, holistic urban design language for place-making The findings are as follows: 1) all of the case study areas except London have more space in front of the buildings by setback, 2) streets in Seoul have the most wide variations in setback and those in London have little variations forming orderly street-wall, 3) street-and-building interface type in Seoul is mostly simple street-widening vis-a-vis those in New York, Boston, London and Tokyo are mostly front-access·front-connecting type and plaza type, 4) street furniture, subway station and facilities in interface area are a lot and privately occupied in Seoul cases rather than the other case causing obstruction of pedestrian flow and pedestrian activity. 5) commercial streets in New York, Boston, London and Tokyo well maintained ground level land use such as small shops, restaurant, cafe and cultural use etc to facilitate pedestrian activities. Seoul regulates the street and street-and-building interface by a general statute such as Building Law and Planning Law. However New York, Boston, London and Tokyo regulate them by a special statute such as special district control and design guideline. Recently New York Zoning Ordinances strictly regulate by incentive a plaza type front space and prohibit ambiguous space for better pedestrian activity and orderly street wall. This is strictly implemented especially for important commercial corridors such as 5th Avenue. Policy recommendations are as follows, 1) deliberation of street-and-building interface on the basis of street characters, 2) set-up the design guildeline for the variety of semi-public space which deliver Seoul`s livability and dynamic of social activities, 3) incentive differentiation Strategy according to the contribution of pedestrian flow and pedestrian activity, and 4) street-and-building unitary design-management system.
