        교정적 치아이동 후 치아동요도 변화에 관한 연구

        올바른 교정치료계획이나 보정계획 수립을 위해서는 교정력 제거후 치주조직 재형성 양상 및 시기의 규명이 필요하다. 동적교정치료가 종료된 환자 6명을 대상으로 교정장치 제거일과 제거일로부터 1주, 2주, 3주, 4주, 6주, 8주, 10주, 12주, 16주, 20주, 24주에 상악의 경우 중절치부터 제1대구치까지, 하악의 경우 중절치부터 견치까지 좌우 치아의 동요도를 Periotest를 사용하여 측정하고 비교분석한 결과 다음과 같은 결론을 얻었다. 1. 교정장치 제거시 치아동요도는 개인에 따라 매우 다양하게 나타났으며 견치, 소구치, 대구치보다 절치에서 크게 나타났다. 2. 교정장치 제거 후 동요도는 시간 경과에 따라 감소하였는데 그 감소양상은 24주까지 지속되었다. 3. 상악절치는 장치제거 후 24주간 내내 지속적 감소양상을 보인 반면 나머지 치아는 처음 12주간은 급격한, 자중 12주간에서는 완만한 감소양상을 나타내었다. 4. 교정장치 제거 후 동요도 감소양상은 상악 제2소구치에서 가장 일관성있게 나타났다. 5. 연구기간 중 전치부 교합접촉 과다 정도에 따른 좌우 치아간 동요도는 통계학적 유의차를 보이지 않았다. 이상의 연구 결과는 치아이동 후 치주조직 재형성이 장치제거 6개월 후에도 완성되지 않음을 시사하였다. The purpose of the present study was to evaluate the change of tooth mobility following orthodontic tooth movement. Six orthodontic patients which had been treated with edgewise appliance were used. Tooth mobility was measured with Periostest at the time of the removal of orthodontic appliance and 1, 2, 3, 4, 6, 8, 10, 12, 16, 20, 24 weeks after appliance removal. Following results were obtained : 1. Tooth mobility upon the removal of orthodontic appliance showed individual variation while incisor showed greater mobility than the other teeth. 2. Tooth mobility showed continued decrease pattern until 24 weeks after appliance removal. 3. While maxillary incisors showed continued decrease pattern during the study period, the other teeth showed steep decline pattern during the first 12 weeks and gentle slope during the second 12 weeks. 4. The tooth mobility of the maxillary second premolar showed the most typical change in terms of the consistency of the decline. 5. There were no significant differences of tooth mobility between heavy- and light-contacted anterior teeth during experimental period. The results of the present study suggested that periodontal reorganization is not completed even in 24 weeks following orthodontic tooth movement.

        칼슘실리케이트 계열 치근단 역충전 재료의 체적 변화에 대한 micro-CT 분석

        The purpose of this study was to investigate the effect of blood contact and tooth mobility on volumetric changes of calcium silicate-based root-end filling materials using a micro-CT. Three calcium silicate-based materials (ProRoot MTA, Biodentine, and RetroMTA) were used in this study. Seventy-two extracted human single-rooted premolars were obturated with gutta percha. Root-end resection and root-end preparation were performed. After root-end filling with tested materials, the tooth specimens were immersed in saline or blood for 5 days in a 37 ℃ incubator (n=8). The tooth specimens were mounted in a chewing simulator to simulate tooth mobility with a force of 30 N and 20,000 cycles. Micro-CT scans were performed immediately after root-end filling and after exposure to storage media or simulation of tooth mobility. The volume loss (%) was obtained from difference in the percentage of defects of materials between first and second micro-CT scans. Apical volume loss (%; volume loss from resected surface to 1 mm from the surface) was calculated for tooth mobility simulating groups. Biodentine showed larger total volume loss than ProRoot MTA and RetroMTA in saline and blood. ProRoot MTA had smaller total volume loss in blood than in saline. Under the condition simulating tooth mobility, total volume loss was similar among materials, and apical volume loss of Biodentine was larger than that of RetroMTA. In conclusion, ProRoot MTA or Retro MTA is recommended in clinical situation of intentional replantation where tooth mobility or direct contact with blood may occur.

        A Case Report on the Incremental Dental Health Care for Periodontal Disease Patient Through Professional Mechanical Tooth Cleansing and Tooth Pik Method of Tooth-Brushing

        Periodontal disease is a prevalent disease for aged people and the most of the treatment has been done by operation or extraction of tooth, even though the patient didn’t want to do it. But in some cases, it can be controlled by preventive cares and maintaining by incremental dental health care system with continuous and periodic management for periodontal disease. A patient age 75 woman who had a severe periodontal problems with severe mobility and pain, swellow and redness on lower left Molar tooth which were already treated on Endodontic treatment, was controlled by PMTC after Oral Prophylaxis at dental clinic and trained to do the tooth-brushing with Tooth-Pik method, .at home care. It showed the sign of the gingival subside with disappears of inflammatory symptoms and regeneration of alveolar bone from 2 months later and remarkable alveolar bone regeneration by X-ray findings and decreased the mobility. It was concluded that some of the cases for severe periodontal disease patient can be controlled by preventive measures and continuous oral check with successfully.

        Longitudinal measurements of tooth mobility following orthodontic treatment

        Objective: The aim of the present study was to evaluate the changes in tooth mobility following orthodontic treatment and to obtain information regarding the guideline of retainer wear duration during the post-treatment period. Methods: The sample consisted of twenty patients who had been treated with edgewise appliances. The mobility of the maxillary teeth from the central incisor to the first molar was measured bilaterally by way of the Periotest?, a non-invasive, electronic device that provides an objective measurement of the reaction of the periodontium to a defined impact load. Tooth mobility was monitored at the time of the removal of the orthodontic appliances and subsequently at three-month intervals during the two years following appliance removal. Results: Tooth mobility decreased rapidly for the first six months and then decreased at a slower rate during the next six months; no statistically significant decrease in mobility was observed during the second year following appliance removal. Conclusions: The results of the present study suggest that adequate tooth stabilization is critical during the first six months following appliance removal and that continued wearing of retainers is recommended at least until twelve months after the completion of orthodontic treatment. 본 연구는 교정치료 종료 후 치아동요도의 종단적 계측을 통해 치아 주위조직의 안정화가 언제 나타나는지 살펴봄으로써 교정치료 후 유지장치 장착기간 설정에 도움이 되고자 시행되었다. 고정식 교정장치로 치료가 종료된 20명의 환자를 대상으로 교정장치 제거 직후 그리고 2년까지 3개월 간격으로 Periotest를 이용하여 상악 양측 중절치부터 제1대구치까지 치아동요도를 계측하고 교정치료 후 시간 경과에 따른 동요도 감소 양상을 비교 분석한 결과, 처음 6개월은 급한 양상으로, 다음 6개월은 보다 완만한 양상으로 감소하였고, 이후는 계속 감소하는 양상을 보였으나 통계적으로 유의한 차이를 보이지 않았다. 이상의 결과는 교정치료 후 치아 주위조직의 안정화는 1년이 지나서야 어느 정도 이루어짐을 그리고 처음 6개월은 매우 불안정함을 나타내는 것으로, 교정장치 제거 후 유지장치가 치료 후 1년간은 필요함을, 특히 처음 6개월 동안은 매우 중요함을 시사하였다.

        3D모델링 기술을 활용한 모바일 튜토리얼 방식의 치아카빙 실습지원도구 개발

        치의학 분야에 입문한 학습자가 치아 형태를 이해하고 임상의 기반 기술을 익히기 위해서는 치아카빙 실습이 필요하다. 치아카빙 실습은 단계별 모델을 필요할 때마다 관찰할 수 있을 때 더 효과적이다. 본 연구의 목적은 치아카빙 실습모형을 제안하고, 카빙 단계에 대한 3D모델링 정보를 제공하는 튜토리얼 방식의 모바일 실습지원도구를 개발하는 것이다. 연구결과 치아카빙 실습 모형은 치아형태학 강의 및 실습을 포함하는 강의실 활동과 학습자의 실습과 일상적 학습을 모바일 실습지원도구로 연계하는 모바일 심리스러닝 형태로 도출하였다. 모바일 실습지원도구는 치아형태학 사전, 치아카빙 실습 튜토리얼, 3D 치아 모델링으로 구현되었다. 개발된 콘텐츠에 대한 전문가 평가 결과 내용 및 기능이 타당하게 설계된 것으로 분석되었다(내용 타당도: 5.0, 인터페이스 타당도: 4.53). 따라서 개발된 모바일 실습지원도구도구는 치아카빙 실습의 모바일 심리스 러닝을 지원하는 데 적합한 학습도구로 판단된다. 본 연구를 기반으로 치의학 분야에 ICT를 활용하는 수업모형과 지원도구인 모바일 콘텐츠 개발 연구가 촉진되기를 기대한다. Tooth carving practice is required for novice learners in dentistry to understand dental morphology and acquire clinically underlying skills. Tooth carving practice is more effective when sequential models can be observed. The purpose of this study is to suggest a tooth carving practice model and to develop a mobile practice supporting tool based on tutorial providing 3D modeling information about steps of tooth carving. As a result, tooth carving practice model consists of the class activity including tooth information lecture and practice and mobile seamless learning connecting learners’ practice and regular learning with the mobile tutorial tool. The mobile tutorial tool is implemented with tooth morphology dictionary, tooth carving practice/training tutorial, and 3D tooth modeling. The experts’ evaluation on the developed contents shows that the content and function are valid(content validity: 5.0, interface validity: 4.53). Therefore, the mobile tutorial tool is suitable for supporting mobile seamless learning for tooth carving practice. Further researches are expected to be conducted to develop instructional models utilizing ICT and mobile contents in dentistry.

        개에서 periodontal splint를 이용한 치아유동성의 치료

        A 2-year-old, intact and a 10-year-old, castrated male Maltese were referred for treatment of progressivetooth mobility and periodontal disease. The first case was presented with tooth mobility of mandibular incisors (Grade2-3) and the second case was also presented with tooth mobility of maxillary incisors (Grade 2-3) by periodontal disease.The treatment plan included supragingival scaling, closed root planing, subgingival curettage and removable-fixedperiodontal splinting of the mandibular (case 1) and maxillary (case 2) incisors to stabilize them. Three months aftertherapy, oral examinations were performed for evaluation of success of therapy. In both cases, oral malodor, periodontaldisease and tooth mobility were resolved and periodontal splints were remained rigidly.

        Association between dental implants in the posterior region and traumatic occlusion in the adjacent premolars: a long-term follow-up clinical and radiographic analysis

        Purpose: The aim of this retrospective study was to determine the association between dental implants in the posterior region and traumatic occlusion in the adjacent premolars, using data collected during from 2002 to 2015. Methods: Traumatic occlusion in the adjacent premolars was assessed by examining clinical parameters (bleeding on probing, probing pocket depth, fremitus, and tooth mobility) and radiographic parameters (loss of supporting bone and widening of the periodontal ligament space) over a mean follow-up of 5 years. Clinical factors (gender, age, implant type, maxillary or mandibular position, opposing teeth, and duration of functional loading) were evaluated statistically in order to characterize the relationship between implants in the posterior region and traumatic occlusion in the adjacent premolars. Results: The study inclusion criteria were met by 283 patients, who had received 347 implants in the posterior region. The incidence of traumatic occlusion in the adjacent premolars was significantly higher for splinted implants (P=0.004), implants in the maxillary region (P<0.001), and when implants were present in the opposing teeth (P<0.001). The other clinical factors of gender, age, and duration of functional loading were not significantly associated with traumatic occlusion. Conclusions: This study found that the risk of traumatic occlusion in the adjacent premolars increased when splinted implants were placed in the maxillary molar region and when the teeth opposing an implant also contained implants.

        3차원 멀티미디어를 활용한 모바일 과제정보 제시가 치아카빙에 관한 지식, 수행 및 수업만족도에 미치는 효과

        본 연구는 3차원 멀티미디어를 활용한 모바일 과제정보 제시가 치아카빙지식, 수행 및 실습만족도에 미치는 효과를 검증하고자 하였다. 이러한 연구목적을 달성하기 위하여 치의학 전공 대학생 66명을 대상으로 3D 모델링 어플리케이션을 활용한 실험집단과 텍스트와 삽화로 구성된 2차원 수업자료를 활용한 통제집단에 각각 33명씩 배정하여 실험을 하였다. 연구결과 첫째, 2차원 수업자료를 활용한 과제정보 집단(통제집단)이 3차원 멀티미디어를 활용한 모바일 과제정보 집단보다 치아형태에 대한 이해가 높은 것으로 나타났다. 둘째, 3차원 멀티미디어를 활용한 수업이 학생들의 치아카빙 수행 능력 향상에서 유의미한 효과가 나타났다. 셋째, 3차원 멀티미디어를 활용한 모바일 과제정보 제시 집단의 수업만족도가 통제집단보다 더 높은 것으로 나타났다. 따라서 치아형태학 수업에서 3D 모델링을 제공하는 것은 수업만족도를 높여주며, 치아카빙 수행 향상을 돕는 교육콘텐츠로 활용이 가능할 것으로 보인다. The purpose of this study was to investigate the effect of mobile task information presentation using 3D multimedia on tooth carving knowledge, performance and class satisfaction for dentistry. To accomplish this purpose, we divided 66 dental students into the two groups: The experimental group was presented with mobile task information using 3D modeling. and the control group was presented task information using textbook. As a result for the study, First, Mobile task information presentation using 3D multimedia in class has significant effect on the tooth carving performance and class satisfaction. Second, tooth morphology knowledge in control group presenting task information using the text book showed relatively higher than in experimental group which presenting mobile task information using the 3D modeling. The conclusion of this study is that the class presenting mobile task information using 3D modeling can be enhances class satisfaction and, teaching and learning strategy to improve tooth carving performance.

      • Teeth recognition based on multiple attempts in mobile device

        Most traditional biometric approaches generally utilize a single image for personal identification. However, these approaches sometimes failed to recognize users in practical environment due to false-detected or undetected subject. Therefore, this paper proposes a novel recognition approach based on multiple frame images that are implemented in mobile devices. The aim of this paper is to improve the recognition accuracy and to reduce computational complexity through multiple attempts. Here, multiple attempts denote that multiple frame images are used in time of recognition procedure. Among sequential frame images, an adequate subject, i.e., teeth image, is chosen by subject selection module which is operated based on differential image entropy. The selected subject is then utilized as a biometric trait of traditional recognition algorithms including PCA, LDA, and EHMM. The performance evaluation of proposed method is performed using two teeth databases constructed by a mobile device. Through experimental results, we confirm that the proposed method exhibits improved recognition accuracy of about 3.6-4.8%, and offers the advantage of lower computational complexity than traditional biometric approaches.
