        It is important to determine the normal range of the spinal cord dimension for screening or localizing spinal pathologic processes which could change the size of cord, such as intramedullary tumor, syringomyelia, radiation and trauma. Although CT has been used extensively for the evaluation of spinal disease, the spinal cord cannot be defined from th subarachnoid space and dura without intrathecal injection of a contrast medium. A few studies have been reported that determined the dimensions of normal spinal cord and subarachnoid space, and moreover, a cross-sectional area of the thoracic cord has not yet been known. CT measurements of coronal and sagittal diameters including the coss-sectional area in the thoracic spines were obtained in 31 cases of normal Korean adults who were undergoing the contrast (Isovist) myelography without any pathology in the thoracic spines. The results were as follows: 1. The transverse diameters of the thoracic cord were 9.1mm(SD 1.1) in level (T1), 7.9mm(SD 0.8) in level 2(T5) and 7.3mm(SD 0.7) in level 3(T9). The sagittal diameters were 7.0mm(SD 0.9) in level 1, 6.1mm(SD 0.8) in level 2, and 6.3mm(SD 0.7) in level 3. 2. The transeverse diameters of the thorthratic subarachnoid space were 15.8mm(SD 1.4) in level 1, 14.3mm(SD 1.6) in level 2, and 13.9mm(SD 1.3) in level 3. The sagittal diameters were 12.9mm(SD 1.5), 12.3mm(SD 1.6) and 11.6mm(SD 1.5) in order of the levels. 3. The cross-sectional areas of the thoracic cord and the subarachnoid space were 58.0㎟(SD 7.5) and 165.4㎟ (SD 27.5) in level 1, 46.4㎟(SD 6.2) and 147.1㎟(SD 27.3) in level 3 separately. 4. The shape of the thoracic cord was elliptical in the upper thoracic spines, then became more round in the lower thoracic levels. The cross-sectional area of the thoracic cord is relatively smaller in the midthoracic spines than those of the upper and lower thoracic spines.

        Effects of Natural Selection, Mutagenesis, and Protoplast Formation and Cell Wall Regeneration on the Production of Aminoglycoside Antibiotics

        High producers or blocked mutants of aminoglycoside antibiotic-producing Streptomyces spp. were selected by application of an agar plug method and by culturing individual colonies in broth. The productivities of aminoglycoside antibiotic producing organisms were increased by selection of a high producer from colonies obtained by spreading spores of wild strain, or survived from treatment of a mutagen or from the colonies regenerated from protoplast-formation and cell-wall regenerations. Some mutagen treated colonies lost the ability to produce antibiotics (5-8%). Some A-factor negative and deostreptamine or streptidine negative mutants were obtained by N-methyl-N'-nitro-N-nitrosomethylguanidine (MNNG) treatment. Many of the survivors from the MNNG treatment lost the ability to produce antibiotics. Major colonies produced less amount of antibiotics ; only few survived colonies produced more antibiotics than the parent. Resistance of Streptomyces spp. against the antibiotics produced by itself was also markedly affected by mutagen treatment.

        한국인 간동맥의 정상변이에 관한 동맥조영술적 연구

        Anatomic structure of the hepatic artery is very complicated and also very variable. And so it is an essential knowledge to the surgical intervention of the liver and trans-arterial embolization of hepatoma. But almost of aoo the anatomic textbook or articles are dealing with the statistics of Caucasians, not Koreans. Study of the hepatic arterial anatomy and its variations in Koreans is a little. Some are from cadaver and the other from angiographic findings. And there are several different methods of classification of hepatic arterial variations without general agreement. We want to propose a new classification method of the hepatic arterial variation by branching patterns from celiac axis and patterns of the accessory hepatic artery, or variable origin of individual hepatic arteries. We want to report of results from angiographic findings from 72 Koreans by the new classification method.

        포진성 뇌염 1례

        We experienced a case of Herpes Simplex Encephalitis in a 3 year-old boy. The diagnosis was made by clinical symptoms, CSF study, serologic test, EEG and brain CT. Herpes simplex virus specific antibody was detected from serum and cerebrospinal fluid on 36th hospital day. Patient was treated with acyclocvir and improved in association with neurological sequallae. Brief review of literatures was made.

        Compound K Rich Fractions Regulate NF-κ<sub>B</sub>-dependent Inflammatory Responses and Protect Mice from Endotoxin-induced Lethal Shock

        In the previous studies, we isolated the compound K rich fractions (CKRF) and showed that CKRF inhibited Toll-like receptor (TLR) 4- or TLR9-induced inflammatory signaling. To extend our previous studies,1) we investigated the molecular mechanisms of CKRF in the TLR4-associated signaling via nuclear factor (NF)-${\kappa}B$, and in vivo role of CKRF for induction of tolerance in lipopolysaccharide (LPS)-induced septic shock. In murine bone marrow-dervied macrophages, CKRF significantly inhibited the induction of mRNA expression of proinflammatory mediators such as tumor necrosis factor-${\alpha}$, interleukin-6, cyclooxygenase-2, and inducible nitric oxide synthase. In addition, CKRF significantly attenuated the transcriptional activities of TLR4/LPS-induced NF-${\kappa}B$. Nuclear translocation of NF-${\kappa}B$ in response to LPS stimulation was significantly abrogated by pre-treatment with CKRF. Furthermore, CKRF inhibited the recruitment of p65 to the interferon-sensitive response element flanking region in response to LPS. Finally, oral administration of CKRF significantly protected mice from Gram-negative bacterial LPS-induced lethal shock and inhibited systemic inflammatory cytokine levels. Together, these results demonstrate that CKRF modulates the TLR4-dependent NF-${\kappa}B$ activation, and suggest a therapeutic role for Gram-negative septic shock.

        瓔兒性 肝血管內皮腫 2例 報告

        Hemangioendothelioma of the liver is a rare neoplasm which may be responsible for hepatomegaly or abdominal mass in infancy. We reports two cases of infantile hepatic hemangioendothelioma with exophytic growth from lateral segment of the left lobe. CT showed solitary, large, homogeneous circumscribed area of lower attenuation than that of the liver and spleen in left upper abdomen displacing the stomach medially, and dense peripheral enhancement was shown after bolus injection of contrast medium.

      • 내시경적 점막절제술로 치험한 식도 과립상 세포종 1례

        과립상 세포종은 Schwann 세포 기원으로 생각되며 인체에 비교적 드물게 발생한다. 과립상 세포종은 전신 어느 곳에서나 발견될 수 있으나 주로 혀, 구강, 피부 혹은 유방 등에서 호발하며 드물게 위장관에서 발견된다. 위장관에서는 식도에서 가장 호발하며 다음으로 위, 대장 순이다. 과립상 세포종은 대부분, 특히 위장관에서는 양성이며 소수의 악성 병변이 보고되었다. 이러한 이유와 함께 수술 전의 진단이 어렵기 때문에 과립상 세포종에 대한 근본적인 치료는 현재까지 외과적 절제술이다. 최근에 시도되는 치료방법들로는 내시경적 레이저 치료, 용종절제술, 내시경적 점막 절제술 등이 있다. 저자들은 상부 소화관 내시경검사를 시행하여 식도 과립상 세포종을 진단하고 내시경적 점막 절제술을 시행하여 합병증 없이 퇴원하여 현재 재발없이 경과 관찰중인 1례를 경험하였기에 보고하는 바이다. Granular cell tumors, which occur infrequently, are probably of Schwann cell origin. They can occur almost anywhere in the body but usually affect the tongue, oral cavity, skin, or breasts and are rarely found in the gastrointestinal tracts. The esophagus is the most frequent gastrointestinal site, followed by the stomach and the colon. Granular cell tumors are generally benign, especially in the gastrointestinal tract, some malignant lesions have been reported. For this reason, and also because preoperative diagnosis is difficult, the standard treatment for granular cell tumor has until now been surgical excision. In recent years, other therapeutic methods is endoscopic laser therapy (ELT), polypectiomy, endoscopic mucosal resection (EMR). We report a case of esophageal granular cell tumor which was diagnosed by an endoscopy and managed using an endoscopic mucosal resection without complication.

        Helicobacter pylori 감염진단을 위한 13C - 효소호기검사의 유용성 평가 : 다기관 공개 비비교 임상시험

        N/A Objective : Helicobacter pylori(H. pylori) infection plays an importanat role in the pathophysiology of gastrointestinal diseases, and its diagnosis is crucial in clinical practice. Currently many diagnostic methods are introduced for its simple, and accurate diagnosis. 13C urea breath test(13C-UBT) is a convenient, non-invasive and reliable test for the diagnosis of H. pylori infection. To assess the clinical value of the 13C-UBT, we examined the sensitivity, specificity of 13C-UBT with regard to other tests. Methods : A prospective multicenter study was done in 107 subjects(mean age 36 years, 55 males, 52 females) presenting for endoscopy from three university hospitals. We tested for H. pylori infection by 13C-UBT, serum IgG antibody level, and multiple biopsies for rapid urease testing and histology. Either a positive histology result or a combined positive CLO test and IgG ELISA in the presence of a negative histology results, was interpreted as a positive finding for H. pylori infection. 13C-UBT was performed after 4hrs fast. Breath samples were collected at 0 and 30 minutes after giving 75mg 13C-urea. Results: Results were analysed by isotope ratio mass spectrometry and expressed as units of delta( 13C, 13CO2 /12CO2) and considered as positive for H. pylori if delta value was greater than 4.0. Sensitivity and specificity of 13C-UBT were 94.5%, and 100% respectively. No significant adverse events were noted. Conclusions : The 13C-UBT offers an easy, safe and accurate approach to the diagnosis of H. pylori infection in routine clinical practice

      • 이중 박동형 생명 구조장치의 개발(T-Pls)

        심혈관 질환 특히 협심증과 급성 심근 경색증 등 관상동맥 질환이 급증함에 따라 병원 밖이나 응급실 중환자실 심도자 검사실등에서 급성 심장 쇼크에 빠지거나 심장마비가 발생하는 경우가 많이 발생하고 있다. 이러한 심혈관 질환에는 정류성 체외 생명보조장치가 널리 사용되고 있으나 이러한 장치는 그 무게와 크기로 인해 응급상황에 대처할 수 있는 능력이 작고, 막형 산화기의 전단에 인가되는 압력이 크며, 시스템 구성상 혈액이 공기에 노출된다. 그리하여 박동성 혈류를 공급할 수 있는 공기에 노출되지 않는 시스템(Close System)이 필요하다. 박동성 혈류를 공급하기 위하여 단일 구동펌프를 이용한 시도가 있었으나 회로내의 순간 압력 상승등으로 인한 단점이 도출되었다. 이러한 기존의 체외생명보조장치의 단점을 극복하고자, 한국형 인공심장의 원리를 이용한 박동식 이중 구동형 혈액펌프를 개발하였고, T-Pls(Twin-Pls)라 명칭하였다. 이 시스템의 구동원리는 막형산화기를 중심으로 전 ㆍ 후에 혈액펌프를 위치하여 혈액펌프를 Actuator가 한번씩 짜주는 것이다. 이런 방법으로 구동할 경우 막형산화기 후단에 있는 펌프가 음압을 동시에 일으킴으로, 막형산화기에 인가되는 큰 양압을 줄인다는 가정하에 이 시스템을 개발하였고, in vitro 실험을 통하여 각 부분의 압력을 측정하고, 총 12건의 동물실험(대형동물 : 6건, 소형동물 : 6건)을 통하여 그 장점을 검증하였다. Many cases of Acute cardiac shock and Cardiac arrest in Emergency room and ICU have been increasing. In this case, ECMO with Centrifugal pump has been used generally. However, due to the heavy weight and big size, the system is not adequate for Emergency cases. And other defects of this system are that Membrane Oxygeantor's pressure is high and blood are exposed to the air. There was some tries of ECMO using pulsatile pump, but it was found that the weak point of these system is high peak pressure and hemolysis. To overcome these defects, we have developed Twin Pulsatile Life System and we named 'T-Pls'. The mechanism of 'T-Pls' is that Membrane oxygenator Outlet Pump(MOP) make negative pressure when Membrane oxygenator Inlet Pump(MIP) provides high positive pressure, and the negative pressure will decrease positive pressure of Membrane Oxygeantor. Our group analyzed this advantage through In-vitro and 12 Cases In- Vivo test(Big animal : 6 cases, small animal : 6 cases).

        기존해 있는 다수의 멀티미디어 아키텍쳐에서 Motion Estimation Block은 ASIC에 의해 구현되어 왔다. 하지만 유연성의 결여와 NRE cost의 증가로 인해 ASIC에 의한 구현 외에 다양한 방법이 소개되고 있는 추세이다. 본 논문에서는 ASIP을 이용하여 Motion Estimation을 효과적으로 구현할 수 있음을 보이고 있다.
