      • Correlation between skeletal and dental changes after mandibular setback surgery-first orthodontic treatment

        Objective: To investigate skeletal and dental changes after application of a mandibular setback surgery-first orthodontic treatment approach in cases of skeletal Class III malocclusion. Methods: A retrospective study of 34 patients (23 men, 11 women; mean age, 26.2 ± 6.6 years) with skeletal Class III deformities, who underwent surgery-first orthodontic treatment, was conducted. Skeletal landmarks in the maxilla and mandible at three time points, pre-treatment (T0), immediate-postoperative (T1), and post-treatment (T2), were analyzed using cone-beam computed tomography (CBCT)-generated half-cephalograms. Results: The significant T0 to T1 mandibular changes occurred ?9.24 ± 3.97 mm horizontally. From T1 to T2, the mandible tended to move forward 1.22 ± 2.02 mm, while the condylar position (Cd to Po-perpendicular plane) shifted backward, and the coronoid process (Cp to FH plane) moved vertically. Between T1 and T2, the vertical dimension changed significantly (p < 0.05). Changes in the vertical dimension were significantly correlated to T1 to T2 changes in the Cd to Po-perpendicular plane (r = ?0.671, p = 0.034), and in the Cp to FH plane (r = 0.733, p = 0.016), as well as to T0 to T1 changes in the Cp to Poperpendicular plane (r = 0.758, p = 0.011). Conclusions: Greater alterations in the vertical dimension caused larger post-treatment (T2) stage skeletal changes. Studying the mandibular position in relation to the post-surgical vertical dimension emphasized the integral importance of vertical dimension control and proximal segment management to the success of surgery-first orthodontic treatment.

        암세포 증식에 대한 YB-1 안티센스 올리고핵산염의 영향

        목적: YB-1은 PCNA, DNA 중합효소 및 MDR 유전자 등의 전사인자로 작용한다. YB-1 유전자는 정상 어른의 간에서는 발현되지 않지만 태아의 간이나 재생 중인 간에서는 발현이 현저히 증가되어서 세포의 증식과 밀접한 관련이 있음이 보고되었다. 본 연구에서는 YB-1 유전자의 안티센스 올리고핵산염을 이용하여 YB-1의 발현을 억제함으로써 암세포 증식을 차단 할 수 있는지를 실험하고 암치료를 위한 유전자 요법으로서의 가능성을 알아보았다. 방법: 세포주로는 Chang liver, HepG2, CT-26 세포를 사용하였고, 사람의 정상세포로는 섬유아세포와 내피세포가 혼재된 조직을 사용하였다. YB-1 유전자의 안티센스 올리고핵산염으로는 YB-1 cDNA의 변역 시작 부위에 상보적인 21mer 올리고핵산염을 제작하여 사용하였다. 세포의 성장은 MTT assay를 이용하였고, 유전자 발현은 Northern blot으로 분석하였으며, 세포주기 변화는 propidium iodide로 염색하여 유식세포분석기로 분석하였다. 동물실험에서는 CT-26 세포를 1.0×10(5)개씩 Balb/c 생쥐의 피하에 접종하여 종양을 유도하였다. 종양이 유도된 Balb/c 생쥐에 YB-1 안티센스 올리고핵산염을 꼬리정맥이나 종양조직에 주사한 후 종양의 크기를 측정하여 종양억제 효과를 관찰하였다. 결과: YB-1 안티센스 올리고핵산염은 CT-26 세포에서는 50 nM 이상 농도에서 Chang liver와 HepG2 세포에서는 10 nM 이상 농도에서 세포주의 성장을 강하게 억제하였지만 정상 조직세포의 성장에는 아무런 영향을 미치지 않았다. 안티센스 올리고핵산염을 DOTAP에 담지한 경우 세포성장 억제에 미치는 효과가 안티센스 올리고핵산염 단독 처치 보다 강하였다. 이때 YB-1의 발현은 증식이 억제된 세포주(Chang liver 및 CT-26)에서는 감소하였으나 정상조직세포에서는 변화가 없었다. 증식이 억제된 세포주에서 세포주기를 살펴보면 초기에 S phase가 감소함을 관찰 할 수 있었다. 꼬리정맥이나 종양조직에 YB-1 안티센스 올리고핵산염을 주입 시 종양의 크기가 유의하게 감소하였다. 결론: 이상의 실험 결과 YB-1 안티센스 올리고핵산염은 암세포의 성장을 저해하며 종양 동물모델에서 종양의 성장을 억제할 수 있음을 시사하였다. Background: Human YB-1 is a transcription factor that binds to the inverted CCAAT box in the promoter region of a variety of genes such as PCNA, DNA polymerase and MDR. In this study we evaluated the effect of YB-1 antisense oligonucleotides on tumor cell growth. Methods: Chang liver, HepG2 and CT-26 cells were cultured as immortalized cell lines. The MTT (3-[4,5-dimethylthiazol-2-yl] 2,5-diphenyltetrazolium bromide) assay, Northern blot and flow cytometric analyses were used to determine cell growth, gene expression and cell cycle changes. In an animal model, CT-26 cells were injected into Balb/c mice to induce tumor; YB-1 antisense oligonucleotides were injected into the tail vein or tumor tissue of the mice; change of tumor size was then measured. Results: Phosphorothioated YB-1 antisense oligonucleotides suppressed the proliferation of the immortalized liver cells (Chang liver cells) and a variety of cancer cells (HepG2 and CT-26 cells); however, it did not inhibit normal cell growth. The DOTAP/antisense oligonucleotide mixture showed stronger effects on cell proliferation than did the antisense oligonucleotide alone. The YB-1 antisense oligonucleotide decreased specific expression of the YB-1 mRNA in the immortalized cancer cell lines. Flow cytometric analysis revealed that the inhibition of cell proliferation might have been due to a decrease in the S phase of the cell cycle. We found that in an animal tumor model, the administration of the YB-1 antisense oligonucleotide, in the vein or tumor tissues, decreased the tumor size significantly. Conclusions: These results suggest that the YB-1 antisense oligonucleotide may inhibit growth of a variety of cancer cells.(Korean J Med 71:293-301, 2006)

      • Projections of summertime ozone concentration over East Asia under multiple IPCC SRES emission scenarios

        <P><B>Abstract</B></P> <P>We have developed the Integrated Climate and Air Quality Modeling System (ICAMS) through the one-way nesting of global–regional models to examine the changes in the surface ozone concentrations over East Asia under future climate scenarios. Model simulations have been conducted for the present period of 1996–2005 to evaluate the performance of ICAMS. The simulated surface ozone concentrations reproduced the observed monthly mean concentrations at sites in East Asia with high R<SUP>2</SUP> values (0.4–0.9), indicating a successful simulation to capture both spatial and temporal variability. We then performed several model simulations with the six IPCC SRES scenarios (A2, A1B, A1FI, A1T, B1, and B2) for the next three periods, 2016–2025 (the 2020s), 2046–2055 (the 2050s), and 2091–2100 (the 2090s). The model results show that the projected changes of the annual daily mean maximum eight-hour (DM8H) surface ozone concentrations in summertime for East Asia are in the range of 2–8 ppb, −3 to 8 ppb, and −7 to 9 ppb for the 2020s, the 2050s, and the 2090s, respectively, and are primarily determined based on the emission changes of NO<SUB>x</SUB> and NMVOC. The maximum increases in the annual DM8H surface ozone and high-ozone events occur in the 2020s for all scenarios except for A2, implying that the air quality over East Asia is likely to get worse in the near future period (the 2020s) than in the far future periods (the 2050s and the 2090s). The changes in the future environment based on IPCC SRES scenarios would also influence the change in the occurrences of high-concentrations events more greatly than that of the annual DM8H surface ozone concentrations. Sensitivity simulations show that the emissions increase is the key factor in determining future regional surface ozone concentrations in the case of a developing country, China, whereas a developed country, Japan would be influenced more greatly by effects of the regional climate change than the increase in emissions.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Change in ozone level is determined by the combination of NO<SUB>x</SUB> and NMVOC emissions. </LI> <LI> Ozone level over East Asia would get worse in the near future (the 2020s) than far future. </LI> <LI> A high-concentration event generally increases and more frequently occurs in the future. </LI> <LI> Change in ozone air quality in a developing country depends more on emissions change. </LI> <LI> Change in ozone air quality in a developed country depends more on climate change. </LI> </UL> </P>

        Causes and Predictive Factors Associated with "Diagnosis Changed" Outcomes in Patients Notified as Tuberculosis Cases in a Private Tertiary Hospital

        Background: The aim of our study was to evaluate the "diagnosis changed" rate in patients notified as tuberculosis (TB) on the Korean TB surveillance system (KTBS). Methods: A total of 1,273 patients notified as TB cases on the KTBS in one private tertiary hospital in 2011 were enrolled in the present study. Patients were classified into three groups: "diagnosis maintained", "diagnosis changed" (initially notified as TB, but ultimately diagnosed as non-TB), and "administrative error" (notified as TB due to administrative errors). Results: Excluding 17 patients in the "administrative error" group, the "diagnosis maintained" and "diagnosis changed" groups included 1,097 (87.3%) and 159 patients (12.7%), respectively. Common causes of "diagnosis changed" were nontuberculous mycobacterial (NTM) disease (51.7%, 61/118), and pneumonia (17.8%) in cases notified as pulmonary TB, and meningitis (19.5%, 8/41) and Crohn's disease (12.2%) in cases notified as extrapulmonary TB. Being older than 35 years of age (odds ratio [OR], 2.18) and a positive acid-fast bacilli stain (OR, 1.58) were positive predictors and a TB-related radiological finding (OR, 0.42) was a negative predictor for a "diagnosis changed" result via multivariate logistic regression analysis in pulmonary TB cases. Conclusion: Because of a high "diagnosis changed" rate in TB notifications to the KTBS, the TB incidence rate measured by the KTBS may be overestimated. Considering the worldwide trend toward increased NTM disease, the "diagnosis changed" rate may increase over time. Thus, when reporting the annual TB notification rate in Korea, the exclusion of "diagnosis changed" cases is desirable.

      • A two-photon ratiometric probe for hydrogen polysulfide (H<sub>2</sub>S<sub>n</sub>): Increase in mitochondrial H<sub>2</sub>S<sub>n</sub> production in a Parkinson’s disease model

        <P><B>Abstract</B></P> <P>Hydrogen polysulfide (H<SUB>2</SUB>S<SUB>n</SUB>, n>1), which is primarily generated during the crosstalk between H<SUB>2</SUB>S and reactive species (ROS and RNS), is receiving increasing attention in biochemical research. H<SUB>2</SUB>S<SUB>n</SUB> is mostly generated in the mitochondria, and abnormal mitochondrial function and oxidative stress are directly related to many disorders including Parkinson's disease (PD). We now report a two-photon fluorescent probe (<B>SPS-M1</B>) for in situ detection of H<SUB>2</SUB>S<SUB>n</SUB> and its application to a PD model to account the H<SUB>2</SUB>S<SUB>n</SUB> levels. The probe exhibited selective and fast response to H<SUB>2</SUB>S<SUB>n</SUB> along with a marked blue-to-green color change. <B>SPS-M1</B> is sensitive enough to quantitative detection of endogenous H<SUB>2</SUB>S<SUB>n</SUB> content in mitochondria using two-photon microscopy (TPM). Ratiometric TPM imaging of live neurons and brain slices using <B>SPS-M1</B> revealed that H<SUB>2</SUB>S<SUB>n</SUB> production is increased to a greater extent in the A53 T α-synuclein (α-syn) overexpressing model than in the wild-type control. These findings suggest that the interactions of H<SUB>2</SUB>S and the increased ROS caused by α-syn overexpression may generate more H<SUB>2</SUB>S<SUB>n</SUB>. By employing our recently published TP probe for mitochondrial H<SUB>2</SUB>S, we also found the relationship between the H<SUB>2</SUB>S<SUB>n</SUB> and H<SUB>2</SUB>S; increased H<SUB>2</SUB>S<SUB>n</SUB> and decreased H<SUB>2</SUB>S levels, indicating that H<SUB>2</SUB>S and H<SUB>2</SUB>S<SUB>n</SUB> may play a significant role in the pathogenesis of PD.         Necrotizing soft tissue infection

        Purpose: Necrotizing soft tissue infection is the infection of the soft tissue with necrotic changes. It is rare, but results in high mortality. We analyzed the characteristics of patients, prognosis, and mortality factors after reviewing 30 cases of a single hospital for 5 years. Methods: From January 2009 to December 2013, 30 patients diagnosed with necrotizing fasciitis or Fournier’s gangrene in Pusan National University Hospital were enrolled for this study. The following parameters were analyzed retrospectively: demographics, infection site, initial laboratory finding, initial antibiotics, isolated microorganisms, number of surgeries, time to first operation, length of intensive care unit, and total hospital stays. Results: The overall mortality rate was 23.3%. Mean body mass index (BMI) of the survival group (24.7 ± 5.0 kg/㎡) was significantly higher than the nonsurvival group (22.0 ± 1.4 kg/㎡, P = 0.029). When BMI was less than 23 kg/㎡, the mortality rate was significantly higher (P = 0.025). Two patients (6.7%) with chronic kidney disease requiring hemodialysis died (P = 0.048). Initial WBC count (>13×10<SUP>3</SUP>/μL), CRP (>26.5 mg/dL), and platelet (PLT) count (<148×10<SUP>3</SUP>/μL) were found to have negative impact on the prognosis of necrotizing soft tissue infection. Factors such as potassium level, blood urea nitrogen (>27.6 mg/dL), serum creatinine (>1.2 mg/dL) that reflected kidney function were significant mortality factors. Conclusion: Patients with low BMI or abnormal values of WBC count, CRP, and PLT count reflecting the degree of infection or abnormal renal function will need more intensive care.

        <P>A novel synthesis method for microspheres using micromolds based on the Laplace pressure difference is presented by Jinkee Lee, Chang‐Soo Lee, and co‐workers on page 5078. This simple micromolding technique makes it possible to synthesize highly monodispersed microspheres with the flexibility of various reaction schemes, such as photopolymerization, sol‐gel reactions, and colloidal assembly. This method is promising for the mass production of various sizes of monodispersed microspheres without the need for complicated control of fluids. </P>

        Multi-GNSS Standard Point Positioning using GPS, GLONASS, BeiDou and QZSS Measurements Recorded at MKPO Reference Station in South Korea

        The Global Navigation Satellite System (GNSS) is undergoing dramatic changes. Nowadays, much more satellites are transmitting navigation data at more frequencies. A multi-GNSS analysis is performed to improve the positioning accuracy by processing combined observations from different GNSS. The multi-GNSS technique can improve significantly the positioning accuracy. In this paper, we present a combined Global Positioning System (GPS), the GLObal NAvigation Satellite System (GLONASS), the China Satellite Navigation System (BeiDou), and the Quasi-Zenith Satellite System (QZSS) standard point positioning (SPP) method to exploit all currently available GNSS observations at Mokpo (MKPO) station in South Korea. We also investigate the multi-GNSS data recorded at MKPO reference station. The positioning accuracy is compared with several combinations of the satellite systems. Because of the different frequencies and signal structure of the different GNSS, intersystem biases (ISB) parameters for code observations have to be estimated together with receiver clocks in multi-GNSS SPP. We also present GPS/GLONASS and GPS/BeiDou ISB values estimated by the daily average.
