        Viewers are supposed to response in cognition and affect toward the advertisement. Attitude toward the ad is considered an evaluation of stimulus based on the cognition to the ad. Some scientists identified it including affect, others characterized it affect itself. This study accepts the identification of the attitude toward the ad as affect in cognitive process of consumers while assumes pure affect in affective response which is not contaminated by the cognition. Therefore we can try to find out the effects of the affect toward the ad in both sides, attitude toward the ad and affect itself. This propose is clued on the dual mediation process of attitude toward the ad and the studies on affective response and its effectiveness. Finally we will get the findings that consumers experience affect from viewing the ad and form their attitudes toward the brand and purchase intention including cognition to the brand through the attitude toward the ad and affective response. Further project can be tried to do with topic finding affect's dimensions and which one acts on which specific effects and planning to induce affective response to get effective results through the ad.

        Background: Probe-based confocal laser endomicroscopy (pCLE) is a novel method that provides in-vivo histology of colorectal neoplasms. However, there has been no study to evaluate in-vivo histology in colorectal submucosa. The aim of this study was to assess the feasibility and safety of pCLE in evaluating colorectal submucosa layer and suggest diagnostic criteria of cancer infi ltration in submucosa. Methods: This was a prospective, single-center pilot study. From March 2014 to June 2014, 20 patients who underwent scheduled colonoscopic procedures were enrolled. All procedures were performed by a single expert colonoscopist. The pCLE videos of each lesion were acquired and biopsies were taken for histopathology. Results: Thirty submucosal layers of twenty patients were analyzed. Among 30 lesions, real-time video sequences were obtained in all lesions with mosaic images. Most of the images were adequate for evaluation of cellular morphology and structure of the submucosal layers. We classifi ed submucosa into normal, fi brosis, and cancer infi ltration with representative images. In the layer which was dissected into the deep submucosa, muscles that were just below were shown as longitudinal structure of single myofi brils of a cylindrical shape with bright stained capillaries (fi gure 1(A)). In superfi cial submucosa, small and round cells were scattered singly (fi gure 1(B)). Bright linear shape structures were woven irregularly without cellular structure in submucosal fi brosis without carcinoma infi ltrations (fi gure 1(C)). Dark and irregular cell nests which were contained irregular cell architecture with little or no mucin were shown at submucosa with invasion of adenocarcinoma (fi gure 2). No adverse events occurred during the procedures. Conclusions: Submucosal assessment of pCLE during colonoscopy is feasible and safe. Prospective large scale studies are mandatory for further evaluation of the clinical impact of this technology during endoscopic procedure.

      • Poster Session : PS 0869 ; Lower GI Tract : Long-Term Outcome of Endoscopic Treatment for Early Colorectal Cancer with Submucosal Invasion Compared to Surgery

        Background: Endoscopic treatment for early colorectal cancer (ECC) with submucosal invasion has been regarded as one of therapeutic options. However, the long-term efficacy has not been validated. Methods: A total of 428 patients with ECC who was treated with endoscopic procedures (n=224) or surgery (n=204), as an initial treatment, at Severance Hospital between January 2005 and March 2012 was included. The patients were subdivided into four groups according to the standard indication (SI) of each therapeutic modality. The SI was defined as achieving negative resection margins and satisfying three of the following criteria; (a) within 1,000 μm from muscularis mucosa; (b) no lymphovascular invasion (LVI); (c) well or moderately-differentiated histology. For prognosis evaluation, short-term outcomes (resection margin and complications) and long-term outcomes (recurrence and cancer-specific mortality) were evaluated. Results: The baseline characteristics were not different between initial endoscopic therapy and surgery in ECC patients with SI except tumor size (18.1±9.2 mm vs. 23.7±14.4 mm, p=0.005). In a total of 224 endoscopic therapies, en-bloc resection was achieved in 86.6%. For complications, delayed bleeding was occurred in one (0.4%) and bowel perforation was occurred in two (0.9%). In 204 initial surgery cases, there were seven of anastomosis site leakage (3.4%), two of intestinal obstruction (1.0%), and one case of urinary dysfunction, pneumonia and mortality (0.5%, respectively). There was no difference in recurrence and cancer-specific mortality between endoscopic therapy and surgical therapy groups with or without SI. In patients with ECC, lymph node metastasis was independently related with positive LVI and poorly-differentiated histology in multivariate analysis (p<0.001 and p=0.001, respectively). Conclusions: Endoscopic treatment with SI is a safe and an effective therapy for ECC as an initial therapeutic option with favorable long-term clinical efficacy compared to surgery.

        Overlooked Management and Risk Factors for Anemia in Patients with Intestinal Behcet`s Disease in Actual Clinical Practice

        Background/Aims: Anemia in patients with inflammatory bowel disease significantly affects the quality of life. The aim of this study was to investigate the frequency of and risk factors for anemia and to describe the management of anemia in patients with intestinal Behcet’s disease (BD) in actual clinical practice. Methods: We included 64 patients with intestinal BD who visited the outpatient clinic of a tertiary referral center in June 2011 and had available laboratory data for the subsequent 6 months. Results: Anemia was detected in 26 patients (40.6%). After 6 months, anemia was still present in 14 of these patients (53.8%). The cause of anemia was investigated in eight patients (30.8%), and oral iron supplementation was prescribed to four patients (15.4%). Of these four patients, two (50%) recovered completely within 6 months. Anemia was associated with a high Disease Activity Index for Intestinal Behcet’s Disease (DAIBD, p=0.024), erythrocyte sedimentation rate (p=0.003), and C-reactive protein (p=0.049) in univariate analysis. In multivariate analysis, the factor predictive for anemia in patients with intestinal BD was a higher DAIBD (≥40, odds ratio, 4.08, 95% confidence interval, 1.21 to 13.71, p=0.023). Conclusions: Although anemia is common in intestinal BD patients, its clinical importance is overlooked in daily practice. Moderate to severe disease activity is predictive of anemia.         The Rebleeding Risk and Prognostic Factors of Acute Hemorrhagic Rectal Ulcer

        Background/Aims: Acute hemorrhagic rectal ulcer (AHRU) is an important etiology of lower gastrointestinal bleeding in intensive care unit patients and hospital inpatients. Moreover, with increasing elderly populations, and improved survival in critically ill patients, the incidence of AHRU has increased. The aim of this study is to determine rebleeding risk and prognostic factors of AHRU patients. Methods: We retrospectively reviewed 32 patients with AHRU in Severance Hospital from February 2006 to October 2010, collected clinical data, and analyzed their association with the recurrence of bleeding and mortality of patients. Results: The mean age of patients was 65.5 years, and 27 patients (84.4%) showed Eastern Cooperative Oncology Group performance status 3-4. Nineteen patients (59.4%) had recurrent bleeding. Hypoalbuminemia (≤2.5 g/dL) was a risk factor of rebleeding in univariate and multivariate analysis. For patients with chronic liver disease, hypoalbuminemia (≤2.5 g/dL), renal dysfunction (>2 mg/dL) and thrombocytopenia (<150,000/μL) showed relatively earlier rebleeding than those without (P=0.007, P=0.009, P=0.027 and P=0.043, respectively). The endoscopic hemostasis at the first bleeding event was associated with lower early rebleeding rate (P=0.048). In univariate analysis, chronic liver disease, hypoalbuminemia (≤2.5 g/dL) and the prolongation of activated partial thromboplastin time (>40 seconds) increased mortality (P=0.028, P=0.008 and P=0.027, respectively) and the patients with rebleeding showed a tendency toward higher mortality, compared to those without (57.9% vs. 23.1%, P=0.051). Conclusions: In AHRU patients, hypoalbuminemia was a risk factor of rebleeding, and chronic liver disease, hypoalbuminemia, renal dysfunction, thrombocytopenia and no endoscopic treatment at the first bleeding event was correlated with relatively earlier rebleeding.       • Peroxide-Mediated Alkyl-Alkyl Coupling of Dialkylzinc: A Useful Tool for Synthesis of ABA-Type Olefin Triblock Copolymers

        <P>A practical and simple method for the preparation of ABA-type olefin triblock copolymers, e.g., PE-<I>b</I>-poly(ethylene-<I>co</I>-propylene)-<I>b</I>-PE, has been described. The performance of the so-called “coordinative chain transfer polymerization” (CCTP) by sequential feed of ethylene and ethylene/propylene mixed gas generated a Zn-bound diblock copolymer (i.e., (PE-<I>b</I>-poly(ethylene-<I>co</I>-propylene)yl)<SUB>2</SUB>Zn). Treatment of the Zn-bound diblock copolymer with lauroyl peroxide (CH<SUB>3</SUB>(CH<SUB>2</SUB>)<SUB>10</SUB>C(O)O-OC(O)(CH<SUB>2</SUB>)<SUB>10</SUB>CH<SUB>3</SUB>) led to a C(sp<SUP>3</SUP>)-C(sp<SUP>3</SUP>) coupling reaction between the two diblock chains bound on the zinc site and an ABA-type olefin triblock copolymer, PE-<I>b</I>-poly(ethylene-<I>co</I>-propylene)-<I>b</I>-PE, was formed. Upon the treatment with lauroyl peroxide, the number-average molecular weight increased by 1.5-1.7 times. The occurrence of the coupling reaction was verified using a model compound, (Oct)<SUB>2</SUB>Zn. The ABA-type triblock copolymer exhibited thermoplastic elastomeric properties and dramatically improved mechanical properties (twice the tensile strength and 10-fold increase in the elongation at break) as compared to the diblock congener.</P> [FIG OMISSION]</BR>

      • An injectable cationic hydrogel electrostatically interacted with BMP2 to enhance <i>in vivo</i> osteogenic differentiation of human turbinate mesenchymal stem cells

        <P><B>Abstract</B></P> <P>We have designed and characterized an injectable, electrostatically bonded, <I>in situ</I>–forming hydrogel system consisting of a cationic polyelectrolyte [(methoxy)polyethylene glycol-<I>b</I>-(poly(ε-caprolactone)-<I>ran</I>-poly(L-lactic acid)] (MP) copolymer derivatized with an amine group (MP-NH<SUB>2</SUB>) and anionic BMP2. To the best of our knowledge, there have been hardly any studies that have investigated electrostatically bonded, <I>in situ</I>–forming hydrogel systems consisting of MP-NH<SUB>2</SUB> and BMP2, with respect to how they promote <I>in vivo</I> osteogenic differentiation of human turbinate mesenchymal stem cells (hTMSCs). Injectable formulations almost immediately formed an electrostatically loaded hydrogel depot containing BMP2, upon injection into mice. The hydrogel features and stability of BMP2 inside the hydrogel were significantly affected by the electrostatic attraction between BMP2 and MP-NH<SUB>2</SUB>. Additionally, the time BMP2 spent inside the hydrogel depot was prolonged <I>in vivo</I>, as evidenced by <I>in vivo</I> near-infrared fluorescence imaging. Biocompatibility was demonstrated by the fact that hTMSCs survived <I>in vivo</I>, even after 8 weeks and even though relatively few macrophages were in the hydrogel depot. The osteogenic capacity of the electrostatically loaded hydrogel implants containing BMP2 was higher than that of a hydrogel that was simply loaded with BMP2, as evidenced by Alizarin Red S, von Kossa, and hematoxylin and eosin staining as well as osteonectin, osteopontin, osteocalcin, and type 1α collagen mRNA expression. The results confirmed that our injectable, <I>in situ</I>–forming hydrogel system, electrostatically loaded with BMP2, can enhance <I>in vivo</I> osteogenic differentiation of hTMSCs.</P> <P><B>Highlights</B></P> <P> <UL> <LI> An injectable, electrostatically bonded, <I>in situ</I>–forming hydrogel system consisting of a cationic polyelectrolyte copolymer derivatized with an amine group and anionic BMP2 was prepared. </LI> <LI> The hydrogel features and stability of BMP2 inside the hydrogel were significantly affected by the electrostatic attraction between BMP2 and cationic polyelectrolyte copolymer. </LI> <LI> The electrostatically loaded hydrogel enhanced osteogenic differentiation of human turbinate mesenchymal stem cells better than one with simple loading of BMP2. </LI> </UL> </P>

      • Proximal disease extension and related predicting factors in ulcerative proctitis

        <P><B><I>Objective.</I></B> Ulcerative colitis usually involves the rectum, may extend in a proximal and continuous fashion to involve varying portions of the bowel. However, the risk factors predictive of proximal extension have yet to be determined. The aim of this study was to evaluate both the natural course of disease and the risk factors influencing the proximal disease extension in ulcerative proctitis. <B><I>Material and methods.</I></B> We retrospectively analyzed 98 patients with ulcerative proctitis at the time of diagnosis who were regularly followed and underwent sigmoidoscopy or colonoscopy between January 2000 and December 2007. <B><I>Results.</I></B> The mean duration of follow-up was 109.2 ± 49.5 months. A total of 27 (27.6%) patients experienced proximal disease extension. Mayo scores were significantly higher in the extension group compared with patients whose ulcerative proctitis did not extend proximally (<I>p</I> < 0.001). Corticosteroid use at initial diagnosis was also more frequent in the extension group (<I>p</I> = 0.026). In addition, chronic, continuous disease activation within 6 months of the initial diagnosis was significantly higher in the extension group (<I>p</I> < 0.001), as was disease relapse and the number of hospitalizations over the entire follow-up period (<I>p</I> < 0.001 and <I>p</I> = 0.002). According to multivariate analysis, disease extension after the initial diagnosis was associated with chronic disease activation, disease relapse and hospitalization (<I>p</I> = 0.030, <I>p</I> = 0.042 and <I>p</I> = 0.044, respectively). <B><I>Conclusion.</I></B> Increased severity of disease upon diagnosis of ulcerative proctitis was associated with a higher probability of proximal disease extension during the follow-up period. Moreover, those with disease extension were more likely to experience relapse and to be hospitalized, indicating poor prognosis.</P>
