Backgrounds/Aims: Reported incidence of extrahepatic bile duct cancer is higher in Asians than in Western populations. Korea, in particular, is one of the countries with the highest incidence rates of extrahepatic bile duct cancer in the world. Although research and innovative therapeutic modalities for extrahepatic bile duct cancer are emerging, clinical guidelines are currently unavailable in Korea. The Korean Society of Hepato-Biliary-Pancreatic Surgery in collaboration with related societies (Korean Pancreatic and Biliary Surgery Society, Korean Society of Abdominal Radiology, Korean Society of Medical Oncology, Korean Society of Radiation Oncology, Korean Society of Pathologists, and Korean Society of Nuclear Medicine) decided to establish clinical guideline for extrahepatic bile duct cancer in June 2021. Methods: Contents of the guidelines were developed through subgroup meetings for each key question and a preliminary draft was finalized through a Clinical Guidelines Committee workshop. Results: In November 2021, the finalized draft was presented for public scrutiny during a formal hearing. Conclusions: The extrahepatic guideline committee believed that this guideline could be helpful in the treatment of patients.

Clinical Significance of CLDN18.2 Expression in Metastatic Diffuse-Type Gastric Cancer

Purpose: Isoform 2 of tight junction protein claudin-18 (CLDN18.2) is a potential target for gastric cancer treatment. A treatment targeting CLDN18.2 has shown promising results in gastric cancer. We investigated the clinical significance of CLDN18.2 and other cell-adherens junction molecules (Rho GTPase-activating protein [RhoGAP] and E-cadherin) in metastatic diffuse-type gastric cancer (mDGC). Materials and Methods: We evaluated CLDN18.2, RhoGAP, and E-cadherin expression using two-plex immunofluorescence and quantitative data analysis of H-scores of 77 consecutive mDGC patients who received first-line platinum-based chemotherapy between March 2015 and February 2017. Results: CLDN18.2 and E-cadherin expression was significantly lower in patients with peritoneal metastasis (PM) than those without PM at the time of diagnosis (P=0.010 and 0.013, respectively), whereas it was significantly higher in patients who never developed PM from diagnosis to death than in those who did (P=0.001 and 0.003, respectively). Meanwhile, CLDN18.2 and E-cadherin expression levels were significantly higher in patients with bone metastasis than in those without bone metastasis (P=0.010 and 0.001, respectively). Moreover, we identified a positive correlation between the expression of CLDN18.2 and E-cadherin (P<0.001), RhoGAP and CLDN18.2 (P=0.004), and RhoGAP and E-cadherin (P=0.001). Conversely, CLDN18.2, RhoGAP, and E-cadherin expression was not associated with chemotherapy response and survival. Conclusions: CLDN18.2 expression was reduced in patients with PM but significantly intact in those with bone metastasis. Furthermore, CLDN18.2 expression was positively correlated with other adherens junction molecules, which is clinically associated with mDGC and PM pathogenesis.

<P>Embolization is a nonsurgical, minimally invasive procedure that deliberately blocks a blood vessel. Although several embolic particles have been commercialized, their much wider applications have been hampered owing mainly to particle size variation and uncontrollable degradation kinetics. Herein we introduce a microfluidic approach to fabricate highly monodisperse gelatin microparticles (GMPs) with a microshell structure. For this purpose, we fabricate uniform gelatin emulsion precursors using a microfluidic technique and consecutively cross-link them by inbound diffusion of glutaraldehyde from the oil continuous phase to the suspending gelatin precursor droplets. A model micromechanic study, carried out in an artificial blood vessel, demonstrates that the extraordinary degradation kinetics of the GMPs, which stems from the microshell structure, enables controlled rupturing while exhibiting drug release under temporary chemoembolic conditions</P>

Aims: Liver stiffness (LS) is an emerging imaging-based prognostic biomarker for patients with chronic liver disease. We investigated whether LS quantified using magnetic resonance elastography (MRE) could predict the prognosis of advanced hepatocellular carcinoma (HCC) patients treated with sorafenib. Methods: We selected 50 sorafenib-treated advanced HCC patients who underwent MRE within 3 months before drug administration from a prospectively maintained cohort of chronic liver disease patients, according to the inclusion and exclusion criteria. Univariate and multivariate analyses were performed to evaluate the prognostic role of laboratory data, tumor characteristics, and MRE-assessed LS for overall survival (OS), progression-free survival (PFS), and significant liver injury (≥grade 3) after sorafenib administration. Results: High MRE-assessed LS was significantly associated with poor OS (kPa; hazard ratio [HR], 1.54; 95% confidence interval [CI], 1.23-1.92; P<0.001) as well as higher serum alpha-fetoprotein (AFP, ≥400 ng/mL) and advanced tumor stage (modified Union for International Cancer Control [mUICC] IVb). Higher MRE-assessed LS was also significantly associated with the development of significant liver injury after sorafenib administration (kPa; HR, 1.62; 95% CI, 1.21-2.17; P=0.001). PFS analysis identified higher serum AFP (≥400 ng/mL) and advanced tumor stage (modified UICC IVb) as significant risk factors for early disease progression, whereas LS was not associated with PFS. Conclusions: Higher MRE-assessed LS is a potential biomarker for predicting poor OS and significant liver injury in advanced HCC patients treated with sorafenib.

본 연구는 김소월과 서지마의 애정시가 한국과 중국의 근대 문학사에서 서구 문화와의 충돌과 융합을 통해 독특한 질서를 형성하며 중요한 문학 형식으로 자리 잡았음을 살펴보았다. 이 과정에서 사랑, 이별 등의 감정 표현이 시 창작의 핵심이 되었음을 고찰하였다. 김소월과 서지마의 시는 사랑과 이별의 감정을 담고 있으며, 후기 낭만주의의 서정적인 감정 표현을 보여준다. 그러한 양상을 보여주는 대표적 시집으로는 김소월의 『진달래꽃』과 서지마의 『지마의 시(志摩的詩)』가 있다. 두 시집에 담긴 작품들은 자연과 일상생활을 배경으로 진실하고 깊은 사랑을 표현한다. 비록 두 시인이 서로 다른 국가에서 활동했지만, 그들의 시에는 많은 유사점이 존재한다. 두 시인은 혼란스러운 시대 속에서 창작 활동을 하였으며, 사랑과 자유라는 주제에 천착했다. 또한, 개인적인 감정을 표현하는 동시에 사회와 문화적 배경을 반영하였다. 본 연구는 김소월과 서지마의 애정시를 비교 분석하여 두 시인의 사랑 표현 방식에서의 공통점과 차이점을 탐구하고, 그것은 그들의 작품과 그 배경에 있는 사회 문화적 요소들을 더욱 심층적으로 이해하는 데 필요한 통찰을 제공하기 위한 것이다. This study compares the love poems of Kim Sowol and Xu Zhimo, highlighting the crucial role of poetry and emotional expression in modern Korean and Chinese literary histories. Both poets, emblematic of their respective literatures, integrated Western influences to craft a unique poetic order. Emotional expression is central to their work, reflecting deep cultural and personal themes. Kim Sowol's “Azaleas” captures the emotions of love and separation with lyrical Romanticism, while Xu Zhimo's “Zhimo's Poems,” including “Love's Death,” conveys sincere love set against nature and daily life. Despite different national contexts, both poets shared a focus on love, freedom, and personal emotion amidst social upheaval. This study explores their commonalities and differences, emphasizing the unique significance of love poetry in their literary contributions.
Comparison of Factors affecting prediabetes by gender in 30-50yr Korean adult; the 2019 KNHANES

Aim(s): This study conducted to compare the factors affecting prediabetes by gender. We compared health behaviors, health related quality of life(HINT-8), and clinical characteristics in prediabetes men and women. Method(s): This study was an observational study with a cross-sectional design based on data from the 2019 Korea National Health and Nutrition Examination Survey (KNHANES) that used stratified, multi-stage-cluster-sampling design to obtain a nationally representative sample. Multiple logistic regression analysis was employed to compute the odds ratios of health behaviors, health related quality of life (HINT-8), and clinical characteristics to identify the risk factors for prediabetes. Result(s): In the prediabetes, 65.3% were men and 34.7% were women (X2=80.64, p<.001). In multiple logistic regression by gender, the factors associated with IFG in men were as follows: perceived health status (OR 1.48, 95% CI 1.11–1.98), climbing stairs (HINT-8) (OR=1.48, 95% CI 0.01–0.92), high BMI (OR=1.93, 95% CI 0.38–0.70), and triglycerides (OR=1.86, 95% CI 0.38-0.76) were influencing factors. On the other hand, in women were as flows: education level (OR=1.7, 95% CI 1.15-2.51), lifetime smoking (OR=1.15, 95% CI 0.06-0.78), energy (HINT-8) (OR=1.03, 95% CI 0.21-0.91), high BMI (OR=1.34, 95% CI 0.22-0.44). Conclusions: Strategies should be targeted to improve health behaviors and health related quality of life, and clinical characteristics for those in their 30s and 40s, men in bad perceived health status, those who overweight men, women with low education level, and less physical activities. Moreover, healthcare providers should understand the barriers to health behaviors and health related quality of life to effectively deliver healthcare service.

This study was conducted to develop a screening method using Colilert-18 and a device for the detection of E. coli from agri-food production environments and fresh vegetables. The specificity and sensitivity of Colilert-18 by temperature (37°C and 44°C) were evaluated with 38 E. coli and 78 non-E. coli strains. The false-positive rate was 3.8% (3/78) and 0% (0/78) at 37°C and 44°C, respectively. The detection limit of E. coli at 37°C at <1.0 log CFU/250 ml was lower than that at 44°C. The efficiency of the developed device, which comprised an incubator equipped with a UV lamp to detect E. coli in the field, was evaluated by measuring the temperature and UV lamp brightness. The difference between the set temperature and actual temperature of the developed device was about 1.0°C. When applying the developed method and device to various samples, including utensils, gloves, irrigation water, seeds, and vegetables, there were no differences in detection rates of E. coli compared with the Korean Food Code method. For sanitary disposal of culture samples after experiments, the sterilization effect of sodium dichloroisocyanurate (NaDCC) tablets was assessed for use as a substitute for an autoclave. The addition of one tablet of NaDCC per 50 ml was sufficient to kill E. coli cultured in Colilert-18. These results show that the developed protocol and device can efficiently detect E. coli from agri-food production environments and vegetables.
