        긴장증의 임상적 특성에 대한 고찰

        26 cases(9 males and 17 females) with catatonia features on admission were analyzed in terms of its clinical characteristics. The results obtained were as follows : 1) In the diagnostic distribution, more than a half of the cases were schizophrenic disorders(57.5%) and the others were major depressions(26.9%) and schizophreniform disorders(15.4%). 2) The catatonia signs were nonspecific and evenly distributed among a variety of clinical diagnostic entities. But the signs of negativism, mutism, and posturing appeared frequently in all cases. 3) Acute onset was prevalent(65.4%), ECT was more frequently applied(61.5%) and treatment response was better(70.9%) in the catatonia group than non-catatonia. Although the definition of catatonia is not clear yet, above results support the opinions of previous studies that catatonia should be understood as a heterogenous symptom complex. And not only schizophrenia but also affective disorder are associated with catatonia features. This suggests that we have to re-evaluate the concept of Kahlbaum's old description.

        Influence of CYP2D6 genetic polymorphism on pharmacokinetics of active moiety of tolterodine

        Tolterodine is metabolized to an active 5-hydroxymethyl tolterodine (5-HMT) by CYP2D6. This study investigated the relationship between CYP2D6 genotypes and pharmacokinetics of tolterodine and its active metabolite in healthy Korean subjects. All volunteers were genotyped for CYP2D6 and divided into four different genotype groups (CYP2D6*wt/*wt [*wt = *1 or *2], CYP2D6*wt/*10, CYP2D6*10/*10, and CYP2D6*5/*10). Each subject received a single oral dose of tolterodine tartrate (2 mg) in single-dose phase of the study. After the single-dose phase of the study, the same subjects received a single oral dose of tolterodine tartrate (2 mg) once daily for 1 week during multiple-dose tolterodine administration phase. Plasma concentrations of tolterodine and 5-HMT were measured by using liquid chromatography-tandem mass spectrometry method. Our study demonstrated that plasma exposure of tolterodine in CYP2D6*10/*10 and CYP2D6*5/*10 group significantly increased, compared with CYP2D6*wt/*wt group (P < 0.001). The pharmacokinetic parameters of 5-HMT were not significantly different in relation to CYP2D6 genotype, as 5-HMT itself is also metabolized by CYP2D6. With regard to active moiety (tolterodine + 5-HMT), Cmax and AUC0–24 was significantly increased in CYP2D6*10/*10 group, compared with CYP2D6*wt/*wt group (P < 0.001). Thus, our study showed the pharmacokinetics of tolterodine and its active moiety was significantly different in relation to CYP2D6 genotype.

        In Situ Coverslip 배양을 이용한 임신중기 양수 천자 785 예의 세포유전학적 고찰

        목적 : 산전 세포유전학 검사를 위해 임신중기에 시행하는 양수 천자의 적응증과 염색체 핵형 결과를 평가하고, 적응증에 따르는 염색체 이상의 발생빈도를 분석하여 양수 천자의 효용성을 알아보고자하였다. 연구 방법 : 1995년 1월부터 2003년 3월까지 동아의료원 산부인과에서 산전 유전질환 진단을 위하 세포유전학 검사의 적응증에 해당되어 in situ coverslip 배양을 시행한 양수천자 785예를 대상으로 염색체 이상의 빈도와 종류를 분석하고, 적응증에 따른 염색체 이상의 결과에 대해 살펴보았다. 결과 : 본원에서 양수 천자를 시행한 785예에서, 양수 천자의 적응증으로는 비정상 모체혈청 삼중표지자 선별검사가 421예 (53.7%)로 가장 많았으며, 다음으로는 고령임신이 233예(29.7%), 비정상 초음파 소견이 61예 고령산모는 261명 (33.2%)를 차지하였다. 염색체 핵형 검사를 시행한 785예 중에 정상 변이형이 28예, 이상 핵형이 40예, 염색체 이상에서는 수적 이상 27예, 구조적 이상 13예 상염색체 이상 중에는 다운 증후군이 13예로 가장 많았으며, 다음으로는 에드워드 증후군 2예였다. 성염색체 이상 중에는 터너 증후군이 3예, 크라인펠터 증후군이 3예 등이었다. 연령별로는 30-34세에서 14예 (35.0%)로 염색체 이상이 가장 많이 발견되었으며, 25-29세와 35-39세가 각각 12예 (30.3%), 7예 (17.5%)였다. 적응증에 따라 분류해 보면 비정상 모체혈청 삼중표지자 선별검사 15예 (37.5%) 및 고령 임신에서 12예 (30.0%), 비정상적인 초음파 소견에서 8예 (20.2%) 등의 순이었다. pseudomosaicism도 5예 (0.7%)에서 관찰되었다. 결론 : 고령산모나 모체혈청 삼중표지자 선별검사 그리고 초음파 검사 등을 통하여 유전 질환의 위험성이 높은 임부를 선별하고, 이들 임부를 대상으로 임신 중기 양수천자를 통한 태아의 세포유전학적 검사는 산전 유전 질환을 확진하는 진단법으로 매우 유용한 검사이다. 또 이러한 세포유전학적 연구 결과는 산전 진찰 및 유전학적 상담의 중요한 자료로 활용될 수 있을 것이며, 이에 대한 지속적인 연구와 보고가 필요할 것이다. Objective : Amniocentesis is the most commonly used invasive method for prenatal diagnosis of genetic disorders. We performed this study to analyze the indications, distributions of maternal age and cytogenetic results of midtrimester amniocentesis. Methods : We retrospectively analyzed 785 cases of midtrimester prenatal genetic amniocentesis which were performed in the cytogenetics laboratory using in situ coverslip culture at Dong-A University Hospital from January 1995 to March 2003. Results : Amniocentesis was practiced mostly from 15 weeks to 20 of gestational ages. Requested indications of amniocentesis were abnormal maternal serum screening (421, 53.7%), advanced maternal age (233, 29.7%) and abnormal ultrasonographic finding (61, 7.8%) in the order of decrease. The overall incidence of chromosome abnormalities was 5.1% (40 cases), and it contains 27 cases (3.4%) of numerical abnormalities and 13 cases (1.7%) of structural abnormalities. Among autosomal abnormalities Down syndrome was most common (13 cases) and followed by Edward syndrome (2 cases). Of the sex chromosomal abnormalities, three cases of Turner syndrome and three cases of Kleinefelter syndrome were found. Chromosomal abnormalities were most frequently noted in the maternal age of 30 to 34 years old (14 cases, 35.0%), 25 to 29 years old (12 cases, 30.0%), followed by 35 to 39 years old (7 cases, 17.5%). The frequency of pseudomosaicism were 5 cases (0.6%). Conclusion : Maternal serum screening advanced maternal age and antenatal ultrasonographic finding must be important screening methods for amniocentesis which is considered to the most effective diagnostic procecdure for prenatal cytogenetic studies. I conclude that th karyotyping analysis of midtrmester amniocentesis efficacious method for detection of chromosomal aberration and genetic counselling for parents.

        Effects of steady-state clarithromycin on the pharmacokinetics of zolpidem in healthy subjects

        Zolpidem is extensively metabolized byCYP3A4, CYP2C9 and CYP1A2. Previous studies demonstratedthat pharmacokinetics of zolpidem was affected byCYP inhibitors, but not by short-term treatment of clarithromycin. The objective of this study was to investigate theeffects of steady-state clarithromycin on the pharmacokineticsof zolpidem in healthy subjects. In the control phase,33 subjects received a single dose of zolpidem (5 mg). One week later, in the clarithromycin phase, the subjectsreceived clarithromycin (500 mg) twice daily for 5 daysto reach steady state concentrations, followed by zolpidem(5 mg) and clarithromycin (500 mg). In each phase,plasma concentrations of zolpidem were evaluated up to 12 hafter drug administration by using liquid chromatographytandemmass spectrometry method. In the clarithromycinphase, mean total area under the curve of zolpidem (AUCinf) was 1.62-fold higher and the time to reach peak plasmaconcentration of zolpidem (tmax) was prolonged by 1.95-fold compared to the control phase. In addition, eliminationhalf-life (t1/2) of zolpidem was 1.40-fold longer duringco-administration with clarithromycin and its apparentoral clearance (CL/F) was 36.2% lower with clarithromycinadministration. The experimental data demonstrate the significant pharmacokinetic interaction between zolpidemand clarithromycin at steady-state.

        N/A Objectives : The present study has been undertaken to investigate the effects of and Ginseng Radix Palva on osteoporosis in ovariectomized rats. Methods : In this expaiment, the rats were ovariectomihed. Rats were administered by 2 kinds of medicine, Estradiol and Ginseng Radix Palva. The levels of bone mineral density, osteocalcin, ALP, phosphorus, calcium in serum, deoxypyridinoline in urine, calcium, phosphorus, ash weight of bone, body weight and uterus weight were measured. results : The levels of bone mineral density were not significantly increased in comparison with OVX group in Ginseng Radix Palva group. The levels of serum didn`t show significant change in comparison with OVX group in Ginseng Radix Palva group. The levels of urine deoxypyridinoline didn`t show significant decrease in comparison with OVX group in Ginseng Radix group. The levels of femoral and fibula-tibial ash weight didn`t show significant increase in comparison with OVX group in Ginseng Radix Palva group. The levels of femoral and fibula-tibial calcium showed significant increase in comparison with OVX group in Ginseng Radix Palva group. The levels of femoral and fibula-tibial phosphorus showed significant increase in comparison with OVX group in Ginseng Radix Palva group. The levels of body weight didn`t show significant changes in comparison with OVX group in Ginseng Radix Palva group. The levels of uterus weight didn`t show significant changes in comparison with OVX group in Ginseng Radix Palva group. Conclusions : Reviewing these experimental results, it appears that Ginseng Radix Palva have efficacy on osteoporosis. And further study should be conducted to illustrate in depth the curing osteoporosis.

        Relationship between plasma exposure of zolpidem and CYP2D6 genotype in healthy Korean subjects

        Zolpidem, a widely prescribed hypnotic agent,is extensively metabolized by cytochrome P450 (CYP) 3A4,and CYP2C9, CYP1A2 and CYP2D6 are also involved inthe metabolism of zolpidem. The aim of the study was toinvestigate the effects of CYP2D6 genotypes on the exposureof zolpidem. The healthy male volunteers were divided intothree different genotype groups (CYP2D6*wt/*wt [*wt = *1or *2], CYP2D6*wt/*10, and CYP2D6*10/*10). Each subjectreceived a single oral dose of zolpidem 5 mg with orwithout a steady-state concentration of clarithromycin (apotent inhibitor of CYP3A4), and plasma concentrations ofzolpidem were measured up to 12 h after zolpidem dosingby using liquid chromatography-tandem mass spectrometrymethod. When zolpidem was administered alone, theexposure of zolpidem (the total areas under the curve andthe mean peak plasma concentrations) was not significantlydifferent among three different genotype groups. Even withthe steady-state concentration of clarithromycin, a potentCYP3A4 inhibitor, there were no significant differences inthe exposure of zolpidem in relation to CYP2D6 genotypes.
