        골성 사비 환자의 코성형술에 있어 비골 3차원 전산화단층촬영을 이용한 절골술

        Background and Objectives Correction of deviated nose is considered one of the most difficult and serious surgical operations in rhinoplasty. The correction of bony deviation, in particular, requires osteotomy. Although there are individual differences in the shapes or features of bony deviations of each patient, the techniques of osteotomy are limited. Therefore, for proper correction of bony deviation, it is necessary to set up a precise pre-surgical plan. Subjects and Method We categorized 187 patients with bony deviated nose into 5 types based on the analysis of preoperative nasal bone 3D computer tomography, and studied surgical method for each type. Results Of 187 patients, 92 (49%) patients are categorized into type I, 62 (36%) patients into type II, 12 (6%) patients into type III, 2 (1%) patients into type IVa, 7 (4%) patients into type IVb and 7 (4%) patients into type V. Only did eight patients (5%) undergo revision surgery due to unsuccessful outcomes. Major complications like infection, abscess, arteriovenous fistula, epiphora, and saddle deformation did not occur at all. Conclusion Preoperative planning under the analysis and categorization using 3D CT is considered helpful for the precise and individualized surgery in corrective rhinoplasty.         만성 전두동 전벽 골수염 환자의 수술 후 발생한 전두동 전벽 결손을 TutoplastⓇ(Allograft Cancellous Bone Chip)를 이용하여 치험한 1예

        Osteomyelitis is a severe infection of bone that can arise from a variety of mechanisms. Osteomyelitis is an acute, subacute, or chronic inflammatory process of the bone, secondary to pyogenic bacterial infection, which differs according to patient’s age and infection mechanism. Treatment requires isolation of the pathogens, significant debridement for removal of all infective and necrotic material, and then bony and soft tissue reconstruction. We recently experienced a case of frontal sinus anterior wall defect caused by chronic frontal osteomyelitis in a 62-year-old male patient. The frontal sinus anterior wall defect was repaired using TutoplastⓇ (allograft cancellous bone chip).

        Nasal septal perforations are relatively common, affecting up to 0.9% of general population. From early reports, the main cause (66%) of septal perforation was iatrogenic in connection to septal surgery. When septal perforation occurs, it is wise to repair perforation immediately and to insert a connective tissue or acellular dermal graft. Alloderm, in particular, has gained popularity as an interpositional graft in septal perforation repair due to its availability and ease of use. we recently experienced a case of septal perforation caused by previous septoplasty in a 21-year-old-male patient.         비중격 교정술 후 안장코 변형이 오는 위험 인자에 관한 분석

        Background and Objectives Surgeons operate carefully to prevent nasal deformity during septoplasty, however, rarely unfavorable esthetic outcomes such as saddle nose deformity may occur. This study was designed to investigate patient factors associated with post-septoplasty saddle deformity. Subjects and Method Of 588 patients who underwent endonasal conservative septoplasty from Jan. 2011 to Dec. 2014, a total of 183 patients were enrolled in this study group after exclusion. Patients were divided into two groups: 12 patients who developed saddle deformity of nasal dorsum after septoplasty belonged to the ‘deformity group’, and the remaining 171 patients were enrolled in the ‘non-deformity group’. We investigated preoperative external nose status, patterns of septal deformity, dorsal septal thickness, the angle of axial and coronal deviation of caudal sepum on CT scan. Results The postoperative saddle deformity was not found to correlate with the location of the most deviated septum and the thickness of dorsal septum (p>0.05). 50% of patients in the deformity group had saddle nose preoperatively, showing statistical differences between the two groups (p=0.000). 75% of patients in the deformity group had severe deviation at the most deviated site, also showing a significant difference compared with the non-deformity group (p=0.011). The axial and coronal deviation angle of caudal septum in the deformity group were significantly increased compared with the non-deformity group (p<0.01). Conclusion The preoperative saddle nose, severity of deviation, and angle of axial and coronal deviation of caudal septum are all possible risk factors of postoperative saddle deformity. We recommend that the surgeon should check these factors before septoplasty to prevent postoperative saddle nose deformity.         연구보문 : 지역 한우 브랜드의 개체관리 현황과 발전방향

        1. "녹색한우"사업단은 광역 한우 브랜드로서 규모가 크고 품질 고급화를 위하여 사양관리를 통일하고 품질의 균일성 확보를 위하여 중장기 육종전략을 수립하여 한우 브랜드를 육성시키고 있다. 2. 한우 개량을 위한 기본적인 개체정보 수집을 위하여 개체관리 자동화시스템을 구축하여 참여 대상축에 대한 DB를 구축함과 동시에 개체별, 출생정보, 혈통정보, 번식정보, 발육능력정보 등 다양한 정보를 정확하게 수립·관리할 수 있는 기반을 구축하였으며, 이를 근거로 한우 축군의 유전능력평가를 실시하여 우량 암소축군을 구축하고 관리할 수 있는 기반을 마련하였다. 그러나 상당수의 한우 브랜드는 이러한 체계적 관리시스템을 구축하지 못한 실정이며 특정 보증씨수소에 의한 개량의존도가 심화되고 있어 빠른 문제해결을 위한 방안을 마련해야 할 것이다. 3. 유전적으로 능력이 우수한 암소의 경우도 한우산업의 특성상 2산 이상의 경우 경제적 가치의 저하로 인하여 조기에 도축하는 경우가 빈번하게 일어나고 있으며, 이는 정부차원에서 우량 암소의 다산장려 정책을 추진하고 있음에도 농가의 현장적용의 효과가 부족한 것은 유전능력 평가에 의한 검증된 우량 암소 축군의 구축보다는 표현형적 자료에 근거한 표현형 선발이 진행되어 다음 세대의 송아지 능력 예측이 어려운 점 등 육종전략과 계획교배 부재 등으로 인하여 한우 브랜드의 축군 관리지침이 없다는 현실을 반영한 것이라 할 수 있다. 4. 따라서 한우 브랜드의 성공적 육성을 위하여 한우 브랜드에 참여하는 암소 축군에 대한 유전적 평가를 실시하여 우량 암소 축군을 조성한과 동시에 한우 브랜드의 차별화를 위한 자구적 육종전략 수립이 필요하다. The objectives of this study were to establish an automatic individual management system using electronic ear tag with RFID chip in Hanwoo and to evaluate an inbreeding coefficients of herd to counsel the breeding scheme and its development strategies for "Green Hanwoo" brand. The individual management system for the "Green Hanwoo" brand consisted of the 3 factors such as an ear tag with RFID chip, a mobile device for recognizing an ear tag, and a body weight measurement scale with indicator including wireless functions to send the individual information to the mobile device. This system was especially applied to 16 breeding farms only and its herds in "Green Hanwoo" brand and collected automatically the informative individual information such as birth date, sex, pedigree, and growth performance and reproductive records in "Green Hanwoo" herds. All data collected from individuals of Hanwoo herds were analyzed to calculate an inbreeding coefficients based on a pedigree information. Some of individuals in each farm had somewhat higher inbreeding coefficients due to an inappropriate mating scheme which was resulted in a decrement of genetic gain or improvement in entire herds of "Green Hanwoo" brand. Therefore, we should consider the genetic diversity of Korean proven sire and the breeding scheme with the long term development strategies for a distinguished local Hanwoo Brand, "Green Hanwoo".
