        귀밑샘에 생긴 다형선종의 수술적 치료로써의 제한적 국소절제술

        Purpose: Pleomorphic adenoma is the most common benign neoplasm in parotid gland. Superficial parotidectomy was usually used to remove the pleomorphic adenoma. But, this method has to remove tumor with normal parotid tissue. Authors did lumpectomy to remove pleomorphic adenoma in parotid gland, because pleomorphic adenoma is wrapped in a capsule as it grows. The purpose of this study is to evaluate the efficacy of lumpectomy as a treatment of pleomorphic adenoma in parotid gland. Method: From 2002 to 2008, 8 patients underwent the lumpectomy of the pleomorphic adenoma in parotid gland. Occurrence of the complications and recurrance were evaluated. Result: Patients were followed-up for a mean 45 months. There were no recurrance or no complication after lumpectomy. Conclusion: Authors suggest that the lumpectomy lead to decrese complications, recurrane and can be used as a procedure for the resection of pleomorphic adenoma in parotid gland.

        Tail model의 기계적 이질통에 대한 전침 자극의 효과 및 교감신경계의 관여기전

        Objective : This study was intended to investigate the analgesic effects of electroacupuncture(EA) on mechanical allodynia according to the frequency and intensity of EA. Also to know if mechanical allodynia and the analgesic effects of EA is related to the sympathetci nervous system and/or the purinergic system. Methods : mechanical allodynia-induced rats were produced by resecting S1-S2 nerve. The zusanli(ST36) was used for acupoint and the rats were divided into 4 groups. Each group was given different stimuli[low frequency low intensity-EA(LFLI-EA), low frequency high intensity-EA(LFHI-EA), high frequency low intensity-EA(LFHI-EA), high frequency high intensity-EA(HFHI-EA)]. Futhermore, to make sympathectomy6-OHDA and phentolamine were administered intraperitonially and the concentration of norepinephrine(NE) were measured. As a ATP blocker, suramin was applied for this study. Results : Comparing to control group, each of the 4 groups(LFLI-EA, LFHI-EA, HFLI-EA, HFHI-EA) showed a significant reduction of response frequency of mechanical allodynia. LFHI-EA was more effective than that of LFLI-EA. The LFHI-EA group also had longer lasting effects from the stimulation than the other groups. Sympathectomy didn't show any reduction of response frequency of mechanical allodynia.(Each n=6, n=4). Nor did both sympathectomy and ATP block. The response frequency wasn't reduced by sympathectomy or by sympathectomy and ATP block, but was significantly reduced with LFHI-EA Conclusions : These results suggest that EA has a significant analgesic effect on mechanical allodynia which has no connection with NE and/or ATP.

        일측성 광대뼈골절 환자에서 수평계와 자를 이용한 변위 교정의 간단한 파악법

        Purpose: The zygoma is a key element which composes the facial contour. Zygomatic fracture induces facial asymmetry. We use radiologic evaluation or inspections mainly for identification of symmetry after reduction depressed zygomatic fracture. But the disadvantages of such methods are time-consuming and complicated process. So we tried to develop a new testing method with a ruler and a level. Methods: In unilateral depressed zygomatic fracture patient, parallel to the patient's head to make sure lay horizontaly. Put the leg of a ruler on the malar eminence so that it is at the same distance from the facial midline. Then take the level of malar eminence as put the level above the ruler. This process was performed before and after the reduction. Results: We were able to fix with plate and screw after checking the results of reduction fast and easily. Good results were obtained at post-operative radiologic evaluation. Conclusion: We can easily get the ruler and level around life. This method is not only simple but also shorttime process compared with other method-radiologic evaluation or inspection. And the operator can explain the results to the patients easily and objectively. Authors obtained the good results with this new method, and would introduce it for another method of identifying the result of reduction in depressed zygomatic fractures.

        DEA/Window 모형을 이용한 국내 생명보험산업의 상대적 효율성 분석

        우리나라의 생명보험산업은 IMF 사태와 방카슈랑스의 도입 등 많은 변화를 겪어왔으며 앞으로 시행될 자본통합법의 시행도 추후에는 생명보험산업에 많은 영향을 미칠 것으로 보이며 생명보험회사의 경쟁력 강화를 위해서는 효율성의 분석이 필수적이다. 본 논문은 DEA 모형을 이용하여 우리나라에서 영업 중인 생명보험회사들의 효율성을 분석했다. 이를 위해 투입변수는 두 가지 범주로 나누어서 분석을 시도하였고 동태적 효율성을 분석하기 위해서 DEA/WINDOW 모형을 도입하였다. 이를 통하여 생명보험회사들의 CCR, BCC, 규모효율성을 알 수 있었고 7년간(1998년~2004년)의 효율성의 추세와 안정성을 파악할 수 있었다. With many changes such as the increase in telemarketing, internet marketing and enforcement of bancassurance, the Korean life insurance companies have undergone a startling transformation. The purpose of this paper is to measure and analyse the static/dynamic efficiency of Korean life insurance companies employing Data Envelopment Analysis(DEA). As the result of the static efficiency analysis, we provide CCR, BCC and scale efficiency, return to scale, and reference set of Korean life insurance companies in 2004. And we also describe about the trend and stability of their efficiency for 7 years(1998-2004) in the dynamic efficiency analysis.

        뇌졸중 환자에서 배우자 사이에 뇌혈관 질환의 위험요인 일치성이 있는가?

        혈관 질환 위험인자의 가족력은 이미 잘 알려져 있으며 최근 심혈관 질환의 위험인자를 대상으로 한 연구에서는 가족력 뿐 만 아니라 배우자간의 위험인자 일치성이 있다고 밝혔다. 이는 혈관 질환 위험인자에서의 가족력은 유전적인 이유뿐 만이 아니라 환경적 요인으로 인해서도 발생한 다는 것을 암시한다. 그러나 뇌졸중이 발생한 환자에서 그 배우자간 위험인자의 일치성을 알아본 연구는 없었다. 이 연구는 본 연구를 진행하기 앞서 시행한 연구로서 뇌졸중으로 입원했던 환자를 대상으로 위험인자간의 관련성을 조사해서 그 경향성을 파악하고자 하였는데 고혈압의 유무(p=0.025)와 경동맥 내막-중막 두께(r=0.479, p=0.001)에서 의미있는 연관성을 보였으나 나이를 보정하는 경우 의미가 없는 것으로 드러났다. 일반 인구 대상이거나 심혈관질환자가 대상이었던 종전의 연구 결과와는 다른 결과였다. 의미 있는 경향성을 보인 위험인자에 대해서 추후 보다 큰 규모의 연구가 필요할 것이다. Familial concordance of vascular risk factors is well understood and there are many studies about inter-spouse concordance of the risk factors for coronary heart disease. However, there are no studies for inter-spousal relation of the risk factors in cerebrovascular disease patients. We analysed inter-spouse correlation of cerebrovascular risk factors in patients admitted our hospital due to stroke and their spouses. There was statistically significant inter-spouse concordance in the presence of hypertension(p=0.025) and carotid IMT(r=0.479, p=0.001). However, the further age-adjusted analysis revealed no significant result. Differently from the results of previous studie,, the present study shows no significant spousal concordance. Althought there are tendencies of spousal concordance in some risk factors, the age is major determinant.

        덕트-음향 시스템에서 소거용스피커 방향에 따른 소음감소효과

        In this paper, we studied on an attenuation effect of automobile exhaust noise according to the direction of canceling speaker in duct-acoustic ANC system. Automobile exhaust noise was recorded at 800rpm, 3500rpm and 5000rpm of a diesel engine. Directions of canceling speaker can be set to 30°, 90° and 150° against the primary noise flow by acrylic ducts to be made for the experimentation. DSP board used to control the ANC system. The algorithm of this ANC system applied the Filtered-x-LMS algorithm that is modified to compensate for a property of DSP input signal and the secondary-path effect. As an experiment result, the direction of canceling speaker was proved to influence the reduction effect of noise. The 150° duct in the attenuation effect of noise showed a better result than the 90° or 30° duct.

        지역별 특성을 반영한 빈집의 활용방향: 전국 252개 기초 지방자치단체를 중심으로

        최근 들어 저출산, 고령화, 구도심의 쇠퇴 등 다양한 문제로 인하여 빈집이 발생하고 있다. 이러한 빈집은 범죄발생으로 인한 지역 주민들의 정주여건 악화, 지역슬럼화, 도시미관 저해 등 다양한 사회ㆍ경제적인 문제를 야기하는 시발점이 되고 있다. 하지만 이러한 빈집은 최근 들어 정부에서 추진하는 도시재생사업 및 각종 정비사업에서 활용이 가능한 현실적인 대응방안으로 주목받고 있다. 이에 본 연구는 전국 252개 기초 지방자치단체별 특성에 따른 군집분석을 통하여, 지역별 특성과 특성에 따른 유형을 살펴보고자 하였다. 분석을 위해 2018년을 기준으로 한 인구ㆍ가구, 물리, 사회ㆍ경제 부분을 나타내는 지표 17개를 선정하였으며, 이 지표들을 표준점수하여 군집분석을 실시하였다. 기초 지방자치단체별 252개 기초지방자치단체를 5개의 유형으로 군집화하여 군집별 특성을 분석하였다. 이러한 특성을 바탕으로 다양한 정비사업에서 빈집의 활용 시 지역별 특성을 고려한 빈집의 활용방향을 제안함으로써, 도시재생사업 및 각종 정비사업에서 효율적인 빈집의 활용방향을 제시하고자 한다. Various societal issues such as low birth rates, aging population, and the deterioration of old downtowns have resulted in increasing property vacancies. These vacant properties have contributed to a variety of social, economic issues, including but not limited to local residents experiencing an increase in crime, sluggish economic activity, and the disfigurement and neglect of the urban landscape. However, in order to turn around these declining living standards, these vacant properties have received recent attention as a possible foreground for the implementation of the government’s Urban Renewal Project and Redevelopment Projects. Thus, the purpose of this study was to analyze the clustering of these vacant properties by regional characteristics and types, depending on the various characteristics of 252 basic local governments across the country. 17 indices which stood as variables for population measure, household characteristics, physical environment differences, and socio-economic measures during the year of 2018 were transformed into standard scores, and then used to map the clustering of different housing vacancies. 252 basic local governments were classified into five types, and were then analyzed by each set of clustering characteristics. This study aimed to cluster vacant properties while taking regional characteristics into consideration, and suggests the appropriate and efficient application scheme for allocating Urban Renewal Project and Redevelopment Projects within different regional contexts.
