        혈소판유래성장인자를 함유한 Chitosan/Calcium Metaphosphate의 골조직재생효과에 관한 연구

        Chitosan is biodegradable natural polymer that has been demonstrated its ability to improve wound healing, and calcium metaphosphate(CMP) is a unique class of phosphate minerals having a polymeric structure. In this study, chitosan/CMP and platelet derived growth factor(PDGF-BB) loaded chitosan/CMP sponges were developed, and the effect of the sponges on bone regeneration and their possibility as scaffolds for bone formation by three-dimensional osteoblast culture were examined. PDGF-BB loaded chitosan/CMP sponges were prepared by freeze-drying of a mixture of chitosan solution and CMP powder, and soaking in a PDGF-BB solution. Fabricated sponge retained its 3-dimensional porous structure with $100-200\;{\mu}m$ pores. The release kinetics of PDGF-BB loaded onto the sponge were measured in vitro with $^{125}I-labeled$ PDGF-BB. In order to examine their possibility as scaffolds for bone formation, fetal rat calvarial osteoblastic cells were isolated, cultured, and seeded into the sponges. The cell-sponge constructs were cultured for 28 days. Cell proliferation, alkaline phosphatase activity were measured at 1, 7, 14 and 28 days, and histologic examination was performed. In order to examine the effect on the healing of bone defect, the sponges were implanted into rat calvarial defects. Rats were sacrificed 2 and 4 weeks after implantation and histologic and histomorphometrical examination were performed. An effective therapeutic concentration of PDGF-BB following a high initial burst release was maintained throughout the examination period. PDGF-BB loaded chitosan/CMP sponges supported the proliferation of seeded osteoblastic cells as well as their differentiation as indicated by high alkaline phosphatase activities. Histologic findings indicated that seeded osteoblastic cells well attached to sponge matrices and proliferated in a multi-layer fashion. In the experiments of implantation in rat calvarial defects, histologic and histomorphometric examination revealed that chitosan/CMP sponge promoted osseous healing as compared to controls. PDGF-BB loaded chitosan/CMP sponge further echanced bone regeneration. These results suggested that PDGF-BB loaded chitosan/CMP sponge was a feasable scaffolding material to grow osteoblast in a three-dimentional structure for transplantation into a site for bone regeneration.

        커튼월 설치 로봇 컨트롤러의 설계 및 사용성 평가에 관한 연구

        A construction robot has been developed for higher productivity and better safety in various construction fields. Especially, curtain wall is suitable for outer wall material of tall commercial building and apartment complexes. This heavy material is, however, hard to install with a manpower and outdated equipment. For this reason, the prototype of ASCI (Automation System for Curtain wall Installation) was developed. This system has a robot controller(i.e. hand-held remote control unit) for the transfer information signal between human operator and robot system. Although study has been conducted on manual controller of ASCI, hardly any information is known about the operator's opinion. In this study, a questionnaire was completed by operator to get their opinion about aspects which need to design a more comfortable and productive manual controller of construction machinery, robot included. Through the result of study, it is expected that this technical data is contributed to the robot controller design for comfort and productivity of various industrial machinery.

        중족골통에서의 소족지 중족 절골술

        Metatarsalgia means the pain under the lesser metatarsal heads. The many causes of metatarsalgia can be categorized into three groups: local disease in the region, altered forefoot biomechanics, and systemic disease affecting the region. Surgical options need to be considered if nonsurgical treatment fails. The metatarsal osteotomies are designed primarily to reduce the weightbearing forces on the metatarsal head by elevating or shortening the metatarsal. Many lesser metatarsal osteotomies have been described, and their success depends on many factors. Regardless of the method employed, it is important to maintain or restore the metatarsal cascade to maintain an even pressure under the lesser metatarsal heads and prevent transfer lesions. The surgeon must understand the effects of the metatarsal osteotomy on the forefoot patho-biomechanics and decide, using a combination of clinical examinations and imaging, whether the desired effect of the osteotomy is to shorten or elevate the metatarsal head or both.

        Marfan증후군의 수술 교정 1례

        Pectus excavatum occasionally occurs in patients who have underlying cardiac disease, especially Marfan syndrome. This report describes a patient with pectus excavatum who had ascending aortic aneurysm with aortic regurgitation and anterior leaflet prolapse of mitral valve. This patient underwent replacement of aortic valve and ascending aorta with 25 mm SJM valved conduit graft[Bentall operation with Cabrol shunt , and mitral valve replacement with SJM 31 mm, the pectus excavatum was corrected at the time of completion of the intracardiac operation with the modified sternal turnover. This procedure offered excellent operative exposure for the inracardiac operation with prevention of low cardiac output after operation due to depressed sternum and maintained chest wall stability resulting good cosmetic chest wall appearance. This patient recovered and discharged in good postoperative result with minimal temporary peroneal nerve palsy in his left leg.

        천수근 약침을 이용한 퇴행성 슬관절질환 치험 1례

        Objectives : The purpose of this clinical case is to evaluate the efficacy of Harpagophytum radix. Pharmacopuncture on Degenerative diseases of knee joint. Methods : We used Korean Medicine treatment and Harpagophytum radix. pharmacopuncture, for this patients. This case is measured and assessed by Neumerical Rating Scale(NRS), Korea Western Ontario MacMaster Universities Osteoarthritis Index(KWOMAC) and Physical examination of knee joint. Results : The NRS decreased from 8 to 5(Left knee joint) & 5 to 4(Right knee joint) and KWOMAC total points decreased from 77 to 41. Also Physical examination of knee joint improved. Conclusions : Harpagophytum radix. pharmacopuncture with Korean Medicine treatment is proved to be helpful to the pain relief and function recovery of knee joint.

        북한 후계체제 구축의 "세 가지 결정요인"에 대한 분석 -경로의존성을 중심으로-

        포스트 김정일, 북한의 3대 세습이 어떻게 결정되는가는 앞으로 북한 문제를 예측하는데 매우 중요한 것이다. 이 논문은 김정일 후계체제의 세 가지 결정요인이 제도의 경로의존성에 의해 북한의 3대세습 과정에서도 가장 중요한 결정 요인이 될 것이라는 것을 강조하고 있다. 경로의존성이란 시대환경의 흐름 속에서 제도가 일단 형성되어 경로의존적인 패턴을 형성하면, 그 기저의 환경조건이 비록 달라진다고 해도 그 제도는 잘 변화하지 않고 그대로 지속되는 경향이 있다는 것이다. 북한의 후계자론 또한 이미 김정일의 후계구축과정에서 제도화되었다. 후계자의 정통성 확립과 후계자의 영도체계 확립, 그리고 후계자에 대한 인민대중의 지지 확립은 북한 후계체제의 세 가지 결정요인이다. 따라서 후계체제의 세 가지 결정요인은 김정은의 3대 세습과정에도 적용될 것이다. 그러나 내용에 있어서는 분명한 변화가 발견된다. 첫째, 김정일 후계체제에서 후계자의 정통성의 핵심은 후계자의 능력과 자실을 증명하는 ‘인물본위’이다. 반면, 김정은 후계체제에서 후계자의 정통성의 핵심은 만경대와 백두를 기반으로 한 ‘혈통승계’이다. 둘째, 김정일 후계체제에서 후계자의 영도 체계는 ‘당적 영도체계’를 의미했다. 그러나 선군정치가 주도하는 3대 세습의 과정에서는 후계자의 ‘선군 영도체계’ 확립이 가장 중요한 내용이다. 셋째, 김정일 후계체제에서 후계자에 대한 인민대중의 지지는 ‘지도와 대중의 결합’을 통해 집단주의적 가치로 나타났다. 그러나 경제난으로 인해 집단주의적 가치가 무너진 3대 세습의 과정에서는 ‘시장의 재허용’을 통한 인민생활의 향상이 후계자에 대한 대중의 지지를 얻을 수 있는 가장 중요한 방법이다. 결과적으로 북한의 3대 세습은 경로의존성이 갖는 ‘의도하지 않는 결과’를 보여주고 있다. 북한의 3대 세습이 경로의존성을 따라 반복되지만, 후계자의 취약한 정통성과 폭압적 영도체계 그리고 시장을 통한 인민 대중의 지지는 김정일 이후 북한체제 전환의 새로운 토대가 될 수 있다. In the post-Kim Jong-il era, how North Korea`s third succession will be determined is very important in predicting North Korean problems. This paper emphasizes that the three determinants of Kim Jong-il`s succession system are to be the main determinant factors in the process of the third succession due to the system`s path-dependence. Path-dependence implies that once a system is formed within the environments of the era and a path-dependence pattern develops, then despite changes in the circumstantial conditions of that basis, the system will not change much and will tend to continue. The succession theory in North Korea was already institutionalized in the course of establishing Kim Jong-il`s succession system. The three determinants of North Korea`s succession system are confirming the legitimacy of the successor, establishing the leadership system of the successor, and building people`s support for the successor. Therefore these three factors will be applied in the third succession process of Kim Jong-eun. Yet, there are distinct differences discovered in the contents. First, the core in confirming the legitimacy of a successor in Kim Jong Il`s succession system puts stress on `character` that proves the successor`s capability and qualification. On the other hand, the essential legitimacy of a successor in the youngest son Kim Jong-eun`s succession system lays emphasis on `blood` that is based on Mankyungdae and Baekdu. Second, in Kim Jong-il`s succession system, the leadership system of the successor meant the leadership system of the (Workers`) party. However, in the course of the third generation succession led by the `seongun(military first)` policy, establishing the seongun leadership system of the successor has become most important. Third, in Kim Jong[il`s succession system, the peoples` support for a successor appeared as a value of collectivity through the union of leaders and people. Yet, in the course of the third generation succession in which such value collapsed due to economic hardships, improving the living standard of people by allowing markets again would be the most significant way to earn the peoples` support. Consequently, the third generation succession in North Korea shows an ‘unexpected result` that path-dependence has. Even though the third generation succession repeats itself by following path-dependency, a successor`s weak legitimacy, oppressive leadership system and people`s support through market may become the new groundwork for the transformation of the North Korean regime, after the Kim Jung-il era.

        나노 크기 매킨나와이트로 코팅된 규사를 이용한 아비산염의 흡착

        나노 크기 매킨나와이트(FeS)는 높은 환원력, 흡착성, 그리고 비표면적을 지니고 있어, 염소유 기물의 분해와 중금속 및 비금속의 제거에 유용하다. 하지만 매킨나와이트 나노입자는 콜로이드 안정성(colloid stability)의 변화로 지하수 흐름에 따라 쉽게 확산되거나, 입자집적(particle aggregation)에의해 대수층의 공극을 막을 수 있다. 따라서 투과반응벽(permeable reactive barrier)에 적용하기 위해서 적절한 공학적 변형이 필요하다. 본 연구에서는 코팅법을 적용해 나노크기 매킨나와이트를 변형시 킴으로써 본래의 반응성을 유지하고 또한 경제적인 투과반응벽의 설치에 활용하고자 한다. 이를 위해 화학적 처리를 하지 않은 규사(non-treated silica sand, NTS)와 화학적 처리에 의해 불순물이 제거된 규사(chemically treated silica sand, CTS)를 사용해 매킨나와이트를 코팅시켰다. 두 규사 모두 약 pH 5.4에서 매킨나와이트가 최대로 코팅되었으며, 이 pH는 (1) 매킨나와이트의 용해도, (2) 규사 및 매킨나 와이트의 표면전하(surface charge)에 의해 영향받았다. 최적 pH에서 NTS와 CTS에 의한 코팅량은 각각 0.101 mmol·FeS/g, 0.043 mmol·FeS/g으로, NTS 표면에 존재하는 산화철 등의 불순물에 의해 매킨나와이트의 코팅이 현저히 증가했다. 한편 혐기성 조건에서 코팅되지 않은 규사 2종과 최적 pH에서 코팅된 규사 2종을 이용해 아비산염(arsenite)의 흡착실험을 실시했다. pH 7에서 코팅되지 않은 NTS와 코팅된 NTS에 의한 아비산염의 상대적 제거율은 아비산염의 초기 농도에 따라 달라졌다. 낮은 농도에서 코팅되지 않은 NTS가 높은 아비산염의 제거율을 보였으나, 높은 농도에서는 코팅된 NTS가 상대적으로 높은 제거율을 보였다. 이런 차이는 아비산염은 낮은 농도에서 규사 표면에 존재하는 산화물과의 표면배위결합(surface complexation)에 의해 제거되었고, 높은 농도에서 코팅된 매킨나와이트와 반응해 황화비소(arsenic sulfides)로 침전되었기 때문이다. pH 7에서 코팅된 NTS에 비교해 코팅된 CTS는 현저히 낮은 아비산염 제거율을 보였는데, 이는 CTS의 상대적으로 낮은 매킨나와이트 코팅량에 기인했다. 따라서 코팅된 NTS는 코팅된 CTS보다 아비산염의 제거를 위한 투과반응벽의 설치에더 적합한 물질이며, 특히 아비산염의 오염도가 심한 지하수의 복원에 유용하게 적용될 수 있다. Due to the high reduction and sorption capacity as well as the large specific surface area, nanosized mackinawite (FeS) is useful in reductively transforming chlorinated organic pollutants and sequestering toxic metals and metalloids. Due to the dynamic nature in its colloid stability, however, nanosized FeS may be washed out with the groundwater flow or result in aquifer clogging via particle aggregation. Thus, these nanoparticles should be modified such as to be built into permeable reactive barriers. This study employed coating methods in efforts to facilitate the installation of permeable reactive barriers of nanosized mackinawite. In applying the methods, nanosized mackinawite was coated on non-treated silica sand (NTS) and chemically treated silica sand (CTS). For both silica sands, the maximum coating of mackinawite occurred around pH 5.4, the condition of which was governed by (1) the solubility of mackinawite and (2) the surface charge of both silica and mackinawite. Under this pH condition, the maximum coating by NTS and CTS were found to be 0.101 mmol·FeS/g and 0.043 mmol·FeS/g respectively, with such elevated coatings by NTS likely linked with impurities (e.g., iron oxides) on its surface. Arsenite sorption experiments were performed under anoxic conditions using uncoated silica sands and those coated with mackinawite at the optimal pH to compare their reactivity. At pH 7, the relative sorption efficiency between uncoated NTS and coated NTS changed with the initial concentration of arsenite. At the lower initial concentration, uncoated NTS showed the higher sorption efficiency, whereas at the higher concentration, coated NTS exhibited the higher sorption efficiency. This could be attributed to different sorption mechanisms as a function of arsenite concentration: the surface complexation of arsenite with the iron oxide impurity on silica sand at the low concentration and the precipitation as arsenic sulfides by reaction with mackinawite coating at the high concentration. Compared to coated NTS, coated CTS showed the lower arsenite removal at pH 7 due to its relatively lower mackinawite coating. Taken together, our results indicate that NTS is a more effective material than CTS for the coating of nanosized mackinawite.
