Early Cardiac Valvular Changes in Ankylosing Spondylitis: A Transesophageal Echocardiography Study

Background: This study was conducted to determine the early cardiac valvular changes in young male ankylosing spondylitis (AS) patients. Methods: A total of 70 AS patients on treatment without clinical cardiac symptoms were divided into group I (< 10 years, n =50) and group II (≥ 10 years, n = 20) depending on their disease duration after first diagnosis. Twenty-five healthy volunteers were selected as control subjects. All the subjects underwent transthoracic and transesophageal echocardiography, electrocardiography, and rheumatologic evaluation for AS patients. Results: The thickness of both the aortic and mitral valve was more increased in AS patients than in controls. Aortic valve thickness over 1.3 mm could predict AS with a sensitivity of 73% and specificity of 76%. The prevalence of aortic valve thickening was higher in the AS group compared to the controls. The prevalence of aortic and mitral regurgitation was very low and there was no difference between the controls and the patients. The aortic valve thickening was related to longer disease duration, high blood pressure, disease activity and inflammatory markers. Conclusion: Thickening of the aortic and mitral valve was observed without regurgitation in male AS patients early in the course of their disease without clinical cardiac manifestations. This subclinical change of aorto-mitral valve in early AS should be considered and followed up to determine its prognostic implication and evolution.
Fresnel Prism Affects the Balance in Hemiplegic Stroke Patients without Neglect

The purpose of this study was to investigate to effects of temporary visual feedback training using Fresnel prism glasses on balance in hemiplegic stroke patients without neglect. 19 hemiplegic stroke patients were randomly assigned to the experimental group (n=10) and the control group (n=9). Subjects of the control group performed balance and gait training, and subjects of the experimental group performed balance and gait training with a Fresnel prism glasses for 30 minutes each. All subjects performed motor-free visual perception test (MVPT) before intervention to measure visual perception. Functional reaching test (FRT) and gait parameters were performed before and after intervention. As a result, MVPT was no significant difference between the groups (p>0.05). FRT showed significant difference between the groups (p<0.05). In gait parameters, the swing time decreased and the single support time on the affected side increased. In summarizing the results of this study, the application of temporary Fresnel prism glasses had the effect to improve the balance ability in hemiplegic stroke patients without neglect. Therefore, Fresnel prism glasses could be favorably used as an intervention tool for improving balance in hemiplegic stroke patients without neglect.

<B>Background.</B><P>Papillary thyroid microcarcinoma (PMC; ≤1 cm) is thought to take a benign course during the lifetime. However, recent studies showed high recurrence rates for PMC.</P><B>Methods.</B><P>We analyzed the clinicopathological features, long-term prognosis, and some molecular characteristics including BRAF<SUP>V600E</SUP> mutation by retrospectively reviewing the records of 1150 patients with papillary thyroid carcinoma (PTC), 278 with PMC, and 868 with PTC >1 cm.</P><B>Results.</B><P>The prevalence of extrathyroidal invasion (52.2%) and initial nodal metastasis (34.9%) in patients with PMC was surprisingly high and almost as high as that for patients with PTC (72.4% and 51.8%, respectively). The rate of recurrent or persistent disease did not differ between patients with PMC and PTC (recurrent or persistent disease, 6.1% vs 14.1%; 53.4- vs 84.2-month follow-up; n = 98 vs 647; corrected p = .112). The frequency of BRAF<SUP>V600E</SUP> mutation was similar in patients with PMC and PTC (65.6% vs 67.2%). Immunohistochemical staining showed no different expression pattern according to the tumor size.</P><B>Conclusion.</B><P>These results suggest that PMC is not an occult cancer and it can act like larger PTC. Lead time at the manufacturing site is one of the most important performance indicators to meet the customer"s delivery date or to establish a production schedule plan. Lead time prediction is a very important role in establishing production schedule plans in most manufacturing sites. However, there are many uncertainties in the actual manufacturing system, such as machine failure, product reprocessing, and fluctuations in preparation time and processing time disrupt the prediction of accurate lead time. Therefore, this paper aims to predict and analyze the lead time of hybrid flow-shop with machine failure and reprocessing through a step-by-step experiment. In addition, in order to generate training data in predictive analysis, we intend to build hybrid flow-shop using manufacturing modeling simulation and apply lead time predictive analysis using data generated through it. In addition, we intend to show a predictive model with high accuracy in each situation by comparing and analyzing the variables through a step-by-step experiment in consideration of whether they are selected or reprocessed within the data generated in the simulation.

Objective: Through the literature on the effect of Scolopendra subspinipes mutilans L. Koch, we are finding out the clinical possibility and revealing the more effective to untractable disease. Method: We investigated the literatures of Oriental Medicine and experimental reports about Scolopendra subspinipes mutilans L. Koch. Results: 1.The taste of Scolopendra subspinipes mutilans L. Koch is hot, wann and toxic, and the effect is dispelling wind, spasmolytic action and detoxication so it has been used for C.V.A, facial palsy, numbness of hands and feet, wounds and arthritis. 2. A toxic constituent of Scolopendra subspinipes mutilans L. Koch is mainly located in the 1st limbs, and we can prevent toxic symptoms, if taken a dosage moderately. 3. The pharmacological action of Scolopendra subspinipes mutilans L. Koch is anti-convulsive action, analgesic action, lowering blood pressure, anti-inflammatory action, anti-tumor action and microbe inhibition. On the study of Scolopendra subspinipes mutilans L. Koch, we thought it is effective to intractable disease, and it may be needed variable studies on toxicity and clinical effects.
