        Pulse diagnosis refers to the process of diagnosing a patient by feeling an artery on the wrist based on the shape that the pulse takes while the hold-down pressure increase. The styloid process artery on the wrist is usually felt, and the pulse is taken on Chon, Gwan and Check using three fingers. This study is to examine the structural difference in the location of pulse diagnosis by measuring and analyzing blood diameter, blood depth, and blood flow velocity of the location of pulse diagnosis by using ultrasonic wave (VOLUSION730 PRO, GE Medical, U.S.A). This study also attempted to grasp whether the characteristics of blood vessels differ depending on Body Mass Index (BMI) and analyzed their correlation with Oriental medical pulse diagnosis. The male subjects without cardiovascular diseases were divided into the normal BMI group, the underweight group and the overweight group and 10 people of each group were measured. Blood depth, blood diameter and blood flow velocity at the location of pulse diagnosis (Chon, Gwan, Check) of the wrists of left and right hands were measured and the pulse wave was measured by using pulse diagnosis instrument (3-D Mac, DaeyoMedi, Korea). The results of this study showed that the characteristics of blood vessels differ depending on the degrees of obesity, and the characteristics of floating pulse and sinking pulse of Oriental medical pulses were related to the degrees of obesity. This shows that the characteristics of the blood vessels of subjects and BMI information are the major indicators for diagnosis and are the matters that must always be considered when developing the algorithm of pulse diagnosis.

      • 부ㆍ침맥 진단에 유용한 맥상 파라메터 및 대표맥상 분석

        Pulse feeling is one of the most important diagnosis method in Oriental medicine. But it is not easy to make an objective and standardized diagnosis. In this study, we found how to quantify the pulse diagnosis. Specially the high practicality in clinic, we search some parameters especially well-related to floating and sinking pulse by statistical analysis. By extension, we find the pulse patterns of the floating and sinking pulse. We choose 15 subjects diagnosed as floating pulse and 15 subjects diagnosed as sinking pulse by oriental doctors. And their pulse signals were acquired by pulse analyzer which has piezoresistive pressure sensor. For the quantification of the floating and sinking pulse, at first, we examined the parameters which were highly correlated with oriental doctor's diagnosis. And then we derived pulse patterns of the floating-sinking pulse from preprocessed signal and its ensemble average. We also looked trend variation (PH-Curve) between contact and pulse pressure. As a result, statistically there is the biggest difference between contact pressure, the maximum pulse pressure, diastolic area (Ad) and floating and sinking data. Through the PH-Curve, which represented the relationship between contact and pulse pressure, we could divide the floating and sinking pulse clearly. As a basic research of pulse diagnosis algorithm, we can contribute to select essential parameters in diagnosis algorithm. And using these diagnosis method, we expect to find typical pulse patterns and some useful parameters about other pulses like slow/rapid, large/fine pulse and so on. We hope that this study will contribute pulse objectification.

        Columnar variant of papillary carcinoma in the thyroglossal duct cyst with progression to lung metastasis

        Thyroglossal duct cyst (TGDC) carcinoma generally shows a favorable prognosis. If metastasis is present latently, it may not threaten the patient’s life immediately. It has been shown, however, that larger than 1 cm papillary carcinoma (PC), level VI metastasis to the lymph node (LN), which is the nearest to the thyroid, independently predicts a worse prognosis. In the case presented herein, a 61-year-old female patient was diagnosed with an about 3 cm PC in the TGDC, particularly the columnar variant subtype, one of the aggressive variants. She had occult papillary thyroid microcarcinoma, but no LN metastasis. Even though she underwent the Sistrunk procedure and total thyroidectomy with central compartment neck dissection followed by high-dose radioactive iodine remnant ablation, however, the cancer cells spread to level IV neck LN, and finally to the lung. Therefore, when a patient is diagnosed with an aggressive histologic variant of PC in the TGDC, even without LN metastasis, the invasive surgical approach and close postoperative surveillance are necessary, with consideration of the risk of disease progression. Therefore, if it is possible to stratify the risk for patients, higher-risk patients can be offered a more invasive therapeutic approach.

        <P>Obesity is an important and preventable risk factor for renal disease. The administration of an antioxidant with a lipid-lowering effect is an important therapeutic approach for kidney disease in obese patients. The present study was conducted to examine whether methanolic extract of Dendrobium moniliforme (DM), one of the most famous traditional medicines used in many parts of the world, has an antioxidant effect in vitro and an ameliorative effect on high-fat diet (HFD)-induced alterations such as renal dysfunction and lipid accumulation in vivo. The 1,1-diphenyl-2-picrylhydrazyl radical scavenging activity of DM extract (IC(50) = 29.6 μg/mL) was increased in a dose-dependent manner. The LLC-PK1 kidney cell damage induced by oxidative stress was significantly inhibited by the treatments with DM extract. In the animal study, DM extract (200 mg/kg) was orally administered every day for nine weeks to HFD-fed mice, and its effect was compared with that of metformin. The administration of DM extract decreased the elevated serum glucose, total cholesterol concentration and renal lipid accumulation in HFD-fed mice. It also ameliorated renal dysfunction biomarkers including serum creatinine and renal collagen IV deposition. Taken together, these results provide important evidence that DM extract exhibits a pleiotropic effect on obesity induced parameters and exerted a renoprotective effect in HFD-fed mice.</P>

        태평양전쟁 전후 캠프 쇼의 계보와 미군의 동아시아 인식

        이 논문은 세계 2차 대전 시기 태평양 지역 군사 충돌이 심화되고 주둔 규모가 커지면서 등장하는 캠프 쇼(Camp Show)라는 새로운 엔터테인먼트 장르와, 이 캠프 쇼가 1945년 이후 미군 부대의 ‘현지인 캠프 쇼’라는 형태로 동아시아에 정착하게 되는 과정을 살펴본다. 특히 <지나의 밤>이라는 노래를 통해 미국과 일본의 중첩된 제국의 관계 속에서 형성되는 식민지 전후 한국예술인들의 캠프 쇼 퍼포먼스의 특징과 변화들도 함께 살펴본다. 1945년 이후 일본 제국주의에서 아메리카합중국(미국)으로의 급속한 힘의 재편 과정에서, 그 해 8월을 기준으로 불과 몇 달 전까지 일본 제국을 위한 캠프 쇼를 공연하던 한국예술인들이 미군이라는 새로운 관객을 위한 캠프 쇼를 조직, 새로운 기대에 부흥하는 퍼포먼스를 연출하게 된다. 태평양전쟁 직후 일본을 향한 호의적인 아시아정책에 맞추어진 미국의 새로운 헤게모니 아래서 일제의 식민지 국가들의 관계도 재편이 되는데, 이 과정에서, 한국 현지인 캠프 쇼(Korean Native Show)의 무대는 일본 제국주의의 범아시아(Pan-Asian) 이데올로기와 미국의 오리엔탈리즘이라는 모순적인 논리가 뒤섞이며, 미군들에게 낯설고 이국적이지만 익숙하고 심지어 편안한 퍼포먼스를 제공하는 장소가 된다. 한반도 내 복잡하고 중첩되는 군사주의와 식민주의의 유산은, 그대로 한국인 캠프 쇼 엔터테이너들의 모순적인 혼종(hybrid)의 퍼포먼스를 가능하게 하는 구조적 힘으로서 작용한다. 이 논문은 한국에서의 미군부대 캠프 쇼, 소위 미8군 쇼와 관련된 연구들의 연장선에서, 캠프 쇼의 역사를 한국전쟁 이전으로 확장하고, 미군부대의 캠프 쇼가 동아시아 지역에 정착하게 된 과정을 하나의 맥락에서 살펴보는 것에 의의가 있다. This article examines the ways in which multiple traditions of camp shows and the overlapping and relational layers of Imperial Japan and U.S. presence in Korea shaped Korean entertainers’ lives after 1945, producing their idiosyncratic performances in response to rapid shifts in Korea’s relations with Japan and the United States in the 1940s-1950s. When the United States sought to reposition Japan at the top of the newly emerging American hegemonic order of Asian countries, Korean entertainers who served the Imperial Japanese Army a few months earlier found themselves performing for American soldiers. The stage of the Korean native camp shows became a “strange and exotic” yet “familiar and even comforting” place where inconsistent logics, such as Imperial Japan’s pan-Asian ideology and American Orientalist fantasy, mingled. Under the complicated legacies of overlapping militarization and colonization in Korea, militarization has constituted a structuring force that enabled Korean women camp show entertainers generating their hybrid performance styles in ironies, contradictions, and complexities. Building on postcolonial theorists’ notion of hybridity, I argue that Korean entertainers’ performances were being shaped or negotiated in contact with different audiences and expectations as well as Korean entertainers attempted to navigate the acceptable ground of performances and womanhood in the constantly changing political and ideological environment.

      • 한의사 맥진 가압력 재현에 대한 고찰

        In pulse diagnosis, the indentation pressure is one of the most important factors as well as the change of pulse shape and the distribution of pressure via time. But, on the oriental medical doctor's indentation pressure control, the understandings of the neurophysiological meanings and mechanisms have been lacked. So, in this paper, we considered on these issues and then proposed a proper system which can imitate the OMD's indentation pressure control mechanisms. As a result, both tactile information and kinesthetic information were found to be essential to the indentation pressure control so that a system, which can measure both the physical indent pressure and the displacement of an indentation arm, has been proposed. With this proposed system, while the indentation was being controlled through the moving step number of the step motor, the physical indentation pressure and displacement of the indentation arm were measured. From these measured data, the relationships between the moving step number and both physical indentation pressure and displacement were revealed to have linear characteristics in early phase and to have nonlinear characteristics in latter phase. Additionally, three types of graph were generated whose X axis means the moving step number, the physical indentation pressure and the displacement respectively and Y axis means the pulse pressure. By comparing these graphs, we come to conclude that different concepts on indentation pressure control cause different diagnostic results on floating/sinking degrees for the same subject. Consequently, an indentation system for the pulse diagnosis should be able to provide both the tactile information and kinesthetic information, that is, the physical indentation pressure and the displacement of the indentation arm. In future, the proposed system should be optimized to the pulse diagnosis environment and how to combine the both information for more reliable diagnosis should be studied.

      • 토노메트리 측정 관점에서의 부침맥 고찰

        In pulse diagnosis, floating pulse and sinking pulse are frequently used for diagnosis about where disease is located and how much severe they are. However, in what mechanism floating pulse and sinking pulse arise is not known well. There are two point of views on substantial of floating pulse and sinking pulse. The first one is the floating and sinking degrees is the expression on the depth of pulsation. And, the second one is floating and sinking pulse is based on the response of pulsation to the indent pressure on radial artery. In this paper, we discussed these two opinions in the view point of tonometric measurement. The process for diagnosis on floating pulse and sinking pulse is similar to the tonometric measurement for non invasive blood pressure or intraocular pressure. We modelled the degrees of depth of pulsation with different indent pressures for initial pulsation feeling and different slopes of indent pressure lines. From this modelling, we can confirm the effect of pulsation depth on P-H curve, that is, in the model where lower pulsation is assumed, the shift of optimal indent pressure to the right was observed. The response of pulse pressure to the indent pressure was tried to be modelled with the degrees of mean blood pressure. Consequently, we tried to model the phenomenon of floating and sinking pulse for the first. And, from this modelling, we can get abundant understanding on how floating and sinking pulse can be caused. In the further study, we want to prove the suitability of this tonometric measurement based modelling with various studies including ultrasound measurement for the depth of pulsation in different BMI subjects.
