Mohsin Hamid`s second novel The Reluctant Fundamentalist can be conveniently assigned to the literary family of what we now call post-9/11 fiction. Changez, a Princeton graduate and the narrator-protagonist of the novel, starts out as a third-world immigrant working as an analyst at a valuation firm in Manhattan. He considers his life in metropolitan New York a great success, and this encourages him to embrace the American dream wholeheartedly. However, his American dream turns sour and nightmarish in the wake of the 9/11 terrorist attacks as the United States launches her global war on terror and wholesale intervention in world affairs-a theme that runs through the pages of the novel. In his reflections on the ways that make possible America`s “constant interference in the affairs of others,” Changez also diagnoses that “finance was a primary means by which the American empire exercised its power,” based, of course, on his various Manhattan experiences. Finding the new social-political atmosphere in the US suffocating and unbearable, Changez gives up his work in Manhattan and returns to his hometown, Lahore, campaigning against American imperialism and devoting himself to the cause of fighting American military and economic hegemony. This paper takes The Reluctant Fundamentalist as a fable of infatuation and disenchantment with the US. I shall analyse how 9/11 becomes a defining moment in the life of Changez, shattering his American dream and turning him into an American-made reluctant fundamentalist. I shall also draw on Judith Butler`s ideas of precarious life and livable life to foreground Changez`s critique of America`s failure to show due respect to, and be bound by, an ethical responsibility towards the Other.
An Integration of Kano's Model and Exit-Voice Theory : A Case Study

The purpose of this study was to examine overall customer satisfaction associated with medical service quality in Taiwan by integrated Kano's model and customer satisfaction index model. Another purpose was to confirmed nonlinear and asymmetric relationship of Customer Satisfaction and Quality Performance by the research outcome. By analyzing 1,100 patients or their family members, this study used the structural equation model (SEM) with AMOS software for data analysis. The results show that must-be attributes, one-dimensional attributes and attractive attributes had a direct effect on overall customer satisfaction, Surprisingly, overall customer satisfaction had positively influenced customer loyalty customer satisfaction had negatively influenced customer complaints. The study also found that customer complaints have direct effect on customer loyalty. Importantly, the study found out the must-be attributes, the attractive attributes and one-dimensional attributes increased, the level of overall customer satisfaction also increased. The customer satisfaction positively influences customer loyalty in medical service quality in Taiwan. The findings might reveal new insights for researchers dealing with quality of medical service and for hospital managers who devote resources exclusively to achieving highest possible levels of patient satisfaction.

Purpose: Metastatic castration-resistant prostate cancer (mCRPC) has a poor prognosis. Abiraterone acetate (AA), enzalutamide, and chemotherapy are first-line treatments for patients with mCRPC. This study examined prognostic factors for AA response in the form of prostate-specific antigen (PSA) kinetics throughout androgen-deprivation therapy (ADT) in chemonaïve patients with mCRPC. Materials and Methods: We retrospectively included data from 34 chemonaïve patients with mCRPC who had received AA at some point between January 2017 and December 2018. We separated patients into two study arms according to the decrease in PSA percentages after use of AA for 3 months. We correlated PSA kinetics parameters with response and compared the two study groups with respect to PSA kinetics. Results: The patients’ median age was 77 years. In the total group of patients, 64% had a response to AA, whereas 35% did not. The ratio of the PSA level at nadir to the level during ADT was significantly higher in the AA-sensitive group (19.78 vs. 1.03, p=0.019). Conclusions: Patients who experienced a dramatic change in PSA level during ADT were more likely to be resistant to AA after progression to mCRPC. Chemotherapy rather than AA might be more suitable as a first-line treatment for these patients.

Objective: Patients with opioid use disorder (OUD) have impaired attention, inhibition control, and memory function. The aldehyde dehydrogenase 2 (ALDH2 ) gene has been associated with OUD and ALDH2 gene polymorphisms may affect aldehyde metabolism and cognitive function in other substance use disorder. Therefore, we aimed to investigate whether ALDH2 genotypes have significant effects on neuropsychological functions in OUD patients undergoing methadone maintenance therapy (MMT). Methods: OUD patients undergoing MMT were investigated and followed-up for 12 weeks. ALDH2 gene polymorphisms were genotyped. Connors’ Continuous Performance Test (CPT) and the Wechsler Memory Scale-Revised (WMS-R) were administered at baseline and after 12 weeks of MMT. Multivariate linear regressions and generalized estimating equations (GEEs) were used to examine the correlation between the ALDH2 genotypes and performance on the CPTs and WMS-R. Results: We enrolled 86 patients at baseline; 61 patients completed the end-of-study assessments. The GEE analysis showed that, after the 12 weeks of MMT, OUD patients with the ALDH2 *1/*2+*2/*2 (ALDH2 inactive) genotypes had significantly higher commission error T-scores (p = 0.03), significantly lower hit reaction time T-scores (p = 0.04), and significantly lower WMS-R visual memory index scores (p = 0.03) than did patients with the ALDH2 1 */*1 (ALDH2 active) genotype. Conclusion: OUD patients with the ALDH2 inactive genotypes performed worse in cognitive domains of attention, impulse control, and memory than did those with the ALDH2 active genotype. We conclude that the ALDH2 gene is important in OUD and is associated with neuropsychological performance after MMT.

This study purposed to look for its uses by analyzing useful substances in kenaf (Hibiscus cannabin us L.) leaves based on the total polyphenol and flavonoid contents in the leaves, DPPH radical scavenging activity of leaf extract, SOD activity, etc. by different growth stage and variety. According to the results of experiment, the total polyphenol content was highest in Everglade-41 (27.9 mg/g dw.), the total flavonoid content was highest in Tainung-2 (42.3 mg/g dw.), and SOD activity was highest in Dowling (96.1 %). However, DPPH radical scavenging activity was similar among the three varieties.
