P252 : Analysis of dermatologic consultations in neonatal intensive care unit patients

Background: There have been several studies about neonatal skin diseases. However, there has been few study analyzing dermatologic consultations of neonate intensive care unit(NICU) patients. Objectives: The aim of this study is to investigate consulted cutaneous disorders of neonates in NICU and their clinical courses after consultation. Methods: The authors retrospectively reviewed the medical records of patients who were consulted at our department of dermatology during their NICU admission periods for 5 years(2009-2014). Results: A total of 44 neonates were enrolled and 25 of them were preterm babies. They were composed of 20 males and 24 females. The mean age was 4.3(0.15-18) weeks and the mean gestational age was 33.3(24-41) weeks. Infectious disease was the most common (n=9, 20.4%), followed by eczema (n=8, 18.2%) and disorder of skin appendages (n=8, 18.2%). Congenital disorders were consulted in 12 patients (27.3%). Pressure alopecia was the most common disease entity (n=6, 13.3%), followed by cutaneous candidiasis (n=5, 11.1%). Conclusion: This study showed dermatologic problem of neonate in NICU showed characteristic distribution. Both understanding about specialized condition of neonate in NICU and collaboration with pediatrician seem to be necessary to solve the dermatologic problem of neonates.

A case of eczema coxsackium in adult patient after direct contact with hand-foot-mouth disease patient

A 22-year-old female presented with erythematous macules and papules on her both palms and erythematous scaly papules on whole body that developed 6 days ago. She had been treated with psoriasis vulgaris since about 10 years ago and kept in good condition recently. Before the skin lesion developed, she had history of direct contact with the infant with hand-foot-mouth disease 1 week ago. On histopathologic examination, spongiosis and exocytosis of inflammatory cells were shown in epidermis and lymphocytic infiltration in dermis. Considering early stageof enterovirus infection, real time polymerase chain reaction was performed and coxsackievirus A16 was identified. Eczema coxsackium caused by coxsackievirus A16 was finally diagnosed. Eczema coxsackium is caused by enterovirus and presented with fever, sore throat and generalized cutaneous eruption. It usually occurred in children but rarely reported in adult who had underlying disease such as Darier’s disease, eczema or human immunodeficiency virus infection . Herein, we report a rare case of eczema coxsackium in adult patient after direct contact with hand-foot-mouth disease patient.

A case of CD30 positive anaplastic large cell lymphoma occurred in patient of mycosis fungoides

CD30 positive anaplastic large cell lymphoma is characterized by constant expression of CD30 and anaplastic cytologic feature. It usually experience an indolent disease course and favorable prognosis. Large cell transformation of mycosis fungoides can show similar clinical and histopathologic findings with CD30 positive anaplastic large cell lymphoma. But it shows poor prognosis and needs more aggressive treatment, so differential diagnosis of those two diseases should be required. Differential diagnosis is made by overall consideration of clinical presentation, past history, histopathologic findings and disease course but the distinguishment is still difficult. A 29-year-old Korean male presented with erythematous nodule on his left leg. He was diagnosed as mycosis fungoides 10 years ago and had been treated with phototherapy continuously. On histopathologic examination and immunochemistry of the nodule, more than 75% of CD30 positive large T cell shown in the specimen. With favorable clinical course, CD30 positive anaplastic large cell lymphoma was finally diagnosed. Herein, we report a case of CD30 positive anaplastic large cell lymphoma in patient with mycosis fungoides differentially diagnosed from large-cell transformation of mycosis fungoides.

A case of bullous dermatomyositis with ovarian cancer

Dermatomyositis is characterized by concurrence of proximal muscle weakness and inflammatory skin lesions occurring in a specific anatomical distribution. Aside from muscular and dermatologic abnormalities, dermatomyositis is known to be associated with internal malignancy in about 10% to 40%. A 57-year-old female presented with symmetric erythematous patch on extensor area of both extremities. Blistering changes were accompanied on the lesion. Gottron’s papules were observed on both hands. Proximal muscle weakness on both extremities was developed with the skin lesion. Laboratory test showed elevated muscle enzymes and electromyography was compatible with myopathy. Histopathologic examination revealed characteristic changes of dermatomyositis with subepidermal blister formation. Dermatomyositis was diagnosed and during the evaluation, ovarian cancer was found that surgical excision and adjuvant chemotherapy were performed. Bullous dermatomyositis is rare variant of dermatomyositis. It seems to be at higher risk of internal malignancy than classic dermatomyositis and intensely linked with gynecologic malignancy. To our knowledge, this is first report of bullous dermatomyositis in Korean patient. Dermatologists should be aware of this rare variant of dermatomyositis that careful observations for malignancy, especially gynecologic tumors should be performed.

P235 : Epidemiologic investigation of skin diseases among sentenced prisoners in Busan through telemedicine

Background: Skin problems are common in prisoners with variable manifestation in specialized environment of cohabitation. However, epidemiologic investigation of skin diseases among prisoners are underresearched. Objectives: This study was aimed to investigate the skin diseases of prisoners in Busan Detention center. Methods: Live interactive teledermatology was performed with prisoners for previous 17 months in our hospital. We reviewed the records of consultation including patient’s demographics, diagnosis made by inspection of dermatologist, treatment and course. Results: A total of 139 patients were consulted for dermatologic problem. Most of the patients were male(97.8%) and median age was 36.2. Folliculitis(20.9%) was most common diagnosis and followed by scabies(17.3%), superficial fungal infection(12.9%). Fifty-one patients(36.7%) consulted more than 2 times and most of them(66.7%) showed clinical improvement. Further evaluation was recommended in one patient. Conclusion: Skin diseases were common health problem among prisoners and environmental factors in detection center may facilitate these problems. Early recognition of the disease can be helpful to provide better health care for prisoners, especially when infectious dermatoses can be suspected.

Background: Mass media plays an important role in shaping public acknowledgement about alopecia with increased accessibility. But there were no report investigating the interpretation of readers. Objectives: This study was aimed to evaluate the reliability of articles about alopecia in newspapers judged by dermatologists and patients. Methods: A total of fifteen articles about alopecia, which were selected from three groups of previous study(one print newspaper(group A) and two online newspapers (group B and C)). The reliability of the articles were measured by 11grade scale (0~10) by patients and dermatologists. Dermatologists also checked the accuracy of content. Results: In dermatologists’ investigation(n=39), group A showed the highest scores in both reliability and accuracy and group C showed the lowest scores with statistical significance. In patient’s investigation(n=39), the result showed similar tendency, which did not showed statistical significance. Also, there were significant gaps between dermatologists and patients in reliability score. Conclusion: Degree of reliability of articles regarding alopecia judged by the patient and dermatologist showed difference according to type of newspaper. It is suggestive that detailed consultation in the clinic and closer monitoring of the media is required.

Cutaneous metastasis of renal angiosarcoma occurred on the face

A 66-year-old male present with multiple erythematous nodules on his forehead, right cheek and left temple developed one month ago. He had been diagnosed as renal angiosarcoma with liver and bone metastasis and underwent right nephrectomy and adjuvant chemoradiation therapy. On histopathologic examination performed on his left temple, infiltration of basophilic epitheloid cells and irregularly anastomosing vascular structures dissecting collagen bundles and extravasated red blood cells were shown in entire dermis. High-power view showed atypical endothelial cells lining vascular lumen and pleomorphic, heterochromic tumor cells. Immunohistochemical stain of CD34 and factor VIII-related antigen showed positivity. On the basis of clinical and histopathological examinations, metastatic angiosarcoma was finally diagnosed. Angiosarcoma is highy malignant tumor which usually occurs on the skin of the head and neck area and is related with old age, lymphedema, trauma or radiotherapy. It frequently metastasizes from skin to internal organ, but metastasis from internal organ to skin is rarely reported. In Korean dermatologic literature, 2 cases of metastatic cutaneous angiosarcoma, from heart and lung relatively, were reported. Therefore, we report a rare case of metastatic cutaneous angiosarcoma from kidney.

A case of multiple nevoid hypertrichosis accompanied with clinodactyly

Nevoid hypertrichosis is a rare disorder, characterized by localized terminal hairs on normally pigmented skin. It usually presents at birth or soon after with marked female predilection. Etiology of this disease is not yet clarified but mosaicism during embryogenesis was suggested as possible cause of this phenomenon. Accompanied partial lipoatrophy and bony abnormalities has been reported occasionally. It usually presents as solitary lesion, but rarely, multiple patches can develop. We called it multiple nevoid hypertrichosis and need differential diagnosis with other disease such as congenital hypertrichosis lanuginose and congenital generalized hypertrichosis terminalis. An 18-month-old female child visited our clinic due to black, coarse hairs on her left periocular area, left upper back and left leg. Histopathologic examination demonstrated terminal hair follicles in normal skin. Except clinodactyly of left fifth finger, no other neurologic or bony abnormalities were found. She was diagnosed to multiple nevoid hypertrichosis and following-up regularly without other treatments.

pH-Independent Sustained Release Matrix Tablet Containing Doxazosin Mesylate: Effect of Citric Acid

The aim of this study was to develop a pH-independent sustained release matrix tablets of doxazosin mesylate. The matrix tablets were prepared by direct compression technique using polyethylene oxide, sodium alginate and citric acid as a pH modifier. Formulations were evaluated for an in vitro drug release study, erosion study, and the microenvironmental pH was studied using the pH indicator methyl red. For formulations without citric acid, the extent and rate of drug release in simulated gastric fluid were much higher than those in simulated intestinal fluid. By adding the citric acid, the drug release rate in simulated intestinal fluid was increased, and microenvironmental pH values within the tablets were maintained at low pH during drug release. Furthermore, drug release from the matrix tablet containing 20% w/w citric acid was comparable to that from a commercial product, Cardura® XL, and a pHindependent release could be achieved. Therefore, the incorporation of citric acid as a pH modifier to Polyethylen oxide-sodium alginate matrix tablets effectively produced pHindependent doxazocin mesylate release profiles.
