        WoeiWan Tan graduated with the Bachelor of Engineering (Electrical, Honours) degree and theMaster of Engineering degree from the National University of Singapore. After earning the Doctor of Philosophy degree from the University of Oxford for her work on self-learning neurofuzzy control of nonlinear systems, she joined the National University of Singapore where she is currently an Associate Professor. She is also serving as an Associate Head (Undergraduate Programmes) in the Department of Electrical & Computer Engineering, and as an Assistant Dean for the Faculty of Engineering External Relations Office. Her recent research interests are theoretical development of type-2 fuzzy systems, and the application of type-2 fuzzy systems to intelligent modelling, control and fault diagnosis. She also has interests in engineering education research, promotion of engineering education and experience with outcome-based accreditation processes.

        Immediate breast reconstruction following nipple-sparing mastectomy in an Asian population: Aesthetic outcomes and mitigating nipple-areolar complex necrosis

        Background Nipple-sparing mastectomies (NSMs) are increasingly performed to obtain the best aesthetic and psychological outcomes in breast cancer treatment. However, merely preserving the nipple-areolar complex (NAC) does not guarantee a good outcome. Darkly pigmented NACs and a tendency for poor scarring outcomes are particular challenges when treating Asian patients. Herein, we review the reconstructive outcomes following NSM at Singapore General Hospital. Methods All breasts reconstructed following NSM over an 11-year period from 2005 to 2015 were reviewed. Information was collected from the patients’ records on mastectomy indications, operative details, and complications. Patient satisfaction, breast sensation, and aesthetic outcomes were evaluated in 15 patients. Sensation was quantified using the Semmes-Weinstein monofilament test. Results A total of 142 NSMs were performed in 133 patients for breast cancer (n=122, 85.9%) or risk reduction (n=20, 14.1%). Of the procedures, 114 (80.2%) were autologous reconstructions, while 27 (19.0%) were reconstructions with implants. Complications occurred in 28 breasts (19.7%), with the most common complication being NAC necrosis, which occurred in 17 breasts (12.0%). Four breasts (2.8%) had total NAC necrosis. The overall mean patient satisfaction score was 3.0 (good). The sensation scores were significantly diminished in the skin envelope, areola, and nipple of breasts that had undergone NSM compared to non-operated breasts (P<0.05). Half of the subset of 15 patients in whom aesthetic outcomes were evaluated had reduced nipple projection. Conclusions Immediate reconstruction after NSM was performed with a low complication rate in this series, predominantly through autologous reconstruction. Patients should be informed of potential drawbacks, including NAC necrosis, reduced nipple projection, and diminished sensation.

      • Retrodirective RF System for Wireless Power Transfer

        This paper presents the overall wireless power transfer RF system with circulator for phase conjugating Retro-directive array (RDA). This work consists of two main parts. The former presents the Retrodirective System & Feed-forward-Cancellation (FFC) part while focus on beam of 1 x 4 RDA system & the improved transmitter (Tx) to receiver (Rx) isolation. The latter includes design of integrated circuits for power amplifier and drive amplifier. This system operates with Tx frequency of 2.4 GHz and Rx frequency of 2.5 GHz. The beamforming accuracy and minimum sensitivity of the RF system depends on the Tx-Rx isolation characteristic of the circulator. Hence, to enhance the isolation, FFC technique is incorporated. The isolation performance at 2.4 GHz and 2.5 GHz is improved by 18 dB and 21 dB, respectively, in this work.

        Predictors of early postoperative hypocalcemia after total parathyroidectomy in renal hyperparathyroidism

        Purpose: Patients with secondary hyperparathyroidism are at high risk for developing postoperative hypocalcemia. However, there are limited data regarding predictors of postoperative hypocalcemia in renal failure patient with secondary hyperparathyroidism. This study aimed to determine the clinical presentations of renal hyperparathyroidism and the predictors of early postoperative hypocalcemia after total parathyroidectomy. Methods: Data of patients with renal hyperparathyroidism who underwent total parathyroidectomy between January 2007 to December 2014 were reviewed retrospectively. Patients were divided into 2 cohort groups according to their serum calcium levels within 24 hours of parathyroidectomy: the hypocalcemia group (calcium levels of 2 mmol/L or less), and the normocalcemia group (calcium levels more than 2 mmol/L). With the use of multivariable logistic regression analyses, the predictors of early postoperative hypocalcemia after total parathyroidectomy in patients with renal hyperparathyroidism were investigated. Results: Among 68 patients, 56 patients (82.4%) were symptomatic preoperatively. Fifty patients (73.5%) presented with bone pain and 14 patients (20.6%) had muscle weakness. Early postoperative hypocalcemia occurred in 25 patients (36.8%). Preoperative alkaline phosphatase level was the predictor of early postoperative hypocalcemia (adjusted odds ratio, 1.004; 95% confidence interval, 1.001–1.006; P = 0.002). Conclusion: Results from our study show that most of the patients with renal hyperparathyroidism were symptomatic preoperatively and the most common clinical presentations were bone pain and muscle weakness. The significant predictor of early postoperative hypocalcemia after total parathyroidectomy was the preoperative alkaline phosphatase levels.

      • Intensity-Modulated Radiotherapy for Nasopharyngeal Carcinoma: Penang General Hospital Experience

        Purpose: To study the overall treatment time (OTT) and acute toxicity of intensity-modulated radiotherapy (IMRT) treatment for nasopharyngeal carcinoma (NPC). Methods: This retrospective study covered all NPC patients who underwent radical IMRT treatment at the Penang General Hospital from June 2011 to February 2012. Patients of any age and stage of disease with histologically proven diagnosis were included. Information was collected on patient demographics, clinical stage, treatment received, including any neoadjuvant and/or concurrent chemotherapy, acute toxity and completion of IMRT within the OTT. Results: A total of 26 NPC patients were treated with IMRT during the study period; 88.5% had stage III/IV disease. 45.2% received neo-adjuvant chemotherapy while 50.0% were given concurrent chemo-irradiation. All patients completed the treatment and 92.3% within the 7 weeks OTT. Xerostomia was present in all patients with 92.3% having grade 2. Severe grade III/IV acute toxicity occurred in 73.1% of patients, the commonest of which was oral mucositis (57.6%). This was followed by dysphagia which occurred in 53.8%, skin reactions in 42.3% and weight loss in 19.2%. However, haematological toxicity was mild with only one patient having leucopaenia. Conclusion: IMRT treatment for NPC is feasible in our center. More importantly, it can be delivered within the 7 weeks OTT in the majority of patients. Severe grade 3/4 toxicity is very common (73.1%) and thus maximal nutritional and analgesic support is required throughout the treatment.

        Distally Based Sural Artery Adipofascial Flap based on a Single Sural Nerve Branch: Anatomy and Clinical Applications

        Background: The distally based sural artery flap is a reliable, local reconstructive option forsmall soft tissue defects of the distal third of the leg. The purpose of this study is to describean adipofascial flap based on a single sural nerve branch without sacrificing the entire suralnerve, thereby preserving sensibility of the lateral foot. Methods: The posterior aspect of the lower limb was dissected in 15 cadaveric limbs. Fourpatients with soft tissue defects over the tendo-achilles and ankle underwent reconstructionusing the adipofascial flap, which incorporated the distal peroneal perforator, shortsaphenous vein, and a single branch of the sural nerve. Results: From the anatomical study, the distal peroneal perforator was situated at an averageof 6.2 cm (2.5-12 cm) from the distal tip of the lateral malleolus. The medial and lateral suralnerve branches ran subfascially and pierced the muscle fascia 16 cm (14-19 cm) proximal tothe lateral malleolus to enter the subcutaneous plane. They merged 1–2 cm distal to thesubcutaneous entry point to form the common sural nerve at a mean distance of 14.5 cm(11.5–18 cm) proximal to the lateral malleolus. This merging point determined the pivot pointof the flap. In the clinical cases, all patients reported near complete recovery of sensationover the lateral foot six months after surgery. All donor sites healed well with a full range ofmotion over the foot and ankle. Conclusions: The distally based sural artery adipofascial flap allowed for minimal sensory loss,a good range of motion, an aesthetically acceptable outcome and can be performed by asingle surgeon in under 2 hours.
