Solar urticaria in Korean patients

Background: Solar urticaria (SU) is an uncommon type of photodermatoses characterized by erythematous wheal with pruritus after sunlight exposure, which shows a female predominance. Only a few studies to analyze the clinical features of SU were reported in Asia. Objectives: We aimed to analyze the clinical and photobiological characteristics of SU in Korea. Methods: We retrospectively investigated the medical records of patients with SU who visited the department of dermatology, Konkuk university medical center from January 2005 to January 2020. Results: Of a total of 44 patients, 36 (81.8%) were male and 8 (18.2%) were female. The mean age at onset was 17.6 years (range, 5-47) and the mean threshold time was 19.7 minutes. Among 32 patients who underwent phototesting, the action spectrum most commonly implicated was ultraviolet (UV) A only in 12 (37.5%), and then visible light (415nm) only was responsible for SU in 8 (25.0%). Three patients (9.4%) were induced by both UVA and visible light (415nm). Even 4 patients (12.5%) were triggered by only natural sunlight. Serum immunoglobulin E levels were measured in 33 patients, and 17 (51.5%) showed values higher than normal. The treatment most widely used were oral antihistamine (43.2%), followed by phototherapy (13.6%). Conclusion: SU is a rare photodermatosis in Korea, which shows a male preponderance. In this study, UVA and visible light (415nm) were predominant action spectrum of SU.

Objective: The aim of this study is to evaluate oncologic outcomes of total abdominal radical hysterectomy (TARH), total laparoscopic radical hysterectomy (TLRH) and laparoscopic-assisted radical vaginal hysterectomy (LARVH) in early cervical cancer patients (stage IB2-3, tumor size≥2cm), analyzing the risk factors that may affect the clinical results. Methods: A total of 756 patients underwent radical hysterectomy between 2000 and 2019 from three university hospitals (Pusan National University Hospital, Yang-san Pusan National University Hospital, Ulsan University Hospital) were enrolled. Among them, 172 patients with stage IB2-3 were analyzed retrospectively. Results: Seventy-seven patients were treated with TARH, and 95 were managed via minimally invasive surgery (MIS), consisting of 35 with TLRH and 60 with LARVH. TARH group had better disease five-year disease free survival (DFS) and overall survival (OS) compared with MIS group (86.4% vs. 73.4 for DFS and 91.7 vs. 82.2 for OS), but not statistically significant in both (p=0.058 for DFS and p=0.119 for OS). When TARH and the LARVH group were compared, both five-year DFS and OS were not significant different (86.4 vs 82.2%; p=0.288 for DFS and 91.7 vs 84.4%; p=0.206 for OS). However, when TARH and the TLRH group were compared, TLRH group had significantly worse five-year DFS than TARH (86.4 vs 54.5%; p=0.008), but five-year OS was not significant different (91.7 vs 77.5%; p=0.127). Multivariate analysis showed that LN metastasis and TLRH surgical approaches were an independent prognostic factor for DFS in IB2-3 cervical cancers. Conclusion: Our findings provided the evidence that TLRH surgical approach was associated with worse DFS in IB2-3 cervical cancers but that of LARVH was not. Such results may have important implications of colpotomy method for MIS in cervical cancer.

Soil salinization driven by groundwater distribution in the reclaimed land is one of the major limiting factors for crop cultivation and soil management. In this study, the effects of two different water table depth (G1 site with intermediate and deep vs. G2 site with shallow depth) of groundwater (on soil salinity, water and crop productivity were investigated. The rainfall event greatly impacted the groundwater recharging rate and salt leaching. The falling groundwatr table depth from 30 to 80 cm together with higher evaporation at G2 site caused soil salinization in the rhizosphere, whereas increases in soil salinity at G1 site was negligible. Overall, soil water content at G2 site was higher than that at G1 site. The positive correlation of soil salinity with soil water content (r² = 0.69) and negative with groundwater table (r² = 0.89) indicated hydraulic connection between groundwater table and soil surface. The deeply falling groudwater table with negative water balance (Evaporation > Rainfall) induced drought-combined salt stress to crop. Soybean was much more susceptible to soil salinty stress and excess-moisture injury than maize, suggesting that the crop selection and practices to reduce crop vulnerability to abiotic stresses(salt and water) should be considered for sustainable and stable agriculture in reclaimed land. Our results highlight that groundwater table depth would play a crucial role in founding cropland suitability and the optimum groundwater table to avoid salinization and excessve wetting might be 1.5 - 2.0 m.

<P><B>Abstract</B></P> <P>Novel monodispersed spherical TiO<SUB>2</SUB> aggregate with a diameter of 100nm (SP100), which is the smallest TiO<SUB>2</SUB> spheres (or beads) reported thus far, has been prepared by a controlled hydrolysis and hydrothermal reaction. The obtained SP100 is a highly porous structure with an internal pore size of 12nm and a surface area of 112m<SUP>2</SUP>/g, and is shown to be an ideal building block of mesoporous TiO<SUB>2</SUB> layer for CH<SUB>3</SUB>NH<SUB>3</SUB>PbI<SUB>3</SUB>–based perovskite solar cells (PSCs). The optimized PSC employing SP100 exhibits photovoltaic conversion efficiency of 18.41% with <I>J</I> <SUB> <I>SC</I> </SUB> of 22.91mA/cm<SUP>2</SUP>, <I>V</I> <SUB> <I>OC</I> </SUB> of 1,049mV and <I>FF</I> of 0.759. Furthermore, fabricated PSCs exhibit reproducible PCE values with little hysteresis in their <I>J–V</I> curves. Time-resolved photoluminescence measurement indicates that the PSC with SP100 (PSC-SP100), compared to the devices with the nanoparticles of 30nm or 50nm size shows notably faster electron injection from CH<SUB>3</SUB>NH<SUB>3</SUB>PbI<SUB>3</SUB> to TiO<SUB>2</SUB> layer, resulting from the extended contact area between perovskite and mesoporous TiO<SUB>2</SUB> layer. <P>Summary</P><P>The roles of intracellular reactive oxygen species (ROS) and related signalling pathways in mycobacterial infection are largely unknown. Here we show that tuberculin purified protein derivative (PPD)/Toll-like receptor (TLR) 2/ROS signalling through activation of apoptosis-regulating signal kinase (ASK) 1 and p47phox pathways is responsible for the induction of proinflammatory responses during tuberculosis (TB) infection. Tuberculin PPD stimulation resulted in rapid activation of mitogen-activated protein kinases (MAPKs) and an early burst of ROS in monocytes/macrophages in a TLR2-dependent manner. PPD-induced ROS production led to robust activation of ASK1 upstream of p38 MAPK, via TLR2. Interestingly, phosphorylation of the cytosolic NADPH oxidase subunit p47phox and ASK1 activation are mutually dependent on PPD/TLR2-mediated signalling. Furthermore, active pulmonary TB patients showed upregulated ROS generation, as well as enhanced activation of ASK1/p38/p47phox pathways in their primary monocytes compared with healthy controls, which suggests a systemic primed status during TB. Taken together, these results indicate that activation of the ASK1/p38 MAPK/p47phox cascade plays a central role in PPD/TLR2-induced ROS generation and suggests the existence of a ‘ROS/ASK1’ inflammatory amplification feedback loop in monocytes/macrophages. The altered regulation of this axis with an increasing free-radical burden may contribute to the immunopathogenesis of human TB.</P>
A Novel Five-Photomask Low-Temperature Polycrystalline Silicon CMOS Structure for AMLCD Application

<P>A novel five-mask low-temperature polycrystalline silicon (LTPS) CMOS structure was verified by manufacturing the thin-film transistor test samples using the proposed five-mask LTPS CMOS process. In integrating the five-mask CMOS structure, a selective contact barrier metal formation process was developed, without additional photomask steps, to solve the issue of high-contact-resistance problem encountered inevitably in the contact between the indium tin oxide and doped polycrystalline silicon (poly-Si) source-drain layers. The five-mask CMOS technology was also confirmed by manufacturing a five-mask CMOS panel for the active-matrix liquid-crystal-display application.</P>

Background/Aims: The role of Helicobacter pylori eradication in patients with functional dyspepsia (FD) is still uncertain. We originally planned a randomized clinical study to observe dyspeptic symptoms after H. pylori eradication therapy. However, we failed to complete the study; therefore, we analyzed the factors that affected the failure of the study. Methods: Interviews and questionnaire surveys were conducted to analyze the factors that induced early termination from the study. Results: Many patients were screened by gastroenterologists at 11 tertiary referral hospitals between July 2009 and August 2010; however, only 4 patients met the enrollment criteria. Most patients who visited our clinics had been experiencing FD symptoms for less than 6 months or were already taking medication. They also demanded to continue taking medications and using other drugs. Only 3 of the 4 patients signed informed consent. Conclusions: The application of the current Rome III criteria to FD is difficult to evaluate in Korean patients with dyspeptic symptoms because of the early medical evaluation. Most Korean patients who were diagnosed with FD by the Rome III criteria did not overcome their fear of being unable to use rescue medications during the study period. (Gut Liver 2011;5:468-471)
Case Report : Delayed Viral Clearance of Chronic Hepatitis C in Patients after Treatment Failure

Hepatitis C virus (HCV) infection usually progresses to chronic hepatitis, with rare cases of spontaneous viral eradication. We present herein four cases involving patients that were initially declared to have failed to respond to treatments, based on the presence of HCV RNA that was still detectable after completion of the standard treatment for chronic hepatitis C with genotype 2. However, the HCV RNA became undetectable, with a delayed response, after discontinuation of therapy. Two of the four patients were diagnosed as treatment failures after extended treatment, and the other two received no further treatment after the standard treatment. All four patients maintained a sustained virological response during the periodic follow-up after delayed viral clearance. (Gut Liver 2011;5:110-114)
