      • 농어촌 지역의 한방의료 이용 실태에 관한 조사 : 전남 신안군 장산면을 중심으로

        The prevalence of disease is affected by cultural factors. Changing cultural and social pattern may modify patterns of disease. Today majority of labor force in rural community is elderly population with a concomitant increase in woman. We investigated prevalence of problems in rural population utilizing oriental medical clinic in Changsan-myon, Shinan-gun, Chunnam. The majority of patients in this area were female and in the 40 to 60s. Muscoskeletal problems had the highest incidence. Low back pain was leading complaints in the muscoskeletal problems and knee pain was the most common complaints in the digestive problems, headache in the neurologic problems. cough in the respiratory problems, hypertension in the circulatory problems, ear problems in the sense organ problems, pruritys in the dermatologic problems, leukorrhea and postmenopausal syndrome in the gynecologic problems, and dysuria and enuresis in the urinary problems. These results suggest that oriental medical physicians have attention to health care and management of the expanding population of elderly and female patients with high incidence of muscoskeietal problems in the rural community.

      • 濟生腎氣丸 煎湯液이 家兎 腎臟機能에 미치는 影響

        Shen(腎) plays an important role in the body fluid regulation in Oriental Medicine. Malfunction of Shen(腎) has been claimed to cause edema. The function of Shen(腎) includes renal function. Body fluid regulation in the kideny is dependent on the renal function, which is influenced by renal, neural, humoral and physical factors. Jesaengsinkeehwan (JSH) has been applied for the treatment of edema and recently of chronic glomerular niphritis, renal failure and hypertension etc. This study was carried out to evaluate the effects of JSH on the renal function and the renin and aldosterone release. Intravenous administration of JSH water extract in the unanesthetized rabbit caused significant increase in urine volume, glomerular filtration rate free water clearance, electrolyte excretion and renal plasma flow. The JSH water extract infusion resulted in significant decrease in plasma renin activity and plasma aldosterone concentration. Mean arterial blood pressure showed significant increase after administration of 0.1㎖/㎏ of JSH water extract, the effect being restored to the original state, but decreased remarkably after administration of 0.4㎖/㎏. These results suggest that JSH has diuretic effect resulted from renal hemodynamic change and reduction of plasma renin activity and plasma aldosterone concentration.

      • 遺尿의 治療에 대한 臨床的 硏究

        Enuresis is relatively common disorder, affecting approximately 20% in 5 year old children. Although the spontaneous cure rate has been reported to be about 15% per year, 1% at the age of fifteen have still bed wetting problems. 50 to 80% of enuresis is reported to be related to physilolgic or psychlolgic cause. Only eneuresis caused by organic disturbance occupies 1 to 205 at most. A variety of therapies have been attempted in most patients including water deprivation in the night, eneuresis alarm clock, and drugs such as imipramine or desmopressin, without success or with side effects. The follow up study was carried out to evalute the efficacy of Kamisamki Tang on eneuresis in 17 patients. They were 9 females and 8 males ranged 4 to 54 years old. The extract of Kamisamki Tang was administered three times a day about 30 minutes after meal. The patients kept water deprivation in the night, urine voiding before sleep, and also did bladder retension exercise and conscious stream interruption exercise. Three patients did not have wetting problems after administration. Good result was appeared in 6 patients and fair in 4. Other 4 patients were not responded. The three with excellent effect were administered for mean 10.0 days, 4 with good effect were for 16.7 days, 4 with fair were for 26.7 days and 4 with poor effect were for 27.5 days. These result suggests that Kamisamki Tang could be applied on eneuresis effectively.

        PWR 발전소에서의 방사선장 특성

        국내 원전의 주종인 가압경수로를 대상으로 스펙트로메트리 기법에 의한 감마, 중성자 그리고 베타 방사선장애 대한 특성조사를 수행하였다. 감마선장의 경우, HPGe 반도체 검출기로 구성된 휴대용 감마 스펙트로메타를 사용하여 측정한 결과, 평균 에너지 값의 분포는 가동정지중일 때 원전의 격납용기내에서 440∼780keV, 가동중일 때 원전의 보조건물내에서 280∼760keV로 나타났다. 가동중인 원전의 격납용기내 operation deck를 중심으로 5개 지점에 대하여 BMSS(Bonner Multiple Spectrometry System)을 사용하여 중성자선장을 조사하였다. BUNKI 코드를 사용하여 측정스펙트럼을 분해하였으며, 측정 장소에서의 평균 에너지 값의 분포는 20∼210keV이었다. 베타피폭원인 오염에 대하여 가동정지 상태인 원전의 격납용기내와 보조건물의 총 14개 지점의 고 방사성 오염지역에서 smearing 시료를 채취하여 표면장벽형 Si 반도체 검출기를 사용하여 에너지 스펙트럼을 측정하였다. 그 결과 부식생성물 60Co에 의한 베타 에너지가 지배적인 것으로 나타났다. Pheton, neutron and beta radiation fields were measurement at PWR plants which are the representative types of nuclear power plant operated in Korea. The photon energy spectra were measurement at locations in the auxiliary building during operation period and in the containment vessel(C/V) during shutdown period using a portable gamma spectrometer with a HPGe detector. The distribution of average energy was found to range from 440 to 780 keV in the C/V and from 280keV to 760keV in the auxiliary building respectively. The average neutron energy measured at the five locations round the operation deck in the C/V in operation using a BMSS(Bonn Multi-Sphere Spectrometer)ranged from 20 keV to 210keV. A computer code, BUNKI was used to unfold the spectrum. The beta energy spectra in the C/V asnd in the auxiliary building in annual outage were determined using 14 smear samples taken from the highly contaminated areas. The analysis showed that representative corrosion product, ??Co made main contribution to the beta energy field.

        八味地黃丸과 六味地黃丸의 效能에 關한 文獻的 考察

        八味地黃丸과 六味地黃丸은 漢代 張仲景의 〈金궤要略〉과 宋代 錢乙의 〈小兒樂證直訣〉에 각각 처음 收錄된 이래 歷代 醫學에 八味地黃丸은 腎陰虛에 사용되어 왔으며 六味地黃丸은 腎陽虛에 사용되는 處方으로 기록되어 왔다. 이에 저자는 腎陽虛 및 腎陰虛에 광범위하게 應用되는 八味地黃丸과 六味地黃丸의 效能, 主治, 現代 臨床活用, 藥理作用에 대해 考察하였다. 그 결과 八味地黃丸 效能은 溫和腎氣, 溫補腎陽하며, 六味地黃丸 效能은 滋腎陰 補肝血 制火로 要約할 수 있었으며, 八味地黃丸은 腎氣不足, 腎陽不足으로 인한 面色蒼白, 頭昏, 耳鳴, 腰酸脚軟, 身半以下常有冷感, 小便不利 或 頻數 脈沈遲 或虛弱, 消渴, 水腫, 久泄, 轉胞, 男子陽위 또는 女子子宮虛寒으로 인한 不姙, 舌淡笞白 等의 諸般 證狀을 治療하는데 사용되어 왔으며, 八味地黃丸은 腎陰不足이나 肝腎不足으로 인한 陰虛發熱을 포함한 骨蒸潮熱 或 手心煩熱, 頭目眩??, 耳??, 咽喉燥痛, 齒牙不固, 腰腿수軟, 遺精, 消渴, 舌紅少苔 或 無苔 脈細, 少便淋閉, 五臟虧損, 自汗溫汗, 諸血 等의 諸般 證狀을 治療하는데 활용되었음을 알 수 있었다. 최근에는 泌尿器系疾患, 高血壓, 糖尿病, 精神疾患, 老人性 眼疾患, 慢性 呼吸器疾患 等에 八味地黃丸과 六味地黃丸이 공통적으로 活用되고 있으며, 性機能 및 生殖器疾患 등에는 八味地黃丸이 活用되며, 甲狀腺機能亢進症, 消花器系, 癌類, 小兒 營養不良 等에는 六味地黃丸이 活用되고 있는 것으로 나타났다. 아울러 八味地黃丸과 六味地黃丸의 藥理作用은 모두 血壓降下, 糖代謝 改善, 肝機能 改善, 腎機能 改善, 혈중 脂質代謝 改善, 免役機能 增强作用이 있으며, 八味地黃丸은 그 외에 Glutathion代謝 影響, 호르몬 影響, 耐寒性 增加, 腦下垂體-副腎皮質機能 改善 等이 있으며, 六味地黃丸은 그 외에 癌誘發 抑制作俑, Interferon 誘導作用 等이 있음을 알 수 있었다. Palmijihwanghan(PJH) and Yukmijihwanghan(YJH) has been widely used as the basic priscriptions for the deficiency of Yin and Yang of kidney. So the study has been carried out to investigate the effects, symptoms, indications and pharmacology of the PJH and YJH. The result of this study were as follows: 1. The effect of PJH is warming and regulation the kidney-Qi and warming and strengthen the kidney-Yang. While the effect of YJH is nourishing the kidney-Yin, nourishing the liver-blood and managing the fire. 2. PJH is used for the treatment of such symptoms caused by dificiency of the kidney-Qi or the kidney-Yang as facial pallidness. dizziness and tinnitus, back pain and leg weakness, cold sensation in lower body, dysuria or urinary frequency, deep and slow or feeble pulse, thirst, edema, persistent diarrhea, painful urination, impotence, dizziness and tinnitus, dry and painful throat, weakness of the teeth, back pain and leg weakness, emmision, thirst, flushed tongue without fur, feeble pulse, dysuria, weakness of the five viscera, spontaneous sweating, perspiration during sleep, and bleeding, which are caused by deficiency of the kidney-Yin or the function of kidney and liver. 3. Both PJH and YJH have been commonly used for treatment of disease of urinary tract disease, hypertension, diabetic mellitus, psychopathy, chronic pulmonary disease. While PJH is applied for the sexual and genital disorders, YJH is for hyperthyrodism, mass in the digestive system and children's malnutrition. 4. The pharmacology of PJH and YJH is commonly the lowering the blood pressure, promoting the glycometabolism, renal function and lipid metabolism, and increasing the immune function. Besides PJH affects on the glutathion metabolism, and hormonal function, increases the cold tolerance ability and improves the function of the pituitary-adrenal cortex, while YJH has the effects of inhibiting the carciogenic action and inducing the interferon secreation.

      • 牧丹皮가 Collagen 誘發 關節炎에 미치는 影響

        류마티스관절염은 활막관절 부위의 만성 염증을 특징으로하며 병이 진행될에 따라 이환된 관절의 연골 및 골과 관절주위 연체조직의 파괴를 초래하는 전신성 자가면역질환이다. 전세계에 걸쳐서 주로 젊은층에서 발병하며 전 인구의 약 2-4%가 이환되는 것으로 알려진 류마티스관절염의 병인에 대하여는 아직도 완전히 규명되지 못하고 있으며,이 질병을 완치 시키거나 질병의 질행을 막을 수 있은 치료법이 정리되지 않은 실정이다. 牧丹皮는 淸熱凉血 活血祛瘀의 효능이 있어 임상적으로 발열,도한,월경불순등 이나 급성충수염 또는 跌打瘀血 등에 응용되어 왔으며 최근에는 진정,최면,진통작용과 항염 및 항알레르기 작용 등이 보고되고 있고 adjuvant 관절염에서 염증반응을 억제하는 것으로 보고되고 있다. 목단피가 콜라겐유발관절염(CIA)에 미치는 영향을 알아보기 위하여 bovine type II collagen을 랫트의 꼬리에 주사하고 목단피를 투여하여 CIA의 유발과정중의 면역학적 변화에 미치는 영향에 대하여 관찰하였다. 그 결과 목단피는 생체내와 생체외에서 담식세포에 의한 탐식능을 증가시켰으며 활막세포에 의한 반응산소중간물질 생성능을 감소시켰다. 또한 목단피는 림프구의 증식을 억제하였고,CD4+세포의 비율을 증가시키고 CD8+세포의 비율을 감소시켰다. 아울러 목단피는 요증 미tric oxide(NO)의 분비를 억제시켰고, 활막세포에서의 NO생성을 약간 증가시키는 효과를 보였으나 고농도의 목단피를 투여할 경우 다시 감소하는 결과를 보였다. 이상의 결과로 볼 때 collagen에 의한 관절염의 진행시 목단피의 투여는 면역반응을 조절하여 ROI와 NO의 생성을 억제하고 탐식능을 증가시켜 외부 항원에 대한 탐식을 효과적으로 하여 관절염의 진행을 완화시킨 것으로 생각되며,따라서 목단피는 류마티스관절염의 치료에 임상적으로 활용할 수 있을 것으로 사료된다. Rheumatoid arthritis(RA) is an autoimmune disorder characterized by a chronic inflammatory, proliferative polyarthritis with joint destruction. The disease occurs throughout the world. Most cases are common in young adult age, and 2-4% of the population are affected with female predominance. However the curative therapy of RA is not yet available, so the development of successful management is required. Moutan Cortex is an herb drug applied for such disease as fever, sweating, menstrual irregularity, acute appendicitis and trauma. Recently it is reported that Moutan Cortex has the antiinflammatory, antimicrobial and antihypertensive actions. It also has analgesic, sedative, and anticonvulsant effects. And it is reported to inhibil swelling of rat's paw and reduce arthritic joint swelling. So as to investigate the effects of Moutan Cortex on collagen-induced arthritis, its water extract was administered in the rat immunized by collagen and the immune responses were measured. The phagocytosis was increased in vivo and in vitro after administration of Moutan Cortex, which was strikingly increased after secondary immunization in vivo. Moutan Cortex decreased lhe reactive oxygen intermediate(ROI) formation and nitric oxide(NO) secretion in urine. While formation of reactive nitrogen intermediate was slightly increased after administration, it showed tendency of decrement in high dose. Moutan Cortex also inhibit lymphocytes proliferation. The ratio of CD4+ T cell was decreased and CD8+ T cell was increased. These results suggest that Moutan Cortex might inhibit arthritic changes by regulating immune response, inhibiting ROI and NO formation and increasing phagocytosis. So Moutan Cortex could be applied for the treatment of RA.

      • 薏苡仁의 投與가 마우스의 細胞性 및 體液性 免疫機能에 미치는 影響

        Coicis Semen is one of the oriental medicine that has been used for the treatment of the disease such as pulmonary abscess, periappendicular abscess and wart since ancient times. However, the mechanism of the action of the drug is not well studied. This study was done to investigate the effects of Coicis Semen on the host defense mechanism. Effects of Coicis Semen on the immune responses were analyzed by measuring the contact gypersensitivity, hemagglutinin, hemolysin and rosette formation, cytotoxicity, and reactive oxygen intermidiates production. As the results, water extract of Coicis Semen administration enhanced the antibodies (hemagglutinin and hemolysin) formation and the appearance of rosette forming cells of the spleen. Also Coicis Semen increased the allogeneic immune response in the mouse, showed cytotoxic activity against human leukemia cell line(K562) and decreased the contact hypersensitivity against dinitroflurobenzene. Also administration of Coicis Semen slightly increased NK cell activity and enhanced the production of such reactive oxygen intermidiates as superoxide and hydrogen peroxide from the macrophages in vivo and in vitro. The above results demonstrate that Coicis Semen has enhancing effects on cellular and humoral immune responses against disease.

      • 枳實에 의한 아나필락시의 抑制效果

        The effect of water extract of Poncirus trifoliata fruit (ATFE) on compound 48/80 induced anaphylaxis was studied in the rat. The following results have obtained. 1. Optimal lethal dose of compound 48/80 was 8ug/gm B.W. 2. PTFE was found to exhibit an inhibitory activity on the anaphylaxis induced by compound 48/80. 3. Administration of PTFE 60 min before or 5 min after had a better result than 10 min after anaphylaxis induced by compound 48/80. 4. PTFE also inhibited significantly the serum histamine release in anaphylaxis induced by compound 48/80 in the rat. 5. Administration of PIFE before 30 min decreased PCA reaction induced by anti DNP-IgE and DNP-HSA. This result indicates that poncirus trifoliata suppresses the anaphylaxis induced by compound 48/80 with decreasing the serum histamine release.

        인천 인현동 호프집 화재 피해자 분석

        Purpose : This study was conducted to develop field triage, transportation, distribution, and prehospital care at a fire disaster by analyzing the victims of the fore that broke out at a bar in Incheon. Method : We analyzed the cases of the victims of a fire in Incheon in Oct. 1999. We determined the primary care hospital, the arrival time, the burn size, the outcome, and the injury type from the medical records, the concerned organ records, and interviews with concerned persons. Result : The total number of victims was 137: 56 prehospital deaths, 1 hospital death, and 80 survivals. The Pearson correlation coefficient between the burn size and the severity was -0.175. There were 121(89.6%) cases of inhalation injury, 59 (43.7%) cases of flame burns, 66 (48.9%) cases of hypoxic brain damage, and 16 (11.9%) cases involving other types of injury. Conclusion : The causes of death of the fire victims were inhalation injury and hypoxic brain damage due to CO poisoning and other toxic inhalants. We propose the use of a simple triage and rapid treatment(START) system and a reassessment the delayed category in fire disasters.
