        Laparoscopic Hepatic Resection Versus Laparoscopic Radiofrequency Ablation for Subcapsular Hepatocellular Carcinomas Smaller Than 3 cm: Analysis of Treatment Outcomes Using Propensity Score Matching

        Objective: To compare the therapeutic outcomes of laparoscopic hepatic resection (LHR) and laparoscopic radiofrequency ablation (LRFA) for single subcapsular hepatocellular carcinoma (HCC). Materials and Methods: We screened 244 consecutive patients who had received either LHR or LRFA between January 2014 and December 2016. The feasibility of LRFA in patients who underwent LHR was retrospectively assessed by two interventional radiologists. Finally, 60 LRFA-feasible patients who had received LHR and 29 patients who had received LRFA as the first treatment for a solitary subcapsular HCC between 1 cm and 3 cm were finally included. We compared the therapeutic outcomes, including local tumor progression (LTP), recurrence-free survival (RFS), and overall survival (OS) between the two groups before and after propensity score (PS) matching. Multivariable Cox proportional hazard regression was also used to evaluate the difference in OS and RFS between the two groups for all 89 patients. Results: PS matching yielded 23 patients in each group. The cumulative LTP and OS rates were not significantly different between the LHR and LRFA groups after PS matching (p = 0.900 and 0.003, respectively). The 5-year LTP rates were 4.6% and 4.4%, respectively, and OS rates were 100% and 90.7%, respectively. The RFS rate was higher in LHR group without statistical significance (p = 0.070), with 5-year rates of 78.3% and 45.3%, respectively. OS was not significantly different between the LHR (reference) and LRFA groups in multivariable analyses, with a hazard ratio (HR) of 1.33 (95% confidence interval, 0.12–1.54) (p = 0.818). RFS was higher in LHR (reference) than in LRFA without statistical significance in multivariable analysis, with an HR of 2.01 (0.87–4.66) (p = 0.102). Conclusion: There was no significant difference in therapeutic outcomes between LHR and LRFA for single subcapsular HCCs measuring 1–3 cm. The difference in RFS should be further evaluated in a larger study.

Pulsed dye laser (PDL) has been suggested to be used adjunctively with oral and topical rosacea regimens for more effective symptom resolution. However, it is not known which treatment method can reduce the recurrence of rosacea effectively. Objectives: The primary objective was to identify whether 595nm-PDL treatment reduced recurrent rate among rosacea patients who were treated with oral minocycline. The secondary objective was to evaluate that the factors such as sex, age, rosacea type, topical agents, and the treatment duration with oral minocycline were associated with the hazard of recurrence of rosacea. Methods: 107 Korean patients with rosacea who started treatment with oral minocycline (100mg per day) with or without 595nm-PDL (total 2-4 sessions) between January 2013 and May 2015 were evaluated retrospectively. The follow-up period was 1 year from the end of the treatment. Results: The recurrence-free survival analysis revealed that the group with oral minocycline plus PDL was significantly different compared with the group with oral minocycline alone. Treatment of oral minocycline for 4 or less weeks without PDL remained a statistically significant risk factor compared with treatment of oral minocycline for more than 4 weeks with PDL. Conclusion: Pulsed dye laser can be used in combination with oral minocycline for more than 4 weeks to reduce relapses of rosacea.

        <노처녀가>에 드러난 우울과 애도 -감정의 치유 과정을 중심으로-

        본 연구는 우울증이 사회적 문제로 대두되는 가운데 이를 야기하는 요소로 상실에 주목하였다. 상실은 고전시가에서 쉽게 찾아볼 수 있는 제재인 바, 감정 진술이 용이한 가사의 특성에 기대어 원인 요소-감정-애도의 관계를 <노처녀가>를 통해 살펴보고자 했다. 작품에 드러난 ‘상실’과 그것을 극복하는 과정을 구체적으로 살펴봄으로써 텍스트 안에서 그려지는 감정 제시 수준에서 나아가 치유문학으로서의 전망을 보여줄 수 있을 것이라고 판단했기 때문이다. 이에 Ⅱ장에서는 혼인이라는 목적에 좌절된 주체가 이를 극복하기 위해 이용하는 기제를 제시하였다. 그 과정에서 상실로 인해 형성된 우울이라는 감정의 양상과 화자의 태도를 함께 다루었다. Ⅲ장에서는 꿈이라는 비현실적인 요소를 현실에서의 상실과 좌절을 극복해내는 환상적 장치로 보고 꿈을 통해 화자가 지닌 욕망이 표출 및 해소되는 과정을 분석하고자 하였다. 그 결과 상실로 인한 혼인 기회의 박탈감이 우울이라는 감정으로 나타났고, 이 감정이 현실과 비현실의 교차를 축으로 해소되고 있음을 밝혔다. The purpose of this study is to describe the relationship of loss-emotion-lamentation in <Nocheonyeoga>, with the focus on loss being one of the causes of depression. The pattern of loss faced by the narrator, and an analysis of the emotions experienced while overcoming it, reveal not only the primary emotions, but also the interaction of emotions. Chapter Ⅱ analyzes the depression caused by the failure to marry, and the multiple mechanisms used by the narrator to overcome it. Chapter Ⅲ clarifies the role of dreams, which are unreal, and considers the possibility of overcoming loss and frustration through fantasy mechanisms. These can be used to examine how the narrator’s desire is realized and expressed. The results reveal that the deprivation of the opportunity to marry, caused by the loss of youth and good health, leads to depressive emotions, which emerge in the axis between reality and unreality.

        일반논문 : 천주가사와 순교의 감정

        이 글은 입교(入敎)가 곧 죽음을 의미함에도 불구하고 천주교 신자로서 살아갈 것을 선택하였던 이들의 마음에 대한 의문으로부터 시작한다. 조선에 천주교가 유입되고 집단적 순교가 일어났던 시기와 맞물려 발생한 천주가사를 분석하며 순교할 수 있었던(혹은 순교하는) 그들의 감정은 무엇이었던 것인지 논의하고자 하였다. 이를 위하여 먼저, 순교자에 의해 제작된 천주가사를 대상 텍스트로 삼아 순교의 열망이 구성되는 지점으로 내세관과 원죄의식에 대해 논구했다. 그리고 3장에서 그것이 공동체 형성에 어떠한 영향을 미쳤는지 살폈으며 특히 기쁜 죽음이라는 관념의 역설에 대해 논의하였다. 더불어 매체로써 천주가사가 감정의 연대와 지속 과정에 준 영향도 보았다. 그 결과 천주가사가 제작되어 유통되면서 그것을 받아들이는 집단(공동체) 에게 매우 강력한 특정 감정을 불러일으켰다는 사실을 알았다. 일련의 과정은, 천주가사를 감정의 형성과 전파라는 측면에서 살핀 것이며, 이를 통해 죽음이라는 실존적 사건이 공동체에 어떠한 의미로 다가오는가에 대한 논의까지 가능하였다. This study began from a question where the mind of those who resolved to live as Catholic nevertheless entering the church(入敎) meant death began. Here, this study intended to discuss what was the emotion that they could face death, that is, could face (or suffered) martyrdom in the persecution. First, this study investigated afterlife view, original sin consciousness, and belief in God as the point of generating passion for martyrdom, by selecting the works of the writers who suffered martyrdom as the target text. Also, explained that the a paradoxical emotion, pleasant death, is possible based on such spirit. Lastly, this research presumed that Catholic-Gasa could transmit special emotion to a group (community) that received it, while it was distributed. As the result, this research could determine that Catholic-Gasa could take a mediating role of formation of a certain emotion. Simultaneously, this research investigated that the community was maintained through that emotion. That is, this research intended to investigate the emotion to reach martyrdom by researching Catholic-Gasa. As a series of process, it would be also possible to discuss the significance of death (emotion) in a community.

      • The Extract of Herbal Medicines Activates AMP-Activated Protein Kinase in Diet-Induced Obese Rats

        <P>Our study investigated whether the extract of six herbal medicines (OB-1) has an inhibitory effect on obesity. High-fat diet-(HFD-) induced rats and controls were treated with 40 mg/100 g body weight of OB-1 or saline once a day for 5 weeks. After significant changes in body weight were induced, OB-1 and saline were administered to each subgroup of HFD and control groups for additional 5 weeks. No statistically significant decrease of body weight in OB-1-treated rats was found compared to controls. However, OB-1-treated rats were found to be more active in an open-field test and have a reduction in the size of adipocytes compared to controls. We observed no changes in the mRNA expressions of leptin and adiponectin from adipocytes between OB-1- and saline-treated rats with HFD-induced obesity group. However, OB-1 treatments were shown to be inversely correlated with accumulation of lipid droplets in liver tissue, suggesting that OB-1 could inhibit a lipid accumulation by blocking the pathway related to lipid metabolism. Moreover, the phosphorylation of AMP-activated protein kinase (AMPK) was significantly increased in OB-1-treated rats with HFD compared to controls. These results suggest that OB-1 has no direct antiobesity effect and, however, could be a regulator of cellular metabolism.</P>

        송순의 <면앙정가>에 나타난 장소성과 그 의미

        이 글은 장소성에 초점을 맞춘 대개의 작품론이 특정 장소가 지니는 일반적인 의미나 의의들을 부여하는 데 그치고 있다는 점에서, 장소와 장소의 접점 혹은 장소 안의 장소(대상)가 개별적으로 논의되어야 하는가에 대한 의문으로부터 출발했다. 따라서 이 글은 송순의 <면앙정가>를 대상으로 장소성이 실제 텍스트 속에서 어떤 방식으로 조직되고 있는지 밝히고자 했다. 먼저 송순에게 자연은 이상향, 즉 코스모스임을 밝혔다. 이를 바탕으로 <면앙정가>에 나타난 토포필리아에 초점을 맞추어 작품을 분석하였다. 이 과정에서 장소성을 드러내기위하여 구체적 지명과 장소를 구상(具象)화 시키는 표현들을 집중적으로 검토했다. 더불어 장소를 지속시키려는 바람이 순환적 시간관념을 통하여 이루어졌다는 점 역시 살폈다. 마지막으로 담양과 면앙정이라는 장소의 유기적인 관계망과, 작품과 장소가 형성하는 심미적 자질과 그 관계망에 대하여 논의했다. This research began from a curiosity about whether the contact point between the places, or place (object) within the place should be individually discussed, considering that the discussions that focused on placeness merely gave general meaning or significances that a certain place has. Here, this research aimed at investigating how the placeness is organized in the actual text by selecting Song Soon``s <Myeonangjeongga> as a research object. First, this research determined that Song Soon``s nature is the ideal, that is, cosmos. Based on this, the researcher analyzed the work by focusing on topophilia that is felt in <Myeonangjeongga>. In this process, this research examined the expressions that concretized(具象) concrete name of region and place in order to reveal placeness. Also, investigated the point that the wish to maintain the place was realized through a circular time conception. Lastly, this research discussed organic relational net of the place, Damyang and Myeonangjeong, and esthetic talent and relational net that the work and the place formed.

        천주가사 <사향가>의 감성적 설득기법

        천주교가 학문이 아닌 신앙으로서 전파되었을 무렵, 천주가사는 암송해야할 교리의 단순한 나열 형태일 수는 없으며 읽는 이의 마음을 움직이는 요소들을 반드시 포함할 수밖에 없었다. 바로 이러한 조건이 가사 장르 자체의 진술적 특징과 결부된다고 할 수 있다. 즉 설득기법의 다양한 요소들 중에서도 특히 파토스(pathos)의 요소에 집중함으로써 신자들의 마음을 사로잡을 필요가 있었고, 이러한 목적을 실현하기 위해서는 여타의 장르보다 가사 장르를 채택하는 편이 더 용이했을 것이다. 본고는 이러한 점에 착안하여 천주가사인 <사향가>를 설득의 기법이라는 측면에 비추어 분석해 보고자 한다. 곧 텍스트 내의 감정들이 어떠한 작가적 의도와 더불어 발현되었으며, 혹은 이것이 독자들에게 전해졌을 때에 어떻게 수용되었는지를 논구하고자 하는 것이다. 이를 위해 작품이 생산되었던 배경을 감안해서 작품 속에 발현된 감정들의 양상에 대한 분류 기준을 `칠정`에 두었다. <사향가>는 설득의 원초적 요소인 신뢰를 형성하기 위해 신부를 작자로 내세웠으며 전연령의 신자 혹은 예비신자라는 확실한 독자(상대)를 설득의 대상으로 삼았다. 그리고 작품 곳곳에 `본향`이라는 단어를 노출시킴으로써 독자들은 모두 같은 본향을 공유하는 형제이자 자매라는 점을 강조한다. 이러한 과정을 통해 <사향가>를 읽는 이들은 가사 작품을 읽는 개인이 아니라 천주교 신도라는 집단 구성원으로 탈바꿈된다. 한편, <사향가>는 다양한 감정선이 교차 구성된, 다분히 의도적으로 재구성된 감성 내러티브를 구축하고 있다. 또한 이러한 극적 구성에는 당시의 어지러웠던 사회상이 적극적으로 반영되고 있다. 그래서 작품의 감정 발현 양상이 작자의 일방적인 표현이 아니라 청자의 처지가 고려된 것으로써 신자들을 결집시키는 데 용이하게 작동하고 있음을 포착했다. 천주교를 배척하는 이들을 분노의 대상으로 삼아 공공의 적으로 돌려 연대감을 가지게 하는 결과를 가져온 것이다. 더불어 내세에 대한 약속된 행복(천당)은 신자들을 궁극적으로 독실한 신자로 이끌었고, 박해세력에 대항해서 그들을 연대하고 저항하게 만들었다. When Catholicism was spread not as study, but as faith, Catholic Gasa had no choice but to include elements moving readers` mind, instead of a simple form of listing doctrines to be recited. This condition might be connected to the testimonial characteristics of the whole Gasa genre. In other words, it would be necessary to capture believers` heart by focusing on elements of pathos among diverse elements of persuasive technique, and in order to realize this purpose, it would be easier to choose the Gasa genre than other genres. Thus, this study aims to analyze a Catholic Gasa < Sahyang-ga > in the aspect of persuasive technique. In other words, it aims to discuss the author`s intention that was expressed with feelings within texts, or how it was accepted by readers. Considering the background of the work creation, the aspect of feelings expressed in the work was classified based on `the seven passions`. Putting up a priest as the author to form trust which is the basic element of persuasion, < Sahyang-ga > targeted definite readers(opponents) such as believers or future believers of all ages for persuasion. Also, frequently exposing the word, `native home`, it emphasizes that all the readers are brothers and sisters sharing the same native home. Through this process, readers of < Sahyang-ga > are changed to group members of Catholic believers, instead of individuals reading the Gasa. Meanwhile, < Sahyang-ga > has established the emotional narrative mingled with diverse feelings, which was quite intentionally reconstructed. Also, the disorderly social aspect of the time was actively reflected to this dramatic composition. Thus, the aspect of expressing feelings of the work was used for uniting believers, by considering listeners` circumstances, instead of the author`s unilateral expression. It created the solidarity by targeting people excluding Catholicism as a public enemy for anger. Moreover, the promised happiness(heaven) in the afterlife eventually changed believers to devout believers, and they were united to resist persecutors.
