Background: After the Fukushima Daiichi Nuclear Power Station (FDNPS) accident, a model was developed to estimate the external exposure doses for residents who were expected to return to their homes after evacuation orders were lifted. However, the model’s accuracy and uncertainties in parameters used to estimate external doses have not been evaluated. Materials and Methods: The model estimates effective doses based on the integrated ambient dose equivalent (H*(10)) and life patterns, considering a dose reduction factor to estimate the indoor H*(10) and a conversion factor from H*(10) to the effective dose. Because personal dose equivalent (Hp(10)) has been reported to agree well with the effective dose after the FDNPS accident, this study validates the model’s accuracy by comparing the estimated effective doses with Hp(10). The Hp(10) and life pattern data were collected for 36 adult participants who lived or worked near the FDNPS in 2019. Results and Discussion: The estimated effective doses correlated significantly with Hp(10); however, the estimated effective doses were lower than Hp(10) for indoor sites. A comparison with the measured indoor H*(10) showed that the estimated indoor H*(10) was not underestimated. However, the Hp(10) to H*(10) ratio indoors, which corresponds to the practical conversion factor from H*(10) to the effective dose, was significantly larger than the same ratio outdoors, meaning that the conversion factor of 0.6 is not appropriate for indoors due to the changes in irradiation geometry and gamma spectra. This could have led to a lower effective dose than Hp(10). Conclusion: The estimated effective doses correlated significantly with Hp(10), demonstrating the model’s applicability for effective dose estimation. However, the lower value of the effective dose indoors could be because the conversion factor did not reflect the actual environment.

This study focused on the measurement of the actual charging state of ambient aerosol particles, which is important for understanding the intricate process of adverse health effects caused by particulate matter (PM). The net electrostatic charging state of PM2.5 collected on filter media was measured in this study. The Faraday cage method and surface potential measurements were used in this study. The results showed that the polarities of the net charging state measured using these two methods were in agreement for 42 out of 48 samples (87.5%), and 36 samples (75%) were negatively charged. The filters were not significantly charged by friction between the filters and air not containing PM. Charge addition to or leakage from the filters was not observed over a two-month storage period. Net charging state of PM2.5 collected on the filters was concluded to be negative in most cases, based on data’s support of the assumption that aerosol charging state is not altered by the process of PM collection using filter.

Background/Aims The association between clinical symptoms, gastric emptying, quality of life and sleep disorders in distinct functional dyspepsia (FD) patients has not been studied yet in detail. Methods We enrolled 79 FD patients (postprandial distress syndrome [PDS], n = 65; epigastric pain syndrome [EPS], n = 47; EPS-PDS overlap, n = 33) and 44 healthy volunteers. Gastric motility was evaluated. We used Rome III criteria to evaluate clinical symptoms and State-Trait Anxiety Inventory (STAI) scores to determine anxiety status. Sleep disorder was evaluated using the Pittsburgh Sleep Quality Index scores. Results There were no significant differences in age, sex and Helicobacter pylori positivity between FD subtypes and healthy volunteers. The scores of Glasgow dyspepsia severity scores (GDSS), SF-8 and Pittsburgh Sleep Quality Index (PSQI) in distinct subtypes of FD patients were significantly different from those in healthy volunteers. However, there were not significant differences in these scores, Tmax and T1/2 among 3 subtypes of FD patients. PSQI score was significantly (P = 0.027, P = 0.002 and P = 0.039, respectively) associated with GDSS among EPS, PDS and EPS-PDS overlap patients. In addition, 8-item short form health survey (SF-8; Physical Component Score and Mental Component Score) was significantly associated with global PSQI score in PDS and EPS-PDS overlap patients. In contrast, SF-8 (Mental Component Score) only was significantly linked to global PSQI score in EPS patients. Conclusions Prevalences for sleep disorders, gastric motility and quality of life in 3 subtypes of FD patients were similar levels. In PDS and EPS-PDS overlap patients, SF-8 was significantly associated with global PSQI score. (J Neurogastroenterol Motil 2014;20:104-112)

Purpose: Deciding on treatment carcinoma of the tongue when the tumor has a thickness of 1.5 cm or more is difficult. Surgery often requires wide resection and re-construction, leading to considerable functional impairment. A cesium implant is an attractive option, but according to the Manchester System, a two plane implant is needed. Materials and Methods: According to the textbook, a tumor is sandwiched between the needles, which are implanted at the edge of the tumor. This may cause an unnecessarily high dose to the outer surface of the tongue,which sometimes leads to a persistent ulcer. To avoid this complication, we invented a modified implantation method, and applied the method to five consecutive patients. Results: With a minimum follow-up of 2 years, all primary tumors in 5consecutive patients have been controlled. No complications occurred in soft tissue of the tongue or in the mandible. Conclusion: Our modified Manchester System was feasible and effective for tumors that has a thickness of 1.5 cm or more.

Massive environmental monitoring has been conducted continuously since the Fukushima Daiichi Nuclear Power accident in March of 2011 by different monitoring methods that have different features together with migration studies of radiocesium in diverse environments. These results have clarified the characteristics of radiological environments and their temporal change around the Fukushima site. At three months after the accident, multiple radionuclides including radiostrontium and plutonium were detected in many locations; and it was confirmed that radiocesium was most important from the viewpoint of long-term exposure. Radiation levels around the Fukushima site have decreased greatly over time. The decreasing trend was found to change variously according to local conditions. The air dose rates in environments related to human living have decreased faster than expected from radioactive decay by a factor of 2-3 on average; those in pure forest have decreased more closely to physical decay. The main causes of air dose rate reduction were judged to be radioactive decay, movement of radiocesium in vertical and horizontal directions, and decontamination. Land-use categories and human activities have significantly affected the reduction tendency. Difference in the air dose rate reduction trends can be explained qualitatively according to the knowledge obtained in radiocesium migration studies; whereas, the quantitative explanation for individual sites is an important future challenge. The ecological half-lives of air dose rates have been evaluated by several researchers, and a short-term half-life within 1 year was commonly observed in the studies. An empirical model for predicting air dose rate distribution was developed based on statistical analysis of an extensive car-borne survey dataset, which enabled the prediction with confidence intervals. Different types of contamination maps were integrated to better quantify the spatial data. The obtained data were used for extended studies such as for identifying the main reactor that caused the contamination of arbitrary regions and developing standard procedures for environmental measurement and sampling. Annual external exposure doses for residents who intended to return to their homes were estimated as within a few millisieverts. Different forms of environmental data and knowledge have been provided for wide spectrum of people. Diverse aspects of lessons learned from the Fukushima accident, including practical ones, must be passed on to future generations.

Background/Aims There are no available data about the relationship between ghrelin gene genotypes and early phase of gastric emptying in functional dyspepsia (FD) as defined by Rome III classification. Methods We enrolled 74 patients presenting with typical symptoms of FD and 64 healthy volunteers. Gastric motility was evaluated using the 13C-acetate breath test. We used Rome III criteria to evaluate upper abdominal symptoms and self-rating questionnaires for depression (SRQ-D) scores to determine status of depression. The Arg51Gln (346G>A), preproghrelin (3056T>C), Leu72Met (408C>A), Gln90Leu (3412T>A) and G-protein 3 (825C>T) polymorphisms were analyzed in the DNA from blood samples of enrolled subjects. Genotyping was performed by polymerase chain reaction. Results There was a significant relationship between the Gln90Leu3412 genotype and SRQ-D score in FD patients (P = 0.009). Area under the curve at 15 minutes (AUC15) value was significantly associated with the Leu72Met408 genotype (P = 0.015) but not with entire gastric emptying. Conclusions The Leu72Met (408C>A) single nucleotide polymorphism was significantly associated with early phase of gastric emptying in FD patients. Further studies will be necessary to clarify the association between ghrelin gene single nucleotide polymorphisms and early phase of gastric emptying in FD patients.
