Background: Sugammadex rapidly reverses deep neuromuscular blockade, but owing to cost, questions remain about its optimal utilization. After the unrestricted introduction of sugammadex at Emory University Hospital, we hypothesized that reductions would be demonstrated in the primary outcome of post-anesthesia care unit (PACU) mechanical ventilation (MV) and secondary outcomes of PACU length of stay (LOS) and emergence time (surgery end to anesthesia end time in the PACU). Methods: This retrospective observational study included patients undergoing general anesthesia over a 12-month period. Using multiple variable penalized logistic regression in a one-group before-and-after design, we compared the categorized rates of PACU MV to examine the effect of sugammadex introduction following a post-hoc chart review to ascertain the reason for postoperative MV. Additionally, multiple variable linear regression was used to assess for differences in PACU LOS and emergence time within a propensity-matched set of patients receiving neostigmine or sugammadex. Results: In total, 7,217 surgical cases met the inclusion criteria: 3,798 before and 3,419 after sugammadex introduction. The incidence of PACU MV was 2.3% before and 1.8% after (P = 0.118) sugammadex introduction. PACU MV due to residual neuromuscular blockade (rNMB) decreased from 0.63% to 0.20% (P = 0.005). Ventilation because of other causes was unchanged. PACU LOS and emergence time were unchanged in the propensity-matched set of 1,444 patients. Conclusions: rNMB was an important contributor to PACU MV utilization and its incidence significantly decreased after sugammadex introduction. The selected efficiency measures may not have been sufficiently granular to identify improvements following introduction.

This article probes the relative importance of alignment amongst key stakeholders as one factor in explaining differences in On-The-Field and Off-The-Field performance of professional sporting clubs. Informed by a survey of 130 professional team sporting executives, and evidence from several related sources of evidence on strong and weak alignment, this research highlights the important role of different stakeholders in club performance and assessments of that performance. Evidence of misalignment is presented in an analysis of Coach and GM exits for clubs with very different levels of alignment between fans of clubs and the owners, coaches, and players of those clubs. The results of this article highlight the important role that the Owner takes in both enhancing or destroying the achievement of On-The-Field and Off-The-Field objectives. The crucial role of General Managers in sporting clubs is also showcased in the findings. The General Manager function has been given limited attention in research on sports management and in many sports management programs.

Saxifraga hirculus is classified as a vulnerable plant species in Europe, and it is confined to base-rich flushes in the British uplands. However, a lack of available information about its conservation status hampers the development of adaptive strategies for its in-situ conservation, especially with respect to grazing. To assess the effectiveness of sheep grazing in maintaining viable populations of S. hirculus, we compared the community dynamics of the vegetation in a base-rich flush over 44 years in two plots: one sheep-grazed under business-as-usual sheep grazing densities and the other fenced to exclude grazing. The plots were established in 1972, and the abundances of all vascular plants, bryophytes, and litter were measured at six intervals until 2016. Our results showed that although the presence of S. hirculus was maintained in both plots over the 44 years, it declined and reached a minimum between 1995 and 2010, when it was close to extinction. Since 2013, Saxifraga has recovered only slightly. Interestingly, the S. hirculus response appeared to be independent of grazing treatment, but it mirrored wider changes in the vegetation composition and structure within the flush over the 44 years. These changes are similar to others reported in broader uplands that have been attributed to a combination of reduced nitrogen and sulfur deposition and global warming. Thus, the simple adaptive management approach of "just managing" sheep grazing appeared ineffectual for preserving the S. hirculus population. S. hirculus showed signs of recovery at the end of the study period within this base-rich flush.

The purpose of this research is to introduce and demonstrate the use of an adaptation of the established focus group research method, referred to as the sequential-funnel-based focus group design. This adaptation is described, developed, explained, and tested with an intention of adding to the rigor of the method's application and, by extension, rebranding focus group as a sound method of inquiry. To illustrate the method, the work used the topic of the value of social media as a rela- tionship marketing medium through a conduct of eight focus groups of a three-phase design over a two-month period. An adaptation with increased rigor is proposed as a sound method of inquiry for sport management scholars. The value and chal- lenges of the design is discussed, and an outline for its future use as a method in sport management is presented.

Induced pluripotent stem cells (iPSCs) technology is a method for generating pluripotent stem cells in vitro from fully differentiated cells such as fibroblast cells. The potential applications of iPSC technology in cell therapy and disease modeling could influence current medical practices. Despite current advances in iPSC technology, many patient-derived reprogrammed cells are not suitable for clinical trial because most protocols rely on virus-based techniques, which pose the risk of integration of the viral genome into the chromosomes. Therefore, non-viral methods such as mRNA and protein-based reprogramming are promising alternatives when generating clinically safe iPSCs. In a previous study, we generated human iPSCs using cell extracts with cell penetration peptide (CPP) for the delivery of reprogramming proteins [Kim et al. Cell Stem Cells, 2009]. In here, we show that the expression of reprogramming factors in mammalian cells and subsequent purification of these factors by FLAG-Tag could reprogram fibroblasts into iPSCs.

Backgrounds/Aims: Associating Liver Partition and Portal Vein Ligation for Staged Hepatectomy (ALPPS) has generated controversy due to high morbidity and mortality. We present our series of patients with 30-40% parenchymal transection and minimal hilar dissection. Methods: Patients who had partial ALPPS between April 2015 and April 2016 were included. Patients with colorectal liver metastases (CRLM) had their future liver remnants (FLR) cleared with metastasectomies. The liver was divided along the future line of transection to 30-40%, right portal vein was stapled and divided without extensive hilar dissection, with minimal handling of right liver, which was not mobilised. We preserved the middle hepatic vein. Data were collected prospectively for hypertrophy of the FLR, morbidity and mortality. Results: Among the 8 patients (age 25-68) investigated, one patient with cholangiocarcinoma had portal vein embolization prior to partial ALPPS. All patients completed two stages with adequate FLR hypertrophy at a median of 28 days. No mortality was found. The median length of stay after stages 1 and 2 was 9 and 9.6 days, respectively. The median increase in FLR was 38%. Conclusions: A limited transection of 30-40%, minimal hilar dissection and longer wait between stages yielded adequate FLR hypertrophy with low morbidity and no mortality.
DETERMINANTS AND CONSEQUENCES OF CONSUMERS’ ATTITUDES TOWARD ONLINE PRODUCT REVIEWS

In today’s Internet society, consumers value the opinion of other consumers, and regularly seek feedback on products and services prior to making purchase decisions. This consumer-to-consumer communication (C2C) commonly occurs online, and is referred to as electronic word-of-mouth (eWOM). One popular source of eWOM is online product reviews. The current research investigates the determinants of consumers’ attitudes toward online product reviews. Results indicate that perceived value and credibility of online product reviews positively impact consumer attitudes towards online product reviews. More specifically, consumers’ attitudes toward online product reviews positively influence their reliance on said reviews prior to product purchase. Additionally consumers’ attitudes toward online product reviews are positively determined by their perceptions of the credibility of online product reviews and the value of those reviews. However, the perceived value of online product reviews have a greater impact on online review attitudes than does perceived credibility of online product reviews. Further, men and women exhibit differences in their attitudes and reliance regarding online product reviews.
