Transverse Fracture and Dislocation at the Sacrum

We present a rare case of fracture-dislocation at the level of the first sacral (S1) and second sacral (S2) vertebrae. The S1 was displaced forward into the pelvic cavity and was located just in front of the S2. Because the patient also had extensive neurological injury to the lumbar plexus and instability of the pelvic ring, surgery to stabilize the pelvis and to decompress the lumbar plexus was performed. The surgery was successful and the patient experienced marked improvement in neurological function.
Intraventricular and Subarachnoid Fat after Spinal Injury

The authors report an extremely rare case with intraventricular and subarachnoid fat developed after trauma to spine and soft tissue in a 54-year-old male. The initial computed tomography (CT) showed multiple low attenuation lesions, which were thought to be pneumocephalus. Cerebral magnetic resonance imaging (MRI) showed lesions with high signal intensity on T1-weighted magnetic resonance images and high signal intensity on T2-weighted images, indicating fat globules within the CSF. In this report, the clinical presentation, radiological findings, and a review of the literature are presented.

A case of hemangioblastoma of the left lateral ventricle is presented. Only six other cases of the intraventricular hemangioblastoma have been reported. The literature on supratentorial intraventricular hemangioblastoma is reviewed.

From May, 1989 to December, 1991, 25 patients were diagnosed as acromegaly and treated by surgical and/or radiosurgical procedure. The most common presenting symptom is acromegalic feature(60%), followed by, in order, headache, cranial nerve palsy, dyspnea, general weakness and anemia. Preoperative endocrineological evaluation included plasma level of growth hormone, somatomedin-C. High resolution computed tomography(CT) and/or magnetic resonance imaging(MRI) was done as imaging studies. Preoperative tumor size was above 10㎜ in all cases. The level of hormone was not correlated with extension, site and size of tumors. Of the 25 patients, 14 underwent TSA(2 cases followed by radiation therapy), 4 underwent cranotomy(3 cases followed by radiation therapy), 3 underwent craniotomy and TSA(all 3 cases followed by radiation therapy), 2 underwent craniotomy and Gamma Knife Radiosurgery and 2 underwent Gamma Knife Radiosurgery only. There was no operative mortality. Postoperatively, 11 patients of 23 patients(48%) were normalized in growth hormon level (GH<5ng/㎖) and tumor was disappeared in 8 patients of 21 patients(38%) in follow up MRI or high resolution CT studies. Correlation between GH and somatomedin-C was present. However, GH may be a better indicator in predicting postoperative result than somatomedin-C.

Objective : Two-dimensional fluoroscopy-based computerized navigation for the placement of pedicle screws offers the advantage of using stored patient-specific imaging data in providing real-time guidance during screw placement. The study aimed to describe the accuracy and reliability of a fluoroscopy-based navigation system for pedicle screw insertion. Methods : A total of 477 pedicle screws were inserted in the lower back of 96 consecutive patients between October 2007 and June 2012 using fluoroscopy-based computer-assisted surgery. The accuracy of screw placement was evaluated using a sophisticated computed tomography protocol. Results : Of the 477 pedicle screws, 461 (96.7%) were judged to be inserted correctly. Frank screw misplacement [16 screws (3.3%)] was observed in 15 patients. Of these, 8 were classified as minimally misplaced (${\leq}2mm$); 3, as moderately misplaced (2.1-4 mm); and 5, as severely misplaced (>4 mm). No complications, including nerve root injury, cerebrospinal fluid leakage, or internal organ injury, were observed in any of the patients. Conclusion : The accuracy of pedicle screw placement using a fluoroscopy-based computer navigation system was observed to be superior to that obtained with conventional techniques.

We report a 35-year old female patient with history of seizure and mass which was confirmed as a plasma cell granuloma, arising in the left parietal area. The mass appeared on magnetic resonance imaging as well circumscribed area of decreased signal that markedly enhanced with administration of the contrast. Pathologically, biopsy showed a mixed cellular population with considerable numbers of plasma cells along with eosinophils and lymphocytes and the tumors was characterized immunohistochemically by polyclonal population of lymphoid cells.

The bone formation accompanied with other diseases in brain has been rarely reported. Furthermore, it has not been reported without any specific disease. We report a case of a 27 year old female who was referred to our hospital because of the incidentally found calcified lesion in plain X-ray of the skull. The CT and MRI of the brain showed a calcification with minimal enhancement at left parietal area. The calcified lesion was removed and biopsy was performed with stereotactic guided craniotomy. Pathologically, the lesion was confirmed as the membranous bone which was composed of bony trabeculations with osteocytes and the biopsy from adjacent area to the bone revealed a gliosis without any other disease.

Multiple intracranial tumors of different cell types are rare. We report a case of double primary intracranial tumors of different histologic types : oligodendroglioma, and pituitary adenoma. Von Recklinghausen's disease or other etiologic factors supposedly associated with multiple brain tumors were not recognized in this 36-year-old male patient.
