      상아질 표면처리가 글라스 아이오노머 시멘트의 변연누출에 미치는 영향에 관한 연구

      The purpose of this study was to evaluate the effect of the dentin pretreatment on the marginal leakage of a glassionomer cement. 1n this study, 60 molars with sound and healthy crown portion were used. The dentin surface of these teeth were exposed and polished with 600 grit silicon carbide paper. Square - shaped cavities were prepared on the flattened dentin surfaces and these were divided into 4 groups according to the dentin pretreatment procedures. Group I : Dentin pretreatment with distilled water as a control group. Group II : Dentin pretreatment with 5% sodium hypochlorite solution. Group III : Dentin pretreatment with Ketac conditioner. Group IV : Dentin pretreatment with 40% polyacrylic acid. The degrees of dye penetration in the cavity walls were assessed using a stereoscope at ${\times}40$ magnification according to the maximum dye penetration. The results were analyzed by using Mann - Whitney U test. The results were as follows : 1. All groups showed varying depth of dye penetration. 2. Distilled water group showed the most severe marginal leakage when compared with the other groups(P<0.05). 3. 40% polyacrylic acid group showed the least amount of marginal leakage compared with the other groups (P<0.05). 4. There were significant differences between Goup I(distilled water) and Group IV (40% polyacrylic acid)(P<0.05), but there were no significant differences among Group I(distilled water), Group II(sodium hypochlorite), Group III(Ketac conditioner) (P>0.05).

    • 조사면내 공동의 존재에 따른 선량분포의 변화측정

      When high energy photon beam is incident upon an air cavity interface the effect of ionization build-up observed . This phenomenon is resulting from the surface layers of the lesions are significant deficiency of electrons reaching the layers because of the replacement, of solid scattering material by the air cavity, that is lack of electronic equilibrium. Measurement have been made in an acrylic phantom with a parallel plate chamber and high energy photon beams, CO-60, 4MV, 6MV and 10MV X-rays have been investigated. The result of our study show that a significant effect was measured and was determined to be very dependent on field size, air cavity dimension and photon energy. The reductions were much larger for 10MV beam, underdosage at the interface was 12, 12.2, 16.9 and $20.6\%$ for the CO-60, 4MV, 6MV and 10MV, respectively. It was found that this non-equilibrium effect at the interface is more severe for the higher energy beams than that of lower energy beams and the larger cavity dimensions the larger beam reductions occur. This problem is of clinical concern when lesions such as carcinoma beyond air cavities are irradiated, such as larynx, glottic and the patients with maxillectomy and ethmoidectomy and so forth.

    • EPID의 적정관리 도구로서의 유용성에 관한 연구

      Purpose : The aim of this study is to conform the possibility of the liquid type EPID as a QC tools to clinical indication and of replacement of the film dosimetry. Aditional aim is to describe a procedure for the use of a EPID as a physics calibration tool in the measurements of radiation beam parameters which are typically carried out with film. Method & Materials : In this study we used the Clinac 2100c/d with EPID. This system contains 65536 liquid-filled ion chambers arranged in a $256{\times}256$ matrix and the imaging area is $32.5{\times}32.5cm$ with liquid layer thickness of 1mm. The EPID was tested for different field sizes under typical clinical conditions and pixel values were calibrated against dose by producing images using various thickness of lead attenuators(lead step wedge) using 6 & 10MV x-ray. We placed various thickness of lead on the table of linear accelerator and set the portal vision an SDD of 100cm. To acquire portal image we change the field size and energy, and we recorded the average pixel value in a $3{\times}3$ pixel region of interest(ROI) at field center was recorded. The pixel values were also measured for different field sizes in order to evaluate the dependence of pixel value on x-ray energy spectrum and various scatter components. Result : The EPID, as a whole, was useful as a QA tool and dosimetry device. In mechanical check, cross-hair centering was well matched and the error was less than ?2mm and light/radiation field coincidence was less than 1mm also. In portal dosimetry the wider the field size the the higher the pixel value and as the lead thickness increase, the pixel value was exponentially decreased. Conclusions : The EPID was very suitable for QA tools and it can be used to measure exit dose during patients treatment with reasonable accuracy. But when indicate the EPID to clincal study deep consideration required

    • 소조사면 전량분포의 특성에 관한 고찰

      Purpose : The aim of this study is to investigate the properties of small field size and to measure the penumbra and central axis depth dose varying to the jaw setting and off axis distance for indicate this data to small field sizes radiation therapy. Material and methods : The percentage depth dose, beam profile and central axis output dose was measured by farmer type ion chamber and pinpoint chamber using Primart linac with 6MV energy. Beam quality and penumbra variations according to the central axis shift, from center to every 2cm outside increment, and field size, from $1{\times}1cm$ to $10{\times}10cm$ was investigated and compared with that of the standard geometrical condition's results Results : The differences of measured values between two ion chamber was about $37\%$ at 10cm depth with $1{\times}1cm$ field sizes but as field size increased this differences was diminished gradually. Measured data from various off axis distance with the different asymmetric collimations are not changed significantly but as size decreased the dose variation was increased and at $1{\times}1cm$ field size dose difference among off axis distance was as much as $13\%$, and as shallower the measured depth the central axis dose variations among the OAD was increased, penumbra was not changed noticeably depending on off axis distance but the percentage of penumbra from its initial field sizes was strongly dependant on field sizes and penumbra occupation rates of its own field sizes ranging from $6\%$ at $10{\times}10cm$ to $50\%$ at $1{\times}1cm$ field size. Conclusion : For imrt treatment, there are several numbers of different gentry angles with beams of nonuniform fluences are required and several complex factors involved. Among them the characteristics of beam output varying to the geometrical setting and design of collimators are of important to attaining a good treatment results. As mentioned in results the differences of measured values are changed significantly depends on ion chamber volume, depths and field size. For providing quality radiation treatment, especially at small field size, those factor's should have considering deliberately.

      메가볼트 에너지 전산화 단층 촬영을 이용한 치료계획의 유용성 연구

      고밀도 물질의 존재에 따른 킬로볼트 및 메가볼트 에너지 전산화 단층촬영(kilovoltage & megavoltage- computed tomography, KVCT & MVCT) 영상의 아티팩트 차이를 비교하기 위하여 Cheese 팬텀을 사용하여 KVCT와 MVCT로 얻은 영상자료를 통해 밀도변화에 따른 HU값의 변화를 비교하였다. 또한 각 영상의 sinogram 자료를 치료계획 장비에 입력 후 시행하여 치료선량에 변화 여부와 조사면내 계산값과 실측값간의 차이를 r값으로 비교분석하였으며 이에 대한 실제 환자에 적용하여 임상적용에 관한 유용성을 검정하였다. KVCT와 MVCT간에 HU값 차이는 KVCT의 밀도 3.0에서 역치를 보여 변화가 없었으나 MVCT에서는 밀도 5.0 이상도 구별하는 것을 관찰할 수 있었다. 각 방법의 Sinogram 정보를 통해 일반팬텀으로 계산한 결과 r값이 허용오차인 1보다 낮은 비율은 KVCT와 MVCT에서 각각 94.92%, 93.87%로 큰 차이를 보이지 않았으나 고밀도팬텀을 이용해 아티팩트가 존재하는 자료를 이용한 선량계산의 결과는 KVCT와 MVCT에서 각각 88.25%와 93.77%로 다소 차이를 보였다. MVCT 이용 시에는 아티팩트가 거의 나타나지 않았고 고밀도 물질의 윤곽을 정확히 알 수 있었으며 상대적으로 선량계산의 정확성이 향상되어 척추궁 절제술 후 인공보형물이 삽입된 척추종양과 같이 결정장기와 종양이 인접한 환자에 있어서도 MVCT영상자료를 이용하여 선량 계산 시 보다 정확한 치료계획이 가능하리라 사료된다.

    • 호흡에 따른 간장용적의 변화와 정상조직손상확율에 미치는 영향에 관한 연구

      Purpose: The aim of this study is to investigate geometrical and volumetrical changes of liver due to breathing and its impact to NTCP. In order to attain better treatment results it should be considered deliberately during planning session. Mehtods and Materials : Seven patients were examined in this study who have done TACE for accurate tumor margin drawing. After contrast media injection, C-T scan data were obtained in supine position during breathing free, inhalation and exhalation, respectively. For all patients C-T scan were done with same scanning parameters- 5 mm index, 5 mm thickness and pitch 1. Based on C-T data we have measured differences of each variables between breathing status such as changes of total and remained liver volumes, GTV, beam path length and superior to inferior shift. NTCP were calculated using Lyman's effective volume DVH reduction scheme and for this NTCP calculation, the V50 was computed from DVH and each m, n value were referred from Burmans data. Results : The measured total tilter volume and the remained liver volume changed between inspiration and expiration about $1.2-7.7\%(mean+2.7\%)$ and $2.5-13.23\%(mean=5.8\%)$ respectively, and these results were statistically significant(p>0.1). The GTV difference in each patient varied widely from $1.17\%\;to\;30.69\%$, but this result was not statistically significant. Depending on the breathing status, the beam path length was changed from 0.5 cm to 1.1 cm with the average of 0.7 cm, and it was statistically significant(p=0.006). The measured superior to inferior shifts were ranged from 0.5 cm to 3.74 cm. The NTCPs were changed relatively small in each patient, but the variation was large between the patients. The mean NTCP difference was $10.5\%$, with the variation ranged from $7\%\;to\;23.5\%$. Conclusion : Variations of liver volume and of beam path length were changed significantly depending on the breathing statues and the range of variation itself was very different between the patients. Since this variance could seriously affect the clinical outcomes of radiation treatments, the breathing of patients need to be accounted when a final treatment planning is derided.

    • 전립선암 치료 시 Tomoimage에 기초한 Setup 오차에 관한 고찰

      Purpose: The patient's position and anatomy during the treatment course little bit varies to some extend due to setup uncertainties and organ motions. These factors could affected to not only the dose coverage of the gross tumor but over dosage of normal tissue. Setup uncertainties and organ motions can be minimized by precise patient positioning and rigid immobilization device but some anatomical site such as prostate, the internal organ motion due to physiological processes are challenge. In planning procedure, the clinical target volume is a little bit enlarged to create a planning target volume that accounts for setup uncertainties and organ motion as well. These uncertainties lead to differences between the calculated dose by treatment planning system and the actually delivered dose. The purpose of this study was to evaluate the differences of interfractional displacement of organ and GTV based on the tomoimages. Materials and Methods: Over the course of 3 months, 3 patients, those who has applied rectal balloon, treated for prostatic cancer patient's tomoimage were studied. During the treatment sessions 26 tomoimages per patient, Total 76 tomoimages were collected. Tomoimage had been taken everyday after initial setup with lead marker attached on the patient's skin center to comparing with C-T simulation images. Tomoimage was taken after rectal balloon inflated with 60 cc of air for prostate gland immobilization for daily treatment just before treatment and it was used routinely in each case. The intrarectal balloon was inserted to a depth of 6 cm from the anal verge. MVCT image was taken with 5 mm slice thickness after the intrarectal balloon in place and inflated. For this study, lead balls are used to guide the registration between the MVCT and CT simulation images. There are three image fusion methods in the tomotherapy, bone technique, bone/tissue technique, and full image technique. We used all this 3 methods to analysis the setup errors. Initially, image fusions were based on the visual alignment of lead ball, CT anatomy and CT simulation contours and then the radiation therapist registered the MVCT images with the CT simulation images based on the bone based, rectal balloon based and GTV based respectively and registered image was compared with each others. The average and standard deviation of each X, Y, Z and rotation from the initial planning center was calculated for each patient. The image fusions were based on the visual alignment of lead ball, CT anatomy and CT simulation contours. Results: There was a significant difference in the mean variations of the rectal balloon among the methods. Statistical results based on the bone fusion shows that maximum x-direction shift was 8 mm and 4.2 mm to the y-direction. It was statistically significant (P=<0.0001) in balloon based fusion, maximum X and Y shift was 6 mm, 16mm respectively. One patient's result was more than 16 mm shift and that was derived from the rectal expansions due to the bowl gas and stool. GTV based fusion results ranging from 2.7 to 6.6 mm to the x-direction and 4.3$\sim$7.8 mm to the y-direction respectively. We have checked rotational error in this study but there are no significant differences among fusion methods and the result was 0.37$\pm$0.36 in bone based fusion and 0.34$\pm$0.38 in GTV based fusion.
