      • VIRTUAL CT OTOSCOPY OF THE MIDDLE EAR AND OSSICLES IN DOGS : Virtual CT Otoscopy of the Middle Ear and Ossicles in Dogs

        <P>Virtual otoscopy enables noninvasive 3D endoluminal imaging of the middle ear through postprocessing of computed tompgraphy (CT) data. A standardized imaging approach for the middle ear was established in six normal dogs in an attempt to optimize the clinical application and student education. High-resolution CT data were obtained. Virtual otoscopic images of the middle ear cavity and ossicles were generated using commercially available software. The views of the four different directions (the ear canal, tympanic bulla, eustachian tube, and ossicular chain) were made for virtual otoscopy. The promontory, cochlea window, tympanic bulla, septum bulla, and auditory tube were distinguished easily and clearly. One of the ossicles, the malleus, was visualized accurately. However, small structures such as the incus and stapes always could not be seen. The main advantage of virtual otoscopy is not only to provide diagnostic information but also to enhance the quality and efficiency of student education, because it contributes to an understanding of the anatomy of the middle ear. We describe the normal topographical 3Dimages of the middle ear of the dog using virtual otoscopy.</P>

        Ultrasonographic evaluation of tracheal collapse in dogs

        Tracheal ultrasonography was performed to measure the width of the tracheal ring shadow and to assess the clinical relevance of these measurements for identifying tracheal collapse. The first tracheal ring width (FTRW) and thoracic inlet tracheal ring width (TITRW) were measured on both expiration and inspiration. The mean of the FTRW width (129 dogs) was greater in expiration (10.97 ± 1.02 mm, p = 0.001) than that in inspiration (9.86 ± 1.03 mm). For 51 normal dogs, the mean of the TITRW width was greater in expiration (9.05 ± 1.52 mm, p = 0.001) than in inspiration (8.02 ± 1.43 mm). For 78 tracheal collapse dogs, the mean of the TITRW width was greater in expiration (15.89 ± 1.01 mm, p = 0.001) than in inspiration (14.85 ± 1.17 mm). The TITRW/FTRW ratio of the normal dogs was higher (p = 0.001) in expiration (0.81 ± 0.09) than that in inspiration (0.79 ± 0.10). When compared between the normal and tracheal collapse dogs, the TITRW/FTRW ratio was also increased (p = 0.001) both in expiration (1.54 ± 0.09) and inspiration (1.47 ± 0.08), respectively. Based on these results, the cutoff level of the TITRW/FTRW ratio was statistically analyzed according to the receiver operating characteristic curve and it could be set at 1.16 in expiration and at 1.13 in inspiration. We have demonstrated that tracheal ultrasonography is a useful technique for the evaluation of tracheal collapse and it can be a supportive tool together with the radiographic findings for making the correct diagnosis.

      • Efficacy of the multidisciplinary tumor board conference in gynecologic oncology: A prospective study

        목적: Evidence has shown that multidisciplinary tumor board conferences (MTBCs) improve patient management for various cancer types. However, few retrospective studies have investigated MTBC efficacy for patients with gynecologic cancers. Here, we aimed to prospectively evaluate how MTBCs influence patient management in gynecologic oncology. 방법: This prospective study included 85 consecutive cases that were presented at gynecologic oncology weekly MTBCs of our tertiary university hospital between January 2015 and April 2016. The primary endpoint was treatment plan change rate, which included both major and minor changes. Major changes were defined as exchange, addition, or subtraction of treatment modality. Minor changes included all other, such as intramodality changes or treatment time changes. The secondary endpoints were the change rates of diagnosis, diagnostic work-up, and radiological and pathological findings. 결과: The overall change rate in treatment plan was 27.1%, which included 10.6% major changes and 16.5% minor changes. Among the treatment plan changes, change rate in treatment plan alone was 16.5%, and changes in diagnosis and radiological findings occurred in 7.1% and 3.5% of cases, respectively. Diagnosis and radiological findings were also changed in 9.4% and 10.6% of cases, respectively. However, there were no changes in pathological findings. Moreover, there was a change of diagnostic method for further work-up in 23.5% of cases. 결론: Gynecologic oncology MTBCs resulted in considerable changes in treatment plans. Diagnosis, diagnostic work-up, and radiological findings were also influenced by MTBCs. The data emphasizes the importance of a multidisciplinary team approach for gynecologic cancer management.

        수의학 훈련을 위한 슬개골 수술 햅틱 시뮬레이터

        소동물의 슬개골 수술은 인간의 뼈보다 약하여 수술의 난이도가 어렵기 때문에, 소 동물의 슬개골 탈구 치료 수술 과정의 학습은 수의사에게 매우 중요하다. 소 동물의 슬개골 수술 훈련은 사체를 이용한 훈련을 하고 있으나 소 동물의 사체가 부족하기 때문에 시청각 자료나 수술 참관으로 수술을 학습하고 있어 수의학과 학생 및 전문 수의사들에게 충분한 훈련 환경이 제공되지 못하는 상황이다. 본 논문에서는 이러한 문제를 해결하기 위하여 소동물의 슬개골 수술을 위한 햅틱 시뮬레이터를 제안한다. 제안한 시뮬레이터는 슬개골 드릴링 수술에 적용시켰으며 소동물의 드릴링에 대한 Force Feedback을 모델링하고 햅틱 인터페이스를 제공하고, 사용자가 몰입하여 수술 경험을 할 수 있는 워크 벤치를 제공하였다. 본 논문에서 제안된 슬개골 햅틱 시뮬레이터가 제공하는 드릴링 과정에서 햅틱 피드백에 대한 사용자 평가 결과 유의미한 결과를 얻을수 있었다. Patella surgery of small animal is an important veterinary surgery that the veterinarian should saw and drill the dislocated patella in order to fix the corrected position. However, the animal protection laws restrict the veterinarian students’ chances for the practice and training of the patella surgery. This paper proposed a haptic based patella surgery simulator for veterinarian students. We modelled force feedback methods in order to provide best similar haptic feedbacks to the real drilling feedbacks in the patella surgery. The proposed patella drilling simulator provides haptic interface as a drill and a workbench in order to provide best surgery experiences. We conducted the performance evaluations in order to prove usability of the proposed patella surgery interface.

      • Microfluidic Multifunctional Probe Array Dielectrophoretic Force Spectroscopy with Wide Loading Rates

        <P>The simultaneous investigation of a large number of events with different types of intermolecular interactions, from nonequilibrium high-force pulling assays to quasi-equilibrium unbinding events in the same environment, can be very important for fully understanding intermolecular bond-rupture mechanisms. Here, we describe a novel dielectrophoretic force spectroscopy technique that utilizes microsized beads as multifunctional probes for parallel measurement of intermolecular forces with an extremely wide range of force rate (10<SUP>–4</SUP> to 10<SUP>4</SUP> pN/s) inside a microfluidic device. In our experiments, various forces, which broadly form the basis of all molecular interactions, were measured across a range of force loading rates by multifunctional probes of various diameters with a throughput of over 600 events per mm<SUP>2</SUP>, simultaneously and in the same environment. Furthermore, the individual bond-rupture forces, the parameters for the characterization of entire energy landscapes, and the effective stiffness of the force spectroscopy were determined on the basis of the measured results. This method of determining intermolecular forces could be very useful for the precise and simultaneous examination of various molecular interactions, as it can be easily and cost-effectively implemented within a microfluidic device for a range of applications including immunoassays, molecular mechanics, chemical and biological screening, and mechanobiology.</P><P><B>Graphic Abstract</B> <IMG SRC='http://pubs.acs.org/appl/literatum/publisher/achs/journals/content/ancac3/2012/ancac3.2012.6.issue-10/nn302202t/production/images/medium/nn-2012-02202t_0006.gif'></P><P><A href='http://pubs.acs.org/doi/suppl/10.1021/nn302202t'>ACS Electronic Supporting Info</A></P>

        Preliminary study of presumptive intradural-intramedullary intervertebral disc extrusion in 20 dogs

        Background: Intradural-intramedullary intervertebral disc extrusion (IIVDE) is a rare condition of intervertebral disc disease. However, the diagnosis of IIVDE is challenging because the prognosis and imaging characteristics are poorly characterized. Objectives: We aimed to describe the clinical and imaging characteristics of tentatively diagnosed IIVDE in dogs to assess the prognostic utility of neurological grade and magnetic resonance imaging (MRI) findings. Methods: Twenty dogs were included in this retrospective cohort study. Results: Nonchondrodystrophic breeds (n = 16) were more predisposed than chondrodystrophic breeds. Most dogs showed acute onset of clinical signs. Neurological examination at admission showed predominant non-ambulatory paraparesis (n = 9); paresis (n = 16) was confirmed more frequently than paralysis (n = 4). Follow-up neurological examination results were only available for 11 dogs, ten of whom showed neurological improvement and 8 showed successful outcomes at 1 month. The characteristic MRI findings include thoracic vertebra (T)2 hyperintense, T1 hypointense, intramedullary linear tracts with reduced disc volume, and cleft of the annulus fibrosus. None of the MRI measurements were significantly correlated with neurological grade at admission. Neurological grade did not differ according to the presence of parenchymal hemorrhage, parenchymal contrast enhancement, and meningeal contrast enhancement. Neurological grades at admission showed a statistical correlation with those observed at the 1-month follow-up (r = 0.814, p = 0.02). Conclusions: IIVDE is a rare form of disc extrusion commonly experienced after physical activity or trauma and most frequently affects the cranial-cervical and thoracolumbar regions of nonchondrodystrophic dog breeds. Neurological score at admission emerged as a more useful prognostic indicator than MRI findings in dogs with suspected IIVDE.

        Far lateral lumbar disc extrusion in a dachshund dog

        A 6-year-old Dachshund was presented with acute, non-localized pain without neurological dysfunction. Radiography revealed multiple calcifications of intervertebral discs and narrowing of disc space in the thoracolumbar region. Computed tomography and magnetic resonance imaging revealed calcified disc-like material entrapped in the left extraforaminal area and showed a displaced nerve root. Fenestration and removal of the extruded disc material were performed in a routine manner. Histopathological examination showed degenerative disc materials with severe calcification both in the nucleus pulposus and around the annulus fibrosis. Based on imaging, surgical, and histopathologic results, the dog was diagnosed with far lateral lumbar disc extrusion.
