Long-term outcomes of single-port laparoscopic myomectomy using a modified suture technique

Objective To evaluate the long-term outcomes, including the pregnancy outcome and recurrence rate after single-port laparoscopic myomectomy (LM) using a modified suture technique with a Hem-o-lok clip (Choi’s LM) and conventional 4-port LM. Methods A retrospective study of patients who underwent Choi’s LM (n=55) and 4-port LM (n=102) in a single institutional hospital was conducted. Patients with <3 symptomatic myomas sized <10 cm each and operated on by a single surgeon were included. Recurrence was confirmed when a myoma measuring ≥3 cm was detected. Results The patients in both groups had similar demographic characteristics. Single (76.4% vs. 62.7%) and intramural (52.7% vs. 56.9%) tumors were commonly detected in both groups in the mean diameter (6.8±1.5 cm vs. 7.0±1.6 cm; P=0.40). In Choi’s LM, 16 patients (29.1%) needed an additional port; those who were nulliparous and/or had a large leiomyoma more frequently required an additional port (P=0.023 and 0.04, respectively). During a median follow-up period of 69 months, 17 patients (7.1% vs. 14.6%) had recurrence. The size of dominant myomas at recurrence was significantly smaller in patients who underwent Choi’s LM (3.4±0.7 cm vs. 5.7±2.4 cm; P=0.004). All 13 patients in both groups who successfully conceived had a full-term delivery. No major complications occurred during pregnancy. Conclusion Although an additional port was frequently used, the long-term outcomes of patients who experienced recurrence and pregnancy after Choi’s LM were acceptable. Considering its usability, Choi’s LM is feasible for the treatment of uterine leiomyoma.

<P>Subways are used for public transportation as a commuting medium in Korea. Subway trains operate mainly through tunnels. Fine particles generated by friction between rails and train wheels affect the air quality inside a train because the particles enter the ventilation system of the train passing through a tunnel. The PM10 concentration has been mainly used to evaluate indoor air quality, and the PM2.5 concentration has measured recently. Therefore, in this study, the concentrations of the particles entering the air-inlet with respect to particle sizes including 0.1, 1, 2.5 and 10 mu m were investigated by using numerical analysis with ANSYS FLUENT software. The particle analysis was performed corresponding to 10, 40 and 80 km/h. It is expected that the numerical results would be helpful in studying the effect of particles corresponding to PM2.5 on the ventilation system, to improve the air quality inside a train passing through a tunnel.</P>

<P>CXC motif chemokine ligand 10 (CXCL10) and its receptor CXC motif chemokine receptor 3 (CXCR3), play important roles in the motility of breast cancer cells. <I>Alisma canaliculatum</I> is a herb that has been used as a traditional medicine for thousands of years in Korea and China. Whether <I>A. canaliculatum</I> inhibits the motility of metastatic breast cancer cells is not clear yet. In this study, we show that <I>A. canaliculatum</I> ethanolic extract (ACE) prevented tumor necrosis factor-alpha (TNFα)-induced migration of MDA-MB-231 cells. ACE significantly attenuated TNFα-induced upregulation of CXCL10 and CXCR3 expression at the gene promoter level. Mechanistically, ACE inhibits TNFα-induced phosphorylation of inhibitor of κB (IκB) kinase (IKK), IκB and p65/RelA, leading to the suppression of nuclear translocation of p65/RelA nuclear factor kappa-B (NF-κB). Also, ACE inhibited NF-κB-dependent <I>CXCR3</I> and <I>CXCL10</I> promoter activities. These results suggest that ACE abrogates TNFα-induced migration of MDA-MB-231 breast cancer cells through down-regulation of IKK-NF-κB-dependent <I>CXCR3</I> and <I>CXCL1</I>0 expression. Our results suggest that ACE has potential as a herbal supplement for the inhibition of breast cancer metastasis.</P>

<P>Interleukin-33 (IL-33), a member of the IL-1 cytokine family, is emerging as a new regulator of immune responses and inflammatory vascular diseases. Although IL-33 and its cognate receptor ST2 appear to be expressed in vascular cells, the precise role of IL-33 in the vasculature has not been determined. In this study, we report a novel role of IL-33 as a potent endothelial activator, promoting both angiogenesis and vascular permeability. IL-33 increased proliferation, migration, and morphologic differentiation of human endothelial cells, consistently with increased angiogenesis in vivo. IL-33 also increased endothelial permeability with reduced vascular endothelial-cadherin-facilitated cell-cell junctions in vitro and induced vascular leakage in mouse skin. These effects of IL-33 were blocked by knockdown of ST2. Ligation of IL-33 with ST2 rapidly increased endothelial nitric oxide (NO) production through TRAF6-mediated activation of phosphoinoside-3-kinase, Akt, and endothelial NO synthase. Moreover, pharmacologic or genetic blockage of endothelial NO generation resulted in the inhibition of angiogenesis and vascular hyperpermeability induced by IL-33. These data demonstrate that IL-33 promotes angiogenesis and vascular leakage by stimulating endothelial NO production via the ST2/TRAF6-Akt-eNOS signaling pathway. These findings open new perspectives for the role of IL-33 in the pathogenesis of angiogenesis-dependent and inflammatory vascular diseases.</P>

<P>Herein, we report a <I>de novo</I> synthesis approach to produce bandgap-controlled polyaniline (PAni) nanostructure <I>via</I> Mn-mediated oxidative polymerization at the catalytic nanoreactor. To achieve systemic nanoconfined polymerization, manganese oxide (MnOx) nanoparticles coated with silica were used as the sacrificial nanotemplate. Interestingly, the catalytic nanoreactor simultaneously allowed the nanoconfined oxidative polymerization and controlling of the bandgap. MnOx could be reduced by the addition of aniline monomers and consecutive redox reaction at the nanoreactor. Furthermore, core cavity was generated, and ionized Mn could control the bandgap by coordination at the nanostructures.</P>

Motion Graphic in the movie title Sequence is one of tools. A main objective of tool is to function as a bridge of mutual communication between the movie and the audience. This paper is intended to mention a new value of Korean movie's opening title sequence utilization by Gestalt Theory. In a design field, the Motion Graphic will be continuously utilized as an important means of communication, and it is expected to be shown as a formative various expression method.
A Study on Guideline Regarding Eco Design Expressed on Web Digital Media

As increasing the social understanding on environmental problems, the eco investments are also growing increased. These lead to many activities for harmonizing with environment. Likewise, 'Eco design' which is connected with the concept of eco harmony is getting more important. This phenomenon also take place on the web digital media. The Expansion of the web scope makes a change in the total paradigm of the culture. The role of this web is getting increased more and more, and creates the new paradigm of the communication by integration of the various functions and technologies. In this paper, we analyze the features of existing web designs and Eco designs, and propose the actual method for friendly environment through the case studies of Eco designs on the web.
