Biomass, a carbon neutral fuel, is one of the fastest growing renewable energy sources. In this study, economic feasibility of small scale power generation system using syngas from biomass has been investigated. Effects of important variables such as capital investment, cost of waste wood, renewable energy certificate(REC), system marginal price(SMP) on the benefit of business have been analyzed. Internal rate of return(IRR) was predicted as 16.90%, which implicates the business is promising based on the assumptions such as SMP of 137 Won/kWh, capital cost of 4.7 billion won, and waste wood price of 30 thousand won. Major factors affecting the benefit of business are as follows; system marginal price, operational rate, capital investment, expenditure of waste wood, certified emission reduction.

Edible insects are considered novel protein sources, and the edible insect breeding industry is growing rapidly. However, fungal pathogens are a major threat to this industry, and appropriate identification of causative agents is important for the development of antifungal agents. In 2019, strains of the fungal pathogen Metarhizium were isolated from infected second and third instar larvae of the economically important edible insect Protaetia brevitarsis, which were collected from separate farms in Gimhae-si, Gyeongsangnam-do, Korea. Based on morphological characteristics and multilocus phylogenetic analysis of the internal transcribed spacer region, and 50-region of gene encoding the 1-α elongation factor, the fungal strains were identified as Metarhizium majus. Here, we report the detailed phylogenetic characteristics and morphological description of this species, which is prevalent in Korea. Identification of the principal disease-causing agent can help develop disease prevention and antifungal strategies for application in edible insect-rearing farms.

Background: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a technique developed to allow mediastinal staging of lung cancer and also to evaluate intrathoracic lymphadenopathy. In a tuberculosis-endemic area, tuberculosis should be considered as an etiology of mediastinal lymphadenopathy. The aim of this study was to investigate the utility of the routine culture for tuberculosis from specimens of EBUS-TBNA. Methods: We prospectively performed routine culture for tuberculosis from aspiration or core biopsy specimens got from 86 patients who had undergone EBUS-TBNA due to mediastinal lymphadenopathy between March 2010 and March 2011. Results: A total of 135 lymph node aspiration and 118 core biopsy specimens were included in this analysis. We confirmed the malignancy in 62 (72.9%), tuberculosis in 7 (8.1%), sarcoidosis in 7 (8.1%), asperogillosis in 2 (2.3%) and pneumoconiosis in 2 (2.3%) patients. One lung cancer patient had pulmonary tuberculosis coincidentally and 5 patients had unknown lymphadenopathy. The number of positive culture for Mycobacterium tuberculsosis by EBUS-TBNA is 2 (1.5%) from 135 lymph node aspiration specimens and 2 (1.7%) from 118 core biopsy specimens. Out of eight patients confirmed with tuberculosis, only one patient had positive mycobacterial culture of aspiration specimen from EBUS-TBNA without histopathologic diagnosis. Conclusion: These results propose that routine culture for tuberculosis from EBUS-TBNA may not provide additional information for the diagnosis of coincident tuberculous lymphadenitis. However, if there is any possibility of tuberculous lymphadenopathy or pulmonary tuberculosis, it should be considered to perform EBUS-TBNA in patients who have negative sputum AFB smears or no sputum production.
A case of giant basal cell carcinoma of the cheek

A 59-year-old female patient presented with one year history of an enlarging left cheek mass. The mass was measured about 6 cm by 4 cm in diameter and the lesion accompanied skin color change to red and vascularization. Contrast enhanced neck magnetic resonance imaging (MRI) showed malignant tumor with suspicious involvement of the skin, but no infiltration of bone. A punch biopsy confirmed the diagnosis of basal cell carcinoma (BCC), and total whole body FDG positron-emission tomography (PET) imaging showed no suspicious metastatic hypermetabolic lesions. She referred to department of plastic surgery and was treated with wide surgical excision. BCC is the most common non-melanoma skin cancer, and its incidence is increasing worldwide. BCC generally grows slowly and has favorable prognosis. Giant BCC is a rare type of BCC, with less than 1%, which is defined as a tumor with larger than 5 cm in diameter. Contrary to natural course of general BCC, giant BCC is known to behave aggressive, which can invade and destroy the surrounding structure such as muscle and bone. However, treatment of giant BCC has not yet been established because of rarity of this tumor. Multidisciplinary approach is needed for treatment of giant BCC. We herein report the interesting case of giant BCC of the cheek.

Patients with dysphagia often have difficulty in supplying adequate nutrition orally, and thus they often use gastrostomyfor nutrition support. If the nutrition affects the deterioration of the disease, as in amyotrophic lateral sclerosis,the majority of patients will have a gastrostomy tube for proper nutrition. To prevent complications from gastrostomytubes, it is important to educate the caregiver or patient about how to properly manage it. If these patientsopt for home care because of financial or cultural reasons, it will be difficult for their healthcare team to observethem closely, leading to complications due to lax tube management. In this case, appropriate managementeducation becomes more important. This paper reports an extremely rare case of duodenal intussusception causedby a migrated percutaneous radiologic gastrostomy (PRG) tube in a patient with amyotrophic lateral sclerosis (ALS)using simultaneous oral and enteral nutrition. The patient was treated successfully with urgent tube removal usingthe air reduction maneuver. This case highlights the importance of gastrostomy management.
Flavonoid silibinin induces hair growth in 3-dimensional cultured human dermal papilla cells

Background: Alopecia is a hair loss disorder, which can have a significant psychological impact on the quality of life. Silibinin is a secondary metabolite from Silybum marianum, which is a major active component of silymarin. Previous studies have revealed that silibinin acts as an antioxidant, which can reduce the risk of cutaneous cancers. Objectives: This study aimed to evaluate the effect of silibinin on hair growth in 3-dimensional (3D) cultured human dermal papilla (DP) spheroids. Methods: Activation of serine/threonine kinase (Akt) and the wingless and INT-1 (Wnt)/β-catenin signaling pathway were measured by using the western blotting and the T cell-specific transcription factor and lymphoid enhancer-binding factor (TCF/LEF) promoter-carrying luciferase reporter assay. Expression levels of genes were determined by using quantitative real time PCR analysis. Results: Silibinin significantly increased cell viability through Akt activation in 3D-cultured DP cells. After the treatment with silibinin, the Wnt/β-catenin signaling pathway was highly activated, and the expression levels of the target genes, WNT5a and LEF1, were increased in 3D DP spheroids. Moreover, the expression levels of the signature genes of DP, such as ALPL, VCAN, FGF7 and BMP2, were significantly increased. Conclusion: Silibinin induces hair growth in 3D-cultured DP spheroids through the Akt and Wnt/β-catenin signaling pathways. This study suggests that silibinin can be a potential therapeutic option for alopecia.
A case of calcinosis cutis of the fingertip

A 31-year-old male patient presented our clinic with a 3-month history of a yellowish globular-shaped subcutaneous firm nodule on right third fingertip. He complained of perilesional pain but had no systemic symptoms. He denied any history of trauma in the lesion, and medical past history. A biopsy was performed under clinical suspicion of glomus tumor. The lesion was totally excised, and histopathological examination revealed the presence of calcified deposits in the dermis. Moreover, laboratory examination, including renal function tests, serum calcium and phosphorus showed no abnormality. Idiopathic calcinosis cutis is an uncommon disorder, which is cutaneous calcification without underlying tissue injury or metabolic disorder. Tumoral calcinosis is a type of idiopathic calcinosis cutis, which usually occurs around the large joints, but rare in the fingers. Subepidermal calcified nodule is an another type of idiopathic calcinosis cutis. It is characterized by solitary hard papules on the head and extremities, which is frequently occurs in children. The other type of idiopathic calcinosis cutis is scrotal calcinosis. We herein report the case of idiopathic calcinosis cutis presenting with atypical manifestation, which occurs in fingertip of healthy male adult.
