        Male Attitudes towards Infertility: Results from a Global Questionnaire

        Purpose: In general, men are less likely to seek health care than women. Infertility is a global disease that afflicts approxi-mately 15% of reproductive age couples and the male contributes to 40% of the diagnosable cause. Remarkably, no large or multi-national population data exist regarding men’s perceptions about their infertility. The purpose of this study was to advance our knowledge about the infertile male’s social experience regarding: (1) how they feel about their infertility, (2) what motivated them to seek health care, (3) how likely are they to talk with others about their infertility, (4) their awareness of male infertility support groups, and (5) what their primary source for information is regarding male infertility? Based on the results from this study, these simple questions now have clearer definition. Materials and Methods: An Institutional Review Board-approved, male-directed, anonymous questionnaire translated into 20 languages was made globally available through the Fertility Europe website (https://fertilityeurope.eu). Males (n=1,171) age 20–49 years were invited to complete the online survey after informed consent. Results: Most respondents were European (86%). Of European men, <15.8% were self-motivated to seek medical help. Fur-ther, their physician was not the primary source of information regarding their infertility. While most men (59%) viewed their infertility positively, a large majority were not very likely (73%) to talk about it. Most respondents indicated a lack of aware-ness or absence of male infertility support groups. Conclusions: These are the first multi-national population data revealing men’s feelings about their infertility, what motivates them to seek help and their awareness of resources for peer support and information. These findings also serve to highlight significant gaps that exist in the provision of male reproductive health care and in supportive resources for men suffering from infertility. We offer recommendations on how to address the problem(s).

        Beyond the Transfer Paradigm: New Insights in Bible Translation using the Dutch Nieuwe Bijbelvertaling as a Case Study

        This article focuses on recent advancements in Bible translation, using the Dutch Nieuwe Bijbelvertaling (NBV) as a case study. The NBV was published in 2004 and underwent a revision in 2021. This translation follows the tradition of Nida, but also aims to enhance the method of functional equivalence with innovative elements. The 2021 edition incorporates valuable feedback from readers to improve the translation according to its own standards and draws insights from skopos theory to address various dilemma’s. Consequently, the NBV exemplifies the contemporary stance of Bible translation that emphasizes methodological improvements and acknowledges the additional perspective of skopos theory. However, it still predominantly adheres to the view of translation as a solely linguistic endeavor and remains within the paradigm of translation as a transfer process. Within the field of translation studies, the transfer paradigm has been under scrutiny for several decades. A more current paradigm (frames paradigm) recognizes that translation inherently involves a reconceptualization of the source text influenced by various factors and frames of reference. Furthermore, the translator not only reconceptualizes the source text but also envisions a particular concept of the target text. Embracing these insights entails recognizing that translation is not a straightforward transfer but is rooted in interpretative processes. While glimpses of these insights were evident in the NBV project, the translators at the time were not fully aware of the implications. Bible translators and their organizations tend to present their translations within the framework of the transfer paradigm, conforming to the audience expectations and adhering to the traditional view of translation as reproduction of the same text in new words. However, according to the frames paradigm, the relationship between source text and translation is less direct. A translation can be seen as a contemporary representation of an existing text, fundamentally aligned with the source text but also displaying distinct variations. Translation inherently embodies a certain duality. The current scholarly view of translation does not necessarily conflict with a Christian perspective on Scripture: a translation can be viewed not only as a representation of the source text but also as a recontextualization of the eternal Word.

        A New Dutch Bible Translation in Plain Language

        The Bijbel in Gewone Taal (Bible in Plain Language), published in October 2014 is a Dutch translation made by the Netherlands Bible Society. It is a translation from the biblical source texts, and is designed to make the Bible accessible to as wide a readership as possible. The translation method is based on scholarly research into the comprehensibility of language and texts. Comprehensibility of language is facilitated by the choice of a very limited vocabulary and a strong preference for using well-known words. Furthermore, it requires the use of relatively short and clearly structured sentences. The comprehensibility of texts relates to factors such as the coherence and structure of the text, the organization of the information, and the connection between sentences. All these relevant factors were integrated into our translation method. The BGT is characterized by its explicit rendering of the source text. Aspects that remain implicit in the source text that are essential for understanding the text are rendered in an explicit way in the BGT. Furthermore, biblical imagery is also made comprehensible for today’s readers. Whereas well-known images are retained, others are clarified, generalized, or presented more directly using their implied meaning. Instead of the traditional biblical terms such as ‘righteousness’, the BGT often uses a variety of plain terms and phrases dependent on the contextual meaning of the biblical term. This sheds light on the actual meaning of such biblical terms. The BGT as a translation aims to clarify the biblical text and make it accessible to present-day readers. The target audience explicitly includes those readers who have difficulty in reading and understanding other Dutch translations. But at the same time, the BGT is designed to be useful for any Bible reader who appreciates a clear text. Its general acceptance in the Dutch context shows that it indeed functions in this way. Whereas clarity and comprehensibility always come first, great care has been taken to retain the expressiveness of the text. Thanks to the power and directness of plain language, reading the BGT can be an overwhelming experience for readers of all backgrounds, age groups, and educational levels.

        Enhancing diffusion of consumer innovations on knowledge sharing platforms

        In the past decade, studies showed that many consumers innovate for themselves. Sometimes their innovations are useful to others and potentially enhance social welfare. Unfortunately, diffusion fails as consumers lack incentives to inform others. Online knowledge sharing platforms (OKSPs), which can be stimulated with government support, may alleviate this problem. Platform communication may trigger passive consumers into active knowledge contributors. It is however uncertain if and how platform communication affects consumers who never shared designs before. We conducted a randomised controlled experiment with 715 members of an OKSP in 3D printing. Our intervention included a series of general and personal messages, tailored to various motives to share knowledge: altruism, status, ideology, learning and community. Platform members who never uploaded designs before are positively influenced by our intervention. Specifically, messages tailored to altruism, ideology and learning made platform members upload more designs. Hence, platform communication can improve the availability of innovative designs to potential adopters, and is a useful step to alleviate diffusion failure of consumer innovations

        The Incidence of Acute Traumatic Tendon Injuries in the Hand and Wrist: A 10-Year Population-based Study

        Background:Acute traumatic tendon injuries of the hand and wrist are commonly encountered in the emergency department. Despite the frequency, few studies have examined the true incidence of acute traumatic tendon injuries in the hand and wrist or compared the incidences of both extensor and flexor tendon injuries. Methods: We performed a retrospective population-based cohort study of all acute traumatic tendon injuries of the hand and wrist in a mixed urban and rural Midwest county in the United States between 2001–2010. A regional epidemiologic database and medical codes were used to identify index cases. Epidemiologic information including occupation, year of injury, mechanism of injury and the injured tendon and zone were recorded. Results: During the 10-year study period there was an incidence rate of 33.2 injuries per 100,000 person-years. There was a decreasing rate of injury during the study period. Highest incidence of injury occurred at 20–29 years of age. There was significant association between injury rate and age, and males had a higher incidence than females. The majority of cases involved a single tendon, with extensor tendon injuries occurring more frequently than flexor tendons. Typically, extensor tendon injuries involved zone three of the index finger, while flexor tendons involved zone two of the index finger. Work-related injuries accounted for 24.9% of acute traumatic tendon injuries. The occupations of work-related injuries were assigned to major groups defined by the 2010 Standard Occupational Classification structure. After assigning these patients' occupations to respective major groups, the most common groups work-related injuries occurred in construction and extraction occupations (44.2%), food preparation and serving related occupations (14.4%), and transportation and material moving occupations (12.5%). Conclusions: Epidemiology data enhances our knowledge of injury patterns and may play a role in the prevention and treatment of future injuries, with an end result of reducing lost work time and economic burden.

        SMALL SCALE DAIRYING IN THREE FARMING SYSTEMS IN EAST JAVA II. ECONOMIC ANALYSIS OF DAIRYING

        The indonesian government wishes to increase farmers' by encouraging dairy farming. Since 1980, imported dairy cattle have been distributed on credit. Survey data from 1990 were used to study the average milk sales per cow and the economic parameters of dairy production of a sample of farm households on seven milk cooperatives East Java Province, in three agro-ecological areas, oriented to sugar cane, cassava and horticulture, respectively. in general, dairy production proved not to be economically attractive as returns to labour were similar to the rates for agricultural labour. The returns showed marked differences among the three areas studied. They were highest in the horticultural area. Analysis of the dairy units according to size showed a tendency for inputs as well as revenue per cow to decline as herd size increased. It is argued that a further increase in milk production in East Java can be stimulated by raising the farm gate price of milk, or by expanding dairy production into suitable, new areas.

        SMALL SCALE DAIRYING IN THREE FARMING SYSTEMS IN EAST JAVA I. FARMER'S INCOME AND HOUSEHOLD CHARACTERISTICS

        The annual income (gross margin) in 1989/90 of a sample of 274 farmers in seven milk cooperatives was analyzed in the sugar cane, cassava, and horticulture areas in East Java. On average dairying contributed 42%, crops 29% and off-farm revenue 29%. Dairy income was highest in the cassava area, where it compensated for the low crop income, and lowest in the sugar cane area. Farm area and average milk yield per day per cow correlated positively with farmer's income, whereas crop income increase significantly with farm area and with the number of cows. The level of total cost per cow had a negative impact on dairy and with the number of cows. The level of total cost per cow had a negative impact on dairy and on total income. Government officials and other professionals engaged in dairying had a significantly higher total income than those with their main occupation in dairying, cropping or working as farm labourers. Uneducated farmers obtained a significantly larger income through crops, whereas farmers with tertiary education obtained more income through off-farm work, This study suggests that more attention must be paid to the actual use of labour and the improvement of the dairy output/cost ratio.

        Performance of Kiestra Total Laboratory Automation Combined with MS in Clinical Microbiology Practice

        Background: Microbiological laboratories seek technologically innovative solutions to cope with large numbers of samples and limited personnel and financial resources. One plat- form that has recently become available is the Kiestra Total Laboratory Automation (TLA) system (BD Kiestra B.V., the Netherlands). This fully automated sample processing sys- tem, equipped with digital imaging technology, allows superior detection of microbial growth. Combining this approach with matrix-assisted laser desorption ionization time-of- flight mass spectrometry (MS) (Bruker Daltonik, Germany) is expected to enable more rapid identification of pathogens. Methods: Early growth detection by digital imaging using Kiestra TLA combined with MS was compared to conventional methods (CM) of detection. Accuracy and time taken for microbial identification were evaluated for the two methods in 219 clinical blood culture isolates. The possible clinical impact of earlier microbial identification was assessed ac- cording to antibiotic treatment prescription. Results: Pathogen identification using Kiestra TLA combined with MS resulted in a 30.6 hr time gain per isolate compared to CM. Pathogens were successfully identified in 98.4% (249/253) of all tested isolates. Early microbial identification without susceptibility testing led to an adjustment of antibiotic regimen in 12% (24/200) of patients. Conclusions: The requisite 24 hr incubation time for microbial pathogens to reach suffi- cient growth for susceptibility testing and identification would be shortened by the imple- mentation of Kiestra TLA in combination with MS, compared to the use of CM. Not only can this method optimize workflow and reduce costs, but it can allow potentially life-saving switches in antibiotic regimen to be initiated sooner.
