Objective : The aim of this study was to review systemically clinical trials on the effectiveness and safety of herbal medicines in the treatment of osteoarthritis. Methods : Computerized literature searches were carried out on seven electronic databases, and hand-searching on some chinese medical journals in library of Kyung Hee Medical Center. Trial data were extracted in a standardized, predefined manner and assessed independently. Results : 1. Thirty reports of clinical trials and two reports of meta-analyses concerning herbal medicine were collected and reviewed Among these reports three medical herbs were applied as topical medicine and others as internal medicine. 2. The western studies established NSAIDs or placebo as their control group. Five chinese reports established formulated herb pill(Ruanshnagshenjin pill) as their control group and Six did not establish a control group at all. 3. ACR was the most highly used diagnostic criteria in the western studies while the Chinese used their official criteria established by their government or the criteria of their text books. 4. 20 reports chose the Lequesne functional index, SHAQ, WOMAC OA index, AIMS, and their own unique scoring system as the criteria of analysing the effect. Others chose clinical symptoms, articular functions, and lab finding as their criteria. 5. 7 single herbs and 19 formulated herbs were studied. Among the formulated herbs, Achyranthes japonica was studied in 10 of the studies and Angelica gigantis Radix in 8, making them the most often studied herbs among the studies.
Effect of Cinnamomum Cassia on Cartilage Protection in Rabbit and Human Articular Cartilage

Background & Objective: Articular cartilage is a potential target for drugs designed to inhibit the activity of matrix metalloproteinases (MMPs) to stop or slow the destruction of the proteoglycanand collagen in the cartilage extracellular matrix. The purpose of this study was to investigate the effects of Cinnamomum cassia in inhibiting the release of glycosaminoglycan (GAG), the degradation of collagen, and MMP activity in rabbit and human articular cartilage explants. Methods: The cartilage-protective effects of Cinnamomum cassia were evaluated by using glycosaminoglycan degradation assay, collagen degradation assay, colorimetric analysis of MMP activity, measurement of lactate dehydrogenase activity and histological analysis in rabbit cartilage explants culture. Results: Interleukin-1a (IL-1a) rapidly induced GAG, but collagen was much less readily released from cartilage explants. Cinnamomum cassia significantly inhibited GAG and collagen release in a concentration-dependent manner. Cinnamomum cassia dose-dependently inhibited MMP-1, MMP-3 and MMP-13 activities from IL-1a-treated cartilage explants culture when tested at concentrations ranging from 0.02 to 1 mg/ml. Conclusion : These results indicate that Cinnamomum cassia inhibits the degradation of proteoglycan and collagen through the down regulation of MMP-1, MMP-3 and MMP-13 activities of IL-1a-stimulated rabbit and human articular cartilage explants.
Cytotoxic Alkaloids from the Wood of Picrasma quassioides

Six alkaloid compounds were isolated from the chloroform soluble fraction of the methanolic extract of the wood of Picrasma quassioides Benn (Simarobaceae) as the cytotoxic components against the tumor cell lines, U937 and HepG2. The compounds were identified as 4,9-dimethoxy-1-vinyl-β-carboline (1), 1-carbomethoxy-β-carboline (2), 4,5-dimethoxycanthin-6-one (3), 5-hydroxy-4-methoxycanthin-6-one (4), 3-methoxycanthin-5,6-dione (5) and 4,8-dimethoxy-1-vinyl-β-carboline (6) by spectroscopic analysis. Among them, compound 5 showed the most potent cytotoxicity in a dose-dependent manner against the two tumor cell lines. This is the first report of the cytotoxic effect of those isolated compounds 1-5 on U937 and HepG2 cell lines.
A Study on localized corrosion susceptibility of the welded STS 304 and 316L in acidic solution

In case of stainless steel, uniform corrosion is not of importance. The increased corrosion resistance, compared to low alloy steel, is explained by the chromium content which affects the formation of passivation. However the sensitization could be caused by lack of Cr on the welded area of stainless steel. In this study, therefore, the corrosion resistance characteristics of STS 304 and 316L were estimated to determine the extent of its use in acidic environment (pH 2). From the results, repassivation potentials(Ep) of STS 304 and 316L were below or above to the natural corrosion potential(Ecorr). Therefore, using STS 304 and 316L materials in acidic solution according to the circumstances should be carefully considered.

Aralia cordata Thunb. dose-dependently inhibited MMP-3 and MMP-13 expression and activities from IL-1a-treated cartilage explants cultures when tested at concentrations ranging from 0.02 to 0.2 mg/ml. Aralia cordata Thunb. had no harmful effect on chondrocytes viability or cartilage morphology in cartilage explants. Histological analysis indicated that Aralia cordata Thunb. reduced the degradation of the cartilage matrix compared with that of IL -1a-treated cartilage explants.

A Review of Pharmacopuncture Treatment for Frozen Shoulder: A Literature Review of Clinical Trials

The objective of this study was to review clinical trials of pharmacopuncture treatment for Frozen Shoulder and to evaluate trends in this field. The literature search was performed using PubMed, the Cochrane Library, China National Knowledge Infrastructure, and 5 Korean electronic databases. A combination of "pharmacopuncture," "acupoint injection," "Frozen Shoulder," "adhesive capsulitis," and "periarthritis of shoulder" search terms were used. A total of 9 studies were included in this review. The studies were classified into herbal extract-based (5 types) and animal-based (2 types) pharmacopuncture treatment of Frozen Shoulder. There were 14 different acupoints and Ashi points used in the 9 studies. The total volume of herbal extract-based pharmacopuncture injected was usually between 2 mL and 4 mL, and for animal-based pharmacopuncture it was 1 ml. In most studies of Frozen Shoulder, pain was reduced and function was significantly improved after pharmacopuncture treatment. These results demonstrate that pharmacopuncture alleviates pain and restores function in patients with Frozen Shoulder. Further studies must be conducted on pharmacopuncture for management of Frozen Shoulder.
