        A Case of Reactive Eccrine Syringofibroadenoma

        Eccrine syringofibroadenoma is a rare adnexal tumor of eccrine ductal differentiation with variable clinical features. It manifests as either a solitary or multiple papules and nodules arranged in a symmetrical or linear pattern. The anatomical distribution is wide and includes the face, trunk,extremities, and rarely the nails. Histopathological findings show anastomosing cords and strands of uniform cuboidal cells surrounded by fibrovascular stroma. Herein, we report a case of reactive eccrine syringofibroadenoma which developed on the foot of a 37-year-old woman, after self-paring of tissue and subsequent infection and ulceration.         Systemic Contact Dermatitis from Propolis Ingestion

        Propolis, also known as bee glue, is a substance collected by worker bees and it is used as a material for constructing and maintaining their beehives. It has been used topically and orally by humans for its anti-inflammatory properties. However, the growing use of propolis has been paralleled by reports of allergic contact dermatitis as a reaction to the substance. Contact dermatitis with generalized cutaneous manifestations elicited by propolis ingestion has not been previously reported. Here we report on the first case of systemic contact dermatitis from propolis ingestion in a 36-year-old woman. (Ann Dermatol 23(1) 85∼88, 2011)

        <P>Dendritic cells (DCs) are potent antigen-presenting cells with high sentinel ability to scan their neighborhood and to initiate an adaptive immune response. Whereas chemotactic migration of mature DCs (mDCs) towards lymph nodes is relatively well documented, the migratory behavior of immature DCs (imDCs) in tumor microenvironments is still poorly understood. Here, microfluidic systems of various geometries, including mazes, are used to investigate how the physical and chemical microenvironment influences the migration pattern of imDCs. Under proper degree of confinement, the imDCs are preferentially recruited towards cancer <I>vs.</I> normal cells, accompanied by increased cell speed and persistence. Furthermore, a systematic screen of cytokines, reveals that Gas6 is a major chemokine responsible for the chemotactic preference. These results and the accompanying theoretical model suggest that imDC migration in complex tissue environments is tuned by a proper balance between the strength of the chemical gradients and the degree of spatial confinement.</P>

        어육장의 미생물학적 특성

        예로부터 전통 장류는 먹거리로 많이 이용되어 왔으며, 최근에는 장류의 다양한 영양 성분 및 생리 활성 능력이 밝혀지면서 관심이 증대되고 있다. 본 연구에서는 전통 방식으로 제조된 어육장의 발효에 관여하는 미생물을 숙성 기간별로 분석하고 이를 통해 안전성을 평가하였으며, 이와 함께 국내에서 시판되고 있는 간장의 미생물을 분석하여 어육장의 미생물분석과 비교하였다. 어육장은 2개월 단위로 샘플링하여 분석하였으며, 분리된 미생물은 API kit와 16S rDNA sequencing을 이용하여 동정하였다. 어육장의 발효에 전반적으로 관여하는 미생물은 Bacillus 균류와 Saccharomyces cerevisiae였으며, 숙성 초기에는 Aspergillus flavus가 발효에 관여하는 것으로 확인되었다. 어육장은 최종적으로 섭취 전에 달이는 과정을 거치게 되는데 실험결과 달인 후에는 미생물이 존재하지 않았다. 현재 국내에서 시판되고 간장의 경우 총 균수가 0-42 CFU/mL였고, 동정결과 B. subtilis, B. licheniformis, B. pumilus로 확인되었다. 분리된 Bacillus 균류에 대하여 독소여부를 분석한 결과 어육장 및 시판 장류에서 검출된 Bacillus 균류는 본 실험에서 분석된 3가지 설사형 독소와 1종의 구토형 독소를 생성하지 않는 균주들인 것으로 확인되었다. Fermented soybean foods are an important component of the Korean diet. Eoyukjang is a type of traditional fermented soybean source. Microbial analysis of eoyukjang was conducted during the fermentation period in this study. Microorganisms isolated from eoyukjang were identified by biochemical tests and 16S rDNA sequencing. 17 different microorganisms, including bacteria, yeast, and fungi were detected in eoyukjang during the fermentation period. Even though Aspergillus participated in the early stage of fermentation of eoyukjang, Bacillus species and Saccharomyces cerevisiae were the major microzymes in eoyukjang throughout the maturation period. Eoyukjang is generally consumed after the boiling of the final sample. Therefore, the final sample of eoyukjang was boiled and analyzed. Our results showed that no vegetative microorganisms survived under the boiling conditions for eoyukjang. Fermented soybean products in the domestic market were also assessed for comparison with the results from eoyukjang. The total cell number of kanjang (soy sauce) samples was between 0 to 42 CFU/mL. The isolated microorganisms were identified as Bacillus species. All Bacillus isolates were not found to harbor the three enterotoxin-producing and emetic toxin-producing genes.

        Genotype–Phenotype Analysis in Pediatric Patients with Distal Renal Tubular Acidosis

        <P>Background/Aims: Primary distal renal tubular acidosis (dRTA) in children is a rare genetic disorder, and three causative mutated genes have been identified: SLC4A1, ATP6V1B1, and ATP6V0A4. We analyzed the prevalence and phenotypic differences of genetic mutations in children with dRTA. Methods: A total of 17 children with dRTA were enrolled in the study. All patients underwent genetic testing for all three candidate genes. Results: Pathogenic mutations, including six novel mutations, were detected in 15 (88.2%) patients: dominant SLC4A1 mutations in ten (58.8%) patients, recessive ATP6V0A4 mutations in three (17.6%) patients, and recessive ATP6V1B1 mutations in two (11.8%) patients. Compared to other patients, patients with SLC4A1 mutations showed an older age of onset (3.7 +/- 2.6 years) and less severe metabolic acidosis at initial presentation. All patients developed nephrocalcinosis, and sensorineural hearing loss was observed in two patients with ATP6V1B1 mutations. Three (17.6%) patients had decreased renal function (chronic kidney disease stage 2), and five (29.4%) patients had persistent growth retardation at the last follow-up. Long-term prognosis showed no genotype-phenotype correlation. Conclusions: SLC4A1 is the most common defective gene in Korean children with dRTA. Patients with SLC4A1 mutations show later onset and milder disease severity.         꿀벌에 대한 중요성이 인식되고 도시양봉의 수요가 증가하는 가운데 도시양봉에 최적화된 벌통을 제작하였 다. 꿀벌과 도시양봉에 대한 설문를 진행한 결과, 분봉으로 인한 민원이 도시양봉을 하는 데 가장 큰 저해요소로 꼽혔다. 이에 분봉을 방지하여 시민과 어우러질 수 있으며 친환경적인 벌통을 제작하는 데 초점을 맞췄다. 분봉을 방지하기 위해서는 여왕벌의 이탈을 감지하는 자성 센서와 벌들이 분봉을 하고자 하는 욕구인 분봉열을 감지하 는 온도 센서를 설치하여 효율을 파악하였다. 더불어 최근 이상기후로 인한 잦은 폭우와 고열을 견디기 위하여 밀랍을 이용한 벌통 코팅을 진행하였다. 벌통의 바닥에는 트레이를 서랍처럼 분리 설치할 수 있도록 함으로써 무더운 기온에서는 손쉽게 트레이를 빼서 환기에 중점을 두고, 그 외의 조건에서는 트레이를 끼워둠으로써 바닥 에 떨어진 꿀벌의 사체를 주기적으로 수거하여 유전자 분석을 함으로써 꿀벌의 건강 상태를 점검할 수 있도록 하였다. 꿀벌이 해당 벌통으로 인하여 스트레스를 받는지 꿀벌의 주요 스트레스 마커 유전자의 상대적 발현량을 조사한 결과, 일반적인 벌통에서의 발현량 수준과 큰 차이가 없는 것을 확인할 수 있었다. 이처럼 새로이 디자인된 벌통을 이용한다면 이상기후에서도 도시양봉을 하는 데 기여할 수 있을 것으로 기대된다.

        A Pediatric Case of Inflammatory Bowel Disease with Renal Amyloidosis

        Amyloidosis is a rare disease that results from the deposition of extracellular protein in various body tissues, causing progressive organ dysfunction. Secondary renal amyloidosis is a rare but serious complication of chronic inflammatory bowel disease, particularly in patients with Crohn’s disease or ulcerative colitis. We report a case of secondary renal amyloidosis in a pediatric patient who reported a 16- year history of “very early onset inflammatory bowel disease”. Intensive treatment including repeated infliximab infusions improved clinical parameters of inflammatory bowel disease, although renal dysfunction showed progression. Amyloidosis should be considered in patients with IBD, particularly if they suffered disease progression.

        Case Reports : Giant Vascular Eccrine Spiradenoma

        Giant vascular eccrine spiradenomas (GVESs) are a rare variant of the eccrine spiradenoma that develops from the sweat gland. It is different from the eccrine spiradenoma in its larger size and greater degree of vascularity. Bleeding and/or ulceration are common clinical features of this tumor, and are the reason why it is often clinically confused with a vascular or malignant tumor. Here, a rare case of GVES without bleeding or ulceration is reported. (Ann Dermatol 23(S2) S197~S200, 2011)
