The Efficacy of the Combined Procedure in Involutional Entropion Surgery: A Comparative Study

Purpose: To evaluate the efficacy of the combined procedure in the management of involutional entropion. Methods: In this study, we reviewed 45 eyes of 36 patients who underwent the combined procedure (lateral tarsalstrip, retractor tightening, and everting sutures) for the management of involutional lower eyelid entropionand compared the results with 31 eyes of 25 patients who underwent the Wies procedure. Exclusion criteriaincluded previous lower eyelid surgery and follow-up of less than 6 months. Results: No patients demonstrated entropion on the first postoperative day. The mean follow-up period was18.4 months (6 to 52 months) in the Wies group and 22.6 months (6 to 59 months) in the combined proceduregroup. During the follow-up period, 9 of 31 eyes in the Wies group presented with recurrence and only 1 of 45eyes in the combined procedure group presented with recurrence (p = 0.001). The average time of recurrencewas 4.8 months in the Wies group. Recurrence occurred at 2 months postoperatively in the patient in the combinedprocedure group. Six of the 9 recurrences in the Wies group were managed by the combined procedure. None of these patients had further recurrence after correction. Three patients complained about a visibleincision line after the Wies procedure. Conclusions: The combined procedure seems to be more effective than the Wies procedure in the managementof involutional entropion. The combined procedure addresses the three major causative factors in involutionalentropion and makes it possible to perform the surgery using a small incision.
Throughput and Delay Optimal Scheduling in Cognitive Radio Networks under Interference Temperature

The xed spectrum assignment policy in today's wireless networks leads to inefcient spectrum usage. Cognitive radio network is a new communication paradigm that enables the unlicensed users to opportunistically use the spatio-temporally unoccupied portions of the spectrum, and hence realizing a dynamic spectrum access (DSA) methodology. Interference temperature model proposed by Federal Communications Commission (FCC) permits the unlicensed users to utilize the licensed frequencies simultaneously with the primary users provided that they adhere to the interference temperature constraints. In this paper, we formulate two NP-hard optimal scheduling methods that meet the interference temperature constraints for cognitive radio networks. The rst one maximizes the network throughput, whereas the second one minimizes the scheduling delay. Furthermore, we also propose suboptimal schedulers with linear complexity, referred to as maximum frequency selection (MFS) and probabilistic frequency selection (PFS). We simulate the throughput and delay performance of the optimal as well as the suboptimal schedulers for varying number of cognitive nodes, number of primary neighbors for each cognitive node, and interference temperature limits for the frequencies. We also evaluate the performance of our proposed schedulers under both additive white gaussian noise (AWGN) channels and Gilbert- Elliot fading channels.

This explanatory research uses a mixed method design to investigate the effect of using an online community of practice within teaching practices on science and math pre-service teachers’ teaching self-efficacy beliefs. 102 science and math pre-service teachers participated in the research. “Teaching Self-Efficacy Belief Scale” and “The Questionnaire on Opinions about Using Online Communities of Practice within Teaching Practices” were the data collection tools of the study. Results showed that online communities of practice affected pre-service teachers’ teaching self-efficacy beliefs significantly and positively. Qualitative data of the research also supported this positive change. Science and math pre-service teachers mentioned several benefits of online communities of practice, such as sharing, recognizing common problems, freely expressing themselves, making comparison, realizing their shortcomings, and benefiting from the experiences of others. However, research findings also showed that online communities of practice have some limitations such as the lack of expressions of sincerity, the need for internet access, the need for additional time, and anxiety when it comes to criticism.

Calcinosis cutis is a condition characterized by the deposition of calcium salts in the skin and subcutaneous tissues, and patients suffering from it encounter various connective tissue disorders, such as dermatomyositis (DM),scleroderma, and systemic lupus erythematosus. Although calcinosis cutis is frequently accompanied by juvenile dermatomyositis, rare cases have been reported in adult patients with DM. On the other hand, lichen sclerosus (LS) is a chronic inflammatory disease of the skin and mucosal surfaces. In the present report, we present a rare case of a 71-year-old patient with DM accompanied by ulcerated calcinosis cutis and vulvar LS. (Ann Dermatol 23(S3) S375∼S379, 2011)

Alexithymia and Acne Vulgaris: A Case Control Study

Objective To assess relationship between alexithymia and acne vulgaris in young people. Methods A hundred and eleven subjects between 15 and 25 years of age referred to out-patient clinic of dermatology with acne and 78 subjects applied to family physician for complaints other than acne were included in patient and control groups of the study, respectively. A questionnaire to determine demographic characteristics, an acne classification to determine severity of acne and Toronto Alexithymic Scale (TAS) to assess alexithymia were used. Results The mean scores of TAS were 52.7±10.8 and 51.7±10.7 in patient and control groups, respectively. Alexitymia was determined in 23.4% of the subjects in acne group and in 24.4% of control group. No significant differences were found between groups in terms of alexithymia, intermediate alexitymia and three-factors of TAS. Conclusion Alexithymia does not appear to be related to acne vulgaris.
Evaluation of Managerial Needs for Palliative Care Centers: Perspectives of Medical Directors

Background: Palliative therapies have an important role in increasing the quality of healthcare and in dealing with physical and psychosocial problems due to cancer. We here aimed to evaluate the managerial perspectives and opinions of the hospital managers and clinical directors about specialized palliative care centers. Materials and Methods: This study was conducted in two large-scale hospitals in which oncology care is given with medical directors (n:70). A questionnaire developed by the researchers asking about demographic characteristics and professional experience, opinions and suggestions of medical directors about providing and integrating palliative care into healthcare was used and responses were analyzed. Results: Potential barriers in providing palliative care (PC) and integrating PC into health systems were perceived as institutional by most of the doctors (97%) and nurses (96%). Social barriers were reported by 54% of doctors and 82% of nurses. Barriers due to interest and knowledge of health professionals about PC were reported by 76% of doctors and 75% of nurses. Among encouragement ideas to provide PC were dealing with staff educational needs (72%), improved working conditions (77%) and establishing a special PC unit (49)%. An independent PC unit was suggested by 27.7% of participants and there was no difference between the hospitals. To overcome the barriers for integration of PC into health systems, providing education for health professionals and patient relatives, raising awareness in society, financial arrangements and providing infrastructure were suggested. The necessity for planning and programming were emphasized. Conclusions: In our study, the opinions and perspectives of hospital managers and clinical directors were similar to current approaches. Managerial needs for treating cancer in efficient cancer centers, increasing the capacity of health professionals to provide care in every stage of cancer, effective education planning and patient care management were emphasized.
