        보조기를 이용한 족관절 외과 골절의 비수술적 치료

        목적: 이번 연구의 목적은 보조기를 이용하여 최소 전위된 족관절 외과 골절의 보존적 치료 결과를 알아보는 것이다. 대상 및 방법: 11명 (11예)의 환자들이 수상 후 8주간 전 체중 부하 보행으로 보존적 치료를 받았다. 적응증으로는 3㎜ 이하의 골절 전위, 내과 부분의 동통과 부종이 없거나 있어도 미약한 경우, 거골의 외측 전위가 없는 경우로 삼았다. 환자들은 AOFAS (the American Orthopedic Foor and Ankle Society) Ankle-Hindfoot Scale에 의해 평가받았다. 결과: 평균 103주 (36~192주)의 추시 관찰을 하였으며, 전례에서 발목 관절의 운동 범위는 정상이었다. 평균 AOFAS Ankle-Hindfoot Scale 점수는 95점이었다. 결론: 보조기를 이용한 보존적 치료의 장점은 조기에 전 체중 부하 보행을 하며 일상생활 및 사회생활을 영위할 수 있다는 것, 환자의 편안함, 그리고 재활 치료 기간의 단축이었다. 그러므로 최소 전위된 족관절 외과 골절에 보조기를 이용한 보존적 치료는 권할만하다. Purpose: The purpose of this study was to evaluate the outcome of conservative treatment for minimal displaced lateral mallolar fracture using ankle brace. Materials and Methods: Eleven patients (eleven ankles) underwent conservative treatment with ankle brace for 8 weeks with full weight bearing ambulation. Inclusion criteria were minimal displacement (<3㎜) of fracture, no or mild tenderness or swelling on medial malleolar area and no lateral shift of talus. The patients were evaluated with AOFAS (the American Orthopedic Foot and Ankle society) Ankle-Hindfoot scale. Result: Average follow up was 103 weeks (36~192). All cases had normal range of motion of ankle. The average score of AOFAS Ankle-Hindfoot scale was 95 points. Conclusion: The advantages of conservative treatment with ankle brace were early return to daily activity and work, comfort to the patients and a short period of rehabilitation. Conservative treatment with ankle brace for minimal displaced lateral malleolar fracture is recommended.

      • Implication of Nonalcoholic Fatty Liver Disease, Metabolic Syndrome, and Subclinical Inflammation on Mild Renal Insufficiency

        <P><B>Background</B></P><P> Limited information exists about the impact of nonalcoholic fatty liver disease (NAFLD) on mild renal insufficiency. We compared the relative influence of NAFLD, metabolic syndrome (MetS), and subclinical inflammation, alone or in combination, on mild renal insufficiency. </P><P><B>Methods</B></P><P> This study included 1174 Korean adults. NAFLD was diagnosed using ultrasonography. Mild renal insufficiency was defined as an estimated glomerular filtration rate (eGFR) ≥ 60 and <90 mL/min/1.73 m<SUP>2</SUP>. </P><P><B>Results</B></P><P> In partial correlation analysis, several components of MetS and liver aminotransferase levels, but not high-sensitivity C-reactive protein (hsCRP), were associated with eGFR. Multivariate logistic regression analysis demonstrated the independent association of NAFLD (<I>P</I> = 0.034) and MetS (<I>P</I> = 0.018) with mild renal insufficiency, but not elevated hsCRP (<I>P</I> = 0.885). Furthermore, NAFLD without the MetS group (odds ratio (95% confidence interval) = 1.56 (1.05–2.34)) or MetS without the NAFLD group (1.82 (1.11–3.00)) was associated with mild renal insufficiency after adjusting for confounding variables. However, individuals with high hsCRP showed no relationship with mild renal insufficiency, irrespective of the existence of NAFLD. </P><P><B>Conclusions</B></P><P> This study demonstrated that NAFLD and MetS are independently associated with mild renal insufficiency, whereas subclinical inflammation did not affect the risk for mild renal insufficiency in Korean adults.</P>

        포장두부의 가공공정에서 미생물 분석 및 안전성 평가

        본 연구는 생산 후 3일 이내인 시판두부의 가공공장별, 두 부유형별 위생실태를 파악하여 두부에 대한 미생물 규격의 설정 가능성을 검토하며, 가공단계별 총생균수와 대장균군을 분석하여 오염 원인을 규명코자 실시되었다. 시판중인 두부의 위생성은 동일유형이더라도 가공공장마다 차이가 있었고 동일공장의 시료 간에도 차이를 보였다. 총생균수가 1.0×10? CFU/g 이상을 보인 일부 포장두부에서는 관능적 부패ㆍ변질 현상이 관찰되었다. 총 148개 두부시료 중 총생균수와 대장균군이 각각 1.0×10? CFU/g 이하와 음성에 적합한 시료는 단순포장두부가 32.0%와 12.0%, 포장가열두부가 86.9%와 83.7%인 것으로 조사되었다. 이러한 가공공장에 따른 위생성 차이는 가열과 열처리사이의 공정인 응고, 압착, 성형, 1차냉각, 내포장공정에서 2차오염 정도의 차이에서 기인하는 것으로 생각되었다. 포장두부의 가공공정 중 미생물이 증가하여 각별한 관리가 요구되는 공정으로는 원료대두의 세척ㆍ침지와 마쇄, 응고 후 압착ㆍ성형, 1차냉각공정이었다. 따라서 보다 위생적인 두부를 가공하기 위해서는 이들 가공공정에서 사용되는 각종 설비나 작업도구, 성형포 및 작업자 등에 대한 세척 및 소독 강화가 절실하게 요구되었다. This study was conducted to investigate microbial distribution in the processing steps and to estimate quality index and shelf life of packaged Tofu (soybean curd). Sanitation and safety of Tofu were analysed in aspects of total viable counts (TVCs) and coliforms. Organoleptic deterioration was observed from some packaged Tofu when their TVCs were over 10? CFU/g. The controlled simply packaged Tofu and sterilized Tofu with TVCs of under 10? CFU/g were 32.0% and 86.9% of the total samples, respectively. Also, the controlled simply packaged Tofu and sterilized Tofu with negative coliforms were 12.0% and 83.7% of the total samples, respectively. TVCs and coliforms increased in some processing steps, which include washing and soaking of raw soybeans, and formation and 1st cooling of packaged Tofu. Increases of TVCs and coliforms in the washing and soaking step were due to contamination from the soaking tank and airborne bacteria, whereas increases of TVCs and coliforms in the grinding step were due to contaminations from the grinder, line and reserving tank. TVCs and coliforms increased in the formation and 1st cooling step of packaged Tofu due to contaminations from filter wools, trays, employee's hands, cooling water, formed products and filter wools.

      • High Performance Control of Reluctance Synchronous Motor with Direct Torque Control

        This paper presents an implementation of efficiency optimization of reluctance syuchronons motor (RSM) using a neural network with a direct torque control. The eqnipment circuit in RSM which consider with iron losses is theoretically analyzed and the optimal current ration between torque current and exiting current analytically derived. For RSM, torqne dynamics can be maintained even with controlling the flux level because a torque is directly proportional to the stator current unlike induction motor. In order to drive RSM at maximum efficiency and good dynamics response, the neural network is used and to achieve complex control algorithms, the TMS320F2812 board is employed as control drivers. The experimental results are presented to validate the applicability of the proposed method. The developed control system show high efficiency and good dynamic response features with 1.0 [kW] RSM having 2.57 ratio of d/q.

        Rotor Fault Detection System for the Inverter Driven Induction Motor using Current Signals

        The induction motor rotor fault diagnosis system using current signals, which are measured using an axis-transformation method, is presented in this paper. In inverter-fed motor drives, unlike line-driven motor drives, the stator currents are rich in harmonics; therefore fault diagnosis using stator current is not trivial. The current signals for rotor fault diagnosis need precise and high resolution information, which means the diagnosis system demands additional hardware such as a low pass filter, high resolution ADC, and encoder, etc. The proposed axis-transformation method with encoder and without encoder is expected to contribute to a low cost fault diagnosis system in inverter-fed motor drives without the need for any additional hardware. In order to confirm the validity of the developed algorithms, various experiments for rotor faults are tested and the line current spectrum of each faulty situation using Park transformation is compared with the results obtained from fast Fourier transforms.

      • Thymosin <i>β</i>4 Reduces H<sub>2</sub>O<sub>2</sub> Induced Oxidative Stress in MC3T3-E1 Cells on Titanium Surface

        <P>Thymosin beta 4 (T beta 4) is known to inhibit an inflammatory response and to increase the survival of osteoblasts on titanium (Ti) surfaces. Ti is the most widely used graft material in dentistry; however, an inflammatory response induced following implant placement results in the generation of reactive oxygen species (ROS). The oxidative stress from the production of ROS such as nitric oxide (NO) and hydrogen peroxide (H2O2) can damage surrounding cells, resulting in implant failure by decreasing cell viability. Thus, the aim of this study was to determine the biological effects of T beta 4 on the oxidative stress induced to MC3T3-E1 preosteoblasts on the Ti surface. Based on an MTT assay and bromodeoxyuridine immunofluorescence staining, T beta 4 was found to increase the proliferation of the H2O2-exposed MC3T3-E1 cells on Ti discs. Reverse transcription-polymerase chain reaction and western blot analyses showed that T beta 4 decreased the mRNA and protein expression levels of inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2) in H2O2-exposed MC3T3-E1 cells on the Ti discs. T beta 4 inhibited the synthesis of intracellular ROS and the secretion of NO and prostaglandin E-2 (PGE(2)) from H2O2-exposed MC3T3-E1 cells on the Ti discs. In conclusion, T beta 4 inhibits H2O2-induced iNOS and COX-2 expression with a decrease in ROS, NO, and PGE(2) synthesis, which leads to improved cell survival with low cytotoxicity under an oxidative stress condition in MC3T3-E1 cells on the Ti surface. This suggests that T beta 4 may be a crucial molecule to reduce oxidative stress-induced cell damage or hypoxia, leading to promoted osseointegration on the Ti surface during implant placement.</P>

      • 근치적 위절제술을 시행한 위암 환자에서 보조요법으로서 5-Fluorouracil, Epirubicin과 5-Epirubicil,Cisplatin의 복합화학요법의 효과 비교

        To compare 5-fluorouracil plus epirubicin (FE) to 5-fluorouracil plus cisplatin (FP) chemotherapy as adjuvant therapy for patients with resected gastric cancer. Between August 1995 and March 2000, 46 patients with completely resected gastric cancer received six courses of FE (5-fluorouracil 1000mg/m2/day, D2-D5, epirubicin 70mg/m2, D1) or FP (5-fluorouracil 1000mg/m2/day, D2-D5, cisplatin 70mg/m2, D1) chemotherapy. The 23 patients were assigned to each treatment group. A total of 127 courses of treatment were given both FE and FP group. The FP group tended to show more risk of overall death rate. But there were no differences between FE and FP groups in terms of overall survival or disease-free survival. Both treatment arms were generally well tolerated to chemotherapy. We concluded to be no significant differences between FE and FP groups in terms of overall survival or disease-free survival. To define the efficacy of adjuvant chemotherapy for gastric adenocarcinoma, further prospective randomized trials with large number of patients should be warranted.

      • Spirosoma rigui sp. nov., isolated from fresh water

        <P>A Gram-negative, yellow-pigmented bacterium capable of gliding motility, designated strain WPCB118(T), was isolated from fresh water collected from the Woopo wetland (Republic of Korea). Cells were rod-shaped and sometimes filamentous. The major fatty acids were iso-C(15 : 0) 2-OH and/or C(16 : 1)omega7c (45.6 %), C(16 : 1)omega5c (18.5 %), iso-C(15 : 0) (9.5 %) and C(16 : 0) (8.8 %). The predominant menaquinone and polar lipid were MK-7 and phosphatidylethanolamine, respectively. The DNA G+C content was 53.3 mol%. A phylogenetic tree based on 16S rRNA gene sequences showed that strain WPCB118(T) had an evolutionary lineage within the radiation encompassing the members of the family 'Flexibacteraceae', its closest neighbour being Spirosoma linguale LMG 10896(T) (93.7 % gene sequence similarity). Data from this polyphasic study indicated that strain WPCB118(T) could not be assigned to any recognized species. Strain WPCB118(T) represents a novel species of the genus Spirosoma, for which the name Spirosoma rigui sp. nov. is proposed. The type strain is WPCB118(T) (=KCTC 12531(T)=NBRC 101117(T)).</P>

      • 白血病患者의 果粒白血球에 對한 NBT檢査

        To investigate the morphological and functional changes of the bone marrow, examination of peripheral bolld, bone marrow smear(Wrigth-Giemsa and PAS stain), and histlological findings(H & E, reticutin, and toluidine blue stain of epon-embedded tissue) was performed at 3-day, 1-week, 2-week, 3-week, and 4-week after splenctomy. The results were as follows. 1. In 3-day group, RBC count, hemoglobin, and hematocrit values were markedly decreased and the reticulocyte count was markedly increased at this time but the altered values were almost completely returned to control values at one week after splencetomy. 2. Number of platelets and leukocytes were gradually increased and on differential leukocyte count, percentage of the lymphocytes was also increased gradually during experimental period. 3. On bone marrow smear and histological examination, erythroid precursors was shown one week after splenectomy.

      • <i>Terriglobus aquaticus</i> sp. nov., isolated from an artificial reservoir

        <P>A pink-pigmented, chemo-organotrophic bacterium, designated strain 03SUJ4<SUP>T</SUP>, was isolated from the freshwater of Juam reservoir, Republic of Korea (35° 03′ 43′′ N 127° 14′ 15′′ E). Cells were aerobic, Gram-reaction-negative and non-motile rods. Strain 03SUJ4<SUP>T</SUP> grew at pH 6–7 (optimum, pH 6) and at 15–30 °C (optimum, 25 °C). Phylogenetic analysis based on 16S rRNA gene sequences indicated that the isolate belonged to the genus <I>Terriglobus</I>, showing sequence similarities of 97.09 % and 96.82 % to <I>Terriglobus roseus</I> DSM 18391<SUP>T</SUP> and <I>Terriglobus saanensis</I> SP1PR4<SUP>T</SUP>, respectively. Low <I>rpoB</I> gene sequence similarity with members of the genus <I>Terriglobus</I> and different fingerprints with the repetitive primers BOX, ERIC and REP indicated that the isolate represented a novel species of the genus <I>Terriglobus</I>. The major cellular fatty acids were iso-C<SUB>15 : 0</SUB>, C<SUB>16 : 0</SUB>, C<SUB>20 : 1</SUB>ω9<I>c</I>, C<SUB>14 : 0</SUB> and summed feature 3 (C<SUB>16 : 1</SUB>ω7<I>c</I>/C<SUB>16 : 1</SUB>ω6<I>c</I>). The DNA G+C content of strain 03SUJ4<SUP>T</SUP> was 63.2±0.1 mol% (mean±<SMALL>sd</SMALL> of three determinations). The predominant menaquinone was MK-8. The major polar lipids were phosphatidylethanolamine, diphosphatidylglycerol, phosphatidylglycerol and two unidentified phospholipids. Several phenotypic characteristics served to differentiate the novel isolate from recognized members of the genus <I>Terriglobus</I>. On the basis of the evidence presented in this study, a novel species, <I>Terriglobus aquaticus</I> sp. nov. is proposed for strain 03SUJ4<SUP>T</SUP> ( = KCTC 23332<SUP>T</SUP> = JCM 17517<SUP>T</SUP>).</P>
