<P><B>Abstract</B></P> <P><B>Background</B></P> <P>The dopamine receptor D2 receptor (<I>DRD2</I>) gene and lead exposure are both thought to contribute to the pathophysiology of attention-deficit/hyperactivity disorder (ADHD). ADHD is characterized by delay in brain maturation, most prominent in the prefrontal cortex (PFC). The D2 receptor is also mainly located in the PFC, and animal studies show that lead exposure affects the dopaminergic system of the frontal lobe, indicating an overlap in neural correlates of ADHD, <I>DRD2</I>, and lead exposure. We examined the interaction effects of <I>DRD2</I> rs1800497 and lead exposure on the cortical thickness of the frontal lobe in patients with ADHD.</P> <P><B>Methods</B></P> <P>A 1:1 age- and gender-matched sample of 75 participants with ADHD and 75 healthy participants was included in the analysis. The interaction effects of <I>DRD2</I> and lead exposure on the cortical thickness of 12 regions of interest in the frontal lobe were examined by multivariable linear regression analyses.</P> <P><B>Results</B></P> <P>When we investigated the <I>DRD2</I> ×lead effects in the ADHD and HC groups separately, significant <I>DRD2</I> ×lead effects were found in the ADHD group, but not in the healthy control group in multiple ROIs of the frontal lobe. There was a significant negative correlation between the cortical thickness of the right superior frontal gyrus and inattention scores.</P> <P><B>Conclusions</B></P> <P>The present findings demonstrated significant interaction effects of <I>DRD2</I> and lead exposure on the cortical thickness of the frontal lobe in ADHD. Attention-deficit hyperactivity disorder (ADHD) is one of the most common childhood psychiatric conditions. In 2007, the Korean Academy of child and Adolescent Psychiatry developed Korean ADHD practice parameter. Advances in the scientific evidence of ADHD caused practice parameter to be modified and updated. The present guidelines developed by ADHD translational research center summarize current literature for the treatment of ADHD in children and adults. This parameter includes the clinical evaluation for ADHD, comorbid conditions associated with ADHD, clinical feature and course, research on the etiology of the disorder, and psychopharmacological and nonpharmacological treatments for ADHD.

Objectives:The objective of this study was to evaluate the treatment duration and adherence of osmotic-controlled release oral delivery system (OROS) methylphenidate for treatment of attention-deficit hyperactivity disorder (ADHD). Methods:A total of 843 children with ADHD were recruited : 213 children (25.3%) who had previously taken medications for ADHD and 630 drug-naive children (74.7%) were recruited. The dosage was adjusted according to the clinician’s judgment. The primary efficacy endpoint of this study was treatment retention rate, which was estimated at Week 12 and Week 20 using the Kaplan-Meier curve. The Swanson, Nolan and Pelham-IV (SNAP-IV), Clinical Global Impression-Severity (CGI-S), Clinical Global Impression-Improvement, and the side effect rating scale were measured at every visit. Remission rates were presented based on SNAP-IV and CGI-S, respectively. Results:The treatment retention rate at 12 weeks and at 20 weeks was 76.2% and 66.8%, respectively. Divided according to 6?8, 9?11, 12?14 and 15?18 years of age, younger children tended to show a statistically higher treatment retention rate (p=.02). Based on SNAP-IV and CGI scores, children with better response to medication showed tendencies of statistically higher treatment retention rate. The most common adverse events included loss of appetite (7.1%) and insomnia (3.3%). There was no serious adverse event related to the treatment, such as death. Conclusion:The use of OROS methylphenidate for treatment of ADHD was safe and tolerable for children. In this study, lower age and better treatment response showed a statistically significant relationship with higher treatment adherence. Boys showed a trend of high treatment adherence. The treatment adherence at 20 weeks was satisfactory, however, the treatment adherence after 20 weeks showed a sharp decrease. Therefore, treatment persistence for six months after the beginning of ADHD treatment is important. In addition, the positive role of psycho-education for children and parents is necessary for increasing treatment adherence.
Mental Health Assessment of South Korean Adults on Probation

Objective-The association of offender mental illness (including those on probation) with crime has long been the subject of social interest; however, systematic management has been insufficient. The study purpose was to analyze mental illness prevalence, proportion of first offenses and recidivism with the mental illness of those on probation. Methods-A total of 206 adults on probation were divided into groups (first offenders and repeat offenders) and they completed self-report instruments and clinical interviews designed to diagnose mental illness. Results-The mental illness prevalence among those on probation was considerably higher than that of the general population, and having mental illness was related to re-sentencing probation or recidivism. In particular, alcohol use disorder, major depressive episode, manic/hypomanic episode, and antisocial personality disorder were shown to affect recidivism. Conclusion-To achieve the ultimate purpose of probation, social and systematic intervention on a mental health and medical basis may be required.

This is an analysis report of the “Korean Academy of Child and Adolescent Psychiatry (KACAP), Vision 2033 Survey”. The survey questionnaires were developed by the planning department of KACAP and sent to KACAP members from 2012 to 2013. This survey consisted of six categories : membership, academic activity, journal publication, administrative system, fellowship training program, and future planning. The response rate was 40.5%. In addition to multiple choice questions, responders also described their own ideas and suggestions regarding KACAP. The results of this study can be used as evidence for planning the vision 2033 of KACAP.

Objectives: This study investigated quality of life in Korean juvenile inmates with attention-deficit/hyperactivity disorder (ADHD) and the impact of behavioral and emotional problems on quality of life. Methods: In total, 200 inmates were evaluated using the Korean version of the Mini-International Neuropsychiatric Interview (KMINI) and the Korean version of the Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime (K-SADS-PL-K). We extracted the inmates with ADHD and evaluated their quality of life, behavioral problems, and emotional problems with the Pediatric Quality of Life Inventory (PedsQL) and the Korean Youth Self Report (K-YSR) scale. Descriptive statistics, Pearson correlation analysis, and multiple regression analysis were conducted. Results: Among the 200 total inmates, 68 were diagnosed with ADHD by the K-SADS-PL-K. Most of the correlations between PedsQL scores and K-YSR items were significant. Multiple regression analysis showed that PedsQL could be predicted by affective problems (among the DSM-oriented scales of the K-YSR) and attention problems (among the syndrome scales of the K-YSR). Conclusion: Our results demonstrate that, among juvenile inmates with ADHD, quality of life was negatively correlated with most behavioral and emotional problems. Meanwhile, the significant influence of affective and attention problems on inmates’ quality of life suggests the necessity of comprehensive treatments for this group.

Many child actors have appeared in various movies as the Korean film industry continues to evolve. As more children appear in violent and raunchy scenes, there are more concerns about the movie’s effect on child actors. In some Western countries, many strategies have been developed for child actors, but for the Korean movie industry, the conditions are still poor for them. Although children who enter the concrete operational period are able to think logically and systematically, they are yet limited by their experiences. Adolescents in the formal operational period try to deal with all of the possibilities and assumptions logically and systematically with freedom from realistic contents and experiences. This period is very important because adolescents become more sensitive to others’ feelings and they should develop their ego identity. Several studies have reported the indirect experiences through media including how the movie affected children and adolescents negatively. Depending on the individual’s morality, judgment and emotional status, these effects were variable and inconsistent and could be relieved by several interventions. We could anticipate much bigger emotional effect on child actors who are acting directly and then are confronting themselves in the scene. Therefore, we suggest that the emotional effects of the movies on child actors can be managed properly by considering children’s cognitive ability and emotional status, and establishing protective strategies before they are exposed to problematic scenes. Of course, it should be followed by evaluating them after the exposure and with follow-up management, if necessary.

Objectives: The purpose of this study is to analyze the co-author networks in the Journal of the Korean Academy of Child and Adolescent Psychiatry, a representative journal published by a branch of the domestic psychiatric academy, in order to present the current state of the co-authoring of and developments in child and adolescent psychiatry. Methods: We visualized and estimated the basic characteristics of the co-author networks shown by 564 authors who wrote 251 papers published in the Journal of the Korean Academy of Child and Adolescent Psychiatry between 2005 and 2015, in order to assess their network characteristics, author centrality, and relevance to research performance. Results: The co-author networks in the Journal of the Korean Academy of Child and Adolescent Psychiatry showed the characteristics of a small world and scale-free network. There was a correlation between the author centrality within the network and the research performance of the authors, but less correlation was shown between the centrality and mean paper citation counts. Conclusion: The network structure in the Journal of the Korean Academy of Child and Adolescent Psychiatry showed similarity to the co-authoring of other branches. However, given that the mean paper citation counts were less correlated with the author centrality than those in other branches, it may be necessary to promote an increase in the mean paper citation counts.
