        Effect of Mandibular Advancement Splint on Obstructive Sleep Apnea with Insulin Resistant Diabetes

        Objectives: Obstructive Sleep apnea (OSA) is characterized by complete or partial obstruction of upper airflow despite the effort to breathe, leading to hypoxemia and hypercapnia. The resultant apnea causes sleep fragmentation, which in turn increases sympathetic activity, decreases insulin sensitivity and glucose uptake, and stimulates hepatic gluconeogenesis that ultimately leads to type 2 diabetes. Most studies exploring the effect of continuous positive airway pressure on insulin sensitivity have showed a positive effect. However, there is no evidence on the effect of mandibular advancement device on insulin resistance (IR). This study was aimed to evaluate the effect of mandibular advancement splint (MAS) on IR in patients with OSA. Methods: The present study was conducted at Department of Prosthodontics, Dental College Azamgarh, from June 2015 to July 2017. Sixty eight dentulous patients with type 2 diabetes and mild to moderate OSA and with stable diabetic regimen were included in the study. A MAS was fabricated and fixed at 70% of the maximum mandibular protrusion recorded. Patients that were comfortable with MAS after one month were assessed for apnea-hypopnea index, mean oxygen saturation, and IR at baseline, 6 months, and 1 year after wearing MAS. Results: An improvement in insulin sensitivity was observed at 6 months for mild OSA patients (p=0.001). For moderate OSA patients, no significant improvement was observed following MAS use (p>0.05). Conclusions: The finding suggested that MAS is effective in improving IR in mild OSA patients.

        Photo Physical Studies of PVP Arrested ZnS Quantum Dots

        Monodispersed polyvinylpyrrolidone (PVP) arrested ZnS quantumdots (QDs) having diameter in range ~2-5 nm are synthesized by acolloidal precipitation method using PVP as the stabilizing agent. X-ray diffraction (XRD), transmission electron microscopy (TEM),high resolution transmission electron microscopy (HRTEM),selective area electron diffraction (SAED) and Fourier transforminfrared (FT-IR) spectroscopy are probed to investigate thestructural information. The optical properties are studied usingdiffuse UV-visible reflectance and photoluminescence (PL)spectroscopy techniques. TEM images as well as XRD reflectionpeak broadening indicate the nanometer size particles formationwith cubic (sphalerite) phase within the polymer matrix. Opticalabsorbance studies reveal an excitonic peak at around ~310 nmdictates the effect of quantum confinement effect in the ZnS QDs. PL emission spectra for ZnS QDs in PVP exhibit four emissionpeaks at ~382 nm, ~414 nm, ~480 nm and ~527 nm are observed. These excitonic emissions from ZnS QDs are caused by theinterstitial sulfur/Zn vacancies and surface states.

      • Breast Cancer Statistics and Prediction Methodology: A Systematic Review and Analysis

        Breast cancer is a menacing cancer, primarily affecting women. Continuous research is going on for detecting breast cancer in the early stage as the possibility of cure in early stages is bright. There are two main objectives of this current study, first establish statistics for breast cancer and second to find methodologies which can be helpful in the early stage detection of the breast cancer based on previous studies. The breast cancer statistics for incidence and mortality of the UK, US, India and Egypt were considered for this study. The finding of this study proved that the overall mortality rates of the UK and US have been improved because of awareness, improved medical technology and screening, but in case of India and Egypt the condition is less positive because of lack of awareness. The methodological findings of this study suggest a combined framework based on data mining and evolutionary algorithms. It provides a strong bridge in improving the classification and detection accuracy of breast cancer data.

        Identifying the Critical Risk Factors for Road Crashes based on Large-Scale Safety Audits in India

        Road transportation is considered one of the most essential factors for the growth of a country. Due to the exponential growth in population and fast urbanization, the existing roadway facilities are facing a huge surge in vehicle growth. But this development is also accompanied by the concerning growth of road crashes. To mitigate road crashes, one needs to identify the potential risk factors involved with them. This requires detailed accident data, which is sometimes difficult to acquire and in addition, there is a high probability of accidents in many places due to various hazardous situations, but there may not be an accident record yet. Road Safety Audit is an alternative approach that can overcome these obstacles. Hence, the present study has attempted to highlight various risk factors involved with road crashes for four different road facilities type, viz. Straight, intersection, curve, and road with culvert and flyovers (RCF) by conducting the Road Safety Audit for 216 black spots. Analytic Hierarchy Process (AHP) was performed on the data collected from the audit process to determine the weightage of involvement of each factor in road crashes and identify the most significant ones.

        A sustainable way for surface functionalisation of PET nonwoven with novel chitosan-cinnamaldehyde cross-linked nanoparticles

        In this study, a novel eco-friendly chitosan-cinnamaldehyde cross-linked nanoparticles (CSN) weresynthesised via a green route and deposited, in alternating combination with sodium tripolyphosphate(TPP), over polyester (PET) nonwoven fabric using layer-by-layer (L-B-L) coating technique. Thesuccessful deposition of a nano-level coating (upto 684.7 nm) of 10.5 bi-layers of CSN and TPP, i.e. (CSN/TPP)10.5, with CSN at the top, was achieved at individualfibre surface as confirmed via scanning electronmicroscopy. Fabric coated with (CSN/TPP)10.5 showed remarkable antibacterial activity against both theGram-positive (Staphylococcus aureus) and Gram-negative (Escherichia coli) bacteria, even with a lowconcentration of the nanoparticles (0.1 w/v). Moreover, the coating material possesses very low toxicityas revealed by the cytotoxicity test. The developed fabric could also adsorb metal ions like Pb2+, Cu2+, Fe3+,Ni2+, Zr4+ and Cd2+, etc. from water. The coating was found to be stable in alkaline and neutral pHenvironments. The slight reduction in the pore dimensions, air permeability and tensile strength of fabricafter coating were insignificant with respect to its potential applications in developing health careproducts, medical and health care textiles and dry or wet purification media.

        Gastric Cancer Presenting as a Krukenberg Tumor at 22 Weeks’ Gestation

        Gastric cancer is rare during pregnancy, and often advanced upon presentation. A Krukenberg tumor presents a diagnostic and therapeutic challenge in the pregnant patient. We present a case of a 38-year-old woman at 22 weeks’ gestation who presented with worsening epigastric pain, and was found to have a left pelvic mass on ultrasound, which was confirmed by magnetic resonance imaging. She went into active labor and delivered a viable infant via vaginal delivery. An exploratory laparotomy revealed a large mass originating from her left ovary and diffuse thickening of the lesser curvature of the stomach. Frozen section investigation revealed the presence of signet cell adenocarcinoma. Subsequent upper endoscopy showed linitis plastica, while biopsy confirmed the presence of adenocarcinoma. In conclusion, the occurrence of gastric cancer in pregnancy is rare despite extremely common symptoms. The management poses a challenge because of the need for early treatment, and the continuation of the pregnancy.

        Interleukin-1B (IL-1B-31 and IL-1B-511) and interleukin-1 receptor antagonist (IL-1Ra) gene polymorphisms in primary immune thrombocytopenia

        Background: Immune thrombocytopenia (ITP) is an immune-mediated disease caused by autoanti-bodies against platelets membrane glycoproteins GPIIb/IIIa and GPIb/IX. The etiology of ITP remains unclear. This study evaluated the association of polymorphisms in interleukin (IL)-1B-31, IL-1B-511, and IL-1Ra with ITP. Methods: Genotyping of IL-1B-31, IL-1B-511, and IL-1Ra was performed in 118 ITP patients and 100 controls by polymerase chain reaction restriction fragment length polymorphism and detection of variable number tandem repeats. Results: Genotype differences in IL-1B-31 and IL-1Ra were significantly associated with ITP. Patients showed a higher frequency of the IL-1B-31 variant allele (T) and a 1.52-fold greater risk of susceptibility to ITP (odds ratio [OR]=1.52, 95% confidence interval [CI]=1.04‒2.22, P=0.034). The frequencies of both homozygous and heterozygous variant geno-types of IL-1B-31 were higher (OR=2.33, 95% CI=1.069‒5.09, P=0.033 and OR=2.044, 95% CI=1.068‒39, P=0.034) among patients and were significantly associated with ITP susceptibility. Both homozygous and heterozygous variant genotypes of IL-1Ra were also more frequent (OR=4.48, 95% CI=1.17‒17.05, P=0.0230 and OR=1.80, 95% CI=1.03‒3.14, P=0.0494) among patients and were associated with ITP risk. IL-1B-31 and IL-1Ra also showed significant association with severe ITP. However, IL-1B-511 was not asso-ciated with ITP. Conclusion: IL-1B-31 and IL-1Ra polymorphisms may significantly impact ITP risk, and they could be associated with disease severity, which may contribute to the pathogenesis of ITP.

        Evaluation of Somaclonal Variation for Genetic Improvement of Patchouli ($Pogostemon$ $patchouli$), an Exclusively Vegetatively Propagated Aromatic Plant

        Patchouli ($Pogostemon$ $patchouli$) is an important, exclusively vegetatively propagated aromatic plant, whose essential oil is widely used in perfumery and cosmetic products. Forty $SC_1$ generation (first generation following $in$ $vitro$ phase) somaclones selected randomly from about 400 somaclones developed from the variety Johore, were multiplied through stem cuttings and evaluated in $SC_2$ and $SC_3$ generations to study the extent of somaclonal variation generated for plant height, herb yield, essential oil content, essential oil yield, and seven constituents of the essential oil. Significant or highly significant somaclonal variation was observed for plant height, herb yield, essential oil content, essential oil yield, and contents of patchouli alcohol, ${\alpha}$-guaiene, ${\alpha}$,${\delta}$-patchoulene, and ${\alpha}$-bulnesene in the essential oil. The number of somaclones significantly superior to the parental variety for plant height, herb yield, essential oil content, and patchouli alcohol content in the essential oil ranged from 8 - 16 and the maximum superiority over the parental variety for these traits ranged from 21 - 79%. Broad-sense heritability estimates of plant height, herb yield, and essential oil content were 0.60 - 0.70 while those of essential oil yield and patchouli alcohol content were 0.44 and 0.47, respectively. Heritability estimates of other studied essential oil constituents were generally low (0.12 - 0.38). A high positive correlation was observed between essential oil yield and herb yield suggesting that selection for herb yield would be effective in improving essential oil yield. Patchouli alcohol content in the essential oil was negatively correlated with all the studied traits. Somaclonal variation, heritabilities of traits, and inter-trait correlations are reported for the first time in patchouli.
