Aims: Chronic hepatitis B (CHB) and C (CHC) are leading causes of liver cirrhosis and hepatocellular cancer. Defining of inflammatory activity and severity of disease is important for commencing antiviral treatment and monitoring the patient. Currently noninvasive serological markers to define disease activity are of great interest. In this study we aimed to determine soluble urokinase plasminogen activator receptor (suPAR) levels of patients with CHB and CHC, compare its levels with those of controls and to find if suPAR levels correlated with some laboratory parameters, histologic activity index (HAI) and fibrosis stage. Methods: Twenty-eight patients with CHB, 11 patients with CHC and 41 healthy subjects, eighty subjects in total, were included in the study. suPAR levels of all subjects were determined using commercial ELISA kits according to manufacturer’s instructions. Correlation between suPAR levels and parameters including AST, ALT, leukocyte count, thrombocyte count and liver histology scores were statistically analyzed. Results: Fifty percent of CHB patients were on oral antiviral treatment. Serum suPAR levels of both CHB and CHC patients were higher than controls (p< 0,001). Age, suPAR, HAI, AST, ALT, leukocyte and thrombocyte counts of subjects in different groups (CHB, CHC patients and controls) were shown in table 1. When we divided patients into two groups according to HBV DNA, suPAR levels were not different between patients with HBV DNA >1000 IU/ml and those with HBV DNA<1000 IU/ml. suPAR levels were not found to be correlated with AST, ALT levels, thrombocyte and leukocyte counts or histologic findings. Conclusions: In both CHB and CHC patients, suPAR levels were determined to be higher than healthy controls although suPAR was not found to be correlated with diseae activity or severity related parameters. However new wide scale studies are needed to investigate the relation between suPAR and disease activity or fibrosis stage.

Objective This study compared the effects of cognitive behavioral therapy (CBT), existential psychotherapy (ExP) and supportive counseling (SUP) on facial emotion recognition among mildly and moderately depressed patients. Methods 21 patients for CBT, and 20 each for ExP and SUP groups with 60 healthy controls were investigated. Eight consecutive weekly sessions and following two monthly boosters were performed. Prior to the sessions, all subjects received Sociodemographic Data Form, the Structured Clinical Interview for DSM-IV Axis I Disorders (SCID-1), and Facial Emotion Recognition Test (FERT). Patients received Hamilton Depression Rating Scale (HDRS) and FERT at the onset and after weekly and booster sessions. Results Patients’ ability to recognize surprised and neutral emotions were lower than controls. ExP group improved recognition of almost all emotions, CBT group improved only happy emotions and SUP group did not improve any emotions. HDRS scores declined in all patient groups, ExP and CBT groups had lower scores than SUP. Conclusion MDD patients recognized surprised and neutral emotions lower than controls. ExP improved ability to recognize almost all emotions, CBT improved only happy emotions, SUP did not improve at all. ExP, CBT and SUP all led to a reduction in MDD. ExP and CBT had comparable effects and both were more helpful than SUP.
Fragility Analysis of Atmospheric Storage Tanks by Observational and Analytical Data

In this research, seismic fragility analysis of storage tanks is carried out with a large damage database from past earthquakes and analytical studies. At the fragility analysis, a new damage state has been defi ned. Peak ground acceleration is employed as an intensity measure. Epistemic and aleatory uncertainties are considered. At the observational fragility analysis, logit, probit and cumulative lognormal model and maximum likelihood method are utilized. In this research, Finite Element Analysis is also performed. As a result, new seismic fragility curves for storage tanks obtained and compared with the existing tank fragility curves in the literature.
Endocan and Non-Dipping Circadian Pattern in Newly Diagnosed Essential Hypertension

Background and Objectives: Non-dipper hypertension is frequently accompanied by endothelial dysfunction and activation. Previous studies suggested that endocan may be a novel endothelial dysfunction marker. This study aims to investigate the association between circadian blood pressure (BP) pattern and plasma endocan levels together with high-sensitivity C-reactive protein (hsCRP) in patients with newly diagnosed untreated hypertension. Subjects and Methods: Twenty-four hour ambulatory blood pressure monitoring was recorded in 35 dipper, 35 non-dipper hypertensives and 35 healthy controls. Endocan levels were measured by enzyme-linked immunosorbent assay. Serum levels of hsCRP were also recorded. Results: Despite similar daytime and 24-hour average BP values between dippers and non-dippers, statistically significant high nocturnal BP was accompanied by a non-dipping pattern (Systolic BP: 132±9 vs. 147±11 mmHg; Distolic BP: 80±7 vs. 91±9 mmHg, respectively, p<0.001 for both). Non-dipper patients demonstrated higher endocan levels compared to dippers and normotensives (367 (193-844) pg/mL, 254 (182-512) pg/mL and 237 (141-314) pg/ml, respectively, p<0.001). HsCRP levels were significantly higher in non-dippers than the other groups (p=0.013). In a multivariate logistic regression analysis, endocan (p=0.021) and hsCRP (p=0.044) were independently associated with a non-dipping pattern. Conclusion: Elevated endocan levels were found in non-dipper groups. Endocan and hsCRP were found to be independently associated with a non-dipping pattern. We suggest that elevated levels of endocan in non-dipper hypertensive patients might be associated with a longer duration of exposure to high BP. These results point to the possible future role of endocan in selection of hypertensive patients at higher risk or target organ damage
Combined supraclavicular and superficial cervical plexus block for clavicle surgery

Background: Clavicle fractures occur in 35% of shoulder girdle fractures. Surgical fixation is preferred, especially in young patients for optimal functional outcomes, while nondisplaced fractures are usually treated conservatively. Case: A 38-year-old male patient was admitted to the emergency services with a fracture of the left clavicle following a fall. During the preoperative evaluation, the patient requested to be awake during the surgery. Combined supraclavicular and superficial cervical plexus block was performed under ultrasound guidance without complications and the patient experienced no pain. Conclusions: This technique may avoid possible complications related to interscalene brachial plexus block. Future studies are required to confirm the safety and efficacy of this approach.
