『상한론(傷寒論)』의 의학연구방식에 대한 문헌학적 고찰

Objectives : ShangHanLun is a clinical guideline, and its core is in the part of Six-meridian Diseases. In recent philological sutdies, independent textual analysis of Shanghanlun is essential to define the scope of research. Methods : By the textual study, I defined the Shanghanlun clinic model. And I researched about medical range of Shanghanlun, and relations between Shanghanlun and JinGuiYaoLue, and between Shanghanlun and Huangdineijing. Results : In six topic sentences, the word '-之爲' means 'a process'. In provisions following topic sentences, the word '者' means 'a person'. So Shanghanlun is describing processing factors of Six-meridian Diseases and related human changes with clinical therapy. In the philological studies, meaning of '傷寒' was possibly just the 3rd provision of the greater yang disease part. Practically Shanghanlun's study range is over the concept of Cold damage today. Additionally Jinguiyaolue deals diseases, but Shanghanlun deals human. Gangpyeong-Shanghanlun's phrases can be separated into five part, and oldest part is independent of Huangdineijing. Conclusions : So we need to set a new and independent clinic model of Shanghanlun to verify it. The Shanghanlun's clinic model has a 3 steps. 1st step is to find 6 type defined symptoms, which act as processing factors of Six-meridian Diseases. 2nd step is to confirm a human changes after a Disease appeared. 3rd step is to treat a patient as a provision suggests. Philologically Shanghanlun's clinic range is not limited by the word '傷寒'. And many concepts in Shanghanlun should be independent of Huangdineijing.

비자 열수 추출물의 항산화 활성 및 뇌신경세포 보호효과 연구

Objectives : This study was designed to estimate the antioxidative and neuroprotective effects of Torreyae Semen hot water extracts (TS). Methods : Torreyae Semen was extracted by hot water for 2 hours with a temperature of 105 degrees. Polyphenols and total flavonoid were measured and LC-MS/MS was used to certificate anticipated antioxidative compounds. The antioxidant activities of TS were measured as scavenging effects of 1,1-Diphenyl-2-picrylhydrazyl (DPPH) and Nitrite Oxides (NO). Cell viability and proliferation rate was measured MTT assay. The toxicities to thymocytes and splenocytes were evaluated by the proliferation rate of primary cultured cells of 7 weeks, male Balb/c mice. The antioxidant activities of TS on C6 mouse glioma cells were measured by the analysis of total glutathione contents variation. The neuroprotective effects against oxidative stresses were measured by MTT assay. Results : Polyphenols of TS was $92.00{\pm}1.24{\mu}g/mg$, and total flavonoids was $0.36{\pm}0.14{\mu}g/mg$. TS includes gallocatechin, epigallocatechin, gallocatechin gallate and epigallocatechin gallate. TS included gallocatechin, epigallocatechin, gallocatechin gallate, epigallocatechin gallate. TS showed DPPH and NO scavenging effects as dose-dependent manner at the concentrations of $0-10mg/m{\ell}$. In MTT assay, TS shows no significant toxicity to C6 cells, primary cultured thymocytes and splenocytes of Balb/c mice. TS increased the level of total glutathiones. TS increased cell viabilities of C6 cells against oxidative stresses such as $H_2O_2$, sodium nitroprusside (SNP), Rotenone at the concentrations of $0-0.063mg/m{\ell}$. Conclusions : TS shows the antioxidant and neuroprotecitive effects in these experiments.

8년 동안 진행된 만성치열(Chronic anal fissure) 환자에게 치자시탕과 상담요법을 사용하여 호전된 치험 1례

Objectives : This study is aimed to report the outcomes of the treatment of a patient with chronic anal fissure during a 8-year period. Methods : Basic patient’s data was analyzed as a CARE guideline. And related psychological state was measured with BAI, STAI-X-II test. Results : The treatment is based on Shanghanlun and the patient reported better outcomes. The main symptoms of the patient were chronic constipation, blood in the stool. The main clinical findings were that the patient’s symptoms got worse during the examination period because of the anxiety of the exam. And loss of appetite and digestive disorders are due to Jue-Yin-Bing(厥陰病) and the clue of bleeding can be found in article 375 in Shanghanlun. The main diagnoses was chronic anal fissure and main interventions were Chijasi-tang according to article 375 in Shanghanlun and counseling on anxiety. As a results bleeding has stopped after taking herbal medicine and constipation has been treated at the same time. After 6 months, the patient performed BAI, STAI-X-II test and scores got lowered indicating decreasing anxiety level. Conclusion : Chijasi-tang with counseling achieved valid positive results to the patient with chronic anal fissure.

논문(論文) : 『강평상한론(康平傷寒論)』내 "상한(傷寒)" 개념의 다양성에 대한 고찰

Objectives : Usually medical terminology of oriental mecidine has a multiple meaning. But concept of Shanghan(傷寒) should be simple, because Shanghanlun(傷寒論) is a clinical guideline book. So I researched to suggest many concept of Shanghan, which are suitable for each chapter of Shanghanlun. Methods : I enumerated provisions including Shnaghan from the original texts of Gangpyeong-Shanghanlun(康平傷寒論). And I translated and reviewed them. Results : 1. Shanghan of Preface(序文) means a disease of high fatality. 2. Shanghan of Shanghanrye(傷寒例) means diseases due to physical damage of cold weather. 3. Shanghan of Diagnosis of Daeyang Disease(辨大陽病) - Neck stiffness(痙), Dampness(濕), Sun stroke(□) means certain disease names accompanying fever, chill. 4. Shanghan used in Diagnosis of Diseases is a premise of many provisions of Shanghanlun. And Shanghan is made up of finished fever, expected fever, chill, body pain, loss of appetite, image of tension. Conclusions : We can use a appropriate translation on Shanghan of each chapter of Gangpyeong-Sanghanlun. Especially Shanghan used in 「Diagnosis of Diseases」should have more accurate meaning.

Objectives : The purpose of this paper is to find out the safe dose and clinical use for administration of Glycyrrhizae Radix in Shanghanlun(傷寒論). Methods : Web-databases(OASIS, NDSL, Pubmed, Google) were searched with keywords including 'Licorice', 'Pseudoaldosteronism', 'Glycyrrhizin', 'Testosterone' on 14/10/2016. The searched about 40 papers and books were reveiwed. Results : Glycyrrhizin(GL) and 3-monoglucuronyl glycyrrhetinic acid(3MGA) in Glycyrrhizae Radix are found to be the main compounds vulnerable for inducing pseudoaldosteronism. The dose range of Glycyrrhizae Radix in Shanghanlun prescriptions is from 0.25 g to 12 g as a daily administration, and this dose satisfies the guidelines of WHO, European Union, ABC etc. And risk factors contributing for personal sensitivities are old age(>60), female sex, liver dysfunction, hypokalemia, prolonged gastrointestinal transit time, anorexia nervosa, decreased 11-${\ss}$-hydroxysteroid dehydrogenase-2 activity and hypertension. Conclusions : As a result, dose of Glycyrrhizae Radix in Shanghanlun(傷寒論) is safe. However, the personal sensitivity and unexpected drug interactions are independent from doses of GL, so doctors should monitor those risk factors and symptoms of pseudoaldosteronism when administering Glycyrrhizae Radix.

작약감초탕(芍藥甘草湯)의 연구동향(硏究動向) 분석(分析)

Objectives : Jakyakgamcho-tang has been used to treat muscular tension, spasm and pain. On this study, new clinical uses of Jakyakgamcho-tang were confirmed and the other pharmacological results were summarized by analysing trends of modern experimental studies and clinical studies. Methods : Papers related to Jakyakgamcho-tang were searched on PUBMED, RISS, and National Assembly Library, and classified by categories and effects of clinical application and a variety of experiment around abstract. Also, figures were used for sketching out a timeline of expressing when these papers were read and core research content was described as a digest for identifying research trends. Results : Until today, there are 104 researches about the antispasmodic, analgesic, anti-inflammatory effects of Jakyakgamcho-tang. In total, 91 papers were analyzed in this study. In matter of subject, researches on the antispasmodic effects were 17 cases, researches on the analgesic effects were 14 cases, researches on gynecological therapeutics were 14 cases, researches on pharmacokinetics were 12 cases, researches on the constituent identification were 7 cases, studies about constituents efficacy were 6 cases, studies about interactions with drugs were 6 cases. Conclusion : The traditional use of Jakyakgamcho-tang on muscle tension, spasm and pain has proven to have effects through the experimental evidences. Other effects of Jakyakgamcho-tang such as gynecological therapeutics, anti-inflammatory effects, are now emerging as a new area in clinical use. Futhermore as an auxiliary treatment, Jakyakgamcho-tang is a good alternative for muscle cramps during maintenance of hemodialysis, myalgia or arthralgia induced by anticancer drugs, amenorrhea induced by neuroleptics.

상한론에 근거하여 소화기 이상을 동반한 지루성 피부염 환자에 대한 적석지우여량탕 치험 1례

objective The purpose of this case report is to review the possibility of utilizing Jeokseokji-uyeoryang-tang as a therapy of seborrheic dermatitis, and to consider clinical using possibility of Shanghanlun.method Patient's Diagnosis and progress of seborrheic dermatitis was evaluated based on the patient's chart and picture. And diagnosis based on Shanghanlun was organized by table.results When analyzing the patient's history and symptoms, it can be found in article No. 159 in Shanghanlun. The area and intentsiy of erythema were reduced when viewed photos findings, so diagnosis and therapies used in this case showed valid results in seborrheic dermatitis.conclusion Jeokseokji-uyeoryang-tang is efficient to patient of this case report as a treatment.

천마의 근경과 지상부의 성분 및 항산화 활성 연구

Objectives : This study was carried out to know the necessity of freezing and boiling process of Gastrodiae Rhizoma. Also we need to evaluate Gastrodia elata floral axis as a product ingridients. Methods : Frozen Gastrodiae Rhizoma (GF1, GF2) and Gastrodia elata floral axis (GFA) were prepared. They were divided into samples (GF1 : frozen at the freezer, GF2; frozen and boiled for 10 hours, GFA; dried at 7 0℃ for 120 hours) for experiment. They were extracted using water, freeze dried and powdered. And we analyzed proximate compositions, free sugars, gastrodin, p-hydroxybenzyl alcohol and p-hydroxybenzyl aldehyde content, phenolic and flavonoid, electron donating ability and nitrate scavenging activity and antioxidant activity. Results : In moisture, crude ash, fructose, glucose, sucrose, p-Hydroxybenzyl alcohol GF2 showed lower level than GF1. But GF2 showed higher content than GF1, in crude fat (0.8% > 0.19%), gastrodin (8.84±0.58% > 4.18±0.73%), and p-hydroxybenzyl aldehyde (2.45±0.26% > 2.07±0.16%) content, phenolic (9.98±0.07% > 3.35±0.03%) and flavonoid (3.01±0.06% > 1.09±0.04%) content, electro donating ability (15.21±6.51% > 10.44±4.78%), nitrate scavening activity (20.43±5.30% > 13.62±5.78%). GFA has a relatively lower key indicators component, but has a enough impact on antioxidant effect in phenolic (11.85±0.08%) and flavonoid content (1.45±0.03%), electron donating ability (18.58±9.06%) and nitrate scavenging activity (19.41±9.90%). Conclusions : In the view of proximate compositions, free sugars, functional component and antioxidant activity, the results indicated that boiling process is effective for the frozen Gastrodiae Rhizoma. And Gastrodia elata floral axis has a significantly functional components and antioxidant activity.

『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 대황황련사심탕(大黃黃連瀉心湯) 투여 후 혈압이 안정된 고혈압 증례 보고

Objective : The purpose of this study is to review a case of hypertension. The patient was 41 years, a male. We stabilized blood pressure after administration of Daehuanghuanglianxiexin-tang. In this paper, we review the interpretation of Shanghanlun by analyzing the patient’s progress, diagnosis, treatment. Methods : Blood pressure changes have been monitored during the period. Additionally, We have observed the patients’ compliance and accompanied symptom by the timeline analysis. Results : According to Shanghanlun disease pattern identification diagnostic system, Wediagnosed a Greater yang disease. The patient was treated with Daehuanghuanglianxiexin-tang for 2 months. Daehuanghuanglianxiexin-tang was able to control blood pressure to under 120mmhg / 80mmhg. During the treatment period, the accompanied symptom Stiffness and pain in posterior neck has improved, patient’s compliance was good, and symptoms improved without significant complication Conclusions : Taking of the Daehuanghuanglianxiexin-tang makes patient’s blood pressure controlled to safe and stable range and eases the discomfort of posterior neck pain
