        가미온청음으로 호전된 쇼그렌 증후군 환자 치험 1례

        Objectives: The purpose of this study is to report the clinical effectiveness of Korean medicine, especially Gami-onchung-eum, for treatment of a patient with Sjogren's syndrome. Methods: The patient, diagnosed with Sjogren's syndrome, was suffering from systemic symptoms accompanied by dry mouth, dry eye, and fatigue. We treated her with Korean medicine involving a herbal decoction, acupuncture, moxibustion, and cupping. We used the European League Against Rheumatism $Sj{\ddot{o}}gren^{\prime}s$ Syndrome Patient Reported Index for assessment. Results: Based on the European League Against Rheumatism $Sj{\ddot{o}}gren^{\prime}s$ Syndrome Patient Reported Index, after 28 days of treatment, dry mouth decreased to a score of 5, dry eye decreased to 2, and fatigue decreased to 3. Conclusions: Korean medicine, including Gami-onchung-eum, may be an effective treatment for Sjogren's syndrome.

        야교등의 항산화 및 간보호효과

        Objectives : Polygoni Multiflori Ramulus has been widely used as a traditional medicinal herb for the treatment of insomnia, limb pain and itch. The extract of Polygoni Multiflori Ramulus (PMRE) is known to have a modulatory effect of many inflammatory responses. This study was performed to investigate the hepatoprotective effect of PMRE against arachidonic acid (AA) + iron-induced oxidative stress on HepG2 cell and carbon tetrachloride ($CCl_4$)-induced liver injury on mice. Methods : The effects of PMRE on cell viability was assessed by MTT assay. And flow cytometric analysis was performed to estimate the effects on mitochondrial function. To investigate its underlying mechanism, apoptosis-related proteins were analysed by using immunoblot analysis. In addition, ICR mouse were administrated (po) with the PMRE (30, 100 mg/kg) for 3 days and then, injected (ip) with $CCl_4$ (0.5 ml/kg body weight) to induce acute liver damage. The level of pro-caspase-3 was measured. Results : Treatment of PMRE increased relative cell viability, prevented a cleavage of poly (ADP ribose) polymerase and pro-caspase-3, and also reduced mitochondrial membrane permeability against AA + iron-induced oxidative stress. In addition, PMRE treatment decreased liver injury induced by $CCl_4$, as evidenced by increases in pro-caspase-3 level. Conclusions : These results demonstrate that PMRE has an ability to anti-oxidant and hepatoprotective effect against AA + iron-induced oxidative stress and $CCl_4$-induced liver injury.

        음양곽(淫羊藿) 열수 추출물의 Smad 신호 억제를 통한 간성상세포의 활성 조절

        Objectives : In Traditional Korean Medicine, Epimedium koreanum Nakai has diverse pharmacological activities to treat impotence, forgetfulness, cataract and exophthalmos. Present study investigated anti-fibrogenic effects of E. koreanum water extract (EKE) in hepatic stellate cells (HSCs). Methods : To study anti-fibrogenic effects of EKE, LX-2 cells, a human immortalized HSCs, were pre-treated with $3-300{\mu}g/mL$ of EKE, and then subsequently exposed to 5 ng/mL of transforming growth $factor-{\beta}1$ ($TGF-{\beta}1$). Expression level of ${\alpha}-smooth$ muscle actin was determined by immunoblot analysis. Phosphorylation of Smad, transactivation of Smad, and expression of plasminogen activator inhibitor-1 (PAI-1) were monitored to investigate the effect of EKE on $TGF-{\beta}1-mediated$ signaling pathway. Results : Up to $100{\mu}g/mL$, EKE did not show any cytotoxicity on LX-2 cells. Pre-treatment of EKE ($100{\mu}g/mL$) significantly inhibited ${\alpha}-smooth$ muscle actin expression induced by $TGF-{\beta}1$. In addition, EKE significantly decreased Smad2 and Smad3 phosphorylations, Smad binding element-driven luciferase activity and PAI-1 expression by $TGF-{\beta}1$. Of three flavonoid compounds found in EKE, only quercertin ($30{\mu}M$) attenuated $TGF-{\beta}1-mediated$ PAI-1 expression. Conclusion : These results suggest that EKE has an ability to suppress fibrogenic process in HSCs via inhibition of $TGF-{\beta}1/Smad$ signaling pathway.

        Nuclear factor erythroid 2-related factor 2 활성화를 통한 해간전(解肝煎)의 간세포 보호 효능 및 분자기전을 활용한 해간전(解肝煎) 구성 약물의 최적화 연구

        Objectives : Present study investigated hepatoprotective effect of Haegan-jeon extract (HE) and tried to elucidate molecular mechanism involved. According to molecular mechanism, present study optimized herbal composition of HE (op-HE) and compared in vitro and in vivo hepatoprotective effects of op-HE to HE. Methods : For in vitro experiments, HepG2 cells were exposed to arachidonic acid (AA, $10{\mu}M$) and iron ($5{\mu}M$) for inducing oxidative stress. Cell viability, GSH contents, $H_2O_2$ production, mitochondrial membrane potential, immunoblot and reporter gene assay were performed to investigate cytoprotective effects and responsible molecular mechanisms. For in vivo experiments, hepatoprotective effect of HE and op-HE were assessed on $CCl_4-induced$ liver injury mice model. Results : HE pretreatment prevented AA+iron-mediated hepatocytes apoptosis. In addition, AA+iron-induced mitochondrial dysfunction, $H_2O_2$ production, glutathione depletion were reduced by HE pretreatment. In addition, nuclear factor erythroid 2-related factor 2 (Nrf2) phosphorylation, antioxidant response element (ARE)-driven reporter gene activity, and antioxidant genes expression were increased by HE. Based on reporter gene and MTT assays, we found that op-HE consisting three medicinal herbs also significantly increased transactivation of Nrf2 and reduced the AA+iron-mediated cytotoxicity. Moreover, in $CCl_4-induced$ liver injury mice model, HE-op had an ability to ameliorate $CCl_4-mediated$ increases in serum alanine transferase and aspartate aminotransferase activity, hepatic degeneration, inflammatory cell infiltration, and collagen deposition. Hepatoprotective effects of op-HE were comparable to those of HE. Conclusions : Present study suggests that op-HE as well as HE exhibit hepatoprotective effect against oxidative stress-mediated liver injury via Nrf2 activation.

      • 황련해독탕의 유해미생물에 대한 항균 효과

        Objective : Hwangnyeonhaedok-tang (HHT) has been used to reduce heat caused by the pathogenic substances and to treat influenza, menopausal syndrome, stomatitis, and toothache. In this study, the effects of HHT on the growth of oral bacteria and acne bacteria were evaluated. Methods : HHT was prepared by extracting 160 g of dry sample in 2ℓ of water for 3 h. The antibacterial potential of HHT was determined by the agar diffusion method and minimum inhibitory concentration (MIC) assay. Results : HHT showed strong antibacterial activity against Streptococcus mutan s and Propionibacterium acnes. At the highest amount of HHT tested (2 ㎎/disk), the mean diameter of the clear zone was recorded at 13 ㎜ for S. mutans and 27.5 ㎜ for P. acnes. MIC values for HHT against S. mutans and P. acnes were recorded as 250 and 62.5 ㎍/㎖, respectively. In addition, Phellodendri cortex showed potent antibacterial activity when comparing antimicrobial activity of Scutellariae radix, Coptidis rhizoma and Phellodendri cortex among herbs constituting HHT. Conclusion : The results of the present study suggest that HHT has antibacterial activity against human pathogenic bacterial strains and indicate possible use of HHT as antibacterial agents.

      • 당뇨병성신경병증의 한의학적 치료에 대한 연구 동향 - 국내논문을 중심으로 -

        Objectives : The purpose of this study is to review herbal treatment for diabetic neuropathy by summarizing Korean clinical studies. Methods : The study was based on 8 papers published in Korea since 1997. The papers were searched on six domestic electronic databases including RISS, NDSL, Koreantk, KMBASE, Koreamed and OASIS. Results : 1. The average age of the subjects was 50.6 years. In sexual proportion, male is 72.7% and female is 28.3%. The average duration of diabetic neuropathy was 4.5 years. 2. The most frequent Pattern identification was Eumhu(陰虛). 3. In administration of Korean herbal medicine, UchaShinki-hwan(牛車腎氣丸) was the most frequently used to treat diabetic neuropathy and the next was Yukmijihwang-tang(六味地黃湯), Bogan-tang(補肝湯) and Gamisamul-tang(加味四物湯). 4. The average period of treatment was 43 days and the most frequent used evaluation index was VAS or NRS score. 5. The effects of treatment were evaluated based on VAS, NRS score, FBS, pp2hr and other evaluation indexes. In all studies, VAS or NRS score decreased to 1-3, and FBS, pp2hr and other indicators were significantly more effective than before treatment. Conclusion : To improve the treatment of diabetic neuropathy in Korean herbal medicine, large scale studies will be required to evaluate the authority of Korean medical treatment for diabetic neuropathy.

        인동(忍冬)의 기철(忌鐵) 및 반철(伴鐵) 추출시(抽出時) 추출물(抽出物)의 변화(變化)

        Objectives : In case of herbs decoction, the ceramic or earthware pots was recommended, but not the metals, particularly iron or aluminum, which could cause unknown chemical reactions. In Korean Medical classics, it has been known that some herbs including Caulis Lonicerae Japonicae (CL) were not recommended to boil with iron pot. This study investigates the physical changes of extracts of CL with or without iron. Methods : CL was decocted with reflux cooling extraction system to prevent evaporation and volatilization. Content of polyphenol was detected by Folin-Denis method and the levels of loganin and chlorogenic acid were evaluated by UPLC. Results : The color of extract with glass beads (GB) is yellowish brown, and the iron beads (IB) is blackish brown. Polyphenol and chlorogenic acid levels were reduced in IB extracts. Conclusions : The color of extract was change to blackish brown, and polyphenol and chlorogenic acid levels were reduced in CL extract with iron. Therefore, iron pots is not suitable for CL extraction.

      • 천심련 물 추출물의 간세포보호효과

        Andrographis Herba is used as a traditional herbal medicine in the southeast Asian countries for the treatment of stomatitis, pharyngalgia, fever etc. In this study, we investigated the hepatoprotective effect of water extract of Andrographis Herba (AHE; the lyophilized yield was 11.2%) on tert-butyl hydroperoxide (t-BHP)-induced oxidative stress in human hepatoma cell line HepG2. T-BHP is widely used to induce oxidative stress in a variety of cells. In HepG2, exposure to T-BHP results in excessive ROS generation and depletion of anti-oxidant systems. To investigate the hepatoprotective effect of AHE, cell viability was measured by MTT assay, apoptosis-related proteins, poly ADP ribose polymerase (PARP) and procaspase-3 were detected by immuno-blot assay and then analyzed the expression densities. In addition, the changes of H<sub>2</sub>O<sub>2</sub> production was determined. Pre-treatment with two dosages of AHE (100 and 300 ㎍/㎖) significantly increased the cell viability, PARP and procaspase-3 reduced by t-BHP. In addition AHE also significantly decreased the oxidative stress in t-BHP-nduced HepG2 cells. These results suggest that AHE has hepatoprotective and anti-oxidant effect.

      • RAW264.7 세포에서 우방근(牛蒡根) 에탄올 추출물의 염증 억제 효능 연구

        Objectives:Root of Arctium lappa L. has been used in Korean Traditional Medicine to treat odontalgia, throat pain and dizziness. In the present study, we investigated effect of A. lappa root ethanol extract (ALR_E) on lipopolysaccharide (LPS)- induced acute inflammation in RAW264.7 cells, a murine macrophage cell line. Methods:After root and seed of A. lappa were extracted by using water and ethanol, RAW264.7 cells were pretreated with 30-1000 μg/mL of each extract and then exposed to 1 μg/mL of LPS for 6 or 18 h. Cell viability was determined by MTT assay. Levels of nitric oxide (NO), prostaglandin E2, tumor necrosis factor-α, interleukin-1β, interleukin-6, monocyte chemoattractant protein-1 were measured in the medium. mRNA levels of inducible nitric oxide synthase, cyclooxygenase-2, and pro-inflammatory cytokines genes were determined by real-time PCR analyses. Results:Pretreatment with ethanol extract of seed or root of A. lappa significantly inhibited LPS-stimulated NO production without any cytotoxicity. In addition, ALR_E decreased the mRNA transcription of inducible nitric oxide synthase and cyclooxygenase-2 by LPS. Moreover, ALR_E attenuated LPS- mediated induction of tumor necrosis factor-α, interleukin-1β, interleukin-6, and monocyte chemoattractant protein-1 in RAW264.7 cells. Conclusions:These results suggest that ALR_E has an ability to prevent inflammation in macrophages via inhibiting NO, prostaglandin E2, and proinflammatory cytokines production.
