Purpose: Good results using minimal invasive hallux valgus surgery has been reported recently. We evaluate the usefulness of linear distal metatarsal osteotomy with minimal skin incision in mild and moderate hallux valgus deformity. Materials and Methods: Twenty-eight patients (thirty-one cases) who had mild to moderate hallux valgus deformity and underwent linear distal metatarsal osteotomy using minimal skin incision were evaluated between February 2005 and February 2006. Hallux-metatarsophalangeal-interphalangeal scale of AOFAS (American Orthopaedic Foot and Ankle Society) score was used as clinical evaluation. Preoperative, postoperative, after pin removal, and final follow up plain radiographs were used as radiologic evaluation. Results: Twenty-six cases (83%) among thirty-one cases showed more than average satisfaction, Average AOFAS score were improved from 63.2 points (range 45-74 points) to 86.4 points (range 67-93 points). Preoperative radiologic index of IMA $14.0^{\circ}$ (range $10-18^{\circ}$), HVA $30.2^{\circ}$ (range $19-39^{\circ}$), DMAA $13.8^{\circ}$ (range $5-23^{\circ}$) were improved postoperatively as IMA $8.3^{\circ}$ (range $5-10^{\circ}$), HVA $10.5^{\circ}$ (range $2-20^{\circ}$), DMAA $7.2^{\circ}$ (range $0-14^{\circ}$) correctively. Mean operative time was 15.5 minutes (range 11-18 minutes) and mean operative time was 5.6 days (range 2-8 days). Conclusion: Despite small skin incision and short operative time and admission period, linear distal metatarsal osteotomy with minimal skin incision showed similar results with conventional distal metatarsal osteotomy. Thus, it was thought to be useful operation in mild and moderate hallux valgus deformity.
Al-Li-(Be) 합금 주괴의 미세조직과 기계적 성질

N/A The purpose of this study is to investigate the effect of Be addition on the microstructure and mechanical properties of as-cast and homogenization treated Al-Li-(Be)alloys. The ductility of as-cast Al-Li alloy was increased by the addition of Be and the fracture morphology was changed from brittle to ductile mode. Also, hardness and strength have been decreased by homogenization treatment. The morphology of eutectic structure which consists of α(Al) and α(Be) was changed from lammellae to spherical type by homogenization treatment. The shape of α(Be) phase has been revealed as hollow type by TEM observation. It consists of outer surfaces with well defined crystal facets and the core filled with α(Al). The microstructure of as-cast Al-Li-Be alloys showed coarse δ´, fine δ´, and coarse δ phases. The coarse and fine δ` phases were formed at Be-rich phase /matrix interfaces and in matrix, respectively. By homogenization treatment, the δ phase in Al-Li and Al-Li-Be alloys dissolved and the size of δ phase in Al-Li-Be alloys was finer than that of Al-Li alloy.

Purpose: Nonunion of intra-articular fractures of calcaneus is rarely reported complication. We present our experiences with 4 patients (5 cases) treated operatively for nonunion after intra-articular fracture of calcaneus. Materials and Methods: 4 patients (5 cases) with nonunion of intra-articular fracture of calcaneus after operative treatment were followed for 4 years (from 2002 to 2006). For assessment, the American Orthopedic Foot and Ankle Society (AOFAS) ankle-hindfoot score and Visual Analogue Scale (VAS) were assessed for clinical outcome and the union of fracture site, the talocalcaneal height and the angle of talar declination were determined for radiologic outcome. Results: The mean talocalcaneal height was 6.94 cm (range, 5.9${\sim}$7.6 cm) preoperatively and 7.34 cm (range, 6.0${\sim}$8.3 cm) at last follow-up. The mean angle of talar declination was 5.68 degrees (range, 4.6${\sim}$8 degrees) preoperatively and 13.1 degrees (range, 5.7${\sim}$21 degrees) at last follow-up. The mean preoperative AOFAS score and VAS were 20.4 (range, 14${\sim}$36) and 4 (range, 3${\sim}$6), respectively. At last follow-up, these scores improved to a mean of 59.6 (range, 54${\sim}$68) and 3 (range, 2${\sim}$4), respectively. Unions of previous nonunion site of intra-articular fracture of calcaneus were achieved in all 4 patients (5 cases). Conclusion: The reconstructive procedure for nonunion of intra-articular fracture of calcaneus showed good results in terms of bone union, radiologic results and functional improvement than preoperative state. Because the plantar pain for the inferior angular formation in nonunion site may happen, we will pay attention to reduction of fragment.

Plantar fasciitis is the most common cause of heel pain. The diagnosis of plantar fasciitis is primarily based on the presentation of symptoms and physical examination. Patients usually complain of heel pain at the medial calcaneal tubercle when taking their first step in the morning or when walking after resting. Diagnostic imaging is rarely required for the initial diagnosis of plantar fasciitis; however, it can be used for differential diagnosis. Conservative treatments, such as stretching, rest, ice massage, oral analgesics, foot orthotics, use of night splint, and corticosteroid injection, may be effective. The majority of patients report improvement with conservative treatments, and those who show no response from conservative treatments for a duration of six months or longer can consider extracorporeal shock wave therapy or surgery.

Osteochondral lesion of the talus (OLT) is a broad term used to describe an injury or abnormality of the talar articular cartilage and adjacent bone. Various terms are used to describe this clinical entity, including osteochondritis dissecans, osteochondral fractures, and osteochondral defects. Several treatment options are available; the choice of treatment is based on the type and size of the defect and the treating clinician’s preference. Arthroscopic microfracture (a bone marrow stimulation technique) is a common and effective surgical strategy in patients with small lesions or in those in whom non-operative treatment has failed. This study had the following aims: 1) to review the historical background, etiology, and classification systems of OLT; 2) to describe a systematic approach to arthroscopic bone marrow stimulation for OLT; and 3) to determine the characteristics that are useful for assessing osteochondral lesions, including age, size, type (chondral, subchondral, cystic), stability, displacement, location, and containment of the lesion.

For the purpose of ameliorating of toughness by boundary control, Be was added to Al-Li multiphase alloys. Interface behaviors during ageing were investigated by means of the measurements of hardness and tensile strength, and also the observation of optical and transmission electron micrographs. In order to evaluate the fracture toughness, instrumented Charpy impact test was carried out and fracture surface was observed with a scanning electron micrograph. Grain size was refined by α-Be particles that acted on dragging force at grain boundary in Al-Li-Cu-Be alloy. And PFZ half width in Al-Li-Cu-Be alloy was decreased by early precipitation of T₁ phases compared with Be-free alloy. Double eak phenomenon which was observed at the hardness and tensile tests in Al-Li-Cu-Zr-Be alloy may be estmated by the precipitation of secondary T₁ phases due to the change of δ′, T₁ and θ′ phases stability. And considering toughness and elongation, it is possible to think that the secon peak condition may be the most optimum condition of heat treatment because of the remarkable improvement of elongation in Al-Li-Cu-Zr-Be alloy. In case of Al-Li-Cu-Zr alloy containing minor Be, higher impact absorbed energy than that of Be-free alloy was observed. This remarkable improvement of fracture toughness was attributed to the change of fracture mechanism from brittle to ductile mode. the high energy fracture mode of minor Be added alloy was associated with homogeneous deformation due to various interface controls.
