쯔쯔가무시병은 가을철에 호발하는 열성 질환이다. 저자들은 여름에 중증패혈증 양상을 보인 HLH 환자에서 쯔쯔가무시병을 진단하였으며, doxycycline으로 치료 후 호전을 보인 예를 경험하여 이를 보고하는 바이다. In Korea, scrub typhus usually occurs in October and November. Hemophagocytic lymphohistiocytosis (HLH) is a distinct clinical entity characterized by a high fever, pancytopenia, hepatosplenomegaly, histiocyte proliferation, and hemophagocytosis. We encountered a summertime case of severe scrub typhus presenting as HLH. A 49-year-old female complained of abdominal pain and fever 3 days in duration. On hospital day 3 she was transferred to the intensive care unit because of clinical deterioration accompanied by severe sepsis. As an eschar was evident on the right shoulder, we commenced doxycycline. Her condition improved dramatically and she was discharged on day 14. Although the indirect immunofluorescence antibody test (IFA) for Orientia tsutsugamushi was negative on admission, a repeat IFA test was positive; the antibody titer was 1:5,120 on hospital day 10. Scrub typhus should be considered during differential diagnosis in a patient with severe sepsis in any season except the fall. (Korean J Med 2017;92:321-325)
치과 임플란트 스크루 풀림토크 개선용 DLC 박막 코팅에 관한 연구

In this paper, we studied coatings of the DLC thin film for improving loosening torque of dental implant screw. We used a filtered arc ion plating process which can realize the most dense DLC layer by coating the DLC thin film on the surface of the dental abutment screw. It showed both hardness comparable to diamond and low friction coefficient similar to graphite, and to improve the loosening phenomenon by increasing the screw tightening force Cr/CrN, Ti/TiN or Ti/TiN/Cr/CrN buffer layers were deposited for 5 to 10 minutes to improve the adhesion of the DLC thin film to the surface of the Ti (Gr.5), and then the DLC thin film was coated for about 15 minutes. As a result, the Cr/CrN buffer layer exhibited the highest hardness of 29.7 GPa, the adhesion of 18.62N on average, and a very low coefficient of friction of less than 0.2 as a whole. And we measured loosening torque after one million times with masticatory movement simulator. As a result, the values of the coated screw loosening torque were clearly higher than those of the uncoated screw. From this, it was found that the DLC coating was effective methods improving the loosening torque. In addition, it was confirmed that the cytotoxicity test and cell adhesion test showed high biocompatibility.
광통신용 글라스렌즈 성형 금형의 이형성 코팅에 관한 연구

The Diamond-Like-Carbon (DLC) coating is a new carbon-based amorphous material. Carbon ions in the plasma are electrically accelerated and collide with the substrate to form a thin film. This film has similar properties to diamonds such as high surface hardness, low coefficient of friction, corrosion resistance and durability that do not react with acids and bases. Also, since there is no thermal deformation, it can be printed at room temperature. and coated on almost all materials such as paper, polymer, ceramics and various metals even aspheric lens it is possible to mirror surface coating with excellent surface roughness. In this paper, we have analyzed the DLC film formed by Filtered Arc Ion Plating (Filtered AIP) process.

리튬나이오베이트 결정에 Ti를 열확산시켜 광도파로를 만드는 방법에서 문제가 되는 out-diffusion(삼출) 문제를 해결하는 방안으로 확산과정 이전에 SiO₂ 박막을 시료 위에 입히고 Ti을 열확산시켜 광도파로를 제조하는 방법을 제안하였으며, 기존의 방법에 의해서 만들어진 광도파로와 본 연구의 방법에 의해서 만든 광도파로의 근시야상(mear field pattern)을 비교하였다. We report a new method of fabricating a Ti:LiNbO₃ waveguide with no out-diffusion by coating the waveguide with SiO₂ thin film. It was coated before diffusion process, and the Li₂O out-diffusion was prevented in the diffusion process. We compare the near field patterns of the guided modes between the typical waveguide and the waveguide fabricated by new method proposed here.
1.48 ㎛ 레이저 다이오드로 여기된 Er³+ 첨가 광섬유 광증폭기에 대한 이론적 분석

3준위 레이저 rate equation 및 overlap integral로부터 파장 1.48 ㎛에서 여기된 Er³+ 첨가 광섬유 광증폭기를 위한 광섬유 매개 변수의 최적 조건을 계산하였다. 이 계산으로부터 Er³+ 첨가 광섬유 광증폭기의 소신호 이득(small signal gain) 특성을 개구수 (N.A.), V값, 광섬유 길이, 차단 파장(cutoff wavelength) 등의 함수로 알아 보았으며 또한, 최대 소신호 이득을 갖기 위한 광섬유 매개 변수를 결정하였다. We carried out the useful theoretical calculation for the optimum design of a 1.48 ㎛ diode laser pumped Er³+ doped fiber amplifier. The model we established is based on the rate equations of three level laser system and the overlap integral between fundamental mode LP_(01) and Er³+ doped area.<br/> We determined several fiber parameters (N.A., V value, fiber length, Er³+ concentration, cutoff wavelength etc.) for the optimum design of a high optical gain. We found that our theoretical results are very useful to the design of Er³+ doped fiber used in EDFA.
