      • F-59 Outcome of Regular Inhaled Treatment in GOLD A Chronic Obstructive Pulmonary Disease Patients

        Aims: The 2017 Global Initiative for Chronic Obstructive Lung Disease (GOLD) Report recommends a bronchodilator for all group A patients with chronic obstructive pulmonary disease (COPD). However, evidence in support of this recommendation is lacking. Methods: To determine whether regular inhaled treatment in 2017 GOLD group A patients with COPD improves their health outcomes including exacerbations and symptoms, we recruited patients from two Korean prospective cohorts. Eligible COPD patients had a modified Medical Research Council (mMRC) dyspnea score of less than 2, an St. George’s respiratory questionnaire for COPD (SGRQ-C) total score of less than 25, and had no more than one exacerbation and no hospitalizations during the previous year. Results: After propensity score matching, there were 107 patient pairs, with and without regular treatment. The incidence rates of exacerbations in those with and without regular treatment were not significantly different (incidence rate ratio, 1.24; 95% CI, 0.68-2.25). Significant differences in favor of regular treatment were observed at 6 and 12 months for the SGRQ-C total scores (P = 0.031 and P = 0.025, respectively), and at 12 months for the mMRC score (P = 0.014). Regular bronchodilator monotherapy also resulted in significantly better scores on the SGRQ-C (P = 0.007) and mMRC (P = 0.004) compared with no regular treatment at 12 months of follow-up. Conclusions: Regular inhaled treatment in group A patients with COPD did not reduce the exacerbation rate, but did provide symptomatic benefit. Our findings support the 2017 GOLD.

        Effect of a rapid response system on code rates and in-hospital mortality in medical wards

        Background: To determine the effects of implementing a rapid response system (RRS) on code rates and in-hospital mortality in medical wards. Methods: This retrospective study included adult patients admitted to medical wards at Seoul National University Hospital between July 12, 2016 and March 12, 2018; the sample comprised 4,224 patients admitted 10 months before RRS implementation and 4,168 patients admitted 10 months following RRS implementation. Our RRS only worked during the daytime (7 AM to 7 PM) on weekdays. We compared code rates and in-hospital mortality rates between the preintervention and postintervention groups. Results: There were 62.3 RRS activations per 1,000 admissions. The most common reasons for RRS activation were tachypnea or hypopnea (44%), hypoxia (31%), and tachycardia or brady cardia (21%). Code rates from medical wards during RRS operating times significantly decreased from 3.55 to 0.96 per 1,000 admissions (adjusted odds ratio [aOR], 0.29; 95% confidence interval [CI], 0.10 to 0.87; P=0.028) after RRS implementation. However, code rates from medical wards during RRS nonoperating times did not differ between the preintervention and postintervention groups (2.60 vs. 3.12 per 1,000 admissions; aOR, 1.23; 95% CI, 0.55 to 2.76; P=0.614). In-hospital mortality significantly decreased from 56.3 to 42.7 per 1,000 admissions after RRS implementation (aOR, 0.79; 95% CI, 0.64 to 0.97; P=0.024). Conclusions: Implementation of an RRS was associated with significant reductions in code rates during RRS operating times and in-hospital mortality in medical wards.

        Contents of heavy metals in paddy soils and brown rice of near the Kunsan industrial complex area were investigated. Average total heavy metal contents in paddy soils were Pb 34.33, Cd 0.50, Cu 35.49, and Zn 71.67 ㎎/㎏ respectively. There are no correlation between distances from industrial complex and contents of heavy metals in paddy soils. Average contents of heavy metals in brown rice grown above paddy soil were Pb 1.74, Cd 0.07, Cu 3.54, and Zn 16.53 ㎎/㎏. Relationships between 0.1N HCl soluble heavy metals in paddy soils and contents of brown rice showed positive significant correlations for Pb at 1% level(r=0.6853^(**)) and for Zn at 5% level(r=0.5420^*). Relationships between total contents of heavy metals in paddy soils and contents of brown rice showed only positive significant correlation for Pb at 1% level(r=0.7131^(**)). Contents of heavy metals in paddy soils and brown rice of near the Kunsan industrial complex was polluted partly by the influence of smoke and dust out from industrial complex but not polluted anxious level.

        Contents of heavy metals in sea waters, sediments, fishes, and shellfishes from Kunsan·Changhang coastal areas were investigated. Average contents of heavy metals in sea water were Pb 2.35, Cd 0.52, Cu 2.09, Zn 13.65 and Hg 0.24 ㎍/ℓ. Average contents of heavy metals in sediments were Pb 19.49, Cd 0.15, Cu 9.31, Zn 20.07 ㎎/㎏, and Hg 13.6 ㎍/㎏. Average contents of Pb, Cd, Cu, Zn, and Hg in Acantogobius hasta were 1.08, 0.05, 2.24, 58.14, and 0.151 ㎎/㎏ and those in Mugil cephalus were 1.63, 0.05, 1.51, 32.69, and 0.065 ㎎/㎏. Accumulation ratios of Pb and Cu in Acantogobius hasta were similar to Mugil cephalus but accumulation ratios of Zn and Hg in Acantogobius hasta were higher 2 times than in Mugil cephalus. Average contents of Pb, Cd, Cu, Zn, and Hg in Cycling sinensis were 1.03, 1.23, 6.83, 75.83, and 0.071 ㎎/㎏ and those in Ruditapes philippinarum were 0.93, 0.86, 5.90, 52.16, and 0.074 ㎎/㎏. Accumulation ratios of heavy metals in shellfishes were in the order of Cu$gt;Cd$gt;Zn$gt;Pb$gt;Hg, irrespective of shellfishes kind.
