        일광 노출과 관련된 표피 종양에서 MAPK 발현에 관한 면역조직화학적 연구

        N/A Background : Mitogen-activated protein kinase(MAPK) is an important molecule in transducing extracellular signal from cell surface to the nucleus. MAPK family includes ERK(extracellular signal-regulated protein kinase), JNK(stress-activated c-Jun N-terminal kinase), p38 kinases. Not only various growth factors and cytokines, but also other signals such as UV light are able to activate MAPK, resulting in various cellular responses including proliferation, differentiation and apoptosis. Objective : The purpose of our study was to determine patterns of MAPK expression in epidermal tumors including seborrheic keratosis(SK) on sun-exposed skin and unexposed area, actinic keratosis(AK), Bowen`s disease(BD), acantholytic squamous cell carcinoma(ASCC), and other squamous cell carcinoma(SCC). Methods : Using pan ERK, JNK-2 and p-JNK, we have examined MAPK expression immunohistochemically in epidermal tumors(total 30 cases) including SK on sun-exposed skin(5 cases) and unexposed areas(5 cases), AK(5 cases), BD(5 cases), ASCC(5 cases), and SCC(5 cases). Results : ERK was expressed in AK and ASCC positively but not in BD, and the staining pattern with ERK also showed positivity in microinvasive area and dysplastic cells of SCC. JNK was not expressed in all tumor cells of SK, AK, BD, and ASCC with the exception of dyskeratotic cells in BD and acantholytic cells in ASCC. Conclusions : We suggest that ERK expression may be related to sun exposure and responsible for proliferation of tumor and invasion into surrounding tissue. JNK may not be related with malignant progression of tumor and carcinogenesis cauesd by sun exposure, but with apoptosis of tumor cells. (Korean J Dermatol 2003;41(5) : 578~585)

        멜라닌세포성 모반과 악성 흑색종의 p16과 p21발현에 관한 면역조직화학적 연구

        Background:p16 and p21 belong to the family of cyclin dependent kinase inhibitors, which are important negative regulators of the cell cycle. Loss of normal cell cycle regulation contributes to the development and progression of malignancies. Typical acquired melanocytic nevi appear to follow a life cycle characterized by active growth, maturation, and progressive involution. In contrast, congenital melanocytic nevi have a different course and prognosis with potential for development of melanoma, and metastatic spread is very common in malignant melanoma. Objective:We investigated the expression patterns of p16 and p21 in acquired and congenital melanocytic nevi, and malignant melanoma by immunohistochemical technique. Method:Formalin-fixed and paraffin-embeded tissue from 3 junctional nevi, 5 compound nevi, 7 intradermal nevi, 5 congenital melanocytic nevi, and 5 malignant melanoma were immunolabeled with monoclonal antibodies directed against p16 and p21. Result:The benign melanocytic nevi showed a high expression for p16, and the expression of p21 was low or undetectable in benign melanocytic nevi with the exception of positive staining in junctional nevus, involuting intradermal nevus, and nevus cells in deep dermis of congenital meanocytic nevi. In malignant melanoma, the expression of p21 was higher than p16 expression with variable intensity of staining in different areas of the tumor. These findings suggest that the loss of p16 and expression of p21 seem to be related to tumorigenesis of melanocytic tumors. The expression of p21 which is induced by feedback mechanism in attempt to halt increased cell proliferation, might not be enough to restrict tumor progression and may be involved in the regressive changes of the intradermal nevi.         Papular elastorrhexis is a connective tissue nevus that occurs in the second decade of life as asymptomatic small white-creamy papules scattered over the chest, shoulder, abdomen, or back Histopathologically, there is a decrease in elastic fibers with focal homogenization of collagen. Recognition of this entity is important to prevent use of unnecessary diagnostic procedures and therapy. We present three cases of papular elastorrhexis. All of them occurred in children under the age 10 years. (Korean J Dermatol 1998;36(2): 291-294)

        동맥류성 양성 섬유성 조직구종 1예

        Aneurysmal benign fibrous histiocytoma, which is a rare variant of benign fibrous histiocytoma, usually occurs as a solitary nodule on the limbs of young to middle aged adults. The clinical diagnosis of fibrous histiocytoma is seldom considered in the differential disgnosis, which may include malignant melanoma, hemangioma, neurorfibroma, and nonspecific cyst. Histopathologically, aneurysmal benign fibrous histiocytoma is characterized by the presence of large blood-filled tissue space which is surrounded by histiocyte, fibroblast, hemosiderin pigment, and capillaries. We report a case of aneurysmal benign fibrous histiocytoma in a 15-year-old boy, who had a solitary nodular lesion on the left knee for a 2 year duration.

      • 모반세포성 모반과 악성 흑색종의 CD44 발현에 관한 면역조직화학적 연구

        CD44 is the major human cell surface receptor for hyaluronate and mediate cellmatrix and cell-cell interactions involved in the mechanisms of tumor invasion and metastasis. Typical acquired melanocytic nevi appear to follow a life cycle characterized by active growth, maturation, and progressive involution, In contrast, congenital melanocytic nevi have a different course and prognosis with potential for development of melanoma, and metastatic spread is very common in malignant melanoma. We investigated the expression of CD44 in acquired and congenital melanocytic nevi, and malignant melanoma by immunohistochemical technique. Formalin-fixed and paraffin-embeded tissue from 5 compound nevi, 6 intradermal nevi, 5 congenital melanocytic nevi, and 5 malignant melanoma were immunolabeled with monoclonal antibodies directed against CD44. All acquired melanocytic nevi showed negative or weak staining for CD44. However, all congenital melanocytic nevi exhibited positive staining for CD44 with minimal irregularities according to location. In malignant melanoma, there was variable intensity of staining for CD44 in different areas of the tumor. These findings suggest that the different expression of CD44 in melanocytic nevi may contribute to different biological behavior between acquired and congenital meanocytic nevi. and CD44 seems to be related to metastasis and tumorigenesis of the malignant melanoma.

        Neurofibromatosis is a disorder of varied manifestations primarily affecting the skin, soft tissue, bone and central nervous system. Riccardi recently classified neurofibromatosis into eight categories. One of these types is segmental neurofibromatosis, defined by limitation of neurofibromas or cafe au lait spots to a given region of the body. We report two cases of segmental neurofibromatosis which the lesions were confined to the right T8 to T9 dermatome and right C4 to T2 dermatome respectively. They had no family members with a history of neurofibromatosis.

      • 유육종성 육아종(sarcoidal granuloma) 소견을 보인 피부 결절 2예

        Sarcoidosis is an antigen-mediated disease defined by granuloma formation in different organs. Cutaneous lesions of sarcoidosis may be specific, showing histologically noncaseating granulomas, or nonspecific, showing typically erythema nodosum. If systemic garcoidosis cannot be demonstrated, a long-term follow-up should be undertaken, since some of these patients will develop systemic involvement later in the course of the disease. Some authors classify these lesions as local sarcoidal reactions. However, since skin lesions in the sarcoidal reaction and sarcoidosis do not differ in clinical or histological manifestations, other authors believe that these cases should be accepted as true sarcoidosis limited to the skin. We report two cases of cutaneous sarcoidosis limited to the skin without systemic involvement.

      • 여드름 환자에서 adapalene 겔과 tretinoin 크림의 split-face 비교 연구

        Background : Adapalene is a new synthetic naphtholic acid derivative with potent retinoid and antiinflammatory properties, developed for the topical treatment of acne vulgaris. Objective : This study was designed to compare the efficacy and safety of 0.1% adapalene gel with tretinoin cream(0.025% and 0.05%) in the treatment of grade I to V facial acne vulgaris. Methods : Forty acne patient volunteers were enrolled in this investigatormasked, left-right comparison, randomized, controlled, intraindividual study. 0.1% adapalene gel and tretinoin cream(0.025% or 0.05%) were applied once a day to one halfface by the volunteers for 30 consecutive days. Efficacy and cutaneous tolerance were accessed at baseline and week 1, 2, 3, and 4. Efficacy was determined by investigator counts of inflammatory papules and pustules, and non-inflammatory open and closed comedones, as well as global improvement. Clinical signs(erythema, desquamation, dryness) and subjective symptoms(pruritus, burning) were evaluated and scored weekly. Result : 0.1% Adapalene gel was more effective in treating acne lesions than tretinoin cream(p<0.05). Cutaneous side effect was limited to a mild retinoid dermatitis occurring in all treatment groups. However, patients treated with adapalene gel tolerated this therapy significantly better than those treated with tretinoin cream. Conclusion : 0.1% adapalene gel applied once daily was significantly more effective in reducing acne lesions and was better tolerated than tretinoin cream in the treatment of acne vulgaris.
