대기오염에 따른 천식, 만성폐쇄성 폐질환, 심장질환 환자의 병원 방문 및 입원

Background: Air pollution is an important risk factor for asthma, chronic obstructive pulmonary disease (COPD), and cardiovascular diseases. Air pollution can aggravate the symptoms of asthma, COPD, and cardiovascular diseases. Objective: A retrospective study was undertaken to investigate the relationship between monthly air pollutant concentrations and monthly hospitalizations for selected disease categories in Bucheon, Gyenggi-do. Method: Monthly measurements of particles (PM10), sulfur dioxide, ozone, and nitrogen dioxide (NO2) concentrations were obtained between Jan. 2003 and Aug. 2003 via the metropolitan network of monitoring stations. Hospital morbidity data for asthma, cardiovascular, COPD were obtained and categorized into all ages greater than 10 years. Result: There were significant associations for monthly changes in PM10 concentrations of emergency visits of female asthmatic patients (r=0.715, P=0.046) and outpatients visits of male asthmatic patients (r=0.714, P=0.047). There were significant associations for monthly changes in ozone concentrations and outpatient visits of COPD (r=0.723, P=0.043) and angina pectoris (r=0.802, P=0.017). Changes in NO2, sulfur dioxide concentrations were not significantly associated with any disease outcomes. Conclusion: These data provide useful information on the potential health impact of air pollution on asthma, COPD, cardiovascular diseases, and the control of air pollution is an important issue for prevention of respiratory and cardiovascular diseases. The health impacts of particulate matter (PM) are of global concern, but the implications of the development and exacerbation of bronchial asthma are subject to debate. PM is generated by several sources including vehicle exhaust fumes, the flaring of hydrocarbons at refineries, coal burning at power plants, and thermal treatment of hazardous waste. Recently, the health effects of PM have been documented with increasing frequency due to the greater release of PM from countries such as China. PM is frequently categorized according to mean aerodynamic diameter, with particles ranging in size from coarse (2.5-10 μm in diameter) to fine (< 2.5 μm) to ultrafine (< 0.1 μm). Smaller PM particles are more likely to cause respiratory toxicity and dysfunction due to their propensity to be deposited deep within the lower airways and alveoli. This article briefly reviews the health consequences of PM, with a particular focus on asthma and respiratory disease. (Korean J Med 2015;88:150-155)

후성유전이 천식 및 알레르기 질환에 미치는 영향은 앞으로 많은 연구가 필요하며 이런 연구를 통해서 1) 알레르기를 일으키는 원인인 알러젠, 대기오염, 담배연기, 바이러스 등이 천식에서 후성유전의 변화를 가져올 것인지, 2) 천식의 임상 경과 중 엄마 뱃속에서부터 그 이후로 출생과정, 성장과정 중 어느 과정에 영향을 주는지, 환경요인에 의해 연령에 따라 다른 영향을 주는지, 한 번 생긴 변화가 바뀔 것인지, 3) 후성유전이 유전자와 상호작용은 어떠한지, 4) 후성유전의 변화를 알거나 제어할 수 있는 생체 마커는 무엇인지등에 대한 연구가 이루어져 천식 및 알레르기 질환에 후생 유전이 미치는 영향이 파악되고 이러한 영향을 줄이거나 극복하는 치료제가 나오기를 기대한다. The epigenetics is the study of changes in gene expression or celluar phenotype caused by mechanisms other than changes in the underlying DNA sequence. Epigenetic regulation in asthma and allergic diseases is an interesting area that has obtained a great deal of scientific concern in recent years. Allergic diseases such as asthma and allergic rhinitis in early life are one of the important indicators that the developing immune system is vulnerable to biologic and environmental pollutants. The epigenetic studies are likely to provide clear and deep understandings into how the environment can be modified to drive immune development and change the allergic disease phenotypes. This review includes 1) definition of epigenetics 2) regulation of epigenetics 3) characterization of environmental asthma and allergic diseases triggers that induce epigenetic changes.
