        무증상의 항SSA/SSB 항체 양성 산모에게서 태어난 신생아홍반루푸스 환아 1예

        Neonatal lupus erythematosus (NLE) is a rare autoimmune disease that has a clinical spectrum of cutaneous, cardiac, and systemic abnormalities in neonates. It is caused by transplacental passage of maternal anti-Ro and/or anti-La autoantibodies, which result in skin lesions such as subacute cutaneous lupus erythematosus, congenital heart block, and liver function and hematologic abnormalities. We report a case of NLE in a 31-day-old female infant who was born to a clinically asymptomatic mother with anti-SSA/Ro and anti-SSB/La antibodies. The baby presented with multiple erythematous patches and annular plaques on the face and trunk. The skin biopsy showed slight follicular plugging, focal hydropic degeneration of the basal epidermis and mild edema, telangiectasia, and perivascular and interstitial lymphohistiocytic infiltration in the upper dermis. Her serological tests were positive for antinuclear antibody (ANA), anti-SSA/Ro, and anti-SSB/La. These findings are consistent with NLE. The mother also had a positive autoantibody profile for ANA, anti-SSA/Ro, and anti-SSB/La without clinical symptoms. (Korean J Dermatol 2016;54(6):481∼484)

        전신 스테로이드제 투여 환자에서 빠른 속도로 악화된 카포시육종 1예

        Kaposi’s sarcoma (KS) is a vascular neoplasm originating from vascular and lymphatic endothelial cells. Iatrogenic KS mainly develops in organ transplant patients or after receiving immunosuppressive therapy. An 81-year-old man presented with multiple dark-purplish nodules, plaques, and patches on the right leg for 3 weeks. Previously, the patient was treated with prednisolone 10∼30 mg/day for chronic obstructive pulmonary disease for 3 months, and percutaneous transluminal angioplasty was performed 1 month previously for the treatment of peripheral arterial occlusive disease. A biopsy specimen of the nodule showed closely packed spindle cells forming slit-like vascular structures, which were consistent with KS. Despite the dosage reduction of prednisolone for treatment, the skin lesions progressed aggressively throughout the entire body, and the patient died after 5 months. We report a case of iatrogenic prednisolone-associated KS rapidly progressing to the entire body shortly thereafter. (Korean J Dermatol 2017;55(9):606∼609)

        Association between CD53 genetic polymorphisms and tuberculosis cases

        Background Tetraspanin proteins are expressed in various immune cells, and they play an important role in tuberculosis formation. CD53 is a protein in the tetraspanin family that is expressed in many white blood cells. In particular, it plays an important role in cytokine regulation and interaction between natural killer (NK) cells and antigen-presenting cells (APCs). Objectives The purpose of this study was to investigate whether the single nucleotide polymorphisms (SNPs) difference of CD53 gene could affect TB case. Methods In this study, we investigated the effects of genetic polymorphism of CD53 on the pathogenesis of tuberculosis based on Korean Association Resource (KARE) data. Logistic regression analysis was used to determine the effect of SNPs of the CD53 gene on tuberculosis in TB cases and control groups. We also examined the effect of SNPs on tuberculosis in gene expression. Results Eight SNPs of CD53 were found to be associated with TB case. The SNP showing the greatest significance in this association was rs4839583 (odds ratio = 0.83, 95% confidence interval 0.72–0.96, p = 0.010). These genetic variants might be involved in cytokine regulation through the Jun pathway, and are thought to affect the immune responses and pathogenesis of TB. Discussion CD53 is a type of tetraspanin that is expressed on various immune cells. In this study, we identified eight statistically significant SNPs in CD53 gene, confirming that it could be involved in the regulation of CD53 gene expression. Conclusion Associations between genetic variants and tuberculosis facilitated better understanding of the differences in the incidence of tuberculosis in various populations.

      • 실물크기 모형실을 이용한 벽 일체형 집광장치의 자연채광성능 평가

        Recently, various daylighting systems have been developed and applied to buildings. However, existing daylighting systems are of no practical use due to a high initial cost and difficulties in maintenance. In addition, there is a main problem of securing sufficient additional space for collecting daylight and to be available in the overcast sky. Wall-type daylight system, which is the subject of this study and doesn't need additional space for collecting daylight, is a very useful day lighting system, because it can substitute for a vertical wall of a building. The purpose of this study is to evaluate the day lighting performance of Wall-type Daylight Collecting system using two mock-up models. The performance of the general windows were compared with that of Wall-type Daylight Collecting systems placed on the windows in terms of illuminance distribution on the work plane at the height of 75cm and luminance distribution on the south wall.

        스프링클러설비의 일상적 유지관리 제도 개선방안

        본 연구는 시간 흐름에 따른 스프링클러설비의 일상적 유지관리 제도의 문제점과 개선방안을 제시하기 위해 유지관리의 개념과 제도적 현황을 알아보고, 수도권 내 건축물에 설치된 스프링클러설비의 작동기능점검과 소방안전관리 업무 대행 시 작성한 불량 내용의 분석을 통해 실태조사를 하였다. 이에 따른 개선방안으로 유지관리 지침서제정, 소방안전관리 업무 대행에 관한 업무 규정, 불량 내용 처리 및 업무 대행 감독자의 교육 강화 등의 법률 제정과유지관리행위의 효율성 제고를 위한 공간 수치화 및 부식 및 침수 피해를 방지할 수 있는 공간 구축 등의 관련 법제도의 정비가 필요하다. The maintenance and status of institutional maintenance of sprinkler facilities are examined to identify problems andsuggest improvement plans for the daily maintenance system for sprinkler facilities over time. In addition, the details ofpoor inspection practices of sprinkler facilities installed in buildings in the Seoul metropolitan area and records preparedby the agency for fire safety management are analyzed. Suggested improvement measures include drafting legislation toestablish maintenance guidelines, standardizing work on fire safety management, handling defective contents, strengtheningeducation of business supervisors, and establishing a maintenance space to prevent corrosion and flooding.

        Urea injection nozzles affect the performance of NOx conversion in urea SCR systems because of their different spray characteristics.This study shows the spray characteristics of an air assisted internal mixing nozzle.The effects of pressure of compressed air and orifice diameter of nozzle on thermal decomposition to ammonia were investigated. In addition, the effect of static mixer on urea decomposition was studied.

        연구논문 : 기존 도시의 홍수저감을 위한 우수관거 배수용량 증대 및 지하 빗물저류조 설치효과 비교 분석

        Urban floods are usually caused by the lack of drainage capacity. Hence, sewer capacity expansion methodology by replacing small pipes with bigger ones is primarily applied as a flood control measure. However, this approach is often unreasonable because of the costs and time involved. Thus, the installation of underground rainwater storage tanks with the two advantages of flood control and water conservation is proposed. This study compared the effectiveness of flood control by both the sewer expansion methodology and rainwater storage tanks using the Storm Water Management Model. Three cases were simulated in this study. The first case analyzed flood reduction by the storm sewer expansion methodology. The simulation results indicate that the overflow volume from manholes was reduced by 49% with this methodology. The second case analyzed flood reduction by installation of rainwater storage tank. The simulation results indicate that the overflow volume was reduced by 62%. However, these two cases could not prevent urban floods completely. Hence, the third case analyzed the joint application of the storm sewer expansion methodology and rainwater storage tanks. In this simulation, flooding did not occur. Consequently, the results of this study clearly show that underground rainwater storage tanks are more effective for flood control than capacity expansion of storm sewer. Furthermore, the joint application of these two flood control measures is more effective than their separate application.

        Organic thermal insulation such as cellular polystyrene insulation is mainly used in buildings. Organic thermal insulation is a highly flammable petrochemical material. Because of such characteristics, organic thermal insulation caused fire accident for buildings. The inflammability of cellular polystyrene insulation specified by Korea standard KS M 3808:2011, "Cellular polystyrene(PS) insulation": combustion time ≤ 120 seconds, combustion length ≤ 60 ㎜ through Korea standard KS M ISO 9772:2013, "Cellular plastics - Determination of horizontal burning characteristics of small specimens subjected to a small flame". In this study, horizontal and vertical burning test (KS M ISO 9772 and UL 94 V, respectively) were analyzed about many cellular polystyrene insulations. As a results, horizontal burning test is very little differentiation the flame-retardant performance levels. However, vertical burning test is not like that. To better assess the level of the flame-retardant performance, the current standard needs to be improved.
