        ICT 기술의 발전과 의료서비스의 혁신

        Hospitals have used OCS, PACS, EMR, and OA to improve task efficiency. As a result of advanced telecommunication and sensor network, which escalates gathering and control of big data, user requirements changed to customize prevention healthcare. This study analyzed the characteristics of healthcare systems along with information technology development phase, and described healthcare service strategy based on innovation information technologies, such as IoT and cloud computing.

        DEA를 이용한 종합병원의 효율성 평가

        병원은 이윤추구를 목적으로 하는 기업과 달리 공익성을 위하여 서비스를 제공하는 측면이 강하다. 하지만, 변화하는 의료환경 속에서 지속적인 성장을 위해서는 경영혁신이 필요하다. 병원의 경영혁신에서 핵심적인 내용은 효율성을 제고하는 것이다. 이를 위해 우선 병원 내부적으로 비효율성이 어떻게 나타나고 있으며 이에 영향을 미치는 요인이 무엇인지 파악하는 것이 필요하다. 그러나 병원의 효율성 평가는 민간부문에서 활용되고 있는 평가방식을 적용하기가 어렵다. 따라서 이 연구에서는 DEA 기법을 이용하여 종합병원의 효율성을 평가하였다. 국내 500병상이상 수련병원 74개를 대상으로 2006년 자료를 이용하여 분석하였다. 분석에 사용된 투입변수는 병상수, 의사수, 간호사수, 의료기사수, 인건비, 관리비, 재료비를 산출변수는 연외래환자수, 연입원환자수, 수술건수, 입원 및 외래 수익을 사용하였다. 또한 병원의 설립형태, 소재지, 설립년, 병원유형에 관해서 효율성에 어떤 영향이 미치는 가에 대해서도 검토하였다. Unlike profit-seeking businesses, hospitals provide medical services to promote the public good in a way, and they need a reform of management to ensure sustainable growth in fast-changing medical environments. The key to the managerial reform is enhancing efficiency, and inefficient managerial practices and factors affecting that should be grasped first of all to boost the efficiency of hospital management. But evaluation methods that are prevalent in the private sector are hardly applicable to hospitals. The purpose of this study was to assess hospital efficiency by using a data envelopment analysis(DEA). The 2006 data on 74 residency training hospitals with 500 beds or more were analyzed. The selected input variables included the number of bed, the number of doctors, the number of nurses, the number of medical technicians, personnel expenses, management cost and materials cost. And the selected output variables were the yearly number of outpatients, the yearly number of inpatients, the number of operation cases and earnings. In addition, the influence of the type of hospital establishment, location, the year of foundation and the type of hospital on hospital efficiency was checked as well.

        병원의 수익성 관련 요인 분석

        병원의 수익성이란 병원이 회계년도 기간동안 환자 진료 및 기타 경영활동을 통해 달성한 경영성과를 말한다. 본 연구는 병원의 수익성 측정 지표로 병원을 운영하여 얻은 의료수익에서 의료비용을 뺀 순수한 의료이익인 의료수익의료이익율을 사용하였다. 이를 통해 수익성에 영향을 미치는 다양한 지표를 대상으로 병원 수익성 관련 요인을 도출하였다. 이 결과를 토대로 병원의 수익성 제고 방안을 강구할 수 있는 보다 유용한 기초자료를 산출하고자 하였다. 분석결과 공공병원이 민간병원보다 수익성이 낮았다. 각 지표의 수익성을 적자병원군과 흑자병원군으로 구분하여 분석한 결과 인건비 비율, 관리비 비율, 전문의 1인당 수술건수, 의료장비 회전율와 관련한 지표에서 유의한 차이를 보였다. 관련 지표가 수익성에 미치는 영향을 보면 모형에 따라 설명력은 25%-74.5%였다. The profitability of a hospital refers to business administration results achieved through its medical care and other management activities during applicable fiscal year. This study focused on operating margin as a measurement index of hospital profitability, which is a genuine medical return obtained by subtracting medical expenses from medical profits achieved during business administration of hospital. Based on the index, this study could deduce certain factors on hospital profitability in terms of various indices affecting profitability. And based on those factors, this study sought to provide more useful reference materials which allow us to devise possible ways to improve hospital profitability. As a result, it was found that public hospitals attained lower profitability than private ones. To analyze profitability depending on each index, this study divided hospitals broadly into deficit group and surplus group. As a result, it was found that there were significant differences in hospital profitability between two groups depending upon relevant indices such as labor cost ratio, maintenance expense ratio, number of operations per medical specialist and medical instrument turnover. According to analysis on potential effects of relevant indices upon profitability, it was found that each index had its explanatory power ranging from 25% to 74.5% depending on given model.

        병원의 비정규직 실태조사

        In 1997, in the course of overcoming economic crisis caused by the shortage of foreign currency there was a significant phenomenon, the irregularization of human labour resources. There was no exception in hospital, either. Most hospitals put a lot of thought to solve the problem about irregular workers. Between employee and employer the problem of irregular workers has been raised as a main topic of all. The objectives of this study were to inspect the actual state of irregular workers and to understand the positive effect of protecting irregular workers, recently established and revised, on the human resources management of hospitals. After enacting irregular labours protection acts there were many opinions that the labour rules of hospitals would be changed.(56.7%) The plan to solve irregular worker`s wage issues would be carried out step by step, but some hospitals have not examined the plan yet.(81.9%) Many hospitals had a plan that irregular worker`s wage would be actualized in two or three years.(78.3%) The methods to solve the problem of irregular workers are as follows; 1) the introduction of functional wage system, 2) the convert from automatic rising wage system to annual wage system, 3) the incentive grade system according to management result, 4) lower functional group wage system, 5) non-term contract wage system. From the point of the opening medical market and securing competitive power of hospitals, it is prospected that irregular workers would be increased also in future. So to manage irregular workers effectively would be essential in maintain hospital`s competitive power and improve medical service. The results of this study were as follows: (1) In hospital, the ratio of irregular workers in hospitals was lower than that of all irregular workers in Korea. (2) In hospital, the wage level of irregular workers was higher than that of irregular workers in Korea. (3) In hospital, the social insurance application ratio of irregular workers was higher than that of all workers in Korea. (4) In hospital, there seems to be no appropriate and active measures to improve labour condition of irregular workers, yet. (5) In many hospitals, the policy of irregular workers would be expected to revised for the law standards. In this study the actual conditions and problems of irregular workers were proposed and it was expected to contribute decisions-making in hospital management, especially when using human resources.

        BSC기반의 중소병원 경영성과예측 시스템다이내믹스 모델개발

        Recently, balanced scorecard is widely used in the public sector as well as in the private sector. Balanced scorecard system of a particular hospital is a very useful tool in the sense that it shows various measures for the management including the traditional financial measures. However, since it does not show the impact of action items, managers have to analyze his potential decisions. On the other hand, a system dynamics model for the management of a hospital can be used to predict future management status. In this paper, a balanced scorecard system with a system dynamics model is developed for a small sized hospital, and the feasibility and applicability of the system have been tested. It turns out the combination of the balanced scorecard and system dynamics technique can be comprised each other. The balanced scorecard system can take advantage of the data forecasted by the system dynamics model including the implementation of action items taken today. On the other hand, the key performance indicators and action items from the balanced scorecard system provide guidelines the scope and direction of the system dynamics model. The additional efforts of developing a system dynamics model are well compensated by the additional colorful functionalities of the system. This paper also covers the future research areas for the combined system to be more effectively developed.
