Subbrow Approach as a Minimally Invasive Reduction Technique in the Management of Frontal Sinus Fractures

        이예원,최현곤,신동혁,엄기일,김순흠,김철근,조동인 대한성형외과학회 2014 Archives of Plastic Surgery Vol.41 No.6

Background: Frontal sinus fractures, particularly anterior sinus fractures, are relativelycommon facial fractures. Many agree on the general principles of frontal fracturemanagement; however, the optimal methods of reduction are still controversial. In this article,we suggest a simple reduction method using a subbrow incision as a treatment for isolatedanterior sinus fractures. Methods: Between March 2011 and March 2014, 13 patients with isolated frontal sinusfractures were treated by open reduction and internal fixation through a subbrow incision. The subbrow incision line was designed to be precisely at the lower margin of the brow inorder to obtain an inconspicuous scar. A periosteal incision was made at 3 mm above thesuperior orbital rim. The fracture site of the frontal bone was reduced, and bone fixation wasperformed using an absorbable plate and screws. Results: Contour deformities were completely restored in all patients, and all patients weresatisfied with the results. Scars were barely visible in the long-term follow-up. No complicationsrelated to the procedure, such as infection, uncontrolled sinus bleeding, hematoma, paresthesia, mucocele, or posterior wall and brain injury were observed. Conclusions: The subbrow approach allowed for an accurate reduction and internal fixationof the fractures in the anterior table of the frontal sinus by providing a direct visualization ofthe fracture. Considering the surgical success of the reduction and the rigid fixation, patientsatisfaction, and aesthetic problems, this transcutaneous approach through a subbrowincision is concluded to be superior to the other reduction techniques used in the case of ananterior table frontal sinus fracture.

유아의 기질 및 부정적 정서표현에 대한 모의 반응이 유아의 정서조절능력에 미치는 영향

        이예원,진미경 한국아동가족복지학회 2016 한국가족복지학 Vol.21 No.1

The purpose of this study is to investigate the effects of children's temperament and mothers' reactions to negative emotions on the children's emotional regulation. The participants were 413 mothers of 4 to 6-year-old children. Cronbach's α, correlation analysis and hierarchical regression analyses were conducted using SPSS 22.0. The results of this study were as follows: First, the mothers’ reactions to negative emotions had a separate influence on children's emotional regulation besides children's temperament. Second, problem-focused responses and punitive responses had a common effect on children's emotional regulation. This study verified the effects of children's temperament and mothers' reactions to negative emotions on the children's emotional regulation, and it found out that mothers' reactions to negative emotions had an independent influence on children's emotional regulation.

Superior Effect of Ultrasonic Homogenization to Mechanical Agitation on Accelerating Reaction Rates in Asymmetric Ring Opening of Epoxides

이예원,유지혜,박규준,김건중 대한화학회 2017 Bulletin of the Korean Chemical Society Vol.38 No.7

Double-layered mesoporous silica was prepared for use as an efficient support for the encapsulation of chiral salen Co-BF3 complexes in the pore channels. The heterogenized chiral salen complex was applied successfully to the asymmetric ring opening (ARO) of terminal epoxides using water, alcohols, and carboxylic acids as nucleophiles. Although the homogeneous salen catalyst exhibited higher catalytic activity than the heterogenized catalyst in ARO reactions as expected, the heterogeneous salen catalysts exhibited high catalytic activities that were sufficient for practical applications. This study evaluated the unique contribution of ultrasonic irradiation as a powerful mixing medium to improve the kinetics in comparison to conventional mechanical agitation during the ARO reactions. Both the homogeneous and heterogeneous chiral salen complexes catalyzed enantioselectively the racemic or chiral epoxides under ultrasonication. The adoption of ultrasonic irradiation in the catalysis resulted in a superior mixing effect to vigorous mechanical stirring. The reaction time was shortened to obtain the highest enantiomeric excess (%ee) when ultrasonic irradiation was applied to the reaction system. Through the ARO reaction of epoxide by phenol derivatives, the highly enantio-enriched β-blockers could be synthesized easily from racemic terminal epoxides or chiral ones using chiral cobalt–salen catalysts. The regioselective ARO reaction of (R)-ECH with propionic or butyric acid proceeded smoothly at room temperature under ultrasonic exposure in the presence of the heterogenized catalyst. The corresponding glycidyl alkylates were produced with excellent optical purity of up to 98%ee. The chiral cobalt–salen complexes encapsulated in MCM-41 retained its catalytic activity and enantioselectivity for three recycles. Consequently, the heterogeneous chiral catalysts anchored in MCM-41 offered the practical advantages of facile reuse through simple recovery from the reactant and products.

User Interface Design and Usability Evaluation of a Mental Health Mobile Application for Healthcare Workers: A Nursing Informatics Perspective

        이예원,곽은주,박미진,박지연,김정재,이지산 한국보건정보통계학회 2024 보건정보통계학회지 Vol.49 No.3

Objectives: The critical need for accessible mental health support for healthcare workers during pandemics underscores the importance of tailored interventions. This study aimed to derive user interface (UI) design elements and evaluate the usability of a mobile mental health application specifically crafted for healthcare professionals, focusing on nursing informatics. Methods: The mental healing app (mHealing app) interfaces were developed, empha- sizing UI design elements derived from user requirements and guided by principles of nursing informatics. Usability was assessed using the Health Infor- mation Technology Usability Evaluation Scale (Health-ITUES). Results: Six nurses, including one specializing in nursing informatics, participated in both UI design derivation and usability pilot test, providing valuable insights from a nursing informatics perspective. The mHealing app offers personalized content delivery and facilitates connections to counselors based on users’ mental health states, supported by a self-diagnosis tool. The UI design priori- tizes user-centered features and intuitive navigation, addressing healthcare workers’ unique challenges during pandemics. In the usability pilot test, the app scored high in impact, perceived usefulness, and ease of use, according to Health-ITUES. Conclusions: The mHealing app, developed with a focus on UI design elements informed by nursing informatics principles, effectively addresses the mental health care needs of healthcare workers during pan- demics, particularly COVID-19. Engaging six nurses in usability pilot test, including one specializing in nursing informatics, the app demonstrates a user- centered approach and intuitive UI, promising a meaningful impact in clinical settings.

Impact of perioperative high-intensity statin treatment on the occurrence of postoperative atrial fibrillation after coronary artery bypass grafting: a meta-analysis

        이예원,임소민,강윤진,손석호,장명진,황호영 대한중환자의학회 2024 Acute and Critical Care Vol.39 No.4

Background: This meta-analysis was conducted to evaluate the impact of high-intensity statintreatment on new-onset postoperative atrial fibrillation (POAF) after coronary artery bypass grafting(CABG).Methods: Four databases were searched for studies that enrolled patients who underwent CABGand investigated the impact of perioperative use of high-intensity statins on the occurrence rateof POAF. The primary outcome was the incidence of POAF. Secondary outcomes were operativemortality and perioperative myocardial infarction (PMI). Publication bias was assessed using a funnelplot and Egger’s test. Results: Nine articles (eight randomized controlled trials and one non-randomized study: n=3,072)were selected. Rosuvastatin (20 mg) was used in four studies, while atorvastatin (40–80 mg) wasused in the other five studies. Reported incidences of POAF in the included studies ranged from11% to 48.8%. Pooled analyses showed that the incidence of POAF was significantly lower in patientstreated with high-intensity statins than in patients in the control group patients (odds ratio,0.43; 95% CI, 0.27–0.68; P<0.001). Subgroup analyses showed that the impact of high-intensitystatins was significant in studies using atorvastatin but not in studies using rosuvastatin. Therewas no significant subgroup difference in the primary endpoint between studies using a placeboand those using low-dose statins. Secondary outcomes, including operative mortality and the incidenceof PMI, were not affected by high-intensity statin treatment. Conclusions: Perioperative use of high-intensity statins is associated with a 57% reduction in theoccurrence of POAF among patients undergoing CABG.

      • Singlet Oxygen Mediated Non-diffusive Photopolymers for Hologram Recording Materials

        이예원,고원건,가재원 한국고분자학회 2021 한국고분자학회 학술대회 연구논문 초록집 Vol.46 No.1

Usually photopolymers for 3D holographic recording materials were used refractive index changing by laser induced polymerization of photo-reactive monomers that leads to the diffusion of an initial homogeneous mixture of component. In this study, we present a non-diffusion process using porphyrin sensitized singlet oxygen. When porphyrin absorbs light, oxygen molecules become highly reactive singlet state oxygen via energy transfer mechanism. From the high reactivity of singlet state oxygen, it changes the structure of surrounding chemical and thereby causes a change in the refractive index. This refractive index change makes it possible to record holograms. In this study, we synthesized the acrylated porphyrin that generates singlet oxygen at 600 nm wavelength and characterized the patterning properties by singlet oxygen through NMR, PL and UV-vis spectroscopy.

Unusual Pseudoaneurysm of the Dorsalis Pedis Artery after an Iatrogenic Injury

        이예원,류한영,김영진,구관우 대한흉부외과학회 2018 Journal of Chest Surgery Vol.51 No.3

Aneurysm and pseudoaneurysm of the dorsalis pedis artery (DPA) are rare vascular entities. Pseudoaneurysms of the DPA are commonly due to blunt trauma, sharp penetrating injury, fracture, or iatrogenic injury. Herein, we report the case of a patient with a rare iatrogenic pseudoaneurysm that occurred after blood sampling. The diagnosis was suspected based on palpitation of a pulsatile mass on the dorsal foot and confirmed by color Doppler ultrasound and computed tomography angiography. Surgical treatment was successfully performed by reconstruction with an autologous venous graft. The patient recovered well, with no ischemic complications.
