        외상성 췌십이지장 손상

        Because of anatomic location and enzymatic function of duodenum and pancreas, the diagnosis & management were difficult. We reviewed medical records of 32 cases of traumatic panreactic and duodenal injuries, which had been admitted to the Department of Surgery, Sung Ae Hospital during 5 years from Jan, 1987 to Dec. 1991. We obtained following results; 1) The prevalent age group was males between twenty-one and forty years old, who were occupied vigorous social activity. 2) The blunt injuries were most common, taking about 93.8%. The Lucas class injuries were frequent in traffic accident. The duodenal injuries were frequent in violance and passinger traffic accident but the pancreatic injuries were in violance and pedestrian traffic accident. 3) The laparotomies were performed under the impression of pancreatic or duodenal injuries in about 56%). 4) The overall postoperative complications occurred in 50% of cases of these injuries and the pancreatic or duodenal related complications were about 20.8%. There were only 3 mortality cases. 5) The related factors in general complications were significant in time from injury to operation, co-existance of other organ injuries, and the quantities of transfusions, and in pancreatic or duodenal related complications were in time from injury to operation, the degree of injury and operative methods.

        복부 둔상에 의한 간손상의 비수술적 치료성적의 고찰

        Serial abdominal CT studies are an integral part of the non-operative treatment of blunt hepst- ic injuries and seem to be useful in monitoring resorption of hemoperitoneum and the healing pattern of intrahepatic hematomas and lacerations. The current study was conducted to evaluate the safety of non-operative management of blunt hepatic trauma in hemodynamically stable patients regardless of injury severity. Twenty five cases of hepatic injury caused by blunt abdominal trauma during 1993-1997 are reviewed. The mean age was 24 with a range of 3 to 58; 68% were male and 28% were children. The most frequent etiologic factor was traffic accidents(76%). Abdominal CT scan, AST/ALT level and vital signs were using the initial diagnostic modality in all patients. With using Injury Severity Scale of American Association for the Surgery of Trauma(AAST), 13 cases(52%) were Grade I, 7 cases(28%) were Grade II, 5 cases(20%) were Grade 3. We transfused the packed RBC in 6 cases. Most cases had shown high serum aminotrans- ferase which were normalized after 2-3 weeks. The severity of abdominal CT-Grading system was not statistically correlated with the prognosis of hepatic injured patients(p$gt;0.05). Non-operative management in blunt hepatic trauma could be considered in most hemodynamically stable patients without evidence of other intraabdominal injuries, regardless of the injured CT-Grading system and trasfusion amount.

        교통카드의 네트워크효과에 대한 계량적 접근

        이 글은 교통카드가 가지는 간접적 네트워크효과를 이용하여 그것의 네트워크 효과를 계량화한다. 구체적 방법으로서, 지하철에서만 교통카드가 사용될 경우와 지하철과 버스 모두에서 사용될 경우를 다음의 두가지 방법을 통하여 비교분석하였다. 첫째, 서울시에서 `국민패스카드'라는 교통카드가 지하철에서만 사용되다가 이후에 버스에도 사용가능해짐에 따라, 지하철에서의 사용량이 증가함을 보여준다. 둘째, 위에서 언급한 양 경우에 부산시의 `하나로'라는 교통카드가 주는 소비자 효용의 차이를 설문조사와 컨조인트분석(Conjoint Analysis)을 통하여 계량화하였다. 그 결과는 다음과 같다. 첫째, `국민패스카드'의 지하철에서의 사용량의 증가는 기존사용량의 40%에 달하였다. 둘째, 그 경우의 소비자효용의 증가분은 교통수단의 요금할인율이 5%에서 15%로 증가되는 경우에 느끼는 효용의 증가분보다 크다. 기존의 네트워크 효과에 대한 계량적 연구가 특정상품의 판매량 및 가격의 상관관계를 보여줌으로써 수행된 것에 비하여, 상호보완적인 네트워크가 연동됨으로 인하여 발생하는 네트워크효과를 계량화한 것에 이 논문의 방법론적 기여가 있다. This paper quantifies the indirect network effect of transportation card. The methodology is to compare the case where transportation card is used only in subway with the case where it is used both in subway and in bus, using the following two methods. First, the usage volume of 'Kookmin Pass Subway Card' in Seoul is shown to have significantly increased when it began to be usable in bus network. Second, the conjoint analysis shows that the consumer utility of 'Hanaro Card' in Busan is much higher when it is used for both than one of two means of transportation. The result is as follows. First, the usage volume of 'Kookmin Pass Subway Card' increased by 40%. Second, the increment of consumer utility mentioned above is higher than that in case where the discount rate of bus or subway fare increases from 5% to 15%. The contribution of this paper is to quantify the network effect from two networks being compatible, while previous research focused on continuous correlated increase of the price and volume of one community.
