        서구 각국의 대체의료에 관한 최근동향 : 미국을 중심으로

        Currently, alternative medicine is highly utilized in the western countries. The public as well as health care practitioners have been paying a considerable attention to alternative medicine. This rapid growth in alternative medicine in many western countries suggests a high degree of public dissatisfaction with the limitations of orthodox medicine and concern over the side effects of chemical drugs. There is also a widespread recognition of the growing financial, social and personal cost for orthodox medicine. In addition, there is a growing need for a less fragmented, more participative and humane approach compared with contemporary medicine. Alternative medicine, which is based on holistic approach and lead to less expensive health care, may have the potential to play a significant role in overcoming the limitations of orthodox medicne in the western country. In this context, this study explores the recent trends of research on alternative medicine in western countries, particularly U.S.A. The study can provide us with valuable information on the types, training and education, utilization, insurance coverage, and regulation of alternative medicine in the western countries, thus offer important policy implications for the improvement of the korean health care system.

        대체의료에 대한 한의사들의 인식도

        This study was carried out to examine the attitude and recognition of oriental medical doctors on alternative medicine, which is increasingly used across the country. The data were collected from 264 oriental medical doctors who were working in oriental medical clinics in Daegu City, Gyeongbuk Province, using self-administered questionnaire from 1 March to 15 April 2007. Generally, the respondents have some positive attitude and recognition on alternative medicine including knowledge, interest and usefulness, efficacy of alternative medicine. However, they have some negative view on the expense of alternative medicine. And they also thought that introducing alternative medicine in their clinics was needed for better treatment and actually about 70% of the respondents recommended or applied alternative medicine to their patients. Around 40% of the respondents did not have the chance of education on alternative medicine officially. About 60% of the respondents thought that the oriental medical school was the best institute to provide the education of alternative medicine, followed by the oriental medical doctors's association. For the development of alternative medicine, they thought that the scientific research on alternative medicine by oriental medical doctors and professionals was highly needed.

        한방의료서비스에 대한 인식도 및 개선요구도

        This study was conducted to examine the recognition level of the people on oriental medical Services and the need for it's improvement. Data were collected from 1174 residents in Daegu metropolitan city and Gyungbuk province. According to the satisfaction level with each items of oriental medical services, the respondents had positive views on efficacy, kindness, and side-effects. They, however, had negative view on the cost of oriental medical services. In regarding to the priority of improvement of oriental medical system, 'expansion of insurance benefit package' ranked first. Followed by 'safety of herbal medicine(heavy metal/pesticides)', 'improvement of scientific methods and diagnostic technique' etc. For the further development of oriental medical services in the consumer - focused and evidenced-based health care environment, much attention to implement relevant health policy reflecting user's need positively should be made.

        한방의료의 이용행태 및 이용결정요인 분석 - 일부 대도시 지역주민을 중심으로 -

        This study was conducted to examine utilization patterns and determinants of oriental medical services. Data were collected from 545 residents in Taegu city The results of this study are summarized as follows 1) 37.8% of subjects used oriental medical services in the past year. Especially, the female, the ages of 50 and 60 over, the single. low-educated. high-income class, white-collar class, medical insured tended to use more oriental medical services than another groups. 2) 46.7% of users of oriental medical services reported that number of visits in the past year was 2 - 5 times 37 1% of them was 1 times, and 4.5% over 10 times. 3) According to the reasons to choose the oriental medical facilities, most was 'on their own judgement'(48.8%) and 'by the advice of relatives and friends'(42.0%) Regarding to the objectives of using oriental medicine, 68.3% was 'treatment', 31.7% 'health counselling and promotion'. And among diseases of users, diseases of musculo-skeletal system was the highest(54.5%). 4) 57.9% of oriental medical services users had experience of utilizing western medicine on the same diseases. Among peoples with experiencing western medicine on the same diseases. 54.4% received oriental medical services 'in addition to western medicine', 45.6%'in place of western medicine 'And 41.2% of using both services reported that they had difficulty in deciding to choose the type of services -oriental medical services or western medicine-for their diseases. 37.3% of them answered that 'providing relevant information' was the most desirable measure to solve this problem, 27.3% 'establishment of effective referral system between oriental and western medical facilities '23.6% 'cooperative medical treatment systems in the same facilities', 11.8%'integration of oriental medicine into western medicine 5) According to the satisfaction level with each items of oriental medical services, the respondents had positive views on efficacy, kindness, and side-effects. They, however, had negative view on the cost of oriental medical services. 6) In regarding to the priority of improvement of oriental medical system,'expansion of insurance benefit package 'ranked first. 'expansion of insurance benefit Package 'ranked second, 'improvement of scientific methods and diagnostic technique 'third, and 'safety of herbal medicine' fourth in order. 7) The significant factors influencing the utilization of oriental medical services were kindness of oriental medical practitioners, efficacy , travel time, age To be brief, utilization rate of oriental medical services in urban area generally tends to be high. There, however, have been various barriers to limit oriental medical services, such as incomplete benefit package of oriental medical insurance and lack of coordination and referral system between oriental and western medical services, lack of scientific diagnostic procedures, high price etc . For the development of oriental medical services, Much attention to remove these limiting factors should be placed. In addition, kindness of oriental medical practitioners , which is expected to be more important factor in the consumer - focused health care environment than ever, should be kept high consistently. Since this study was conducted for specific residents of an urban city. further research including more sampling in different urban areas should be required to generalize the results of the study.

        몽골 전통의료의 현황 및 역사적 발달과정

        Objective : The aim of this study is to review the historical development and current situation of Mongolian traditional medicine. Method : Systematic literature review for books, journals, governments statistics, reports on mongolian traditional medicine was carried out. Results : The theory of Mongolian traditional medicine is influenced by the philosophy and medical theories of the ancient orient such as India, Tibet and China-theories of hot and cold, principle of yin and yang and five elements. There are very unique treatments like brain tram concussion, fermented horse milk treatment as well as moxibustion, acupuncture, and blood letting. It appears that they are very closed to nomadic life. Training program of each institutes need to be standardized. Mongolian traditional medicine had been almost destroyed by the influence of communist idea from 1930s until the end of the 1980s. Since 1990, Mongolian government put much emphasis on the development of traditional medicine. And now about 3.7% of total beds is traditional medicine one and that there are six TM schools and the total number of traditional medicine in 2012 is 1696. However, there are still the lack of manpower, facilities, standardized training program and scientific research for traditional medicine. Conclusion : Mongolian traditional medicine has been developed over centuries in response to Mongolia's unique geographical and climate conditions and the lifestyles of its people and that made contribution to the health of mongolian people. However, it needs to be strengthened because there are poor infrastructure and training program.

        몽골의 보건의료체계 현황 및 국민보건수준

        This paper reviews the historical development of Mongolian health care and it's health care systems and looks at the health status of Mongolian people. In addition to it, this one also explores the health care reforms since 1990 when Mongolia introduced market economy. Mongolian health care systems was a centralized and hierarchial system that health care were fully financed and delivered by the government from the 1940s until 1990s. When new market economy introduced after 1990, free health care services with state financing alone were unsustainable and multiple sources of revenue have developed. However, The introduction of social health insurance and the decentralization process since 1990 brought new organizations and policies into health sector creating a more pluralistic environment. However, frequent changes of the government, scarce resources such as chronic under funding of health services as well as lack of coordination seemed to fail to implement both national programs and health care reform process effectively. As to the health status of Mongolian people, there has been continuous improvements in the health indicators in Mongolia since the 1920s although health indicators are still high compared with developed countries. Particularly, considerable progress has been made in reducing vaccine-preventable diseases, the infant mortality rate and the maternal mortality rate through concerted policy efforts. However, cardiovascular diseases, neoplasm and injuries and the poisoning has been currently emerging as the leading main causes of mortality in Mongolia.

        보건의료서어비스 접근에 대한 형평성 보장은 영국보건의료제도( NHS ) 설립원리의 하나이다. 모든 국민은 재산, 성별, 연령. 직업, 거주지역에 관계없이 적정의료서어비스 수혜에 대한 균등한 기회를 정당히 보장받아야 함을 영국의료제도(NHS)는 천명하고 있다. 그러나 NHS 제도 창설이래 이에 대한 법적보장과 보건의료자원의 형평적인 배분에 대한 지속적인 노력에도 불구하고 일부 사회계층은 보건의료 서어비스 접근에 대한 지리적, 문화적, 경제적 장벽을 경험하고 있다. 더우기 최근의 NHS 개혁작업에서도 형평성 원리를 보호하거나 감시하는데 많은 관심과 노력이 주어지지 않고 있는 실정이다. 본 논문은 보건의료 서어비스가 NHS의 설립정신에 바탕하여 얼마나 공정하고 형평에 맞게 일반국민에게 제공되는가를 알아보기 위하여 보건의료자원의 배분과정 , 보건의료 서어비스 이용에 대한 저해요인 등을 분석하고 더 나아가 건강에 있어서의 불평등을 극복하기 위한 NHS의 역할과 이에 대한 정책 및 서어비스 개발 둥을 고찰한다. 보건의료 선진국인 영국의 NHS제도의 형평성에 관한 분석, 고찰은 비록 제도 및 환경여건이 다른 우리나라의 경우에도 많은 정책적 시사점을 제공할 뿐만 아니라 보건의료제도의 형평성 증진을 위한 구체적인 전략수립에도 도움을 줄 것이다.

        Nowadays excessive consumption of alcohol of the people is considered as one of important public health problems as well as societal and economic problems in Korea. There has been a constant increase in rate of consumption of alcohol and in 1998 around 70% of peple aged 20 and over drinks alcohol. In particular, there has been a sharp increase in women and young people. In this context, this paper reviews the trends in consumption pattern of alcohol, and analyzes social and economic cost of alcohol abuse, and finally suggests the direction of alcohol polity in Korea.

        한양방 협진에 대한 한의대생들의 인식도

        The purpose of this study is to examine the recognition of the student of oriental medical school on cooperative system between Oriental and Western medicine and to provide basic information for the development of oriental medicine. In order to look at the level of recognition on cooperative system between Oriental and Western medicine, from September 1st to 15th of 2005, this study had been conducted through personal interview and questionnaires to 600 students who were attending the Department of Oriental Medicine (in both prep and regular courses) in D University, located in the City of Daegu. The data has been analyzed using statistic program, the SPSS WIN 12.0. Statistical analysis tools used for this study were frequency analysis, cross-over analysis and the t-test. The results are as follows; The students of oriental medical school had relatively high level of recognition on the basic concept, interests, necessity and potential for cooperative system between Oriental and Western medicine. However, they had negative understandings on the issue of unification of the two medical systems and it's possibility in the future. The students were optimistic about possible merits of cooperative system between Oriental and Western medicine, especially in the field of rehabilitation. On the reasons for lack of development of cooperative system between Oriental and Western medicine, the students listed prejudices existing on both sides firstly, followed by their contrasting approach on human illness, lack of legal and institutional support system, the indifferences of doctors and indifferences of oriental medical doctors. In addition, students understood that the dualism existing in our current medical system is aggravating mutual distrust between the two sides, causing the confusion of patients on the choice of medical facilities, and raising their medical bills. Therefore, in order to vitalize collaboration between Oriental and Western medicine in new health care environment, the following measures should be needed decreasing the prejudices between the two medical spheres with open mind improving educational programs in Western and Oriental medical schools; promoting joint academic research or exchange programs between the schools, and increasing government effort to minimize legal and institutional restrictions cooperative system between Oriental and Western medicine.

        보건의료에 있어서의 질관리는 세계 각국의 중요한 보건의료정책과제의 하나로 대두되고 있으며 그 중요성에 대한 인식이 크게 고조되고 있는 현실이다. 특히, 영국의 경우도 1980년대 중반이후 전문가중심의 가부장적 보건의료체계에서 벗어나 내부시장(internal market)의 새로운 개념도입과 환자중심(patient-focused)의 보건의료체계를 지향하면서 보건의료서어비스의 질관리 및 향상에 대하여 많은 정책적 관심을 집중시켜 왔다. 이러한 측면에서 본 연구는 신임제도를 비롯하여 영국에서 진행되어온 다양한 서비스 질 관리운동을 서구 각국의 경험과 비교고찰하고, 특히 의료제공자, 의료구매자 및 소비자와 관련하여 신임제도의 영향관계를 분석함으로써 우리나라의 보건의료서어비스 질 관리 및 증진에 많은 정책적 시사점을 제공할 수 있을 것이다.
