        3D프린팅전문운용사 및 3D프린터개발산업기사 국가기술 자격종목 개발에 관한 연구

        본 연구는 3D프린팅 산업에서 3D프린팅 운용과 개발에 사용되어지는 전문기술인력을 효과적으로 양성할 수 있는 3D프린팅전문운용사와 3D프린터개발산업기사의 국가기술 자격종목을 개발하고자 하는데 그 목적이 있다. 연구의 목적을 달성하기 위하여 3D프린팅 분야의 국내외 실태 조사, 문헌조사를 통하여 3D프린팅 관련 교육훈련기관 및 검정 수요 예상 인력을 파악했으며, 유사자격제도를 분석하였고, 직무분석을 통하여 3D 프린팅전문운용사와 3D프린터개발산업기사의 직무 및 교육내용을 분석하였다. 3D프린팅전문운용사의 필기시험은 3D 스캐너, 3D 모델링, 3D프린터 설정, 3D프린터 출력 및 후가공, 3D프린터 교정 및 유지보수 5개 과목과 실기시험의 경우, 3D프린터 작품제작, 3D 모델링, 3D 스캐너, 3D프린터 유지보수 실무 등을 내용으로 검정 방법으로는 작업형이다. 3D프린터개발산업기사의 필기시험은 3D프린터 회로 및 기구, 3D프린터 장치, 3D프린터 프로그램, 3D프린터 교정 및 유지보수 등 4개 과목과 실기시험의 경우, 3D프린터 조립 및 출력점검, CAD 작업, 3D프린터 프로그램, 3D프린터 유지보수 실무 등을 내용으로 검정방법으로는 작업형이다. 또한, 3D프린팅전문운용사와 3D프린터개발산업기사 자격종목 신설에 대한 설문조사를 실시했으며, 3D프린팅전문운용사와 3D프린터개발산업기사 자격종목의 출제기준 및 채점 방법을 제시했고, 필기시험과 실기시험에 대한 모의 검정시험도 실시하였다. 이러한 결과를 토대로 3D프린팅전문운용사와 3D프린터개발산업기사에 대한 교육프로그램을 만들었으며, 자격검정을 실시할 수 있는 출제기준을 제시하였다. This study aims to develop qualification criteria for training and establishing an expert pool of 3D printing operation experts and 3D printing development specialists in the emerging 3D printing industry. We investigate the domestic and international 3D printing industry, as well as 3D printing- related training institutions and quantity of test examinees. Utilizing competency analysis, this study examines the competencies and educational content of both 3D printing operation expert as well as 3D printing development specialists. In addition, a survey was conducted on the issue of institutionalization of qualification criteria for 3D printing operation expert and 3D printer development specialists. This study further proposes evaluation methods for qualification criteria for both 3D printing operation expert and 3D printer development specialists based on simulations of both written and performance tests. Based on the findings, this study proposes instructional program outlines as well as comprehensive guidelines for training and qualification processes for 3D printing operation expert and 3D printer development specialists.

        노인 당뇨환자에서 혈당조절에 따른 식사의 질과 당뇨자가관리상태

        Evidence for the effects of different health behaviors, including diet, in elderly diabetes is currently limited. The aim of this study was to compare the quality of diet and health behaviors in Korean elderly T2DM patients, using a glycemic control. T2DM elders(>65 yr, n=48) were recruited and categorized by the concentration of glycated-hemoglobin HbA1c; subjects with HbA1c<7% were the good control(GC) group, and subjects with HbA1c≥8% constituted the poor control(PC) group. General characteristics, self-management behavior questionnaires, and 3-d diet records were all collected and assessed. No significant differences in general characteristics between GC and PC were detected, with the exception of a higher level of education in GC(p<0.05). A twofold longer duration of diabetes was observed in PC as compared to GC(p<0.01). The GC group did exercise for a longer time(p<0.001), and had an earlier beginning of diabetes self-management education (DSME) by healthcare practitioners using a team teach as compared with the PC group(p<0.05). The total dietary quality index(p<0.001) and individual index for carbohydrate(p<0.001) or vegetables and fruit(p<0.05) were better in GC than in PC. Therefore, the earlier DSME including intensive exercise and balanced diet selection should be expected to improve glycemic control in diabetic Korean elders.

        당뇨병 환자의 임상영양치료를 위한 임상영양사의 직무표준 개발

        This study was conducted to develop job standards for clinical dietitian administering clinical nutrition therapy to diabetic patients in hospitals. Based on DACUM (Developing A Curriculum) analysis of 17 members including clinical dietitians, professors majoring in clinical nutrition and researchers, information on duties, tasks and task elements of clinical dietitians for diabetes care were derived and applied to diabetes mellitus- specific clinical nutrition care in hospitals for evaluation. The final developed job standards for clinical dietitians for diabetes care included four duties, 19 tasks and 56 task elements. The duties consisted of nutrition assessment, nutrition diagnosis, nutrition intervention, and nutrition monitoringㆍevaluation. For application of diabetes mellitus-specific job standards in clinical nutrition care, 108 work activities were developed and classified into 90 basic and 18 recommended types. Performance rates of standardized jobs were 80.2% at nutrition assessment, 99.6% at nutrition diagnosis, 78.5% at nutrition intervention, and 32.9% at nutrition monitoringㆍevaluation. These results can be applied as guidelines to implement jobs for diabetes mellitus-specific clinical nutrition services in clinical settings. In addition, they would be useful for education standards in educational institutions for education and training of clinical dietitian.

        뇌경색 당뇨병 환자와 비뇌경색 당뇨병 환자의 비교연구 : 영양소 섭취, 식사의 질 평가를 중심으로

        뇌경색을 동반질환으로 가지는 당뇨병 환자와 뇌경색이 발병하지 않은 당뇨병 환자의 식·생활습관, 영양소 섭취상태 및 식사의 질을 비교한 결과를 요약하면 다음과 같다. 1) 뇌경색 당뇨병 환자인 DM-CI군과 비뇌경색 당뇨 환자인 DM군의 평균 연령은 65.6, 69.2세, 당뇨병 유병 기간은 10.9, 9.7년으로 두 군 간의 차이가 없었다. 당뇨병교육을 받은 경험은 DM-CI군에서 유의적으로 낮았으며(DM-CI: 21.4% vs. DM: 47.5%) 약물치료에서도 두 군간에 유의적인 차이가 관찰되었다 (p < 0.05). 흡연율 (DMCI: 25.0% vs. DM: 2.5%)과 음주율 (DM-CI: 28.6% vs. DM: 10.0%)은 DM-CI군에서 유의적으로 높았으나 (p < 0.05) 규칙적인 운동과 운동 횟수에서는 DM-CI군에서 유의적으로 낮았다 (p < 0.05). 2) 신체계측에서는 군 간에 차이가 관찰되지 않았으나 여자의 경우, 평균 신장이 DM-CI군에서 유의적으로 높았으며 (p < 0.05) 체질량 지수가 두 군 모두 과체중-비만 범위에 있었다. 혈압측정 결과, 수축기 혈압은 군 간의 차이를 보이지 않았으나 이완기 혈압에서 남녀 모두 DM-CI군에서 유의적으로 높았다 (p < 0.05). 3) 식사를 규칙적으로 하고 있는 경우는 DM군에서 유의적으로 높았고 하루에 한 끼를 거르는 경우는 DM-CI군에서 많았다 (p < 0.05). 식사시간이 불규칙한 경우는 DMCI군에서 유의적으로 높았으며 (p < 0.05) 간식의 섭취빈도 및 외식의 섭취 빈도에서는 두 군 간에 차이가 없었다. 4) 일일 탄수화물, 나트륨 섭취량은 남녀 모두 DM-CI군에서 유의적으로 높았으며, 지방과 콜레스테롤은 여자 DMCI군에서 유의적으로 높았다 (p < 0.05). 비타민 B₁, B₂, C, 엽산, 칼슘 섭취는 DM군에서 높았으며 단백질, 아연의 섭취는 DM-CI군에서 유의적으로 높았다 (p < 0.05). 영양소섭취량을 권장섭취량과 비교한 결과, DM군에서 비타민 A, 칼슘, 아연이 권장섭취량에 미달되었으며 DM-CI군에서는 비타민 B₁, 비타민 B₂, 나이아신, 엽산, 비타민 C, 칼슘이 미달되었다. 5) 식사의 적절성을 평가하는 INQ의 경우, DM-CI군에서 비타민 B₁, 비타민 C, 비타민 B₂, 엽산, 칼슘의 적정도가 유의적으로 낮았다 (p < 0.05). 식품군의 다양성을 평가하는 DDS는 DM-CI군에서 유의적으로 낮았으나 (p < 0.05) 섭취 식품의 다양성을 평가하는 DVS는 두 군 간에 차이가 없었다. 식사 섭취패턴은 두 군에서 유의적으로 다르게 나타났는데 DM군에서는 다섯 가지 식품군을 섭취하는 비율이(GMVDF = 11111) 가장 높았던 반면 DM-CI군에서는 유제품군을 섭취하지 않는 경우가 (GMVDF = 11101) 가장 높았다 (p < 0.05). 식사의 질 평가에서 DM-CI군, DM군이 각각 8.3 ± 1.3, 6.9 ± 1.7로 DM-CI군에서 식사의 질이 유의적으로 낮았다 (p < 0.05). 결론적으로, 뇌경색을 동반한 당뇨병 환자는 뇌경색을 동반하지 않은 당뇨병 환자에 비해 흡연, 음주, 운동 등의 건강관련 습관이 좋지 않았다. 또한 열량, 지방, 나트륨, 콜레스테롤의 섭취는 많은 반면, 비타민과 무기질의 섭취량이 낮았으며 영양소 섭취 적정도와 식품 섭취패턴 및 식사의 질이 저하되어 있었다. 따라서 뇌경색을 동반한 당뇨병 환자는 당뇨병 관리를 위한 건강 관련 습관을 조절함과 동시에 다양한 식품 섭취를 통하여 식사의 질을 높이는 것이 요구된다. 당뇨병은 기본적으로 식사요법을 잘 실행할 때 여러 합병증의 발병을 예방할 수 있으며 약물 복용 시에도 그 중요성은 강조된다. 식사요법은 올바른 영양교육을 통하여 건강관련 생활습관과 함께 습득될 수 있는 부분이므로 당뇨병 환자뿐만 아니라 합병증을 동반한 당뇨병 환자에서도 개개인에 맞는 적극적인 의학영양치료가 필요할 것으로 사료된다. Diabetes mellitus (DM) is a well-established independent risk factor for cerebral infarction (CI). Additionally, the DM as well as CI are influenced significantly by health-related behaviors and diets. The aim of this study was to compare the food habits, nutrient intakes, and dietary patterns and quality in DM patients with CI (DM-CI) and without CI. This study was accomplished with 68 subjects (DM-CI group = 28, DM group = 40). Health-related behaviors, food habits, nutrient intakes and dietary quality were investigated. As a result, the mean ages and durations of diabetes in DM-CI and DM groups were 65.6 ± 9.2, 10.9 ± 8.5 and 69.2 ± 3.0, 9.7 ± 8.4 years, respectively. The health-related behaviors such as, smoking, alcohol drinking, and regular exercising in both groups were significantly different (p < 0.05) showing the number of subjects who were smoking and drinking alcohol was significantly higher in DM-CI group and the opposit result was seen for exercise. In male subjects, the intake of carbohydrate and sodium of DM-CI group were significantly higher than those of DM group (p < 0.05). In female subjects, the intake of calorie, carbohydrate, fat, sodium, and cholesterol were significantly higher in the DM-CI group (p < 0.05). Daily intake of vitamin B1, vitamin B2, folate, vitamin C, and calcium were significantly higher in DM group (p < 0.05). In the comparison with the dietary reference intake for Koreans (KDRI), vitamin B1, vitamin B2, folate, and calcium intakes were lower in DM-CI group whereas calcium and zinc intakes were in DM group. Moreover, index of nutritional quality (INQ) of vitamin B1, vitamin B2, vitamin C, folate, and calcium were lower significantly in DM-CI group (p < 0.05). Dietary quality including dietary diversity score (DDS), GMVDF (grain, meat, vegetable, dairy, fruit), and dietary quality index (DQI) was significantly lower in DM-CI group (p < 0.05). According to the results, diabetic mellitus patients accompanied by cerebral infarction had poorer eating patterns and dietary quality in accordance with poorer health-related behaviors compared with the DM patients without CI.

        파킨슨병 환자의 영양 상태에 따른 식사의 질 및 삶의 질에 관한 연구

        Assessment regarding the nutritional status of Parkinson's disease (PD) patients is important because their nutritional status influences the outcome of disease. The purpose of this study investigated the dietary habits, dietary quality, and quality of life for Korean PD patients according to their nutritional status. Seventy PD patients were recruited from K University Hospital (KMC IRB#0918-07) from February 2010 to October 2011. To diagnose PD, the United Kingdom Parkinson's Society Brain Bank standards were used by a neurology physician. The participants were interviewed and assessed using various tools, including a general questionnaire, anthropometrics, questionnaires for dietary habits, dietary diversity score (DVS), dietary variety score (DVS), diet quality index-international (DQI-I), beck depression inventory (BDI), the Korean version of mini mental state examination (K-MMSE), and 36-item short-form health survey (SF-36). The results of the study were as follows: 1) Most of the PD patients (69.0%) were at risk of malnutrition, and 8.5% and 22.5% of the subjects were malnourished and well-nourished, respectively. 2) Clinical symptoms (swallowing and chewing difficulties) were obvious, and the quality of diet (DDS, DVS, DQI-I) was low in the malnourished group (p < 0.01). 3) The malnourished group had significantly lower scores regarding SF-36 (quality of life) as well as that of cognitive function (p < 0.05). 4) The nutritional status of PD patients was significantly correlated with their depression (p < 0.05) and quality of life (p < 0.01). These results indicate that appropriate nutritional management will improve the nutritional status and quality of life as well as the cognitive function in Parkinson's disease patients.

        암 환자의 임상영양치료를 위한 임상영양사의 직무분석과 직무표준 개발

        The present study was conducted to provide the basis for improvement of clinical nutrition services through development of job standards of clinical dietitian for the clinical nutrition therapy to cancer patients in hospitals. Developing A Curriculum (DACUM) method was used for job analysis and development of job standards for clinical dietitians for cancer care. Based on DACUM analysis, information about duties, tasks, and task elements of clinical dietitians for cancer care was collected. Developed job standards were applied to clinical nutrition care for cancer patients in hospitals for evaluation. Based on DACUM analysis, consultations from professionals, and field application tests, the final job standards were composed of four duties, 18 tasks, and 56 task elements. The duties consisted of nutritional assessment, nutrition diagnosis, nutrition intervention, and nutrition monitoringㆍevaluation. For cancer nutrition care, 109 work activities were developed. They were composed of 75 basic and 34 recommended work activities. The application of developed job standards for clinical dietitians for cancer care at 10 hospitals showed a performance rate of 72.3%. In conclusion, job standards for clinical dietitians for cancer care developed in this study might be effectively used as guidelines for providing clinical nutrition services for cancer patients in hospitals.

        2010 당뇨병 환자를 위한 식품교환표 개정

        A food exchange system for diabetes is a useful tool for meal planning and nutritional education. The first edition of the Korean food exchange lists was developed in 1988 and the second edition was revised in 1995. With recent changes in the food marketplace and eating patterns of Koreans, the third edition of food exchange lists was revised in 2010 by the Korean Diabetes Association, the Korean Nutrition Society, the Korean Society of Community Nutrition, the Korean Dietetic Association and the Korean Association of Diabetes Dietetic Educators through a joint research effort. The third edition is based on nutritional recommendations for people with diabetes and focuses on adding foods to implement personalized nutrition therapy considering individual preferences in diverse dietary environment. Foods were selected based on scientific evidence including the 2007 Korea National Health and Nutrition Examination Survey data analysis and survey responses from 53 diabetes dietetic educators. While a few foods were deleted, a number of foods were added, with 313 food items in food group lists and 339 food items in the appendix. Consistent with previous editions, the third edition of the food exchange lists included six food categories (grains, meat, vegetables, fats and oils, milk, and fruits). The milk group was subdivided into whole milk group and low fat milk. The standard nutrient content in one exchange from each food group was almost the same as the previous edition. Korea Food & Drug Administration's FANTASY (Food And Nutrient daTA SYstem) database was used to obtain nutrient values for each individual food and to determine the serving size most appropriate for matching reference nutrients values by each food group. The revised food exchange lists were subjected to a public hearing by experts. The third edition of the food exchange lists will be a helpful tool for educating people with diabetes to select the kinds and amounts of foods for glycemic control, which will eventually lead to preventing complications while maintaining the pleasure of eating.
