        짧은 안축장의 환자에서 각막곡률의 정도에 따른 인공수정체도수 산출 공식의 정확성 분석

        Purpose: To evaluate the accuracy of intraocular lens (IOL) power calculations between Hoffer Q and other formulas according to corneal curvature by comparing the results of cataract surgery and calculation of chosen formulas in short axial lengths. Methods: We performed a retrospective analysis of patients who underwent cataract surgery from January 1st, 2012 to June 12th, 2012. The patients were selected if their axial length was below 23.00 mm and 77 patients (90 eyes) were included in the present study. The patients were divided into 2 groups according to mean corneal curvature below 44.0 D and over 45.0 D. IOL power was calculated using the Hoffer Q and SRK II, SRK-T and Holladay I formulas and the error between the calculations and refractive outcome of cataract surgery were measured. The accuracy of each formula was evaluated by comparing the error between the 2 groups. Results: Hoffer Q formula showed a higher predictive accuracy than other formulas regardless of corneal curvature in eyes with short axial lengths (p < 0.001, p = 0.023). Particularly, SRK II, SRK-T and Holladay I showed a lower predictive accuracy in eyes with flat corneal curvature than Hoffer Q (p < 0.001, p = 0.215). Conclusions: In eyes with short axial lengths, preoperative predicted IOL power calculations showed better accuracy with Hoffer Q formula than SRK II, SRK-T and Holladay I formulas. SRK II, SRK-T and Holladay I formulas showed a lower predictive accuracy in flat corneal curvature eyes than steep corneal curvature eyes with short axial lengths. We hypothesize that SRK II, SRK-T and Holladay I tend to underestimate effective lens position in eyes with short axial lengths indicating Hoffer Q formula is more accurate.         유행성각결막염의 발병원인과 치료대책

        Epidemic keratoconjunctivitis is the most common type of infectious conjunctivitis, and is caused by adenoviruses. The mode of transmission is mainly through direct contact with ocular secretions. Epidemic keratoconjunctivitis is generally diagnosed based on a patient’s clinical features, and additional measures, such as cell cultures, polymerase chain reaction, and rapid antigen detection tests, can further confirm the diagnosis. The most common symptoms include a foreign body sensation, tearing, and photophobia. The symptoms are usually expressed unilaterally in the initial phase, but gradually become bilateral. Frequently occurring complications include pseudomembrane formation and subepithelial infiltrates. Currently, no antiviral agent has been proven effective to alter the natural course of the disease, and treatment merely has a supportive role instead of a curative role. Therefore, preventive measures in medical offices and in the community are the most important methods of controlling the propagation of this disease.

        원발성 안와 종양에 대한 신경학적 고찰

        Background : Primary orbital tumors are relatively rarely encountered by neurosurgeons. Several approaches to primary orbital tumors have been used depending upon the tumor location, preoperative diagnosis, and the surgeon’s experience. Here, we present our clinical experience with primary orbital tumors from a neurosurgical perspective. Methods : A total of 11 patients with primary orbital tumors who underwent surgical resections between January 2011 and December 2018 were included in the study. All clinical data, including preoperative symptoms, visual function, operation record, histopathology, and radiologic imaging were reviewed. Results : The median age of the patients was 55 years, and diplopia and impaired vision were the most common symptoms. Depending upon the tumor location in the orbit, the intraconal type was seen in eight patients, the intracanalicular type was seen in two, and the extraconal type was seen in one. Surgery on these tumors was performed by the transcranial approach in nine patients and the endoscopic endonasal approach in two, depending upon the meridian of the optic nerve. Gross total resection was completed in five patients, subtotal resection in one, partial resection in one, and biopsies in four. Postoperatively, visual function was improved in two of four (50%) patients, and preexisting vision was preserved in the other nine patients. Conclusions : Although the experience with primary orbital tumors was limited, we suggest that favorable surgical outcomes can be achieved with an appropriate surgical approach by understanding the anatomy of the orbit, especially the meridian of the optic nerve.

        후면토릭 RGP 콘택트렌즈를 이용한 원추각막의 시력교정 증례

        Purpose: To report a case of correcting irregular astigmatism in severe keratoconus with back toric rigid gas permeable (RGP) contact lens. Case summary: A 43-year-old man visited our clinic for correction of decreased visual acuity on the both eyes. The refractive status of both eyes was over mean 5 diopters. The best corrected visual acuity (BCVA) of right was 20/25 and BCVA in left eye was 20/100. The corneal astigmatism was 1.75 diopters in right eye and 6.5 diopters in the left eye. By topography of both eyes, the right eye has minimal degree of keratoconus and the left eye was more advanced stage of keratoconus. He could not wear the RGP contact lens with multi-curved or high eccentricity due to corneal irregular astigmatism. Using back toric RGP contact lens, the right eye’s BCVA was improved 20/25 and left eye’s BCVA was improved 20/40. There were no significant complications or abnormal finding in the corneal surface. Conclusions: Back toric RGP contact lens appears to be effective in improving BCVA and reducing corneal irregular astigmatism of keratoconus when multi-curved or high eccentricity RGP contact lens failed.

        회절성 다초점 인공수정체를 삽입한 백내장 수술안의 시각 및 광학 결과 비교

        목적: 두 종류의 회절성 다초점 비구면 인공수정체인 Acri Lisa 366D와 AcrySof ReSTOR (+3.00D)를 삽입한 환자들의 시각 및 광학적 임상결과를 알아보고자 하였다. 대상과 방법: Acri Lisa 366D 34안과 AcrySof ReSTOR (+3.00D)를 삽입한 34안을 대상으로 수술 후 1주, 1달, 6달째 시력, 대비감도, 고위수차 및 환자만족도에 대해 조사하였다. 결과: 중간거리 logMAR 시력은 6개월째에 Acri Lisa 366D군에서 0.31 ± 0.14로 AcrySof ReSTOR 3SN6AD1군의 0.24 ± 0.11에 비해 유의하게 낮았고 원근거리 시력은 차이가 없었다. Acri Lisa 366D군은 대비감도 검사상 명순응 상태의 6 cpd에서 55.36 ± 7.40으로 47.25 ± 9.67의 AcrySof ReSTOR SN6AD1군에 비해 유의하게 높았고 암순응 상태의 6 cpd에서도 40.26 ± 11.38로 28.97 ± 10.45의 AcrySof ReSTOR SN6AD1군에 비해 유의하게 높았으며 이외의 대비에서는 유의한 차이가 없었다. 구면수차는 Acri Lisa 366D군에서 0.037 ± 0.039 μm로 AcrySof ReSTOR SN6AD1군의 0.105 ± 0.066 μm에 비해 유의하게 낮았고 전체 및 고위수차와 coma 및 trefoil은 차이가 없었다. 결론: 회절성 다초점 인공수정체 중 Acri Lisa 366D는 대비감도와 구면수차 면에서, 그리고 AcrySof ReSTOR SN6AD1는 중간거리 시력에서 상대적으로 나은 결과를 보였다. Purpose: To compare postoperative clinical outcomes, optical quality, and patient satisfaction between two types of diffractive multifocal intraocular lens (IOL, Acri Lisa 366D and Acrysof ReSTOR +3.00 D). Methods: In a total of 68 eyes, one of two diffractive multifocal IOL (Acri Lisa 366D and Acrysof ReSTOR +3.00 D) was implanted after cataract extraction. Visual acuity was measured postoperatively at one week, 1 month, and 6 months. Contrast sensitivity, wavefront aberration, and visual function were determined via questionnaire at postoperative 1 month. Results: Intermediate visual acuity of Acri Lisa 366D and Acrysof ReSTOR at 6 months were 0.31 ± 0.14, and 0.24 ± 0.11 (log MAR), respectively. At 6 months, near and distant visual acuity results showed no significant differences between the two groups. The photopic contrast sensitivity of Acri Lisa 366D at 6 cycles/degree was 55.36 ± 7.40 and showed significant differences with Acrysof ReSTOR (47.25 ± 9.67). The mesopic contrast sensitivity values of Acri Lisa 366D and Acrysof ReSTOR were 40.26 ± 11.38 and 28.97 ± 10.45, respectively, and the spherical aberration values were 0.037 ± 0.039 μm and 0.105 ± 0.066 μm. The spherical aberration of Acri Lisa 366D was significantly lower than that of Acrysof ReSTOR. Total and high order aberration, coma, and trefoil show no significant differences between the two groups. Conclusions: The Acri Lisa 366D multifocal IOL showed better contrast sensitivity and spherical aberration compared to Acrysof ReSTOR multifocal IOL, which had an effective intermediate visual acuity.
