This report presents a case of patent ductus arteriosus complicated with total left lung atelectasis and mitral regurgitation. Her mother complained growth retardation and exertional dyspnea. The 3 year old girl had large patent ductus arteriosus [Qp/Qs=5.6] which resulted in moderate pulmonary hypertension, left atrial hypertrophy and enlargement, consequently the left main bronchus was compressed between the dilated left atrium and aorta. We would like conclude the cause of mitral regurgitation as the result of annular dilatation secondary to left atrial enlargement rather than congenital associated to patent ductus arteriosus. 3 weeks later from ligation of patent ductus arteriosus, the left atrial dimension was markedly reduced echocardiographically [from 3.9cm to 2.7cm], and the left lung progressively aerated by halves.

방송영상 촬영감독의 자기효능감이 직무성과에 미치는 영향: 영상콘텐츠 제작 창의성과 직무만족의 매개효과

본 연구는 방송영상 촬영감독의 자기효능감(자신감, 자기조절 효능감, 과제난이도 선호)이 영상콘텐츠제작 창의성과 직무만족, 직무성과에 미치는 영향을 살펴보고, 제작 창의성과 직무만족의 매개효과를 밝히고자 했다. 이를 위해 지상파, 지역방송사, 종합편성채널, 홈쇼핑, 케이블, 프리랜서 등 국내 방송영상 촬영감독148명을 대상으로 온라인 설문조사를 실시하였다. 연구 결과, 방송영상 촬영감독의 자기효능감 하위요인 중자기조절 효능감이 영상콘텐츠 제작 창의성과 직무성과에 유의한 영향을 미치는 것으로 나타났으며, 과제난이도 선호 역시 제작 창의성에 유의한 영향을 미치는 것으로 확인되었다. 또한 자아효능감의 세 하위요소인자신감, 자기조절 효능감, 과제난이도 선호 모두 직무성과에 영향을 미침에 있어 제작 창의성이 매개효과를갖는 것으로 나타났다. 이와 함께 직무만족은 과제난이도 선호가 제작 창의성을 거쳐 직무성과에 영향을 미침에 있어 직렬다중매개효과를 갖는 것으로 확인되었다. 본 연구는 그동안 부족했던 방송영상 촬영감독을대상으로 자기효능감의 하위 요소들이 직무성과에 미치는 영향을 살펴보고, 영상콘텐츠 제작 창의성과 직무만족의 매개효과가 있는지 고찰한 데 의의가 있다. 또한 분석결과를 바탕으로 실무적으로 적용할 수 있는효율적인 방안을 모색하였다. The present study examined the effects of video program production directors’ self-efficacy (confidence, self-control efficacy, and task difficulty preference) on their job performance, highlighting the mediating role of content production creativity and job satisfaction. The study collected data via an online survey from 148 video program production directors who had worked for broadcasting networks, local broadcasters, home-shopping channels, cable networks, and freelancers in South Korea. Findings indicate that self-control efficacy affected content production creativity and job performance, while task difficulty preference influenced content production creativity. Also, content production creativity mediated the associations between all sub-factors of self-efficacy and job performance. However, job satisfaction mediated only the association between task difficulty preference and job performance as a serial multiple mediator model. The present study contributes to the understanding of the associations between video program production directors’ self-efficacy, content production creativity, job satisfaction, and job performance, which have not been investigated in previous studies. The study also offers practical implications by examining the mediating role of content production creativity and job satisfaction.

전페정맥 이상환류 (TAPVD)의 치험 1례

This is a case report of total anomalous pulmonary venous drainage with Atrial septal defect, which was corrected surgically by intracardiac procedure under total cardiopulmonary bypass. This 9 years old girl, she complained mild cyanosis, exertional dyspnea, and frequent upper respiratory infection from 3 months age. The chest X-ray showed cardiomegaly and a "snow man appearance", she has systolic murmur with splitting of S2. The diagnosis was confirmed with right heart catheterization, the catheter was reached to right pulmonic vein passed through right atrium, right superior vena cava, innominate vein, left superior vena cava, and common venous trunk. On 28th, Dec. 1979, an anastomosis between common pulmonary venous trunk and left atrium, Pericardial patch closure of ASD, and ligation of left superior vena cava were performed with Extra-corporeal circulation. The postoperative course was uneventful and discharged with excellent general condition.Total Anomalous Pulmonary Venous Drainage with A.S.D. with A.S.D.

동측폐에 발생한 선암과 편평상피세포암의 수술 1례

This is one case report of coexisting two histologically different carcinomas in the different lobes of the right lung, which operated the right pneumonectomy in the Dept. of Thoracic and Cardiovascular Surgery Hanyang University Hospital. The 43 years old male patient complained coughing, blood tinged sputum and weight loss. The chest films [PA and lateral view] showed a small round hazy shadow in the posterior segment of the right upper lobe and irregular hazy densities in the right middle lobe and hilar area. Histopathologically, the tumor in the posterior segment of the right upper lobe was consisted of a tissue of adenocarcinoma and the tumor in the right middle lobe was consisted of a tissue of well-differentiated bronchial squamous cell carcinoma.

Endovascular Treatment for Ruptured Distal Anterior Inferior Cerebellar Artery Aneurysm

A 42-year-old woman presented with Hunt and Hess grade (HHG) III subarachnoid hemorrhage (SAH) caused by a ruptured left distal anterior inferior cerebellar artery (AICA) aneurysm. Computed tomography showed a thin SAH on the cerebellopontine angle cistern, and small vermian intracerebral hemorrhage and intraventricular hemorrhage in the fourth ventricle. Digital subtraction angiography revealed the aneurysm on the postmeatal segment of left distal AICA, a branching point of rostrolateral and caudomedial branch of the left distal AICA. Despite thin caliber, tortuous running course and far distal location, the AICA aneurysm was obliterated successfully with endovascular coils without compromising AICA flow. However, the patient developed left side sensorineural hearing loss postoperatively, in spite of definite patency of distal AICA on the final angiogram. She was discharged home without neurologic sequela except hearing loss and tinnitus. Endovascular treatment of distal AICA aneurysm, beyond the meatal loop, is feasible while preserving the AICA flow. However, because the cochlear hair cell is vulnerable to ischemia, unilateral hearing loss can occur, possibly caused by the temporary occlusion of AICA flow by microcatheter during endovascular treatment.

Suppressing the verbal expression of anger is related to increased pain severity. We investigated the relationship between the pain and anger using a questionnaire in patients with back or neck pain. From August 2007 to January 2008, 173 patients visited our hospital due to back and/or neck pain. They answered the questionnaire before physical examination by themselves. Visual analog scale (VAS) was used to measure the intensity of the pain. The same method was used to measure the intensity and frequency of the anger. We also asked the degree of sleep disturbance, fatigue, loss of appetite, and mood. Psychological disturbances were relatively common; moderate to severe sleep disturbance in 66%, fatigue in 79%, loss of appetite in 55%, and depressed mood in 86%. More than a half answered their anger was moderate to severe in both frequency and degree. All psychological factors appeared to be strong contributors to the intensity of pain by a linear regression analysis. More than a half of the patients with back or neck pain have significant psychological problems. Proper evaluation and management of those psychological factors are important. Cervical Epidural Abscess: Rare Complication of Bacterial Endocarditis with Streptococcus Viridans : A Case Report

Although many patients with infective endocarditis (IE) complain of joint, muscle, and back pain, infections at these sights are rare. The incidence of spinal abscess in cervical spine complicating endocarditis is very rare. Although the surgical management is the mainstay of treatment, conservative treatment can get success in selected patients. We report a patient with cervical epidural abscess due to Streptococcus viridans endocarditis. Both epidural abscess and IE were managed conservatively with intravenous antibiotics for 8 weeks, with recovery. It is important to remind spinal epidural abscess can occur in those patients with bacterial endocarditis.
